Abstract

Purpose

This study aims to evaluate the functional readiness of selected hospitals in Tehran in the face of biological events focusing on the Coronavirus.

Design/methodology/approach

The current study is a cross-sectional, descriptive-analytical study, with the research population consisting of four hospitals in Tehran (Ministry of Health, Social Security, Azad and Military University). This study used data collection tools, standardized functional preparedness tools for hospitals using a biological approach and a standardized checklist of biological event preparations for the American Hospital Association. Interviews with the incident and disaster committee director and observation of each hospital’s existing documents, were used to collect data, which was then analyzed using SPSS-16 software.

Findings

According to the results, the average percentage of total hospital preparedness in biological events is 36.9%. With 53.3%, the selected military hospital has the most preparation, whereas the Ministry of Health has the lowest preparation with 28.3%. Surge capacity management and communication had the most remarkable preparedness rate of 68.75% (adequate preparedness), biological consultants, meeting management and post-disaster recovery had the lowest preparedness rate of 0% (extremely weak preparedness).

Practical implications

The average functional preparedness of selected hospitals in Tehran was assessed at an insufficient level in this study. Given the recurrence of disease waves, these results are helpful in increasing hospital preparedness for impending events. Improving preparedness in most areas, especially in post-disaster recovery seems necessary.

Originality/value

Given the COVID-19 pandemic, it is important to assess hospitals’ readiness to increase capacity and respond to this scourge. Few studies have been done in this field in the world. This study investigates this issue in the capital of Iran. The finding of this study suggest authorities’ attention to this issue and the creation of severe and prompt solutions and measures and the use of military hospital experiences to improve biological threat preparedness.

Abstract

Purpose

The regular occurrence of natural disasters elevates the need for an effective method to measure organizational preparedness in responding to the adverse impact of disasters. In this context, this paper presents a new decision support model to assess organizational disaster preparedness using both subjective and objective disaster preparedness criteria in a multi-criteria decision-making context.

Design/methodology/approach

The statistical variance method is integrated with the proximity value index (PVI) technique to determine priority scores in order to rank organizational disaster readiness.

Findings

The results of applying the integrated model developed herein enable decision-makers to make informed decisions for assigning priority ranking of organizational disaster preparedness in a simpler and more efficient way.

Research limitations/implications

Human resource is the most impacting criterion affecting hospital preparedness in undertaking action to cure disaster victims.

Practical implications

This paper offers an exemplar of a simple and efficient decision-making process considering the subjectivity associated with decision-making as well as the objectivity of data used for determining the priority ranking of organizational disaster preparedness.

Originality/value

Integrating statistical variance method with the PVI technique is novel and it has not been presented in previous studies. In fact, this study is the first to integrate both methods for selecting the priority ranking of organizational disaster preparedness.

Abstract

Purpose

The years following the 9/11/2001 terrorists attacks saw a marked increase in community and hospital emergency preparedness, from communications across community networks, development of policies and procedures, to attainment and training in the use of biological warfare resources. Regular drills ensured emergency and health care personnel were trained and prepared to address the next large-scale crisis, especially from terrorist and bioterrorist attacks. This chapter looks at some of the more familiar global health issues over the past two decades and the lessons learned from hospital responses to inform hospital management in preparation for future incidents.

Search Methods

This study is a narrative review of the literature related to lessons learned from four major events in the time period from 2002 to 2023 – SARS, MERS, Ebola, and COVID-19.

Search Results

The initial search yielded 25,913 articles; 57 articles were selected for inclusion in the study.

Discussion and Conclusions

Comparison of key issues and lessons learned among the four major events described in this article – SARS, MERS, Ebola, and COVID-19 – highlight that several lessons are “relearned” with each event. Other key issues, such as supply shortages, staffing availability, and hospital capacity to simultaneously provide care to noninfectious patients came to the forefront during the COVID-19 pandemic. A primary, ongoing concern for hospitals is how to maintain their preparedness given competing priorities, resources, and staff time. This concern remains post-COVID-19.

Abstract

Purpose

The purpose of this paper is to present a qualitative report on the implementation of activities aimed at reducing disaster risks through safer health facilities in the Western Pacific region.

