Abstract

Purpose

Integrated care continues to be a central aim within health and social care policy in England. Personal budgets and personal health budgets aim to place service users at the centre of decision-making and are part of a wider long-term initiative working towards personalised and integrated care. Personal budgets began in social care with the national pilot programme of individual budgets, which aimed to incorporate several funding streams into one budget, but in practice local authorities limited these to social care expenditure. Personal budgets then moved into the health care sector with the introduction of a three-year personal health budgets pilot programme that started in 2009. The purpose of the paper is to explore the post-pilot implementation of personal health budgets and explore their role in facilitating service integration. We examine this through the RE-AIM framework.

Design/methodology/approach

During 2015 and 2016, eight organisational representatives, 23 personal health budget holders and three service providers were interviewed, 42 personal health budget support plans were collected and 14 service providers completed an online survey.

Findings

Overall, personal health budgets continued to be viewed positively but progress in implementation was slower than expected. Effective leadership, clear communication and longer-term implementation were seen as vital ingredients in ensuring personal health budgets are fully embedded and contribute to wider service integration.

Originality/value

The paper highlights the importance of policy implementation over the longer-term, while illustrating how the venture of personal health budgets in England could be a mechanism for implementing service integration. The findings can serve to guide future policy initiatives on person-centred care and service integration.

Abstract

Purpose

The purpose of this paper is to illustrate the potential for service integration by focusing at the level of the individual rather than through structural integration or care coordination. The paper illustrates the potential for integration through the experience of the personal health budgets pilot programme.

Design/methodology/approach

This paper is based on a review of the experience of personal health budget pilot sites, drawing on the national evaluation of the pilot programme and the author's experience of working with pilot sites between 2010 and 2013. It also draws on the experience of similar programmes in the USA.

Findings

Personal health budgets support integration in two distinct ways. First, they can support the delivery of more holistic, whole‐person care in line with the principles of shared decision making. Second, by bringing personal budgets in social care and personal health budgets together, they can provide a vehicle for integration across health and social care systems. If integration starts from, and responds to, what matters most to individuals rather than with the development of joint structures and processes, the result is more likely to be integrated care.

Originality/value

This is one of only a small number of papers that discusses the scope for personal health budgets to improve the integration of care. Integration between the NHS and social care, in particular, has been identified as a central priority for the NHS.

Abstract

Purpose

Current research topics in relation to health information searching focus on challenges faced by health consumers and domains used to perform the health search. Health consumers may not be capable of successfully searching for a health task due to limited medical knowledge. As such search assisting features provided on health domains are important in assisting health consumers during a search session. The purpose of this paper is to perform a preliminary exploratory research study to understand if search assisting features are visible to searchers and the usage of search assisting features when searching on a personal health task.

Design/methodology/approach

A convenience sampling method in a university setting and an observational type study was used. MedlinePlus is used as the search domain for this research study. While participants of this research study were first time users of MedlinePlus, they were not first time medical searchers.

Findings

Results of this research study indicate health consumers do not utilize search assisting features when searching for a personal health task. This is because health consumers are comfortable with their search skills. In other cases health consumers found the search assisting features irrelevant or had no confidence in the search assisting features presented. Key contributions of this research study indicate health consumers do not utilize search assisting features when searching for a personal health task. Research limitations/implications

Results of this research study has implications for health domain and human computer designers in relation to the development of specialized search assisting features and the placement of these features. Theoretical contributions indicate health searchers use search assisting features minimally when searching on a personal health task.

Abstract

Purpose

This paper aims to present the results of a three‐year comprehensive workplace initiative which provided an unprecedented opportunity to explore the potential relationship between organisational health, stress, and health outcomes.

Design/methodology/approach

Data were collected from 325 employees participating in a comprehensive workplace wellness intervention taking place in a large governmental organisation. Organisational health was measured using a 16‐item measure of organisational health indicators and a four‐item measure of health culture. Personal health outcomes were assessed using 12 indicators: personal wellness profile, health age, blood pressure, nutrition, fat intake, fibre intake, alcohol use, fitness, smoking status, cancer risk, stress, and good health practices.

