Abstract

Purpose

The aim of this paper is to demonstrate how a historical event packaged as an iconic heritage cultural brand can be marketized and modified over time to ensure brand longevity and continued emotional commitment and loyalty through the leverage of stories and associations more closely aligned with modern-day audiences. The authors do this through examining the marketization of the New Zealand World War 1 (WWI) nurse to today’s audiences. The periods of study are WWI (1914–1918) and then the modern day. The New Zealand Army Nursing Service (NZANS) during WWI has previously had little attention as a key actor in the Australia and New Zealand Army Corp (ANZAC), Today ANZAC is held as pivotal in the birth of New Zealand’s perception of nationhood and as an iconic heritage cultural brand. The history and legend of the ANZAC plays an important role in New Zealand culture and is fundamental to the “Anzac Spirit”, a signifier of what it means to be a New Zealander.

Design/methodology/approach

A historical case study method is used. The primary source of data is 1914–1918, and includes contemporaneous articles, and personal writings: diaries, letters and published memoirs. More contemporary works form the basis for discussion of marketization as it relates to the NZANS. The article first presents conceptual framing, then the development of the Anzac brand and the history of the NZANS and its role in WWI before turning to discussion on the marketization of this nursing service to today’s audiences and as part of the ANZAC/Anzac brand.

Findings

Today the story of the WWI NZANS nurse, previously seldom heard, has been co-opted and is becoming increasingly merged as an integral part of the Anzac story. The history of the NZANS during WWI has a great deal of agency today as part of that story, serving many functions within it and providing a valuable lever for marketization.

Originality/value

To date, there is a scarcity of marketing analysis that examines the marketization of history. By focusing on New Zealand WWI nursing as a contributor to the Anzac story, the authors contribute to the understanding of how marketers package and contemporize history for appeal to audiences through both sustaining and reworking cultural branding.

Abstract

An enormous amount of change has occurred in the last six years for the mental health system in England and the workforce within it. We have seen the 10‐year National Service Framework for Mental Health (Department of Health, 1999) gradually make its impact felt in the form, in particular, of new community mental health teams and structures for delivering care in the community. We have also, most recently, experienced the passing of the Mental Health Act 2007 (HM Government, 2007), after many turbulent years of controversy and argument, extending to nurses and non‐medical practitioners who have been given statutory powers to act as approved mental health practitioners and approved clinicians.Alongside these important developments has been a gradual revolution in traditional ways of working, in the form of the New Ways of Working initiative. This article considers the impact of New Ways of Working on mental health nursing ‐ the single largest professional group within the mental health workforce ‐ and the continuing implications for the profession. The development of nurse prescribing is used as an illustration of the challenges and opportunities that have commonly arisen when new roles and skill sets have been introduced in mental health settings.

Abstract

Purpose

The purpose of this paper is to examine advertising placed within Kai Tiaki: The Journal of Nurses of New Zealand. The periodization for this study is 1908-1929, a chronological time span and approach that incorporates the first issue of the Kai Tiaki in 1908 and ending 1929, thus representing the launch of the journal and first two decades of its production. During this period, New Zealand nursing was emerging as an organised and professionalised body of health-care workers. At the same time, New Zealand, Britain’s most distant “dominion”, was beginning to realise its own nationhood and growth.

Design/methodology/approach

The primary source of data was digitalised Kai Tiaki issues between 1908 and 1929. Each four issues for every year were analysed. All advertisements were coded (n = 1895), followed by a qualitative content analysis in which key themes were derived.

Findings

Five overarching themes were found. The two main themes were the proliferation of therapeutic “tonics” and the second the relationship of nurses to them that was profiled by advertisers. The three remaining themes were accommodations, lifestyle and the nurses “role” as nurse, handmaiden or mother. Conclusions discuss reasons for key findings, and, in particular, the reinforcement of the traditional nursing role, which was often at odds with the increased technological roles and responsibilities nurses were undertaking. In turn, advertisers saw the nursing profession as a new and potentially lucrative market of women with disposable income and influence in the health field.

