Abstract

Purpose

The purpose of this paper is to develop a workflow for design and fabrication of customized surgical guides (CSGs) for placement of the bidirectional extraoral distraction instruments (EDIs) in bilateral mandibular distraction osteogenesis (MDO) surgery to treat the bilateral temporomandibular joint ankylosis with zero mouth opening.

Design/methodology/approach

The comprehensive workflow consists of six steps: medical imaging; virtual surgical planning (VSP); computer aided design; rapid prototyping (RP); functional testing of CSGs and mock surgery; and clinical application. Fused deposition modeling, an RP process was used to fabricate CSGs in acrylonitrile butadiene styrene material. Finally, mandibular reconstruction with MDO was performed successfully using RP-assisted CSGs.

Findings

Design and development of CSGs prior to the actual MDO surgery improves accuracy, reduces operation time and decreases patient morbidity, hence improving the quality of surgery. Manufacturing of CSG is easy using RP to transfer VSP into the actual surgery.

Originality/value

This study describes an RP-assisted CSGs fabrication for exact finding of both; osteotomy site and drilling location to fix EDI’s pins accurately in the mandible; for accurate osteotomy and placement of the bidirectional EDIs in MDO surgery to achieve accurate distraction.

Abstract

Purpose

This paper aims to provide an overview of applications of medical rapid prototyping (MRP)-assisted customized surgical guides (CSGs) and shows the potential of this technology in complex surgeries. This review paper also reports two case studies from open literature where MRP-assisted CSGs have been successfully used in complex surgeries.

Design/methodology/approach

Key publications from the past two decades have been reviewed.

Findings

This study concludes that the use of MRP-assisted CSGs improves the accuracy of surgery. Additionally, MRP-assisted CSGs make the surgery much faster, accurate and cheaper than any other technique. The outcome based on literature review and two case studies strongly suggested that MRP-assisted CSGs might become part of a standard protocol in the medical sector to operate the various complex surgeries, in the near future.

Practical implications

Advanced technologies like radiology, image processing, virtual surgical planning (VSP), computer-aided design (CAD) and MRP made it possible to fabricate the CSGs. MRP-assisted CSGs can easily transfer the VSP into the actual surgery.

Originality/value

This paper is beneficial to study the development and applications of MRP-assisted CSGs in complex surgeries.

Abstract

Purpose

This study aims to find the usefulness of the customized surgical osteotomy guide (CSOG) for accurate mandibular tumor resection for boosting the accuracy of prefabricated customized implant fixation in mandibular reconstructions.

Design/methodology/approach

In all, 30 diseased mandibular RP models (biomodels) were allocated for the study (for experimental group [n = 15] and for control group [n = 15]). To reconstruct the mandible with customized implant in the experimental group, CSOGs and in control group, no CSOG were used for accurate tumor resections. In control group, only preoperative virtual surgical planning (VSP) and reconstructed RP mandible model were used for the reference. Individually each patient’s preoperative mandibular reconstructions data of both the groups were superimposed to the preoperative VSP of respective patient by registering images with the non-surgical side of the mandible. In both the groups, 3D measurements were taken on the reconstructed side and compared the preoperative VSP and postoperative reconstructed mandible data. The sum of the differences between pre and postoperative data was considered as the total error. This procedure was followed for both the groups and compared the obtained error between the two groups using statistical analysis.

Findings

The use of CSOG for accurate tumor resection and exact implant fixation in mandibular reconstruction produced a smaller total error than without using CSOG.

Originality/value

The results showed that, benefits provided with the use of CSOG in mandibular reconstruction justified its use over the without using CSOG, even in free hand tumor resection using rotating burr.

Abstract

Purpose

This review paper aims to provide an overview of applications of direct rapid manufacturing assisted mold with conformal cooling channels (CCCs) and shows the potential of this technique in different manufacturing processes.

Design/methodology/approach

Key publications from the past two decades have been reviewed.

Findings

This study concludes that direct rapid manufacturing technique plays a dominant role in the manufacturing of mold with complicated CCC structure which helps to improve the quality of final part and productivity. The outcome based on literature review and case study strongly suggested that in the near future direct rapid manufacturing method might become standard procedure in various manufacturing processes for fabrication of complex CCCs in the mold.

Practical implications

Advanced techniques such as computer-aided design, computer-aided engineering simulation and direct rapid manufacturing made it possible to easily fabricate the effective CCC in the mold in various manufacturing processes.