Findings

Analyses indicated that after controlling for gender and age, organisational health was associated with increased personal wellness, lower health age, better overall nutrition, reduced fat intake, increased fibre intake, reduced alcohol consumption, increased fitness, reduced cancer risk, lower stress, and more positive health practices. For several outcome measures, organisational health had a stronger impact on personal health for men. Personal health of correctional workers and youth workers was most influenced by organisational health. Finally, stress mediated the relationship between organisational health and health outcomes for all measures of wellness except for alcohol consumption.

Originality/value

Organisational health is often overlooked by employers when considering the personal health of employees. Interventions aimed at influencing organisational health (generally considered a low cost intervention) can have beneficial influences on personal health.

Abstract

Purpose

Mobile health service (MHS) can provide users with convenient health services and information to reduce their medical costs from aging or other health issues. Previous studies confirm the underdevelopment of the Chinese MHS. The purpose of this paper is to analyze the factors that affect the intention to use MHS.

Design/methodology/approach

This paper develops a research model that integrates personal health differences with theory of planned behavior (TPB) and protection motivation theory (PMT). This model is empirically tested using data from 494 valid questionnaires. Structural equation modeling is used to test the hypotheses.

Findings

Perceived vulnerability to disease, perceived severity of disease, response efficacy and self-efficacy positively affect attitude, thereby exerting a positive influence on the behavioral intention to use MHS. Subjective norms also influence users’ behavioral intention. Personal health status and personal health value have quasi-moderating effects on the relationship between attitude and behavioral intention.

Originality/value

This paper presents an early attempt to conceptualize and validate a research model of MHS acceptance by integrating TPB and PMT in a complementary manner. The integrated model provides a holistic view of people’s intention to use MHS by considering health threat beliefs, individual role (i.e. attitude and self-efficacy) and social influences (i.e. subjective norms). Furthermore, this research highlights the role of two individual health characteristics (i.e. personal health status and personal health value) in MHS adoption. These new findings are beneficial toward an in-depth understanding of technology adoption in the MHS context.

Abstract

Purpose

This paper aims to identify the reasons why cases of leakage of patient personal data often occur in the health sector. This paper also analyzes personal data protection regulations in the health sector from a comparative legal perspective between Indonesia, Singapore and the European Union (EU).

Design/methodology/approach

This type of research is legal research. The research approach used is the statute approach and conceptual approach. The focus of this study in this research is Indonesia with a comparative study in Singapore and the EU.

Findings

Cases of leakage of patient personal data in Indonesia often occur. In 2021, the data for 230,000 COVID-19 patients was leaked and sold on the Rapid Forums dark web forum. A patient’s personal data is a human right that must be protected. Compared to Singapore and the EU, Indonesia is a country that does not yet have a law on the protection of personal data. This condition causes cases of leakage of patients’ personal data to occur frequently.

Research limitations/implications

This study analyzes the regulation and protection of patients’ personal data in Indonesia, Singapore and the EU to construct a regulatory design for the protection of patients’ personal data.

Practical implications

The results of this study are useful for constructing regulations governing the protection of patients’ personal data. The regulation is to protect the patient’s personal data like a patient’s human right.

Social implications

The ideal regulatory design can prevent data breaches. Based on the results of comparative studies, in Singapore and the EU, cases of personal data leakage are rare because they have a regulatory framework regarding the protection of patients’ personal data.

Originality/value

Legal strategies that can be taken to prevent and overcome patient data breaches include the establishment of an Act on Personal Data Protection; the Personal Data Protection Commission; and management of patients’ personal data.

Abstract

Purpose

Building on theoretical foundation of personal information management (PIM) in information science, this paper seeks to understand how activity tracker users manage their personal health information generated by their devices and to elucidate future activity tracking technology in support of personal health information management (PHIM). This paper aims to discuss this issue.