Originality/value

To date, there is a scarcity of marketing analysis that examines advertising placed in trade journals and in particular for early twentieth century health-care professionals. Through a lens of advertising analysis, this study investigates the advertising nursing readership of the day was exposed to and the motives and tactics used by those interested in expanding both health-care worker and consumer markets through a singular strategy.

Abstract

Purpose

The purpose of this paper is to explore the perceptions of new nurses in Singapore of their experiences of role transition and to examine the implications for managers in terms of employee training, development and retention.

Design/methodology/approach

This qualitative study was conducted using a constructivist grounded theory approach. In total 26 novice nurses and five preceptors (n=31) from five different hospitals participated in the study. Data were collected from semi-structured interviews and reflective journal entries and analysed using the constant comparative method.

Findings

The findings revealed that novice nurses remained emotionally and physically challenged when experiencing role transition. Two major constructs appear to play an important part in the transition process; learning how to Fit in and aligning personal with professional and organisational identities. The findings highlight factors that facilitate or impede Fitting in and aligning these identities.

Originality/value

Although the concept of Fitting in and its relation to the attrition of novice nurses has been explored in global studies, that relationship has not yet been theorised as the dynamic alignment of multiple identities. Also, whilst most research around Fitting in, identity and retention has been conducted in western countries, little is known about these issues and their interrelationship in the context of Singapore. The study should inform decision making by healthcare organisations, nurse managers and nursing training institutions with respect to improving the transition experience of novice nurses.

Abstract

Purpose

There have been profound changes in the Portuguese national health system (NHS), instigated under the influence of managerialism and the new public management (NPM) “philosophy”. These changes have been in line with what has happened in other developed countries. At the beginning of the new century, important reforms that emphasised the efficient use of scarce resources were implemented. The objective of this study is to understand how nurses are adapting to a more managerial environment, one in which economic rationalism and market‐driven initiatives are the key principles behind the health reforms.

Design/methodology/approach

A qualitative study was developed, based on semi‐structured interviews with 83 nurses with managerial duties in ten hospitals in Portugal. All interviews were tape‐recorded and each interviewee's discourses were subjected to content analysis.

Findings

Data analysis led to the conclusion that under the new logic of the market and managerialism, these professionals have tried to (re)define their professionalisation route by emphasising the importance of care but also by trying to incorporate management as their dominant role in the social division of work. In reconfiguring their notion of professionalism, nurses were incorporating new practices in their day‐to‐day activities. This empirical study confirms that professionalism can also be conceptualised as a technology of self‐control being able to discipline professionals at the micro level.

Originality/value

This research is an empirical study based on the effects of managerialism on nurses with managerial duties in Portugal. This study contributes to a better understanding of the complex process of the professionalisation of nurses in a context of institutional change.

Abstract

Purpose

The purpose of this paper is to investigate nurses’ attitudes, perceptions, and reactions to a new point-of-care information system for documenting nursing care.

Design/methodology/approach

A design science research methodology (DSRM) was used to examine the feasibility and usability of a novel nursing informatics solution in the context of acute hospital care. Data were collected using focus groups and non-participant observations. Analyses were guided by the theoretical lens of actor-network theory (ANT).

Findings

The findings unpack an understanding of the potential value of a new technology, rather than a binary understanding of positive or negative value. Using the ANT lens, the study reveals the dynamics of the nurse-technology relationships and consequent disruptions throughout the translation process. The findings highlight the central role of negotiation in the socio-technical construction of the hybrid actor-network during the implementation of new technology in acute hospital contexts.

Research limitations/implications

Further studies are needed to investigate the dynamics and complexity of the translation process that occurs during technology adoption, reactions of the involved actors to the emerging network and impacts on their role and work process.

Practical implications

Engaging nurses early during development and testing; aligning the new system’s functionality and interface with nurses’ interests and work practices; and supporting changes to clinical work process to enable an effective heterogeneous actor-network to emerge and become stable.

Originality/value

This study presents a novel use of ANT in a DSRM to understand an enterprise-wide system involving nurses and real clinical settings. The emerged actor-network provides insights into the translation process when nurses adapt to using new technology in their work.

Abstract

Purpose

The purpose of this paper is to lay out how advanced practitioner development occurs in New Zealand primary health care settings. The paper specifically focuses on mechanisms occurring across policy creation and in practice leading to successful role development.

