Abstract

Purpose

The purpose of this paper is to propose that the study, and the promotion, of recovery can be augmented by adopting the model of mental health as a complete state.

Design/methodology/approach

A literature review of the last two decades was undertaken and pathways to complete mental health in recovery are proposed.

Findings

More work is needed to further develop interventions oriented towards the promotion of positive mental health in recovery, targeting the enhancement of positive emotions towards life and a sense of fulfillment in private and social life. Positive mental health also deserves more research attention to assess the full range of recovery outcomes related to the restoration and optimization processes. A better understanding of individual and environmental factors facilitating or hindering the achievement of complete mental health in recovery is warranted as well.

Originality/value

Unlike previous conceptions, the model presented in the paper proposes to redefine recovery from the complete view of mental health and introduces positive mental health as an additional outcome of recovery.

Abstract

Purpose

This paper aims to examine student exercise physiologists (EPs) and student dietitians’ confidence regarding working with people with severe mental illness (SMI) pre- and post-practicum in a mental health service.

Design/methodology/approach

This single-arm, quality improvement project included students completing practicum within a lifestyle programme embedded in mental health services. Student EPs completed 100 h of practicum across 15 weeks as part requirement for their Bachelor of Exercise Physiology degree and student dietitians completed six weeks full-time (40 h/week) for the part requirement of their Master of Nutrition and Dietetics. Students completed the Dietetic Confidence Scale (terminology was adapted for student EPs) pre- and post-practicum.

Findings

In total, 27 student EPs and 13 student dietitians completed placement and returned pre- and post-practicum questionnaires. Pre-practicum confidence scores were 90.8 ± 17.1 and 86.9 ± 18.9 out of a possible 140 points for student EPs and student dietitians, respectively. Confidence scores increased substantially post-practicum for both student EPs [mean difference (MD) = 29.3 ± 18.8, p < 0.001, d = 1.56] and dietitian students (MD = 26.1 ± 15.9, p = 0.002, d = 1.64). There were significant improvements in confidence across all domains of the confidence questionnaire for both EPs and dietitian students.

Originality/value

There is a research gap in understanding the confidence levels of student EPs’ and student dietitians’ when working with people with mental illness and the impact that undertaking a practicum in a mental health setting may play. To the best of the authors’ knowledge, this is the first study to explore student EP and student dietitian confidence in working with people with SMI pre- and post-practicum in a mental health setting.

Abstract

Approximately 90% of prisoners experience mental health problems, substance misuse problems or both. However, prison reception screening tools are not always effective in enabling staff to identify mentally disordered prisoners. Therefore, to ensure that these individuals get access to appropriate care, custodial staff should be trained in recognising the signs and symptoms of mental health disorders, and in effectively working with these individuals. This paper charts the pilot implementation of a mental health awareness workbook designed for use in custodial settings across England. It examines the variety of approaches adopted to implement the workbook, staff views on the usefulness of the workbook, and barriers to implementation encountered in each area. Recommendations made for best practice in delivering the workbook in other areas suggest a need for changes to its format, but also that time should be ring‐fenced for staff to participate in this training, in groups led by experts such as in‐reach team members.

Abstract

Purpose

The purpose of this paper is to investigate Keyes’ (2007) model of mental health, the presence (flourishing) or absence (languishing) of social, emotional and psychological wellbeing, in the context of drug and alcohol misuse and the frequency and pattern of community participation (engaging in society).

Design/methodology/approach

Participants were 1,815 individuals (70 per cent male) who entered residential substance misuse treatment provided by The Salvation Army. Questionnaires were completed at intake assessments with The Salvation Army staff. The data were compared with population norms of community participation utilising t-tests, while multiple linear regression was used to examine continuous mental health.

Findings

Although participants have lower levels of community participation compared to Australian population norms, those participants who were experiencing flourishing mental health had higher rates of community participation than Australian norms. Keeping in touch with friends and family was the most common form of participation. Informal social connectedness and civic engagement were the strongest predictors of mental health over and above more traditional substance use outcomes such as cravings.

