Abstract

Purpose

The purpose of this paper is twofold: first, to present patient-level utilisation patterns of hospital-based mental health services funded by private health insurers; and second, to examine the implications of the findings for planning and delivering private mental health services in Australia.

Design/methodology/approach

Analysing private health insurance claims data, this study compares differences in demographic and hospital utilisation characteristics of 3,209 patients from 13 private health insurance funds with claims for mental health-related hospitalisations and 233,701 patients with claims for other types of hospitalisations for the period May 2014 to April 2016. Average number of overnight admissions, length of stay and per patient insurer costs are presented for each group, along with overnight admissions vs same-day visits and repeat services within a 28-day period following hospitalisation. Challenges in analysing and interpreting insurance claims data to better understand private mental health service utilisation are discussed.

Findings

Patients with claims for mental health-related hospitalisations are more likely to be female (62.0 per cent compared to 55.8 per cent), and are significantly younger than patients with claims for other types of hospitalisations (32.6 per cent of patients aged 55 years and over compared to 57.1 per cent). Patients with claims for mental health-related hospitalisations have significantly higher levels of service utilisation than the group with claims for other types of hospitalisations with a mean length of stay per overnight admission of 15.0 days (SD=14.1), a mean of 1.3 overnight admissions annually (SD=1.2) and mean hospital costs paid by the insurer of $13,192 per patient (SD=13,457) compared to 4.6 days (SD=7.3), 0.8 admissions (SD=0.6) and $2,065 per patient (SD=4,346), respectively, for patients with claims for other types of hospitalisations. More than half of patients with claims for mental health-related hospitalisations only claim for overnight admissions. However, the findings are difficult to interpret due to the limited information collected in insurance claims data.

Practical implications

This study shows the challenges of understanding utilisation patterns with one data source. Analysing insurance claims reveals information on mental health-related hospitalisations but information on community-based care is lacking due to the regulated role of the private health insurance sector in Australia. For mental health conditions, and other chronic health conditions, multiple data sources need to be integrated to build a comprehensive picture of health service use as care tends to be provided in multiple settings by different medical and allied health professionals.

Originality/value

This study contributes in two areas: patient-level trends in hospital-based mental health service utilisation claimed on private health insurance in Australia have not been previously reported. Additionally, as the amount of data routinely collected in health care settings increases, the study findings demonstrate that it is important to assess the quality of these data sources for understanding service utilisation.

Abstract

Purpose

The purpose of this paper is to explore the proportions of health‐related content in non‐program time on Australian television, and how this relates to channel, product category, program genre and whether it is an advertisement or public service announcement (PSA).

Design/methodology/approach

Australian prime time television was recorded across three commercial television stations in Sydney. Non‐program content (NPC) was coded according to the channel, program genre, length of content and product advertised. In total, 44 hours of programming was recorded.

Findings

Not surprisingly, significant differences were found in the percentage of health‐related content between advertisements (22 percent) and PSAs (67 percent). Again unsurprisingly there was also a significant relationship between the product category and health‐related content (χ2=366.601, p=0.000), but also between health‐related content and program genre (χ2=20.594, p=0.024), particularly situation comedies (31 percent) and sport (15 percent). No difference was found in the percentage of health‐related content between the channels.

Research limitations/implications

Differences existing in the amount of NPC across program genre suggest that viewers of programs with high rates of health‐related content in advertising may have higher exposure to product dependant health information.

Originality/value

Health information is examined in a general sample without focus on particular demographics or health topics and the role of program genre is investigated.

Abstract

Purpose

Little is known about health‐related advertising on university environments. Given the power of advertising and its potential effect on health behaviors, the purpose of this paper is to assess the health‐related advertisement environment and policies on university campuses.

Design/methodology/approach

In total, ten geographically and ethnically diverse US university campuses that were trained in using the health‐related advertisement survey tool participated in the study. Inter‐rater reliability with data collectors at each university was established before data commencement began in Spring 2011. The survey tool assessed the types, locations, and prevalence of health‐related advertisements and messages (e.g. nutrition, alcohol, tobacco) on campus, and included both advertisements and messages related to any aspect of health by any sponsor. Current campus health‐related policies from each institution were collected as well.