Design/methodology/approach

The authors applied a realist approach involving interviews, document review and field log observations to create refined theories explaining how successful development occurs.

Findings

Three final mechanisms were found to influence successful advanced practitioner role development: engagement in planning and integrating roles; establishing opportunities as part of a well-defined career pathway; and championing role uptake and work to full scopes of practice.

Research limitations/implications

This research focuses on one snapshot in time only; it illustrates the importance of actively managing health workforce change. Future investigations should involve the continued and systematic evaluation of advanced practitioner development.

Practical implications

The successful development of advanced practitioner roles in a complex system necessitates recognising how to trigger mechanisms occurring at times well beyond their introduction.

Social implications

Potential candidates for new roles should expect roadblocks in their development journey. Successfully situating these roles into practice through having a sustainable and stable workforce supply provides patients with access to a wider range of services.

Originality/value

This is the first time a realist evaluation has been undertaken, in New Zealand, of similar programmes operating across multiple sites. The paper brings insights into the process of developing new health programmes within an already established system.

Abstract

Purpose

The purpose of this paper is to explore the way gender may be used as an instrument to avoid New Public Management (NPM) potential processes of deprofessionalisation in nursing.

Design/methodology/approach

In total, 83 nurses with managerial duties were interviewed in autonomous and corporate public hospitals in Portugal.

Findings

Nurses used gender as an argument to legitimate their presence in management, and in this way, to keep their control over the profession. Gender stereotypes were used to legitimate their position in two different ways. Firstly, nurses reproduced and reinforced gendered inequality by supporting their male colleagues careers. Secondly, they valorised their feminine skills sustaining that women were in better position to manage hospitals as an extended role from the private domain.

Research limitations/implications

The paper uses a sample from only one country and care must be taken when extrapolating conclusions to the wider population.

Practical implications

Acknowledges the way NPM reinforces gender stereotypes and contributes to redefine professionalism.

Originality/value

Recognition of the complexity and diversity of gender issues in the organisational context and in the structuration of professional legitimacy.

Abstract

This paper reports on a comparative qualitative study across four European countries which explored the formation of work identity amongst nurses and other professionals in the field of health care. Within this sector, it identifies trends towards a more flexible, more highly skilled and more mobile workforce. Conversely, however, it is becoming difficult to recruit and retain staff due to increasing workload, decreasing job satisfaction and comparatively low pay. Occupational identity is theorised as a multi‐dimensional phenomenon, with structural, social and individual‐psychological components. A number of emerging common themes across the three dimensions and across the four national settings include structural conflicts between cost efficiency and quality of care, and individual conflicts between the core activity of caring for patients and the increasing demands of administration and other peripheral work. The study identifies a number of strategies used by nurses to balance these conflicting demands. Overall, the professional identity of nurses remains strong, but it is important for policy makers to be aware of the potential negative effects, in terms of staff turnover, mobility and job (dis)satisfaction, of the current state of the health care sector.

Abstract

Purpose

Several issues are reported in the literature regarding the preparation and training of nurses for the preceptor role. The purpose of this study is to explore the experiences, growth and development of nurses transitioning to the preceptor role in allied health contexts.

Design/methodology/approach

A basic interpretive qualitative research method was used for this study. In total, 20 preceptors who were practicing in a variety of healthcare settings participated in in-depth interviews.

Findings

The preceptors of this study found meaning through their teaching and learning encounters with novice nurses. Their meaning making led to identity development and new perspectives on both the nursing and preceptor roles.

Research limitations/implications

This study extends the literature on informal learning and training by focusing on the unique work role of nurse precepting. Conclusions of this study call for additional research that examines other occupational areas in which workers have transitioned from expert to novice again, and how training can enhance these transitions

Practical implications

Participants described several areas of improvement for preceptorships: additional administrative support, guidelines and standards for preceptor training and preparation and additional time and support for transitioning to the preceptor role

Originality/value

Work role transition theory was used in this study to examine the preparation and training of preceptors. This study features the voices of nursing preceptors who have experienced changes in their employment status and major shifts in their work roles transitioning from expert to novice to expert again.
