Abstract

There are a great many people with mental health problems receiving treatments that are aimed primarily at managing aggressive or challenging behaviours by means of sedation, seclusion or restraint. The management of these behaviours is frequently justified under Part IV of the Mental Health Act 1983, which permits treatments ‐ without consent if necessary ‐ for a person's mental disorder. Alternatively, the behaviours have been managed at common law on the basis that the patient is incapable of giving a legally effective consent to behaviour management and the interventions are considered to be in the person's best interests. This article considers the management of difficult behaviours in adults with mental health problems. This issue is particularly timely as there have been a considerable number of legal challenges in this area in recent years. This article reviews the practical contexts in which behaviour management, which here covers sedation, seclusion and restraint, is used with adults with mental health problems and explores the legal justifications for these interventions. Although the justification for the management of challenging behaviours is frequently asserted (or, more likely, presumed) to be therapeutic, a review of the research literature and recent case law casts doubt on the credibility of this therapeutic justification.

Abstract

Purpose

This pilot study aims to assess the feasibility of conducting shared philosophical inquiry with women at risk of re-offending to improve motivation to change. The philosophy sessions aimed to give participants new ways to think about their lives and to help them have more control over their own mind by learning new ways to think differently.

Design/methodology/approach

The pilot study adopted a mixed-methods approach to collect and analyse data pre- and post-intervention. Ten women serving a custodial sentence at the Democratic Therapeutic Community (DTC) in HMP Send were recruited to take part in ten weekly sessions of philosophical discussion. The intervention was adjunctive and not meant to replace other treatments an inmate may already be receiving.

Findings

The results showed that most participants experienced improved levels of well-being and mental health post-intervention, and that the intervention has the potential to help participants better critically assess their own behaviour and ways of thinking. It also suggested that the intervention has the potential to help participants engage more effectively with the therapeutic process.

Research limitations/implications

The results of this study are limited by the small sample size and the lack of a control group. As such this study cannot rule out that the changes observed in participants were a function of time or the specific therapeutic environment they were in (or both).

Originality/value

This pilot study is innovative not just for introducing philosophy classes to the women’s prison estate for the first time in England and Wales, but also in its ambition to contribute to the “what works” agenda in offender rehabilitation.

Abstract

Purpose

This study aims to evaluate a novel mobile therapeutic videogame for adolescents with anxiety disorders (ADs), combining elements of cognitive-behavioural therapy and attention-bias modification, in terms of both its therapeutic efficacy over a controlled intervention and two-month follow-up, as well as the extent and implications of self-directed play.

Design/methodology/approach

A within-groups design with two parallel conditions [clinical anxiety (N = 16) and subclinical/at-risk (N = 15)] were measured on both self-reported anxiety and threat-detection bias (TDB) across three timepoints (pre- and post-intervention and two-month follow-up).

Findings

Significant reductions were observed in both self-reported state and trait anxiety and TDB over the course of the two-week intervention, which were maintained at follow-up. Engagement in self-directed play during the follow-up period significantly predicted outcomes at two-month follow-up for clinical participants.

Originality/value

To the best of the authors’ knowledge, this paper represents the first of its kind to evaluate a mobile therapeutic game designed with and solely for adolescents with ADs. This study also represents the first of its kind to examine the extent and implications of self-directed play for outcomes.

Abstract

Purpose

The purpose of this paper is to examine prisoner experience of an equine assisted psychotherapy (EAP). This paper explores the use of therapeutic interventions; specifically focussing on EAP, within this paper EAP constitutes the use of horses in therapy and involves a team approach from equine and mental health experts.

Design/methodology/approach

This paper took a qualitative approach; due to the exploratory nature of this study a phenomenological approach was adopted. Interpretative phenomenological analysis was deemed appropriate; the intervention took place in an adult, male, open condition prison in England (Category D) however participants who engaged in the equine intervention were from both the open prison and a nearby closed Category C prison. The equine intervention was delivered by qualified therapists who worked to help improve emotional regulation among participants with a history of drug and alcohol abuse.

Findings

The findings within this paper identify a strong correlation between EAP and positive experiences expressed by participants. Alternative approaches such as animal assisted therapies are worthy of consideration when attempting to support the rehabilitation and treatment needs of incarcerated clients. Participants achieved a number of goals and their confidence improved as they felt a sense of achievement.

Research limitations/implications

This paper demonstrates the complexities of therapeutic interventions. Research relating to EAP in the UK is few and far between, consequently understanding is limited. This paper seeks to offer an insight into this topic and build upon this research in the future.

