Abstract

Purpose: In the current work, we provide a portrait of heavy alcohol use, cigarette smoking, mental health, and suicide ideation by sexual orientation among a large sample of US adults aged 25 years and older.

Design/methodology/approach: We produce a repository of information on sexual orientation, substance use, mental well-being, and suicide ideation for adults aged 25 years and older using Behavioral Risk Factor Surveillance System (BRFSS) surveys for nine US states from 2011 to 2018. We establish baseline differences on these outcomes for gay, lesbian, and bisexual (GLB), relative to heterosexual, adults and then use regression techniques to adjust the estimates for important sociodemographic, socioeconomic, and relationship status variables.

Findings: Disparities by sexual orientation across substance use, mental health, and suicide ideation are concerning, some alarmingly so. Bisexuals, particularly women, face pronounced challenges across outcomes. Sexual minority men and women report significantly more poor mental health days and much higher odds of suicide ideation. To illustrate, gay men, lesbians, and bisexual men and women, relative to their heterosexual counterparts, have odds of seriously contemplating taking their own lives that are two to four times higher even after adjusting for relevant controls.

Originality/value: Existing knowledge connecting GLB identity and mental well-being has focused largely on adolescent and young adults. We provide a representative study on older adult differences across four different behavioral health outcomes by sexual orientation. The scale of the disparities we report here, and their implications for overall well-being across groups, deserves national attention and action.

Abstract

Purpose: To understand how lesbian, bisexual, transgender, queer, and asexual (LBTQA+) young women interpret the social construction of “lesbian obesity” in the context of their lived experiences and membership in the LGBTQ+ community.

Methodology: Individual, in-depth interviews were conducted with a convenience sample of 25 LBTQA+ women, ages 18–24, to explore how participants perceive and experience dominant discourses about gender, sexuality, and weight. Interviews were analyzed using a combination of deductive and inductive coding approaches.

Findings: Participants resisted public health discourse that frames obesity as a disease and the implication that their sexual identities put their health at risk. Many participants viewed their sexual identities and membership in the LGBTQ+ community as protective factors for their health statuses in general and their body image in particular.

Implications: Our findings suggest a need to reconsider the utility of the concept of “lesbian obesity” to characterize the significance of elevated rates of overweight and obesity in this population. Public health and clinical interventions guided by body positive approaches may be of greater relevance for sexual minority women.

Originality: This study centers the perceptions and experiences of LBTQA+ young women in order to examine how the intersections of sexual minority identity, dominant cultural ideals about weight, and obesity discourse inform their health.

Abstract

This chapter explores processes of stratification in reproductive healthcare and considers the ways in which mechanisms of inclusion/exclusion shape reproductive opportunities and experiences. First, I consider the process of “selective inclusion” among sexual minority women. This examination questions the schisms that exist within the sexual minority population in regard to their visibility and legibility in medical, scientific, and public health discourses and constructions of reproductive health. The second process I examine is that of “exclusionary inclusion” among substance using pregnant women who have been collectively deemed “bad breeders” by medical and state authorities and whose reproduction is explicitly monitored, regulated, and criminalized. The final process I discuss is “side-stepping inclusion” which describes the healthcare and consumer decisions of women who circumvent medicalized childbirth experiences by employing the services of a midwife for their pregnancy and birth care. This chapter examines how medicalization, biomedicalization, and de-medicalization dynamically work together to expand and delimit inclusionary processes, emphasizing the spectral and interconnected quality of these processes. By exploring various processes of inclusion that shape reproductive experiences of these disparate and differentially marginalized populations, this chapter provides a conceptual and critical meditation on the ways in which “respectable reproduction” is deployed in reproductive care. In considering these processes of inclusion and the ways in which they are co-produced by medical discourses and practices, scholars may more clearly grasp some fundamental mechanisms of stratification in reproductive healthcare and knowledge production.

Abstract

Purpose: We review theory and research to suggest how research on sexual and gender minority (SGM) population health could more completely account for social class.

