Abstract

Purpose

This study examines the osteoconductive and healing capabilities of locally implanted synthetic hydroxyapatite (sHAp) derived from eggshells in the central incisor sockets of rats.

Design/methodology/approach

Toxicity experiments were conducted in vitro and in vivo, to testify the safety dosage of sHAp. Around 24 mature male Sprague–Dawley (SD) rats had their upper central incisors extracted. The rats were placed into three groups of eight rats each: Group 1: the sockets of extracted central incisors were left unfilled (control), Group 2: filled up with commercially available hydroxyapatite (HAp) and Group 3: implanted with sHAp locally retrieved from eggshells. After extraction, four rats from each group were sacrificed at 2nd and 4th weeks. Maxillary tissue sections were obtained and stained with hematoxylin and eosin (H&E) and Masson’s trichome (MT) staining. Anti-osteocalcin (OCN) and proliferating cell nuclear antigen (PCNA) were used primary antibodies for immunohistochemistry (IHC) special labeling.

Findings

The results showed that the locally implanted sHAp was non-toxic and safe in cell lines (human osteoblast and fibroblast) and animals. Histological analysis of H&E, MT and IHC showed that the sockets treated with locally implanted sHAp from eggshells were filled with new bone tissue of comparable thickness to other groups.

Originality/value

This unique technique uses locally implanted eggshell-derived sHAp with osteoconductive characteristics. In an in vivo model, sHAps increased OCN and PCNA expression to improve bone repair.

Abstract

Looks at the recent US court decision in the case of Pepsico v. Redmond. The case centred on the circumstances of Quaker’s hiring of William Redmond, a former Pepsico marketing executive, to co‐ordinate the company’s distribution strategy for its newly‐acquired Snapple product line. At issue in the Redmond case was the unique question of whether or not knowledge of corporate strategy could be considered protectable proprietary information under the American incarnation of trade secret law. To understand the setting for the Redmond case, presents a brief overview of trade secret law in the USA. Then, after an examination of the facts and judicial decision in the Redmond case, concludes with a discussion of the implications of this case for marketing strategic decision making and executive recruitment in the post‐Redmond environment.

Abstract

Purpose

Blueberry contains bioactive compounds which are beneficial to organisms, such as phenolics and flavonoids. The purpose of this paper is to evaluate the potential protective effects of blueberry extracts (BE) on H2O2-induced HepG2 cells.

Design/methodology/approach

Cell protection was evaluated via the survivals of the cell. Reactive oxygen species (ROS) level, antioxidant enzyme and malondialdehyde (MDA) were detected. Western blot was carried out to analysis protein which was related to the cell apoptosis pathway. Changes in morphology including: cell total apoptosis/necrosis and G0/G1 cycle arresting were also concomitant.

Findings

The levels of ROS and malondialdehyde (MDA) reduced after the BE treatment while the contents of superoxide dismutase, and glutathione peroxidase (GSH-Px) increased in HepG2 cells induced by H2O2. Furthermore, mechanistic studies indicated that BE regulated the activation of mitochondrial apoptosis signal-regulating (Bcl-2, Bax). Qu was used as a positive control group. All these results demonstrated that the BE have a potential against oxidative stress in vitro.

Originality/value

Few studies have focused on the bioactivities of blueberry on oxidative stress. Taken together, the results confirm that polyphenol-enriched BE have the ability to protect against oxidative stress in cells. It has a great potential as a functional food ingredient to health benefits. Furthermore, this work showed the value of using simple biological models to screen for compounds that are of interest for food and pharmacological industry.

Abstract

Purpose

This study explored health insurance coverage of genetic testing and potential factors associated with precision medicine (PM) reimbursement in Thailand.

Design/methodology/approach

The study employed a targeted review method. Thirteen PMs were selected to represent four PM categories: targeted cancer therapy candidate, prediction of adverse drug reactions (ADRs), dose adjustment and cancer risk prediction. Content analysis was performed to compare access to PMs among three health insurance schemes in Thailand. The primary outcome of the study was evaluating PM test reimbursement status. Secondary outcomes included clinical practice guidelines, PMs statement in FDA-approved leaflet and economic evaluation.

Findings

Civil Servant Medical Benefits Scheme (CSMBS) provided more generous access to PM than Universal Coverage Scheme (UCS) and Social Security Scheme (SSS). Evidence of economic evaluations likely impacted the reimbursement decisions of SSS and UCS, while the information provided in FDA-approved leaflets seemed to impact the reimbursement decisions of CSMBS. Three health insurance schemes provided adequate access to PM tests for some cancer-targeted therapies, while gaps existed for access to PM tests for serious ADRs prevention, dose adjustment and cancer risk prediction.