Originality/value

This is one of the first studies to describe the relationships between community participation, substance use and mental health in participants seeking treatment for substance misuse. Despite having a drug or alcohol addiction requiring treatment, those participants with flourishing mental health have higher levels of community participation than community norms. Furthermore, community participation predicts mental health. This offers promise for interventions that increase community participation but further research using longitudinal designs is needed to replicate and clarify the direction of these relationships.

Abstract

Purpose

Observations have been made that mental health students receive very little support following observing patients displaying suicidal behaviour. The purpose of this paper is to conduct a small‐scale empirical study to investigate this issue further.

Design/methodology/approach

The approach used in this study is phenomenological. Qualitative data were obtained through semi‐structured interviews consisting of a range of questions asking mental health student nurses about their experiences of support in practice. The data are analysed using thematic analysis.

Findings

As well as issues relating to the support of mental health student nurses in practice, there are many ethical issues raised in this paper. These include student responsibilities while in placement areas; students as having a supernumerary status; and the inclusion of students in supervision and debriefing sessions following traumatic incidents.

Research limitations/implications

This small‐scale exploratory study was conducted with a small number of students in one University. However, the study provides a strong starting point for further research on the support students receive during their mental health nurse training.

Originality/value

This paper makes some recommendations on ways to improve the support of students in practice, including maintaining and supporting the role of Practice Experience Managers who spend a considerable amount of time in placement areas interacting with students and feeding back relevant practice concerns to University staff.

Abstract

Purpose

The purpose of this pilot study was to evaluate an online mental health literacy training program for college female mentors.

Design/methodology/approach

Undergraduate female participants (n = 10) completed in asynchronous mental health literacy training. Participants completed the Heads Up Checkup (HCU) and mental health literacy scale (MHLS). Participant usage data were collected for the training course and analyzed using means and standard deviations. Qualitative data were analyzed by axial coding and thematic categorization.

Findings

The mean MHLS score, based on mean (M) and standard deviation (SD), was high for the participants (M = 145.69, SD = 8.41). Six qualitative themes from the training effectiveness feedback were used: (1) help-seeking, (2) stress management, (3) tailored guidance to resources, (4) understanding warning signs, (5) body image awareness and (6) engaging with mentees.

Research limitations/implications

There are a few limitations to this study including the small sample size, unrestricted time frame for completion of the asynchronous training and the need for long-term follow-up of the intervention effects.

Practical implications

This pilot study provides initial support for the mental health literacy training program when implemented with undergraduate females.

Social implications

Given inadequate mental health literacy in college females regarding such topics, continually training undergraduates about these warning signs is necessary. Future research could consider implementing this program with all gender identities, various age groups and in both synchronous and asynchronous modalities. This program could also be replicated using a pre- and post-test design to evaluate direct impact of the training on outcome variables.

Originality/value

All the participants discussed how the training prepared them in aiding mentees in help-seeking and identifying appropriate mental health resources. Participants also indicated how the training enabled personal and others-oriented stress management, which supports a mental health literacy training program that reduced stress in medical students.

Abstract

Purpose

Inequalities can have a cumulative effect that leads to the presentation and subsequent progression of mental health difficulties. The detrimental effects can be compounded in the healthcare environment if staff lack an awareness of patients’' inequalities, and therefore, educating staff is of particular importance. The development of awareness training requires a deep understanding of staff perceptions of patient inequalities in a secure mental health care setting and the impact that this can have on mental illness.

Design/methodology/approach

The study was carried out using a qualitative design, where staff were asked to complete a 22-question survey from which the output is analysed using thematic analysis. In total, 100 patient-facing staff members working in a secure mental health facility completed the survey.

Findings

The results highlight that staff employed in a secure mental health care setting have an understanding of patient inequalities and how these can impact on patients in both the short and longer terms. The results highlighted the importance of awareness by staff and how an increase can have a significant benefit on the quality of the care provided within secure mental health facilities.

Originality/value

There is an increasing awareness of the impact of inequalities on mental health and how this can influence a patient’s journey. This study involving staff employed in a secure care mental health facility highlights the role of staff awareness of inequalities and also underlines the importance of understanding the key role of staff awareness in mental ill health.