Practical implications

Access to prison for research purposes is challenging. Due to the nature of this study and the resources required sometimes EAP therapy cannot be implemented in or near many prisons in England and Wales. Therefore gaining access to this prison and exploring the data is the first phase of further research in this area.

Social implications

Researching the way individuals experience therapeutic interventions is a “growing phenomenon”. This paper aimed to explore EAP interventions, however due to the sample size it was imperative that the role of EAP was not misrepresented. Therefore this papers intention is to raise awareness of EAP interventions and therapeutic interventions in prisons in England and Wales.

Originality/value

To the authors knowledge no previous study has examined such an intervention using this method and as such the findings of this evaluation are important. Moreover this paper enhances and develops our knowledge about how best to support and treat people with histories of substance use and/or mental health problems and anxiety while in prison, and the vital role such therapies may play.

Abstract

Purpose

This paper aims to describe the evaluation of an innovative pilot mental health service for asylum seeking mothers and their babies in their first year of life, and to highlight the challenges and possibilities when intervening with this group.

Design/methodology/approach

A mixed‐methods evaluation was designed based upon the participatory action research framework, viewing mothers and their infants as active participants in the research process. Evaluation comprised reflective infant‐led session evaluations, CARE Index video microanalysis, and reflective focus group discussions. CARE Index analysis was conducted by a trained Psychologist and an external coder to ensure reliability of findings. Focus group discussions were thematically analysed, and reflective infant‐led session evaluations scored for comparison.

Findings

Evaluation has highlighted the imperative of designing responsive service models able to adapt to cultural nuances and the realities of asylum seekers' lives. Qualitative data provide a rich narrative of the benefits of therapeutic interventions for this group, which are reaffirmed by CARE Index analysis and session evaluations.

Research limitations/implications

As a pilot service the numbers involved in this evaluation are small. Furthermore, a paucity of measurement tools validated in languages other than English forced reliance upon self‐designed tools such as the reflective infant‐led session evaluation designed to complement a “keeping the baby in mind” ethos of the intervention. This has been complemented by CARE Index analysis and qualitative focus group discussions. The combination of measurement instruments and data analysis tools provides a comprehensive indication of the impact of this pilot intervention.

Practical implications

The benefits and challenges of establishing an early‐intervention therapeutic service for refugee and asylum seeking women and their infants are detailed and reflected upon. It is hoped that by chronicling the experience and findings an evidence base is being built to support development of future innovative service models.

Social implications

Policy aspirations to meet the needs of refugee and asylum seeking women and their infants identify the need to provide rights‐based, humane and person‐centred services. The pilot model described here meets these aspirations and can be used as an adaptable and responsive model upon which other services can draw.

Originality/value

The paper provides a comprehensive service evaluation, highlighting key policy and practice implications to support the delivery of health and social care services targeting refugee and asylum seeking women and their infants.

Abstract

Despite a long history of psychodynamic therapy interventions with people with learning disabilities and anger problems, there is little evidence that suggests that this approach is effective. The present case study explored 18 pychodynamic therapy sessions with an adult with mild learning disabilities and anger problems, investigating therapy outcomes and progress along a nine‐stage Malan (1979) model, as well as analysing therapeutic interactions leading to therapy progression. The patient's progression along Malan stages was explored by a thematic analysis matching therapy sessions to the Malan model. The investigation of therapeutic interactions was achieved by discourse analysis of those therapy sessions indicative of Malan‐Stage progression. The results are discussed and suggestions for future research and practice implications are made.

Abstract

Purpose

This study aims to highlight the need for an interdisciplinary intervention approach to bring noteworthy changes in children with an autism spectrum disorder. It proposes to study how holistic individualized therapeutic plans can promote functionality even in the adolescent age. This study aims to channelize the restricted abilities in a positive manner and make it, a strength for the child. Social-emotional development along with academic goals is also proposed.

Design/methodology/approach

The paper is based on an exploratory study where participant observation was the main tool. Unstructured interviews with the therapists were conducted and the background history was taken. Documents and assessments related to the case were referred.

Findings

The paper provides empirical insight into the impact of an interdisciplinary intervention on autism spectrum disorder. It suggests that speech therapy, occupational therapy, special education and counseling taken up as a holistic approach and modified as per the needs and competence of the child prove to be effective. It also becomes evident that intervention can help the child to become functional and meaningful even if the intervention is started a little late in life. Consistency and intensity of interventions along with compassion play a very positive role in the life of children with autism.