Design/methodology/approach

In response to the Global Campaign on Hospitals Safe from Disasters 2008‐2009, the World Health Organization, with support from the European Commission, provided tools that enabled government, communities and institutions to reduce the risks of their hospitals and health facilities from disasters due to natural hazards. Target countries included Cambodia, Lao PDR, the Philippines and Viet Nam. Different activities were implemented based on country needs, contexts and capacities.

Findings

Hospital assessments revealed gaps in infrastructure, essential emergency equipment and functional capacity. Enhancing preparedness and resilience of health facilities is a vital component to ensure risk mitigation and disaster response in communities.

Research limitations/implications

The report calls for further studies on specific gaps in health sector capacity, deficiencies in health infrastructure and economic analysis.

Practical implications

The health sector should implement disaster risk reduction and preparedness strategies through the support of local and national governments.

Originality/value

This is the first report on risk reduction initiatives in the health sector in developing countries in the region. Written at the regional perspective, it engages national and local governments, health policy makers and health facility administrators in the public and private sectors.

Abstract

Purpose

Resilient health facilities, particularly hospitals, are critical for a responsive local health system in post-disaster settings. The purpose of this paper is to look at the experiences of two hospitals (public and private) in Tacloban City, Philippines in the aftermath of a super typhoon and their respective delivery of health services in such setting.

Design/methodology/approach

It described the impact of Typhoon Haiyan on health services delivery capacity and the factors instrumental in the resilience of the case hospitals. Lessons learned from the hospitals’ experiences, both at the level of the hospital staff and the institution, were also drawn. Disaster preparedness of case hospitals were assessed along several domains of resilience. Key informant interviews among stakeholders were conducted with key themes on disaster resilience extracted.

Findings

Disaster preparedness scores for case hospitals were different from each other and were reflected in their experiences of health services delivery in the aftermath of the disaster.

Research limitations/implications

This study on hospital resilience of two case hospitals, in the aftermath of Typhoon Haiyan, is exploratory in nature. The retrospective design of the study made it prone to recall bias. Further, the use of self-report measures for hospital resilience needs to be validated by more objective measures. The lack of baseline pre-disaster resilience indicators and the unpredictability of disasters could perhaps be addressed by a longitudinal study on hospital resilience in disasters in the future.

Originality/value

This study revealed several key findings. Some of the themes that emerged were: public health in disaster is the responsibility of both public and private hospitals; need for flexibility in disaster preparedness and planning, disaster resilience is an emergent process not a static construct, chaos results from zeal without coordination, and the need for integration of disaster preparedness in daily processes and structures of hospital facilities.

Abstract

Purpose

An emerging technology in the primary stages of its life cycle is the blockchain. This research paper aims to evaluate the preparedness of hospitals in using blockchain technology in their electronic health record (EHR) systems.

Design/methodology/approach

In the initial stage, 15 criteria relating to preparedness in using blockchain in EHR systems were identified from the literature and divided into five criteria, namely, technological, legal, financial, environmental and organizational. Then, 17 experts from various specialized fields were invited to form expert panels. After validating the criteria identified by the expert panels, the weights of the criteria were determined through the fuzzy best-worst multicriteria decision-making method. Following that, the preparedness of ten selected hospitals in Tehran to use blockchain in their EHR systems was assessed via the weighted aggregated sum product assessment method. Finally, using sensitivity analysis and examining different scenarios, the robustness of the results of the proposed approach was validated.

Findings

According to expert judgments, the legal criterion (32%) was deemed the most important factor in the preparedness to use blockchain in EHR systems followed by technological (28%), financial (17%), organizational (13%) and environmental (9%) criteria. A sensitivity analysis showed that the proposed approach offers good strength and robustness in evaluating the selected hospitals.

Originality/value

This study can be useful in developing knowledge in the field of technology management for evaluating blockchain implementation in the health-care industry using a novel, coherent and robust approach. In addition, the proposed approach provides comprehensive insight for decision-makers on assessing preparedness in deploying blockchain technology in EHR systems.

Abstract

Purpose

Hospitals are all required to be prepared against crisis, while according to studies, most hospitals are not prepared enough to encounter disaster problems. Therefore, each hospital should have an established programme to face earthquake and other catastrophes. This paper aims to investigate this issue.