Originality/value

This was the first study to explore the situation of access to PMs in Thailand. The evidence alerts public health insurance schemes to reconsider access to PMs. Development of health technology assessment guidelines for PM test reimbursement decisions should be prioritized.

Abstract

Purpose

Dibutyrylchitin (DBC) is an ester derivative of a natural polysaccharide – chitin. DBC is obtained by reaction of chitin with butyric anhydride in the presence of a catalyst. The production methods of DBC have been elaborated and optimized. DBC is easily soluble in common organic solvents and has film – and fibre forming properties. Such characteristics allow obtaining classical fibres from the polymer solutions. DBC is also a raw material for manufacturing yarn and for a broad range of textile dressing materials. Fibres with good mechanical properties are obtained by an optimized spinning process from the DBC solutions. The purpose of this paper is to present a further optimization of the mechanical properties of DBC‐fibres and yarns.

Design/methodology/approach

The excellent biomedical properties of the DBC are confirmed by different experimental results which prove that DBC is a biocompatible and biodegradable polymer and stimulates regeneration of damaged tissues. Tests of these DBC dressing materials under clinical conditions prove the excellent results of DBC‐based dressing materials for the ordered healing of tissues and wounds. The DBC dressing materials accelerate the healing of the wound and are biodegraded during the healing process. From the clinical tests, it can be clearly observed that the DBC dressing materials are absorbed into the fresh tissue formed during the healing process of the wounds.

Findings

The DBC and DBC‐based dressing materials are good bioactive textile materials for wound healing and for understanding the biological properties of chitin derivatives. The obtained results prove the importance of the O‐substitution of the hydroxyl groups present in chitin, not only for the solubility of the derivatives and the mechanical properties of the produced fibres, but still more important for the biological properties of these ester derivatives of chitin containing butyric acid. This development creates a link between textile products, based on material properties and human health, based on the biological properties of the basic material.

Originality/value

The mechanical properties of DBC are further optimized by blending it with poly(ε‐caprolactone). Good transparent and flexible products, such as films, with a high elongation to break are obtained by blending 10‐20 wt per cent of poly(ε‐caprolactone) with DBC. This creates new possible bioactive applications for DBC or poly(ε‐caprolactone).

Abstract

Purpose

Using critical disaster studies and state theory, we assess the disaster aid platform named Lebanon Reconstruction, Reform and Recovery Framework (3RF) that was put in place by international donors in the aftermath of the Beirut Port Blast in August 2020, in order to examine the effectiveness of its inclusive decision-making architecture, as well as its institutional building and legislative reform efforts.

Design/methodology/approach

The paper uses the case study approaach and relies on two original data sets compiled by authors, using desk reviews of academic literature and secondary data, in addition to 24 semi-structured expert interviews and participant observation for two years.

Findings

The aid platform appears innovative, participatory and effectively functioning toward recovery and reform. However, in practice, the government dismisses CSOs, undermines reforms and dodges state building, whereas the 3RF is structured in incoherent ways and operates according to conflicting logics, generating inertia and pitfalls that hinder effective participatory governance, prevent institutional building, and delay the making of projects.

Research limitations/implications

The research contributes to critical scholarship as it addresses an important research gap concerning disaster aid platforms’ institutional design and governance that are under-studied in critical disaster studies and political studies. It also highlights the need for critical disaster studies to engage with state theory and vice-versa.

Practical implications

The research contributes to evaluations of disaster recovery processes and outcomes. It highlights the limits of disaster aid platforms’ claims for participatory decision-making, institutional-building and reforms.

Originality/value

The paper amplifies critical disaster studies, through the reflexive analysis of a case-study of an aid platform.

Abstract

Purpose

Good medical practice, evidence-based medicine (EBM) and clinical practice guidelines (CPG) have been recurring subjects in the scientific literature. EBM advocates argue that good medical practice should be guided by evidence-based CPG. On the other hand, critical authors of EBM methodology argue that various interests undermine the quality of evidence and reliability of CPG recommendations. The purpose of this paper is to evaluate patient related outcomes of CPG implementation, in light of EBM critics.

Design/methodology/approach

The authors opted for a rapid literature review.