Approach: First, we review theory on social class, gender, and sexuality, especially pertaining to health. Next, we review research on social class among SGM populations. Then, we review 42 studies of SGM population health that accounted for one or more components of social class. Finally, we suggest future directions for investigating social class as a fundamental driver of SGM health.

Findings: Social class and SGM stigma are both theorized as “fundamental causes” of health, yet most studies of SGM health do not rigorously theorize social class. A few studies control socioeconomic characteristics as mediators of SGM health disparities, but that approach obscures class disparities within SGM populations. Only two of 42 studies we reviewed examined SGM population health at the intersections of social class, gender, and sexuality.

Research implications: Researchers interested in SGM population health would benefit from explicitly stating their chosen theory and operationalization of social class. Techniques such as splitting samples by social class and statistical interactions can help illuminate how social class and SGM status intertwine to influence health.

Originality: We synthesize theory and research on social class, sexuality, and gender pertaining to health. In doing so, we hope to help future research more thoroughly account for social class as a factor shaping the lives and health of SGM people.

Purpose

The purpose of this paper was to examine the robustness of the findings on educational advantage among sexual minority men.

Methodology/approach

Using nationally representative data (AddHealth) and controlling for other predictors of academic attainment, we examine the educational attainment of sexual minority males by using hierarchical regression and logistical regression for two measures of sexual identity.

Findings

We find robust differences in educational attainment across analyses and sexual orientation constructs. Our results show sexual minority identity predicts up to a year more of education for male respondents and consistently reporting male homosexuals have an even greater advantage, more than one and a half years, compared to inconsistent responders.

Originality/value

Our results extend previous research on educational outcomes for nonheterosexual adolescents, suggesting there are sustained differences in long-term educational outcomes for nonheterosexual adults and supporting earlier analyses of the AddHealth survey data. This study contributes to the existing literature by examining educational attainment as measured by continuous years and cut-points, using two measures of sexual orientation, providing estimates for all Wave 4 sexual minority identities (i.e., not collapsing any sexual minority category), and controlling for adolescent school geography and type. Moreover, we find early identification of sexual orientation and stability of sexual orientation may be an important source of variation in identifying LGBTQ adolescents who are at greater academic risk or who may benefit from increased social support.

Abstract

Purpose

The purpose of this paper is to explore the positive and negative workplace experiences of gay men that they perceive to be a consequence of their sexual identity.

Design/methodology/approach

This paper uses in-depth interviews of a diverse group of university educated white collar men employed full-time in the public and private sector. Its findings are based on a thematic content analysis of these interviews.

Findings

Despite experiencing some negative consequences of being out at work, their positive and neutral experiences show encouraging signs of increasingly tolerant workplaces. Some of the challenges encountered that respondents believe to be a consequence of their sexual identity are, however, not dissimilar to those faced by workers with non-traditional families.

Research limitations/implications

As with any small sample exploratory qualitative research, this paper’s findings cannot necessarily be generalized to larger populations. The uniqueness of the sample (ethnically/culturally homogenous, university educated, public/private sector employees, residents of medium-sized Canadian city) allow for display of certain experiences not representative of the population at large.

Originality/value

This paper contributes to the relatively small but growing body of research on the experience of sexual minorities in the workplace. Its findings challenging the notion that sexual minorities are uniquely advantaged in the workplace, and that research on sexual minorities in the workplace is misguided in focusing on the problematic aspects of sexual identity/orientation.

Abstract

Purpose: The goal of Volume 21 of Advances in Medical Sociology, entitled Sexual and Gender Minority Health, is to showcase recent developments and areas for future research related to the health, well-being, and healthcare experiences of LGBTQA+ (Lesbian, Gay, Transgender, Queer, Asexual, and related communities that do not identify as heterosexual) persons and communities.

Approach: In this introduction to the volume, we trace the historical development of research on sexual and gender minority (SGM) health, discussing how priorities, theories, and evidence have evolved over time. We conclude with brief suggestions for future research and an overview of the articles presented in this volume.