Design/methodology/approach

This descriptive study was carried out in teaching hospitals of Iran University of Medical Sciences. Data collection was done using a checklist including general information, as well as information on demography, security, supplies and equipment, evacuation and communication. This procedure was done through observation and interview with hospital managers, etc. and the data were analyzed by SPSS software.

Findings

In the hospitals four dimensions of planning were studied and the following mean scores were obtained. On security, Hazrat Rasul hospital scored the highest (88.4) while Navab Safavi, Haftome Tir and Ali Asghar hospitals scored the lowest (51.4). On supplies and equipment, Navab Safavi and Haftome Tir hospitals obtained the highest score (66.6) and Hasheminejad, Shafa Yahyaeian, Firoozgar and Hazrat Rasul hospitals got the lowest score (60.0). On evacuation, Hazrat Rasul and Haftome Tir hospitals scored the highest and lowest respectively (64.2), (47.0). On communication, Hasheminejad hospital scored the highest (63.2) while Firoozgar hospital scored the lowest (36.6). In general from among ten hospitals under study the most and the least prepared hospitals were Hazrat Rasul and Navab Safavi respectively (65.65), (54.3).

Originality/value

The paper shows that most hospitals under study were not prepared enough against crisis and communication is found to be the weakest aspect although it is regarded as a basic principle in planning. Hazrat Rasul hospital was well prepared against crisis due to its disaster plan.

Abstract

The possibility of a bioterrorism event haunts all healthcare organizations. We believe a bioterrorism event is more likely in an urban area, and that urban multiprovider systems will be the “first line healthcare responders.” Due to the lack of empirical research on this topic and firsthand experience with a bioterrorism event, this paper will provide the theoretical underpinnings to support the rationale for multiprovider systems as “first responders.” This chapter outlines the nature and challenges of bioterrorism for a healthcare organization, the likely “state of the art” preparations for such events on the part of the four categories of healthcare organizations, and finally, the implications for the structuring of multiprovider systems to enhance their ability to plan for, and respond to bioterrorism events. Potential future research issues are also addressed.

Abstract

Purpose

This study describes preparedness of emergency physicians and general practitioners following chemical, radiological and nuclear incidents.

Design/methodology/approach

Five emergency physicians and six general practitioners were interviewed individually, and data was analysed using qualitative content analysis.

Findings

The study results showed that physicians' preparedness for chemical, radiological and nuclear incidents is linked to one main category: to be an expert and to seek expertise and two categories: preparations before receiving CRN patients, and physical examination and treatment of CRN patients with subcategories.

Research limitations/implications

The results have implications for further research on the complexity of generalist vs specialist competence and knowledge when responding to chemical, radiological and nuclear incidents.

Originality/value

This study provides insights regarding chemical, radiological and nuclear preparedness among physicians at emergency departments and primary healthcare centres.

Abstract

Purpose

Universities and higher education institutes are key pillars of all societies, as the result of their important roles in education, research and training of human resources. Nevertheless, similar to other social structures, universities need to be prepared for natural disasters. This study aims to measure and analyze the level of earthquake preparedness in a higher education institute in Tehran, Iran.

Design/methodology/approach

This cross-sectional study was conducted in 2017 in a higher education institute through three stages of tool, namely, development, preparedness evaluation and strength–weakness–opportunity–threat analysis. The valid and reliable tool was applied for assessment of college preparedness through a field survey. Factors affecting the obtained level of preparedness were categorized into internal (strengths and weaknesses) and external (opportunities and threats) factors to decide on an appropriate strategy for promoting the preparedness level.

Findings

Content validity index and content validity ratio of the tool were 88 and 98.85 per cent, respectively. The reliability of the tool was measured to be 98 per cent using the kappa coefficient. The level of preparedness was estimated to be weak. The final value of the matrix of internal and external factors was higher than average, which can be improved in the form of strength–opportunity strategy.

Originality/value

It is highly suggested to allocate a specific budget for improvement and evaluation of preparedness measures in all parts of education systems. In addition, making the preparedness measures as the mandatory duty for educational structures can be considered by decision-makers and managers at the national and local levels.