Findings

The largest proportion of advertisements on all campuses were for diet/nutrition, exercise/fitness, and alcohol. The majority of advertisements promoted positive health behaviors recommended by health professionals. Unbranded advertisements were more likely to promote positive health behaviors than branded advertisements. Diet/nutrition, tobacco, and drug advertisements were more likely to be positive, whereas alcohol‐related advertisements tended to be negative.

Originality/value

The paper's findings indicate significant gaps in campus health‐related policies with regard to healthy eating and physical activity and lack of policies covering health‐related advertisement content. Benchmark data like those reported here can help campus stakeholders set priorities and work with campus decision makers to advocate for the development and implementation of healthy campus policies that support healthy environments.

Purpose

Over the past few years, the internet has expanded rapidly, and it has been considered a systematic way that consumers use to retrieve health-related information. However, the existing literature does not provide an articulated view of online health information-seeking behaviour through an in-depth understanding of users’ searching-related behaviour. The objectives of this study are to identify the factors affecting consumers’ health-related internet use and recognise the relationships between those specified and health-related internet use. Finally, the recommendations are made based on the findings.

Design/methodology/approach

An amalgamated model of technology acceptance model and health belief model was used to hypothesise health-related internet use behaviour, which is then tested using a cross-sectional survey of 287 Sri Lankan managerial-level employees. The covariance-based structural equation modelling with AMOS was used to check the study hypotheses.

Findings

Findings of this study depict five factors contributing to consumers’ health-related internet use as follows: perceived health risk towards chronic diseases consisting of perceived susceptibility and perceived severity; health consciousness; perceived usefulness of the internet; perceived ease of internet use; and attitude towards health-related internet use. As theorised, the internet’s perceived usefulness was positively and significantly related to consumers’ use of health-related internet and attitude towards health-related internet use. But as hypothesised, perceived ease of internet use did not directly affect consumers’ use of health-related internet. Further, findings reveal that health-related internet use is estimated by perceived health risk than health consciousness.

Originality/value

Findings reveal that Sri Lankan managerial-level employees have a reactive health behaviour driven by the perceived health risk and the desire to seek online health information.

Abstract

Purpose

This paper aims to examine the effect of three types of health-related claims (health, nutrition and ingredient) and product healthiness on situational skepticism toward the claims that appear on the front-of-package of food products. The effect of situational skepticism on the purchase intention of the product is further examined.

Design/methodology/approach

Two experimental studies were conducted with a 3 (health-related claims: health claim vs nutrition claim vs ingredient claim) × 2 (product healthiness: healthy vs unhealthy) between-subjects factorial design. Study 1 investigates the effects within a single product category (Biscuits) and Study 2 the effects across product categories (Salad and Pizza).

Findings

The results demonstrate that situational skepticism is the highest for health claims, followed by nutrition claims and the least for ingredient claims. In addition, situational skepticism is higher for claims appearing on unhealthy products vis-à-vis healthy ones. Finally, situational skepticism mediates the relationship between claim type, product healthiness and product purchase intention.

Research limitations/implications

This study contributes to the field of nutrition labeling by advancing research on information processing of nutrition labels through the lens of the persuasion knowledge model (Friestad and Wright, 1994). Specifically, this study contributes to a nuanced understanding of claim formats on how the language properties of the claim – its vagueness, specificity and verifiability – can affect consumer perception. This study finds that higher specificity, verifiability and lower vagueness of ingredient claims lead to lower skepticism and hence higher purchase intention.

Practical implications

Furthermore, this study incrementally contributes to the ongoing discussion about the claim–carrier combination by showing that health-related claims are better perceived on healthy compared to unhealthy products. Hence, managers should avoid health washing, as this can backfire and cause harm to the reputation of the firm.

Social implications

From a public policy point of view, this study makes a case for strong monitoring and regulations of ingredient claims, as consumers believe these claims easily and hence can be misled by false ingredient claims made by unethical marketers.

