Abstract
PaperChase is a Medline searching system designed for use by physicians and other health care professionals and students. It offers searching by subject headings, title words, author, journal title, year of publication, all of which can be combined using the Boolean operators AND, OR, and NOT. Unlike most Medline searching systems, PaperChase attempts to guide the user in developing a successful search strategy.

Abstract
Purpose
Currently, human hearts destined for transplantation can be used for 4.5 hours which is often insufficient to test the heart, the purpose of this paper is to find a compatible recipient and transport the heart to larger distances. Cooling systems with simultaneous internal and external liquid cooling were numerically simulated as a method to extend the usable life of human hearts.
Design/methodology/approach
Coolant was pumped inside major veins and through the cardiac chambers and also between the heart and cooling container walls. In Case 1, two inlets and two outlets on the container walls steadily circulated the coolant. In the Case 2, an additional inlet was specified on the container wall thus creating a steady jet impinging one of the thickest parts of the heart. Laminar internal flow and turbulent external flow were used in both cases. Unsteady periodic inlet velocities at two frequencies were applied in Case 3 and Case 4 that had four inlets and four outlets on walls with turbulent flows used for internal and external circulations.
Findings
Computational results show that the proposed cooling systems are able to reduce the heart temperature from +37°C to almost uniform +5°C within 25 min of cooling, thus reducing its metabolic rate of decay by 95 percent. Calculated combined thermal and hydrodynamic stresses were below the allowable threshold. Unsteady flows did not make any noticeable difference in the speed of cooling and uniformity of temperature field.
Originality/value
This is the pioneering numerical study of conjugate convective cooling schemes capable of cooling organs much faster and more uniformly than currently practiced.

Abstract
Purpose – To examine the introduction of a practice known as Donation after Cardiac Death (DCD) into Australian hospitals notwithstanding that DCD constitutes a significant shift in medical practice. The shift is from holding off organ donation activities until after death has occurred (the Brain Death Scenario and Uncontrolled Cardiac Death Scenario) to progressing the injured patient to organ donor on the basis of an early prognostic call. The more precise term is ‘Controlled DCD’. What is controlled are the timing, location, mode and criteria of death.Findings – Controlled DCD first appears as an ‘inpatient trial’ in Pittsburgh, United States, in 1992, and has progressed, via various organisations and committees, to being used in a number of countries, including Australia and New Zealand, and being embodied in national, state and hospital protocols. Along the way, concerns that previously precluded such activity have been consistently raised and documented. Operational accounts reported in the medical literature do not acknowledge this history or the ethically problematic aspects of the practice. It is likely that these operational reports and related information sources, together with promotion of the positives of organ donation, have facilitated the practice’s progress through the relevant institutional committees.Social implications – Much is at stake when the quest for more organ donors starts changing what can be done to patients. How this practice has come to be tolerated in 2012, despite a number of irresolvable ethical issues, is a matter of vital community interest.Originality/value of chapter – This chapter is part of an ongoing study by the authors.

Abstract
Purpose
The purpose of this paper is to propose a conceptual framework that highlights transhumanism’s ideals of achieving superintelligence, super longevity and super well-being through human enhancement technologies (HET) and their relations with services marketing principles.
Design/methodology/approach
Framed by the transformative service research (TSR), this conceptual work articulates the 7Ps of the marketing mix with four macro-factors that create tensions at both the marketplace and consumer levels.
Findings
HET has potential for doing good but also tremendous bad; greater attention is needed from services marketing researchers especially in one proprietary research area: bioethics.
Research limitations/implications
The authors contribute to the growing work on TSR investigating how the interplay between service providers and consumers affects the well-being of both. Additionally, the authors call for novel interdisciplinary work in transhuman services research.
Originality/value
To the best of the authors’ knowledge, this is one of the first papers in services marketing research to explore the promises and perils of transhumanism ideals and human enhancement technologies.

Abstract
The Abdur Razzaque Ansari Memorial Weavers Hospital (ARAM) came into effect on 7 April 1996, and is dedicated to the people of Jharkhand and weavers. To deal with the issues of inequity in healthcare services, ARAM was founded for the extension of affordable healthcare services to the needy in and around the area of Jharkhand. Visualised by a great social worker and legend Abdur Razzaque Ansari, it has been successfully run by his eldest son Mr Sayeed Ahmad Ansari for 28 years. This research uses mainly a case-study approach through secondary data from the hospital website and other websites citing ARAM and its functions. Consent to use data for the study was obtained from Mr. Sayeed Ahmad Ansari. Primary information was collected through the patients who availed facilities from the hospital. They were interviewed through a semi-structured questionnaire each taking 30-40 minutes. Taken over by Medanta Group on 8 July 2015 (earlier being managed by Apollo Hospitals Group for 20 years), it is the first super speciality community hospital in Eastern India. Treating over 50,000 patients yearly with state-of-the-art medical equipment and providing discounts to lower-income groups, people from the weaver’s community, freedom fighters and members of ICSI have intrigued people from these sections for affordable treatment and facilities in and near Jharkhand. With a 200 bed-capacity, nine different disciplines and 12 departments spread across the city of Ranchi, the hospital caters to a massive population at a much-subsidised rate. Reaching out to rural villages through free medical camps and awareness campaigns, the hospital showcases how a successful model of healthcare cooperative can be replicated accordingly in similar developing and underdeveloped regions.

Abstract
Purpose
Predictive models implemented in medical procedures can potentially bring great benefit to patients and represent a step forward in targeted treatments based on a patient’s physiological condition. It is the purpose of this paper to outline such a model.
Design/methodology/approach
A multi-scale 0D-3D model based on patient specific geometry combines a 0-dimensional lumped parameter model (LPM) with a 3D computational fluid dynamics (CFD) analysis coupled in time, to obtain physiologically viable flow parameters.
Findings
A comparison of physiological data gathered from literature with flow-field measurements in this model shows the viability of this method in relation to potential predictions of pathological flows repercussions and candidate treatments.
Research limitations/implications
A limitation of the model is the absence of compliance in the walls in the CFD fluid domain; however, compliance of the peripheral vasculature is accounted for by the LPM. Currently, an attempt is in progress to extend this multi-scale model to account for the fluid-structure interaction of the ventricular assist device vasculature and hemodynamics.
Originality/value
This work reports on a predictive pulsatile flow model that can be used to investigate surgical alternatives to reduce strokes in LVADs.
