Abstract

Purpose

To explore how motivational interviewing (MI) training might benefit the practice of COVID-19 contact tracers.

Design/methodology/approach

Following co-production of a MI training package, with a United Kingdom (UK) track and trace organisation, training was delivered virtually to 101 volunteer participants involved in contact tracing. Data were captured via an online survey, incorporating questions from recognised measures of occupational self-efficacy and workplace wellbeing, prior to the training. Open data fields were used to gather feedback about participants' reasons for attending, and views about the training afterwards.

Findings

Although the contact tracers reported high occupational self-efficacy and workplace wellbeing, both quantitative and qualitative data suggested participants saw practitioner value and utility in MI.

Research limitations/implications

The sample was self-selecting and typically involved contact tracers from UK local authorities. The study did not measure impact on compliance with self-isolation guidance and/or providing details of contacts, and larger-scale research would be needed to establish this. This was not a pre-post-test evaluation study, and measures of occupational self-efficacy and workplace wellbeing were gathered to give insight into the sample and to test the feasibility of using this survey for a future large-scale study. The research was conducted during the height of the pandemic. While UK COVID-19 contact tracing services have since been reduced, there are potential implications for infection control more generally.

Practical implications

MI is potentially a useful approach for enhancing contact tracing practice. However, implementation factors should be carefully considered, to ensure effective and sustainable practice.

Social implications

Improved practice in contact tracing could have potential benefits in infection control, through improving compliance with central guidance, although this requires more widespread investigation.

Originality/value

This is the first empirical study to investigate how MI training could benefit COVID-19 contact tracing practice.

Abstract

Purpose

This study was intended to investigate a small-scale School-based Motivational Interviewing (SBMI) pilot with first-year college students. This approach honors student autonomy, supports self-determination and has the potential to impact educational outcomes in higher education. Motivational Interviewing (MI) is an evidence based conversational skill set, defined as “a collaborative conversational style for strengthening a person's own motivation and commitment to change” (Miller and Rollnick, 2013, p. 12). Student perceptions of satisfaction with the faculty-student mentoring intervention were sought. Relational aspects of MI (partnership, empathy and alliance) were also explored.

Design/methodology/approach

A mixed-method approach was used for the SBMI study, focused on college students with recent academic setbacks (N = 19).

Findings

The intervention was deployed with high levels of MI technical fidelity and relational quality. Participants reported high satisfaction with the intervention. The relational aspects and participant perceived alliance with their faculty were highly correlated across the intervention, adding to the discussion of the mechanisms of MI that contribute to its effectiveness.

Research limitations/implications

This work is formative, yet at this point is not generalizable given the scope of the study.

Practical implications

Findings are encouraging for further development of this innovative pedagogical approach. Possible future applications of research are provided.

Social implications

Discussed herein, SBMI has the potential to meet the needs of traditionally underrepresented student groups.

Originality/value

The reported study is the initial portion of a larger intervention development project.

Abstract

Purpose

Brain injury rehabilitation is often complicated or confounded by difficulties with engaging the injured party with the services and input required. Lack of awareness of cognitive and executive impairments is often implicated in this difficulty. Any technique or approach that enhances engagement may then support rehabilitation. The aim of this paper is to examine the current evidence base for the use of motivational interviewing (MI) as a method for increasing engagement by supporting the development of insight.

Design/methodology/approach

The paper is a literature review, taking as its basis Medley and Powell's conceptual review of MI and then examining the published evidence available.

Findings

Although attractive to practitioners in the field because the purpose of MI and the goals underpinning acquired brain injury (ABI) rehabilitation are co‐terminus, the theoretical and research findings to date that address the effective application of MI to ABI are inconclusive. The literature presently available suggests there is no conclusive evidence that MI is a more effective approach than any other, and that which is available makes little reference to the specific difficulties of an ABI population that may confound MI's application in this area.

Research limitations/implications

There is currently no high standard of evidence to support the use of MI with people with ABI.