Originality/value

The scope of research on skepticism has largely been limited to examining a general individual tendency of being suspicious (i.e. dispositional skepticism) in health-related claims as well as other areas of marketing. In this research, the authors extend the scope by examining how specific types of claims (health vs nutrition vs ingredient) and product healthiness jointly impact consumer skepticism, i.e. situational skepticism.

Purpose: The aim of the study is to consider the use of the Internet as a potential facilitator of positive health-related perceptions. Specifically, we propose that online health information seeking fosters positive perceptions of health. Using path modeling, we theorized several mechanisms through which information seeking could be conducive to positive health perceptions, which we conceptualized into the following four dimensions: (1) sense of empowerment in managing health, (2) self-reported ability to take better care of health, (3) sense of improved health-related quality of life, and (4) self-reported improvement of health.

Methodology: Our sample consisted of respondents who have used the Internet as a resource for health information (n = 710), drawn from the largest national probability-based online research panel. Our comparison subsample consisted of older respondents (age ≥ 60; n = 194). We used Internet-specific measures and employed structural equation models (SEM) to estimate the direct, indirect, and total effects of health-related use of the Internet on subjective health perceptions. Based on our review of the literature, competent health communication with healthcare providers and sense of empowerment in managing personal health were modeled as mediator variables. We assessed whether the proposed mediational relationships, if significant, differed across our indicators of positive health perceptions and whether any differential associations were observed among older adults. We run parallel models for each indicator of positive health perception.

Findings: Provider-patient communication informed by the Internet resources were perceived to impart a greater sense of empowerment to manage health among our respondents, which in turn, was associated with perceived contributions to better self-reported ability to provide self-care, increased health-related quality of life, and improvement in self-reported health. The SEM results revealed a good fit with our full sample and subsample.

Research Implications: Conceptualization of the multidimensional aspects of online health information seeking with separate multi-indicator analyses of the outcome variable is important to further our understanding of how technology may impact the pathways involved in influencing health perceptions and as a result health outcomes.

Purpose

Internet is both a medium and a platform for information exchange. This characteristic of internet is gradually metamorphosing it into an e-learning enabler. A significant percentage of internet users access health-related information through the medium of internet, but little is known about the factors that determine such behaviour. This study aims to explore the factors that determine the behaviour of those seeking health-related information in the virtual world.

Design/methodology/approach

The present study analyses the role of perceived ease of usefulness (PEOU), perceived usefulness (PU), self-efficacy (SE) and information quality (IQ) on the intention (IU) of the internet users to seek health-related information. Researcher-controlled sampling was used for data collection from 210 university students. Exploratory factor analysis (EFA), t-test and correlation have been used for data analysis.

Findings

The results of the study demonstrate that except SE, all other factors have a positive relationship with the intention of the users to seek health-related information. The findings suggest that PU and IQ overshadow PEOU in encouraging the users seeking such health-related information over the internet.

Originality/value

The present study extends the understanding of e-learning adoption associated with seeking health-related information. The researchers propose an extended technology acceptance model (TAM) model to study the factors influencing the use of internet in seeking health-related information by the students enrolled in medical and non-medical courses.

Purpose

The police in Trinidad experience extremely stressful job conditions (e.g., elevated rates of violent crime, the rapid spread of organized crime, financial constraints, and staff shortages) which could lead to health-related problems. The purpose of the current study is to examine the relationship between job stress and health-related problems and to determine whether burnout mediates this relationship. Data from police officers in Trinidad was used to test these relationships.

Design/methodology/approach

Using self-report questionnaires from all eight police station districts in Trinidad (N = 331 police officers), this study conducted structural equation modeling (SEM) to examine the relationship between job stress, burnout and health-related problems.

Findings

The results indicated that officers' job stress increased their burnout and health-related problems. As the authors expected, officers' burnout, served as a mediating variable between job stress and health-related problems. In other words, job-related stress can lead to burnout, which in turn, leads to health-related problems.

Research limitations/implications

Despite meaningful findings, a few limitations (e.g., measurement issues and missing values) were present in this study.

Originality/value

Policing studies have paid less attention to Caribbean policing. Findings in this study have implications for addressing officers' health-related problems.

