Abstract
Purpose
Twin to Twin Transfusion Syndrome (TTTS) is a well understood, yet under-recognized, placental disease affecting any given pregnancy at a rate of 1 in 1,000. There is no clustering of TTTS; instead the threat remains pathologically distinctive due to its pervasiveness. However, while incidence rates are random, survival rates are not. Despite compliant acceptance of “routine prenatal care,” sadly, there are many women who for currently unknown reasons are not receiving the advanced prenatal care needed to appropriately screen for, diagnosis and treat TTTS. And these women are paying the ultimate price for such obstetrical oversight.
Methodology
This study hypothesizes that differential care being given by primary obstetricians of TTTS patients is resulting in experienced inequalities. Utilizing social reproduction theory, and through ethnographic and quantitative analyses of primary data, this study seeks to divulge the complex social processes taking place (or failing to take place) within the world of American obstetrics, and begin to understand how they are affecting TTTS mortality and morbidity rates.
Findings
Findings illuminate a profound imbalance of power and influence amongst the following entities: American Congress of Obstetricians and Gynecologists and Society of Maternal Fetal Medicine; obstetrical training and practice; and levels of patient awareness and advocacy.
Value
This study argues that the current social relations being reproduced by these entities are perpetuating a climate that allows for disregard of proper TTTS management. Specifically, this study theoretically explores what social relations and subsequent (in)actions are being reproduced prior to TTTS diagnoses, and applies the effects of those observations.

Abstract
Throughout history, from ballads to requiems, music has played an important social role in reflection on mortality. Not only do musicians articulate their angst and thoughts about death, but, in so doing, they enable listeners to explore their own feelings. While the relationship between music and mental health can be examined from a number of perspectives, two broad approaches can be taken: artist-centred approaches and listener-centred approaches. The first analyses the life and work of artists, focussing particularly on the ways in which they explore death and angst in their music. The second looks at the ways in which the life and work of an artist is interpreted by listeners. Within these general approaches, a complex set of questions emerge – often at the interface of both approaches. How is the music used by listeners in their reflection on mortality? How is music used to manage mental health? Does reflection on the life and work of an artist contribute to suicidal ideation? Is the reception of music altered by an artist’s suicide? Using both these approaches and drawing particularly on the work of Émile Durkheim, this discussion demonstrates the significance of popular music analysis for death studies, focussing particularly on the issues surrounding popular music’s relationship to suicidal ideation.

The importance of alignment between users’ and providers’ expectations of accounting services has long been recognized as paramount in the auditing profession. The importance of expectations, and especially expectations gaps, is even more compelling for new assurance services, where the importance of marketing the service is pronounced. This paper develops the Assurance Gaps Model, which describes expectations gaps in general, defining these holistic differences between users’ and providers’ perceptions of assurance services as assurance gaps. The model suggests that assurance gaps really have a number of components – expectations, evaluations of performance and disconfirmation – all of which impact users’ satisfaction with the service. The magnitude of each of these components, as well as the emphasis placed on each one, is important in describing the nature of the gap. This model is consistent with previous research in auditing as well as a large body of research in marketing studying expectations and the satisfaction process (Oliver, 1997). To illustrate potential applications of the Assurance Gaps Model, hypotheses are developed and tested using an online simulation of the ElderCare assurance service proposed by the AICPA/CICA. Results indicate that users and providers demonstrate similar magnitude of each of the factors in the model, but differ in that users emphasize performance in forming satisfaction judgments while providers emphasize expectations. The study and results illustrate the usefulness of the model for performing detailed analysis of assurance gaps and for suggesting appropriate courses of action to manage the factors that contribute to them.

Abstract
This chapter examines a training programme for facilitators of the ‘A Quiet Place’ project in three residential homes offering space for between 8 and 10 children up to the age of 18. It reviews the impact of a 12-month training programme which involves action learning in the trainees' own personal development, creativity and job satisfaction. It examines the hypothesis of whether the calibre of the person working with young people with a variety of ‘labels’ is a key variable in the successful impact of any intervention. The approach outlined interventions combine the latest in modern brain research with more traditional approaches. A key component of the programme is the training of staff to be able to deliver the programmes for themselves. The trainees reported, quite soon into their programmes, that they notice personal growth; this was not originally an outcome of the training as such. As they noticed a change in themselves, they also noticed change in the children and young people with whom they worked. Changes in the cultures of the homes brought more benefits as one change built upon the next. The case histories trace the journey of three facilitators, the ups and downs, finally ‘getting it’ and achieving their goal with all the wonderful surprises and skills that they developed and learnt to use as part of their own lives outside work.

Late in 2010 the Social Emotional and Behavioural Association (SEBDA), based in the United Kingdom, held an international conference at Keble College, Oxford. The theme was ‘Transforming Troubled Lives’ and 160 delegates arrived from countries around the globe. Around 50 per cent of those attending presented papers. Some of the presentations were short, others longer, some very practically based and others more theoretically focused. A range of papers has already been published in SEBDA's international journal Emotional and Behavioural Difficulties (volume 16, issue 3). The chapters in this volume draw upon other presentations given at the conference, which we, as editors, felt were representative of the good practice, provision and policy to be found amongst professionals working both to transform the sometimes troubled lives of children and young people and to ensure that these students are included in educational settings.

This paper examines the role of firm capabilities, psychic distance and choice of country risk variables in the entry mode decision between developed and emerging countries. The conceptual model extends previous studies built on gravity models by considering the potential effect of colonial ties on the entry mode. The empirical test applies an ordered logit model on a survey sample of 462 Portuguese firms. A subsample of 143 firms with internationalisation in the former Portuguese colonies was rerun to be compared with the total sample results. The findings suggest that firm capabilities maintain their importance in the presence of colonial ties, whereas psychic distance and perceived risk seem to be distorted or absent from the decision process of the market entry.

Michael Holquist (1990), one of the commentators on Mikhail Bakhtin’s monumental work, stated flatly that “human existence is dialogue,” and Ivana Markova (2003) declared that “dialogism is the ontology of humanity.” Bakhtin (1985;1986) himself said that such dialogues are conducted by using “speech genres.” From another angle Kenneth Burke asked, “What is involved when we say what people are doing and why they are doing it?” and claimed – and showed – that this question can be best answered by using what he called the “grammar of motives,” which consisted of a hexad of terms: act, attitude, scene, agent, agency, and purpose. In this chapter, I examine, by using various examples, how the Burkean grammar is used in the construction of one speech genre or the other to achieve rhetorically effective dialogic communication.