Practical implications

There are still very few papers written, or research undertaken, into the effectiveness of MI with people with a brain injury. Most of the work undertaken thus far is concerned with supporting behavioural change in this population when problematic alcohol or drug use is co‐morbid. This paper identifies some of the practical difficulties with the approach whilst recognizing the inherent value in its aims.

Originality/value

The paper provides an opportunity for practitioners who wrestle with the difficulty of engagement on a daily basis to reflect upon how present practice could be altered to increase the likelihood of supporting engagement.

Abstract

Purpose

Motivational interviewing (MI) has developed considerably since its inception, which may have led to diverse practice across contexts and differential understanding of core principles. Concept mapping is one potential method for offering insight into practitioner awareness, understanding and application of MI. The paper aims to discuss this issue.

Design/methodology/approach

In total, 29 professionals from a range of disciplines, including counselling, education and health, completed concept maps about MI, following brief training at the UK regional MI interest network. In total, 17 completed maps were submitted for analysis using quantitative and qualitative methods.

Findings

A total of 186 concepts and 175 propositional links were found within the 17 maps. The most commonly identified concepts were: change, empathy, collaboration, open-ended questions, affirmations, reflections, summaries (OARS), client centred and compassion. The concept maps also suggested differing levels of expertise across network members using concept mapping morphology classification.

Research limitations/implications

The sample was small scale and located in one region of the UK. Maps were submitted anonymously meaning that participant data could not be matched to the maps.

Practical implications

Concept mapping is a potentially useful method for auditing practice and developing skills in MI, as well as exploring participants’ understanding of related concepts and therapeutic mechanisms.

Social implications

MI has a strong evidence-based across a variety of disciplines and contexts. Refining practitioner skills in MI has implications for the integrity of delivery, and improved client outcomes in areas such as substance use, health promotion and educational disaffection.

Originality/value

This is the first study to investigate concept mapping as a means of understanding MI practice. It has potential implications for training, monitoring, supervision and development in MI practice.

Abstract

Purpose

Non-participation in outpatient dual diagnosis services presents a challenge for providers assisting clients in their recovery. To better understand factors that facilitate participation, the purpose of this paper is to examine positive recovery states – hope, meaning, and empowerment – as they relate to motivational interviewing (MI) and service use.

Design/methodology/approach

Six dually diagnosed adults completed four baseline assessments, four MI sessions, a post-MI tape-assisted recall interview, and one-month follow-up measures. Simulation modeling analysis of phone survey responses, comparisons of baseline and intervention phase data, and grounded theory analysis of interviews were conducted to determine MI's relationship to the dependent variables.

Findings

MI was associated with modest improvement in levels of participation, hope, empowerment, and with greater change in life purpose. Key recovery themes were: positive sense of self, increased self-efficacy, and improved relationships. Feelings of safety and trust were tied to greater self-disclosure while more active emotions were more closely linked to the discussion of recovery progress.

Research limitations/implications

The paper's finding are limited by small sample size and phone survey response sets.

Practical implications

To better help dually diagnosed clients sustain treatment involvement, MI practitioners should pay special attention to recovery accomplishments, values, abilities, and self-esteem, while linking these attributes to service participation where appropriate and creating a safe, valuing atmosphere conducive to self-disclosure.

Originality/value

This is the first paper to measure key recovery constructs within MI process, and to explore the role of positive emotions related to MI, recovery, and service participation.

Abstract

Purpose

Motivational Interviewing (MI) is a vocational communication skill from the helping professions. Verbal skills in MI are summarized under the acronyms OARS and EARS (open-ended questions/elaborating, affirmations, reflections, and summaries). The purpose of this paper is to outline how MI provides important skills for engineers, and demonstrate skill assessment by using an observation-based scientific approach.

Design/methodology/approach

Totally, 25 engineering students took part in a skill-based MI training. Quality assurance of the training was assessed by using a repeated measurement design with multiple measures: systematic observations from recorded interactions and self-reported and standardized performance measures. Two external observers reliably coded the recorded conversations using the MI skill code.

Findings

Trainees showed a significant increase of verbal skills in MI. Directive-confrontational behaviors decreased after training. Self-reported and performance measures indicated significant increases in MI post training. Conversational partners in the post-training condition showed significantly more motivation in comparison to partners before the training.