Findings

There are few studies evaluating the effectiveness of CPG in patient-related outcomes. The systematic reviews found are not conclusive, although they suggest a positive impact of CPGs in relevant outcomes.

Research limitations/implications

This work was not a systematic review of literature, which is its main limitation. On the other hand, arguments from EBM and CPG critics were considered, and thus it can enlighten health institutions to recognize the caveats and to establish policies toward care improvement.

Originality/value

The paper is the first of its kind to discuss, based on the published literature, next steps toward better health practice, while acknowledging the caveats of this process.

Abstract

Purpose

The purpose of this study is to determine the blood pressure (BP) lowing potential of some herbs used by the indigenous people of Zango Kataf, Kaduna State, Nigeria, to treat hypertension.

Design/methodology/approach

A total of 17 herbal healers and 15 hypertensive patients who use herbs to treat hypertension in Zango Kataf, Kaduna State, Nigeria, were consulted between 2019 and 2020. Participants were selected via purposeful sampling. BP was measured before and after the ingestion of a herbal preparation. Three plant preparations were screened: Newbouldia laevis, Hibiscus cannabinus and Pavetta crassipes.

Findings

H. cannabinus produced a significant decrease of 16, 12 and 14 mmHg in SBP at 2, 3 and 4 h, respectively (p = 0.001 at 2 h, p = 0.04 at 3 h, p = 0.01 at 4 h). While P. crassipes produced a significant decrease in SBP of 12, 15, 19 and 20 mmHg at 1, 2, 3 and 4 h, respectively (p = 0.04 at 1 h, p= 0.00 at 2 h, p = 0.00 at 3 h and p = 0.00 at 4 h). The findings for N. laevis were not reported due to incomplete information.

Research limitations/implications

The sample size of this study was small. The researcher could not meet with some hypertensive clients owing to the poor cooperation of some herbal healers. Further, some hypertensive patients rejected the offer to participate in the study.

Originality/value

This study reveals that treatment with H. cannabinus and P. crassipes significantly decrease BP in hypertensive patients.

Abstract

Purpose

The purpose of this study was to examine the effect of family involvement in the educational sessions and follow-up meetings on compliance with the low-salt diet for hypertensive older adults.

Design/methodology/approach

Randomized controlled trial was used in this study with a total of 30 hypertensive older adults divided into two groups (n = 15). The intervention group involved one family member during the educational sessions and follow-up meetings. Conversely, no family member was involved in the control group.

Findings

The intervention group showed a significant (p < 0.05) increase in compliance level on the attitude (knowledge) and subjective norm (psychomotor and family support) subscales. The perceived obstacle subscale and the salt concentration in food and urine excretion significantly decreased both after educational sessions and follow-up meetings (p < 0.05). However, the control group did not.

Research limitations/implications

A relatively small number of samples would have affected the results, but in this study, randomization was applied in sample collection.

Practical implications

By encouraging the involvement of family members in the educational sessions and follow-up meetings, it could enhance compliance of low-salt diet among hypertensive older adults.

Originality/value

The findings and outputs provide a combination of family involvement and the Geragogy learning model through educational sessions and follow-up meetings that could enhance a low-salt diet adherence among older adults with hypertension in the community.

Abstract

Purpose

The purpose of this paper is to assess physical activity and nutrient intake and their association with obesity in apparently healthy middle-aged adults in Akuse, a rural community in the Eastern region of Ghana.

Design/methodology/approach

This cross-sectional study assessed demography and anthropometry, nutrient intakes using a 3-day repeated 24 h dietary recall and physical activity using the global physical activity questionnaire (GPAQ). Data were entered into Microsoft excel and analyzed with SPSS version 25.

Findings

There were 118 respondents (55 males, 46.6% and 63 females, 53.4%) in the study with mean age of 45.62 ± 6.88 years. About 90% of respondents were physically active based on WHO physical activity recommendation. Prevalence of overweight/obesity was higher among inactive participants compared to active participants and sitting/reclining hours was significantly (p-value = 0.042) associated positively with BMI after a bivariate correlation analysis, suggesting that physical activity plays a role in obesity. About one in five respondents were obese. Mean energy intake exceeded RDA for females. Sodium intake far exceeded RDA for both males and females, putting the population at potential risk of hypertension.

Practical implications

This study demonstrates the need for intensifying health education and other obesity prevention interventions to curb the rising obesity prevalence in rural communities.

Originality/value

The study revealed that overweight/ obesity in this rural community is higher than previously reported for rural Ghana.