Purpose

The number of university students seeking mental health support is rapidly increasing. To provide additional psychological support to students accessing well-being services, this study aimed to pilot a mental health Web application (app) called Orpheus®.

Design/methodology/approach

Guided by student consultations, a multi-methods approach was adopted, including an examination of in-app data, chart reviews of routinely collected student information and interviews with mental health practitioners. Usage data were analysed descriptively. Changes in mental health outcomes were examined using means, standard deviations and reliable change indices for anxiety and depression scores. Inductive and deductive thematic analysis was conducted on qualitative data from staff interviews and student feedback.

Findings

A total of 26 students registered an account with 39 completed app visits. On 37 of the 39 (94.9%) occasions, students reported reductions in the intensity of unwanted negative emotions. Statistically significant reductions in the average pooled anxiety and depression scores were observed. Of the 15 students who completed pre- and post-routine outcome measures, between 20% and 60% showed reliable and meaningful symptom improvements. Students reported that the app was helpful and easy to set up and use, with no adverse events. Practitioners highlighted barriers and facilitators related to the technology features, situational contexts and individual differences.

Originality/value

Integrating Orpheus in real-world settings resulted in promising implementation processes, potential for future uptake and positive outcomes. However, ongoing research, staff training and app testing are needed to further improve the implementation processes for digital mental health interventions.

Abstract

Purpose

Depression and anxiety are the most common comorbidities in TB patients, adversely impacting TB treatment outcomes. The purpose of this study is to assess the effect of lay counselling in reducing symptoms of depression and anxiety and TB treatment completion.

Design/methodology/approach

The study used a pre-post interventional research design. Patients were screened for depression and anxiety. All symptomatic TB patients were followed up and offered four to six or more lay counselling sessions whenever necessary. The authors assessed changes in depression and anxiety symptoms before lay counselling intervention and after fourth lay counselling session or the completion of TB treatment, whichever occurred the last.

Findings

Approximately 6,974 TB patients were screened for symptoms of depression and anxiety. The mean age was 36.7 ± 14.7 years. Total 25.9% patients were symptomatic. About 99.8% were provided lay counselling and received at least one to two follow-up sessions, while two patients who screened with severe depression were referred to a mental health specialist. Nearly 96.9% TB patients did not report symptoms of depression or anxiety after four lay counselling sessions, and TB treatment completion rate was higher among symptomatic TB patients who completed at least four counselling sessions (92.5%).

Practical implications

Lay counselling services delivered by field coordinators offer a promising approach to address mental health comorbidities among TB patients in resource-limited settings.

Originality/value

It explores a novel approach – lay counselling delivered by field coordinators – in tackling depression and anxiety among TB patients, which is a potentially scalable solution in resource-limited settings.

Abstract

Purpose

Recovery in mental health emphasises the empowerment of clients to discover and develop hope and a more satisfying life often in the presence of ongoing symptoms of mental illness. Work values that are incompatible with values that underpin the recovery philosophy may be contributing to the challenges in implementing recovery values in practice. The purpose of this paper is to explore the types of work values espoused by Australian mental health workers and their degree of congruence with recovery values.

Design/methodology/approach

In total, 65 Australian mental health workers completed an open-ended work values question. Leximancer content analysis was used to generate a thematic work values profile followed by a theory-led thematic analysis of the responses to assess congruence with recovery values.

Findings

This sample valued client-centred practice that supports recovery, making a difference in others’ lives, work competence, being caring and empathic, and meaningful work. Overall, there was substantial congruence between work and recovery values, with less evidence of endorsement of values relating to strengths-based approaches, personal responsibility, and positive self-identity. These values should be targeted in future training initiatives.

Originality/value

The current study is the first study to identify the types of work values espoused by Australian mental health workers and to examine the degree to which they are recovery-consistent. This is an important research agenda given the high national and international priority to adopt a recovery orientation, and the need to identify and modify potential barriers to the implementation of recovery-oriented services.