Abstract

Purpose

The purpose of this paper is to understand the behavior diversities that exist among young millennials’ subgroups in ways they seek health-related information.

Design/methodology/approach

The authors ran several sets of analyses on the 2012–2014 US Program for the International Assessment of Adult Competencies (PIAAC) Data using Stata. The population was stratified into four specific subgroups based on their gender, ethnicity—blacks, Hispanics and whites—immigration status, college status—whether they were enrolled in a program of study at the time of the survey. The outcome variables were sources of health information including print (books/magazines/brochures), traditional media (Radio/TV), internet, family/friends/co-workers and health professionals. The independent variables were gender, ethnicity, educational status and immigration status. The authors utilized the appropriate sample weight derived by Organization for Economic Cooperation and Development so the findings can be generalized to the populations. The analysis included several descriptive statistics and χ2 test of independence.

Findings

Despite similarities, young adults’ health seeking behavior is complex influenced by gender, ethnicity, immigration status and education. The results indicated that while the internet is the primary source of health-related information for all young adults, there are subtle differences in utilizing other available resources. For example while more educated young adults seek help from their family members, the less educated peers use the media to obtain health-related information. Ethnicity has also an effect on young adults’ information seeking behavior. The number of Hispanics and blacks that obtain their information from traditional media is significantly higher than their white counterparts.

Research limitations/implications

This study has several limitations. First, the authors did not consider the effect of young adults’ digital literacy skills, problem solving skills and numeracy skills on their health seeking approach. Including these cognitive skills could reveal key information about young adults approach to information seeking that is not apparent by race, ethnicity and gender only. Another limitation of this study is the lack of the ability to claim causation, PIAAC data are designed strictly for cross-sectional analysis.

Practical implications

Although, behaviors often do not change simply by presenting information, trying to change behavior without improving individuals’ understanding of the issue by providing accurate information is likely to fail. Providing standardized health-related information sources that are accessible to all is vitally important. The results indicate that while the majority of young adults use the internet as their primary source of information only a few percentage of young adults seek information from health professional. Consequently, there is a need for an easily accessible and standardized online health-related source of information.

Social implications

Healthcare facilities and health related industries have the resources and the ability to develop a reliable infrastructure that could potentially provide reliable information that is easy to understand and navigate for adults with a variety of literacy and skills to use. Perhaps adopting the Universal Design for Learning approach and providing information that is accessible to a variety of individuals regardless of their education, learning skills and language skills. Flexible learning resources provided within a standard infrastructure accessible to all can help individuals find trustworthy and consistent information that they can trust.

Originality/value

Despite the unique characteristics of the millennials and the profound change in the way young adults seek information, there is a paucity of research on the ways young adults seek health-related information. Most existing literature is based on locally developed surveys and convenient sampling with limited reliability and validity information. Consequently making a sweeping statement based on their findings is considered as hasty generalization. The PIAAC, on the other hand, is a nationally representative data, extensively examined for its validity and reliability.

Abstract

A policy of surveillance which interferes with the fundamental right to a private life requires credible justification and a supportive evidence base. The authority for such interference should be clearly detailed in law, overseen by a transparent process and not left to the vagaries of administrative discretion. If a state surveils those it governs and claims the interference to be in the public interest, then the evidence base on which that claim stands and the operative conception of public interest should be subject to critical examination. Unfortunately, there is an inconsistency in the regulatory burden associated with access to confidential patient information for non-health-related surveillance purposes and access for health-related surveillance or research purposes. This inconsistency represents a systemic weakness to inform or challenge an evidence-based policy of non-health-related surveillance. This inconsistency is unjustified and undermines the qualities recognised to be necessary to maintain a trustworthy confidential public health service. Taking the withdrawn Memorandum of Understanding (MoU) between NHS Digital and the Home Office as a worked example, this chapter demonstrates how the capacity of the law to constrain the arbitrary or unwarranted exercise of power through judicial review is not sufficient to level the playing field. The authors recommend ‘levelling up’ in procedural oversight, and adopting independent mechanisms equivalent to those adopted for establishing the operative conceptions of public interest in the context of health research to non-health-related surveillance purposes.