Findings: Research on SGM health has flourished in the past two decades. This trend has occurred in conjunction with a period of intense social, political, and legal discourse about the civil rights of SGM persons, which has increased understanding and recognition of SGM experiences. However, recent advances have often been met with resistance and backlash rooted in enduring social stigma and long histories of discrimination and prejudice that reinforce and maintain health disparities faced by SGM populations.

Value: Our review highlights the need for additional research to understand minority stress processes, risk factors, and resiliency, particularly for those at the intersection of SGM and racial/ethnic or socioeconomic marginality.

Abstract

In this chapter, we examine historical and contemporary debates about the position and situation of women in Myanmar (and to a lesser extent gender and sexual minorities). Specific reference is made to the patriarchal character of the military coup of February 1, 2021, and the emergent forms of feminist resistance that turned social norms inside out in protest against lethal repression. The way women, as well as sexual and gender minorities, are unequally positioned and face structural and social discrimination in society serves to contextualize our presentation of the findings of a collaborative case-study conducted in 2018 on issues pertaining to gender and imprisonment in Myanmar, based mainly on interviews with former prisoners in three research sites. The aim was to generate field-based knowledge about the carceral experiences of women and LGBTQIA+ persons in Myanmar – focusing on their needs and vulnerabilities, their capacities and rights, their relationships and identities, and their modes of survival as they encounter penal regimes. Findings are summarized focusing on former prisoners’ experiences of legal bias; perspectives on encounters with the criminal justice system; the inadequacy of health provision (posed as lethal neglect); and the way certain behaviors and identities are criminalized. The chapter concludes with some reflections on the degree to which there might still be space for critical scholarship in the post-coup world and poses some questions for future research.

Abstract

Purpose

The purpose of this paper is to examine the relationship between “locker room” hegemonic masculinities at work and the construction of homophobia, particularly the use of the word “fag” to describe gay men – real or perceived. Although research indicates that men are more homophobic than women, examples are presented which examine some of the reasons why women use the word “fag” at work. Although equal opportunities at work have improved for sexual minorities over the past two decades, studies indicate that some forms of anti‐lesbian, bisexual and transgender (LGBT) behaviour continue, which raises the question whether a hierarchy of inequality exists in some organizations.

Design/methodology/approach

The data used to analyze this under‐researched phenomenon come from the author's observations working for three multinational corporations in the USA.

Findings

The paper shows how men and women engage in locker room culture to construct homophobic narratives.

Research limitations/implications

The issues raised in this article will be useful for empirical studies which examine the relationship between competitive sports and sexuality in the construction of masculine hegemonies in the workplace. Additionally, research should address the workplace experiences of sexual minorities who are also ethnic minorities, and disabled.

Originality/value

The paper contributes to the largely invisible research on the role of sports culture, especially the locker room, and gender and sexuality in non‐sports work environments. It also contributes to the study of masculine embodiments by focussing on sports culture such as the locker room, heteronormative‐masculinities and homophobia.

Abstract

Purpose

The inclusion of LGBTQ + persons (lesbian, gay, bisexual, transgender, queer and having other sexual orientations and gender identities) is a crucial step in improving gender diversity in the workplace; however, till date, it remains a significant challenge for human resource management professionals. The current study critically examines this issue of an inclusive workplace for LGBTQ + people through a systematic review of the existing research that has empirically studied their experiences at the workplace. It also examines the resistance and challenges organizations face in LGBTQ + diversity training and provides future research avenues.

Design/methodology/approach

For systematically reviewing the literature, Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) model has been used. A total of 101 empirical studies have been reviewed.

Findings

The result shows that LGBTQ + people encounter multiple negative workplace experiences, including proximal (hiring discrimination and housing discrimination) and distal workplace discrimination (unsafe work climate, microaggressions and harassment). These aversive experiences lead to work stress while also mandating that people manage their sexual identity and style of dressing. This stress, in turn, impacts their work–family outcomes, job satisfaction and decision-making with regard to their careers.

Originality/value

The paper provides a holistic understanding of the aversive workplace experiences encountered by sexual minorities.