Research limitations/implications

The main limitation of the study is the small sample size. However, training effect sizes showed large effects on verbal skills.

Practical implications

Communication skills in MI can be taught effectively for a technical population. This study suggests that MI is effective within the higher education of technical professions who have to deal with motivational issues. Observational measures can be used for quality assurance purposes, but also serve as a feedback instrument for work-based learning purposes.

Originality/value

This is the first study to evaluate training in MI for engineers using a multi-method approach with observational measures.

Abstract

Purpose

Prisoners have poorer oral health than the general population. Good oral health is essential for both social and physical well-being. For prisoners, poor oral health is also related to drug use after release, whereas good oral health is related to successful reintegration into society. The purpose of this study was twofold: to examine the effect of an intervention based on motivational interviewing (MI) on prisoners’ oral health-related behavior and to assess if the intervention is a good fit for this population.

Design/methodology/approach

In total, 16 prisoners in a Norwegian prison were offered a brief MI-based intervention focusing on changing their oral health-related behavior. An oral examination was also performed and the prisoners received a small package containing oral hygiene aids. Two weeks later, a second oral examination and a semi-structured interview were conducted to explore the effect of the intervention and examine the prisoners’ responses to the intervention. Qualitative data analyzes were guided by thematic analysis.

Findings

The findings indicate that the intervention had positive effects on both the prisoners’ motivation to use oral health-related behavior and their performance of oral health-related behavior. The findings also indicate that the intervention was well adapted to the target population.

Originality/value

This is one of the first studies that explore the effect of an intervention in improving prisoners’ oral health and bridges a knowledge gap in the literature. The findings may increase the understanding of how dental services should be organized and offered to provide dental health care to this vulnerable group.

Abstract

The aim of this randomised intervention study was to test the use of two counselling styles in reducing alcohol consumption in offenders who were hazardous drinkers and who had been charged with alcohol‐related offences. An additional aim was to evaluate the research process itself before embarking on a larger trial. Participants were recruited from a police custody suite in the south east of England and randomised to receive either a motivational interviewing brief intervention (MIBI) or a standard brief intervention (BI). The Alcohol Use Disorder Identification Test (AUDIT) was used to screen offenders for hazardous drinking. Participants were asked to complete a second AUDIT 12 weeks later. Two hundred offenders with alcohol‐related offences were screened over a 10‐month period. Of these, 182 were alcohol dependent and were therefore excluded from the study. Of the 18 who were eligible to enter the study, six refused to participate. Five were randomised to the MIBI group and seven into the BI group (BI). The mean age of the MIBI group was 25 (SD±3.86) years and the mean age of the BI group was 32.4 (SD±7.9). Audit scores were significantly lower at time 2 compared to time 1 for both intervention groups (t(11) = 17.60; p < 0.05). There was no significant difference between the different intervention groups.

Abstract

Purpose

This paper aims to explore how healthcare organizations in Denmark transform modern ideas of patient‐centred preventive care into organizational practice. Specific attention is given to the influence of existing organizational practices.

Design/methodology/approach

A qualitative multiple case study design is used to explore “motivational interviewing”, a health behaviour concept that was introduced in preventive consultations in ten Danish clinics. From an institutional perspective, the concept may be understood as an “organizational recipe” that translates into organizational activities. Data are generated by observations, interviews and document reviews. Theory and data provide the framework for an analytical phase model.

Findings

The paper reveals how abstract ideas on preventive care translate into specific activities in organizations following pre‐existing, general rules of medical practice. Disparities between clinics are related to distinctive local practices, such as clinics' conditions and preventive treatment practices that form local sets of editing rules. Differences in clinic performances result in variations in achieving the ideal of patient‐centred, preventive care prescribed by motivational interviewing.

Research limitations/implications

It is acknowledged in medical practice that there are different conceptions of the ideal of preventive care. The paper points to the value of promoting transparency in clinical practice and of paying attention to the mismatch between external expectations and organizational capabilities.

Originality/value

The paper adds to the understanding of organizational dynamics at the micro‐level.