Design/methodology/approach

The authors conducted a web survey study with a specific group of activity tracker users – amateur runners. This survey collected both quantitative and qualitative data on participants’ engagement with activity tracking technology, their PHIM practices with the information generated by the technology and how their needs were being met by their activity tracking technology use and PHIM practices.

Findings

Amateur runners surveyed in this study exhibit long-term engagement and frequent interaction with activity tracking technology. They also engage in PHIM practices by using a range of PHIM tools and performing various PHIM activities. Furthermore, they use activity tracking technology and engage in PHIM practices to meet various health/fitness-related needs and information needs, while some of these needs such as performance needs and overarching needs are only partially met or unmet.

Originality/value

This research discusses amateur runners as power users of activity tracking technology, provides timely updates to PIM and PHIM research in light of a new type of personal health information, and generates design considerations for future activity tracking technology in support of PHIM. It also brings together previously disparate research regarding everyday life PHIM in information science, human–computer interaction and health informatics.

Purpose

There are several tensions in the disclosure of personal experience of mental health problems. The issue of “otherness” is particularly pertinent when exploring mental health. This paper seeks to draw upon workshop discussion around the theme of educators disclosing personal experience of mental health problems.

Design/methodology/approach

This paper is based around a critically reflective self‐selecting conference workshop for mental health educators, practitioners and others.

Findings

There are risks and benefits to personal disclosure of lived experience of mental health problems. It was found that three areas emerged when considering the dilemmas of self‐disclosure: personal impact, wider ethical issues and student learning.

Practical implications

The paper provides information on how educators can maximise the benefits and minimise the hazards associated within these areas as a means to develop good practice guidance and when and how to disclose personal experience.

Originality/value

The paper finds its value in the inclusion of recognised lived experience of mental health problems amongst educators and the tensions apparent with disclosure about such experiences.

Privacy and security are coming under more and more scrutiny in this age of digital information that can be generated, duplicated, and transferred with increasing facility. Nowhere is this so apparent as in the field of health care, where breaches to security and privacy carry very personal and potentially harmful consequences. Health Information Networks must deal with these issues carefully as they seek to share sensitive information among health‐care providers to improve patient care. This article examines issues related to privacy in Health Information Networks in Canada, provides a summary of relevant federal and provincial legislation, and through a case study offers suggestions for future directions in the arena where health‐care and privacy issues meet.

Abstract

Purpose

Service alignment between health service provider and patient is changing. Instead of placing responsibilities into the hands of a provider, new forms of co-operation are emerging in which patients are regarded as a resource and a partner. In order to see this vision come to life, mechanisms that: first, support patient’s health decision making; and second, integrate matters of health into a wider ensemble that is health space; the overarching state of health-related affairs, are needed. In the following, these kinds of mechanisms are investigated and their applicability is discussed in relation to a national project. The paper aims to discuss these issues.

Design/methodology/approach

The work is exploratory and conceptual, focussing more on people than on technology. In the work, findings related to a concept of a health navigator; an artefact of personal health decision support, are assembled into a framework that bases on key sociological theories. The empirical elements focus on observations made on applicability of the concept, and the underlying framework of citizen-centric electronic health services.

Findings

The authors argue that the discussed concept, when applied to personal health decision making according to the underlying framework, has a potential to change health service provisioning. In addition to stimulating new kind of co-operation between the health service provider and the citizen, the concept gives form to, somewhat idealized, notions of patient choice and empowerment.

Research limitations/implications

The work described here is exploratory and forward-looking. Even though the concept and the framework are tested to a degree in a national project, more practice-oriented work is needed in terms of real-world applicability. It follows from this that the work is a conceptual elaboration on the future of personal health decision making.

Originality/value

The findings, including the discussed challenges and needs, stem from real-world observations; from the needs of citizens. As such, they indicate a direction into which the development of personal health records and health decision support aids should go.