Research limitations/implications

As the approach taken describes one case in depth, there is less probability of generalization of results. Therefore, it suggests a wide scope of testing the proposed propositions further.

Practical implications

This paper includes implications for the children with autism spectrum disorder, who: are not able to get early intervention for some reason and the ones who have special restricted abilities. It also is an inspiration for the service providers to develop comprehensive and interdisciplinary plans of intervention.

Social implications

This will help parents who somehow miss providing interventions at an early age to be hopeful and to seek help. The results are encouraging so as to make children with autism spectrum disorder more functional and acceptable in their lives.

Originality/value

This paper worked on the identified needs of children with autism but found that their restricted abilities that are commonly found can be used and channelized positively to become a strength. There is a scope and hope to guide these children toward a functional life where they can connect with others around them and are accepted and included in society. Individualized and interdisciplinary interventions prove to make these children happier and confident.

Abstract

Purpose

The purpose of this study is to seek answer to the following question: Does participating in storytelling changes participants’ life experiences in their journey of recovery? The study explores participants’ experiences of engaging in a storytelling programme in a community mental health team in a large New Zealand city. The programme aims to provide a safe environment to support and increase participants’ engagement with services and the community. Currently, there is limited literature on consumer’s experiences of engaging in therapeutic storytelling programmes in the international or New Zealand context.

Design/methodology/approach

Following ethical approval, eight adult participants were recruited to the study. Data were collected through semi-structured interviews and analysed using the six-phase process of thematic analysis, developed by Braun and Clarke.

Findings

Three main themes emerged from the data. The first theme, breaking barriers, was described by participants as to how they overcame barriers. The second theme, creating positive memories, uncovered the positive experiences that participants gained from the programme. The final theme, becoming a new person, described some of the transformative experiences that emerged through participation in the programme.

Originality/value

This is the first qualitative study in the New Zealand context that has explored the value of a storytelling programme from a consumer’s perspective. The findings suggest that participating in a storytelling programme can contribute to a participant’s journey of recovery; the use of myths, legends and group work is fundamental to those from collective cultures with oral traditions. The ongoing value of storytelling as a therapeutic tool requires further research and the development of a clearer evidence base to inform practice.

Purpose

The purpose of this paper is to review the purpose of the Community Meeting and how it fits within the Sanctuary Model®, it will outline the tools theoretical underpinnings and finally how the tool is used in other trauma models specifically Therapeutic Communities.

Design/methodology/approach

This paper draws the key literature together related to the Community Meeting within the Sanctuary Model®, making links to theoretical influences between the Community Meeting, Trauma Theory and Attachment Theory. Finally it provides a comparison of the Community Meeting within Therapeutic Communities and the Sanctuary Model®.

Findings

The paper detailed how the Community Meeting tool is underpinned by the norms and theories of the Sanctuary Model®. It detailed the direct link between healing from trauma and building attachments to the Community Meeting. It found the similarities of the Community Meeting within both Therapeutic Communities and the Sanctuary Model® in that they always included all participants and occurring regularly in circular groups. However, it noted the distinct differences including in Therapeutic Communities the Community Meeting forms a significant intervention, whereas within the Sanctuary Model®, the Community Meeting supports the broader intervention of the model for all members of the community including staff and clients.

Originality/value

The Sanctuary Model® is gaining international interest and as such, critical consideration of its theoretical influences, similarities and differences with existing models is critical to understanding the model.

Abstract

Purpose

The purpose of this paper is to discuss Autogenic Training as an effective therapy for children with behavioural and emotional problems. The technique offers children in therapeutic communities strategies in dealing better with stress- and anxiety-related problems. The resulting increased body awareness and ability for self-reflection can also lead to a better engagement in other forms of therapy, decrease anxieties, reduce levels of frustration, increase self-esteem, decrease the fight/flight feeling and supports the work in therapeutic communities.

Design/methodology/approach

International literature and the author's experience are reviewed to provide a better understanding of the technique and its potential in therapeutic communities. The paper explores creative ways of modifying the technique aiming to help children engage in the autogenic process.

Findings

The paper concludes that Autogenic Training is an effective therapy and self-help technique for children with emotional and behavioural problems in therapeutic communities.

Originality/value

Autogenic Training is well known in other European countries and often used as a first intervention in therapeutic settings for children and adults. There seems to be a clear lack of literature, papers and understanding of the technique in the UK. The paper addresses this deficit.