Originality/value

This paper is beneficial to study the direct rapid manufacturing technique for development of the mold with CCC and its applications in different manufacturing processes.

Abstract

Purpose

The purpose of this paper is to apply rapid prototyping (RP) philosophy as a technology transfer in industries to take its time and cost‐effective advantages for development of rapid tooling (RT).

Design/methodology/approach

Experimentations are performed for development of RT for sand casting, investment casting and plastic moulding applications.

Findings

This paper reports the procedures developed for manufacture of production tooling using RP. A cost/benefit model is developed to justify implementation of RP as a technology transfer in industries.

Research limitations/implications

The examples are limited to parts build by fused deposition modelling RP process. However, the concepts experimented may be applied for other RP processes.

Practical implications

RP has proved to be a cost‐effective and time‐efficient approach for development of RT, thereby ensuring possibility for technology transfer in casting as well as plastic industries.

Originality/value

This is the pioneer attempt towards quantifying RP benefits, in view of technology transfer. This paper presents original case studies and findings on the basis of experimentations performed in foundries.

Abstract

Purpose

The main purpose of this paper is to report the successful treatment modality for patients suffering from arthritis of the metatarsophalangeal joint (MTPJ) of the foot which otherwise could not be treated through traditional surgeries.

Design/methodology/approach

The unique capabilities of the computer-aided design and the rapid prototyping (RP) technology are used to develop the customized MTPJ implant (SamKu).

Findings

This approach shows good results in the fabrication of the MTPJ implant. Postoperatively, the patient experienced normalcy in the movement of the MTPJ of the foot.

Practical implications

Advanced technologies made it possible to fabricate the customized MTPJ implant (SamKu). The advantage of this approach is that the physical RP model assisted in designing the final metallic implant. It also helped in the surgical planning and the rehearsals.

Originality/value

This case report illustrates the benefits of imaging/computer-aided manufacturing/RP to develop the customized implant and serve those patients who could not be treated in the traditional way. This is a pioneered attempt toward implementation of a customized implant for patients suffering from arthritis of the MTPJ.

Abstract

Purpose

The purpose of this paper was to find a successful treatment modality for patients suffering from temporomandibular joint (TMJ) ankylosis who could not be treated through traditional surgeries.

Design/methodology/approach

This work integrated the unique capabilities of the imaging technique, the rapid prototyping (RP) technology and the advanced manufacturing technique to develop the customised TMJ implant. The patient specific TMJ implant was fabricated using the computed tomography scanned data and the fused deposition modeling of RP for the TMJ surgery.

Findings

This approach showed good results in fabrication of the TMJ implant. Postoperatively, the patient experienced normalcy in the jaw movements.

Practical implications

Advanced technologies helped to fabricate the customised TMJ implant. The advantage of this approach is that the physical RP model assisted in designing the final metallic implant. It also helped in the surgical planning and the rehearsals.

Originality/value

Abstract

Purpose

The purpose of this study is to highlight the direct fabrication of rapid tooling (RT) with desired mechanical, tribological and thermal properties using fused deposition modelling (FDM) process. Further, the review paper demonstrated development procedure of alternative feedstock filament of low-cost composite material for FDM to extend the range of RT applications.

Design/methodology/approach

The alternative materials for FDM and their processing requirements for fabrication in filament form as reported by various researchers have been summarized. The literature demonstrates the role of various post-processing techniques on surface finish of FDM prints. Further, low-cost materials for feedstock filament have been investigated experimentally to check their adaptability/suitability for commercial FDM setup. The approach was to realize the requirements of FDM (melt flow rate, flexibility, stiffness, glass transition temperature and mechanical strength), necessary for the successful run of an alternative filament. The effect of constituents (additives, plasticizers, surfactants and fillers) in polymeric matrix on mechanical, tribological and thermal properties has been investigated.

Findings

It is possible to develop composite material feedstock as filament for commercial FDM setup without changing its hardware and software. Surface finish of the parts can further be improved by applying various post-processing techniques. Most of the composite parts have high mechanical strength, hardness, thermal stability, wear resistant and better bond formation than standard material parts.

Research limitations/implications

Future research may be focused on improving the surface quality of parts fabricated with composite feedstock, solving issues related to the uniform distribution of filled materials during the fabrication of feedstock filament which in turns further increases mechanical strength, high dimensional stability of composite filament and transferring the technology from laboratory scale to various industrial applications.

Practical implications

Potential applications of direct fabrication with RT includes rapid manufacturing (RM) of metal-filled parts and ceramic-filled parts (which have complex shape and cannot be rapidly made by any other manufacturing techniques) in the field of biomedical and dentistry.

Originality/value

This new manufacturing methodology is based on the proper selection and processing of various materials and additives to form high-performance, low-cost composite material feedstock filament (which fulfil the necessary requirements of FDM process). Finally, newly developed feedstock filament material has both quantitative and qualitative advantage in RT and RM applications as compared to standard material filament.

Abstract

Purpose

This paper aims to propose a new framework on prioritizing and deployment of design for additive manufacturing (DfAM) strategies to an industrial component using Fuzzy TOPSIS multiple criteria decision-making (MCDM) techniques. The proposed framework is then applied to an automotive component, and the results are discussed and compared with existing design.

Design/methodology/approach

Eight DfAM design alternatives associated with eight design criteria have been identified for framing new DfAM strategies. The prioritization order of the design alternatives is identified by Fuzzy TOPSIS MCDM technique through its closeness coefficient. Based on Fuzzy TOPSIS MCDM output, each of the design alternatives is applied sequentially to an automobile component as a case study. Redesign is carried out at each stage of DfAM implementation without affecting the functionality.

Findings

On successful implementation of proposed framework to an automotive component, the mass is reduced by 43.84%, from 0.429 kg to 0.241 kg. The redesign is validated by finite element analysis, where von Mises stress is less than the yield stress of the material.

Practical implications

The proposed DfAM framework and strategies will be useful to designers, R&D engineers, industrial practitioners, experts and consultants for implementing DfAM strategies on any industrial component without impacting its functionality.

Originality/value

To the best of the authors’ knowledge, the idea of prioritization and implementation of DfAM strategies to an automotive component is the original contribution.

Abstract

Purpose

This paper aims to provide a review of four-dimensional (4D) printing using fused-deposition modeling (FDM). 4D printing is an emerging innovation in (three-dimensional) 3D printing that encompasses active materials in the printing process to create not only a 3D object but also a 3D object that can perform an active function. FDM is the most accessible form of 3D printing. By providing a review of 4D printing with FDM, this paper has the potential in educating the many FDM 3D printers in an additional capability with 4D printing.

Design/methodology/approach

This is a review paper. The approach was to search for and review peer-reviewed papers and works concerning 4D printing using FDM. With this discussion of the shape memory effect, shape memory polymers and FDM were also made.

Findings

4D printing has become a burgeoning area in addivitive manufacturing research with many papers being produced within the past 3-5 years. This is especially true for 4D printing using FDM. The key findings from this review show the materials and material composites used for 4D printing with FDM and the limitations with 4D printing with FDM.

Research limitations/implications

Limitations to this paper are with the availability of papers for review. 4D printing is an emerging area of additive manufacturing research. While FDM is a predominant method of 3D printing, it is not a predominant method for 4D printing. This is because of the limitations of FDM, which can only print with thermoplastics. With the popularity of FDM and the emergence of 4D printing, however, this review paper will provide key resources for reference for users that may be interested in 4D printing and have access to a FDM printer.

Practical implications

Practically, FDM is the most popular method for 3D printing. Review of 4D printing using FDM will provide a necessary resource for FDM 3D printing users and researchers with a potential avenue for design, printing, training and actuation of active parts and mechanisms.

Social implications

Continuing with the popularity of FDM among 3D printing methods, a review paper like this can provide an initial and simple step into 4D printing for researchers. From continued research, the potential to engage general audiences becomes more likely, especially a general audience that has FDM printers. An increase in 4D printing could potentially lead to more designs and applications of 4D printed devices in impactful fields, such as biomedical, aerospace and sustainable engineering. Overall, the change and inclusion of technology from 4D printing could have a potential social impact that encourages the design and manufacture of such devices and the treatment of said devices to the public.

Originality/value

There are other 4D printing review papers available, but this paper is the only one that focuses specifically on FDM. Other review papers provide brief commentary on the different processes of 4D printing including FDM. With the specialization of 4D printing using FDM, a more in-depth commentary results in this paper. This will provide many FDM 3D printing users with additional knowledge that can spur more creative research in 4D printing. Further, this paper can provide the impetus for the practical use of 4D printing in more general and educational settings.
