Abstract
Purpose
There is a growing academic interest in the examination and exploration of work intensification in a wide range of healthcare settings. The purpose of this paper is to explore the differing staff perceptions in emergency ambulance services in the UK. It provides evidence on the challenges for the paramedic professionalisation agenda and managing operational demands and work intensity in emotionally challenging circumstances, with significant implications for patient safety.
Design/methodology/approach
Drawing on the evidence from an empirical study in a large National Health Service ambulance trust in England, this paper examines the challenges and differing staff perceptions of the changing scope and practice of ambulance personnel in the UK. Amidst the progress on the professionalisation of the paramedic agenda, individual trusts are facing challenges in form of staff attitudes towards meeting performance targets, coupled with rising demand, fear of loss of contracts and private competition.
Findings
Research findings highlight differing perceptions from various sub-cultural groups and lack of clarity over the core values which are reinforced by cultural and management differences. Need for greater management to explore the relationship between high sickness levels and implications for patient safety including the need for policy and research attention follows from this study. The implications of work intensity on gender equality within the ambulance settings are also discussed.
Research limitations/implications
Ambulance services around the world are witnessing a strain on their operational budgets with increasing demand for their services. Study evidence support inconclusive evidence for patent safety despite the growing specialist paramedic roles. Organisational implications of high staff sickness rates have been largely overlooked in the management literature. This study makes an original contribution while building upon the earlier conceptions of work intensification.
Practical implications
The study findings have significant implications for the ambulance services for better understanding of the staff perceptions on work intensity and implications for patient safety, high sickness absence rates amidst increasing ambulance demand. Study findings will help prepare the organisational policies and design appropriate response.
Social implications
Societal understanding about the organisational implications of the work intensity in an important emergency response service will encourage further debate and discussion.
Originality/value
This study makes an original contribution by providing insights into the intra-organisational dynamics in an unusual organisational setting of the emergency ambulance services. Study findings have implications for further research inquiry into staff illness, patient safety and gender issues in ambulance services. Evidence cited in the paper has further relevance to ambulance services globally.

Purpose
This paper sets out to understand the impact of the ambient environment on perceived comfort, health, wellbeing and by extension productivity in the workplace.
Design/methodology/approach
The research combined an occupant survey considering satisfaction with the ambient environment, health and wellbeing and workplace behaviour with the monitoring of ambient environmental conditions.
Findings
The paper demonstrates that the ambient environment can have a significant impact on occupant comfort, health and wellbeing, which in turn has implications for built asset performance. Within the ambient environmental factors considered, a hierarchy may exist with noise being of particular importance. Occupant behaviour within the workplace was also found to be influential.
Research limitations/implications
The research was limited to a single commercial office building, and a wider range of case studies would therefore be of benefit. The research was also limited to the summer months.
Practical implications
The findings show that an active approach to asset management is required, by continuously monitoring internal environment and engaging with occupants. This must carefully consider how ambient environmental factors and workplace behaviour impact upon occupants’ comfort, health and wellbeing to ensure the performance of the built asset is maximised.
Originality/value
This paper demonstrates that both occupiers’ workplace behaviour and ambient environmental conditions can have an impact on occupant comfort, health, wellbeing and productivity. The paper strengthens the case for the active management of the workplace environment through environmental monitoring and behaviour change campaigns supported by corresponding changes to workplace culture.

Abstract
Many studies have shown that an unfavourable psychosocial environment increases the risk of mental and physical illness, as well as absenteeism, or sickness absence. However, more costly than absenteeism is presenteeism, where a person is present at work even though disabled by a mental or physical illness. We sought to identify factors explaining why workers would come to work even when their health is impaired. In a cross‐sectional design data were collected from 3825 employees of a Canadian organisation. The results show a high occurrence of presenteeism: workers went to work in spite of illness 50% of the time. Presenteeism propensity (the percentage of days worked while ill over total number of sick days) was higher for workers who were ill more often. Heavier workloads, higher skill discretion, harmonious relationships with colleagues, role conflict and precarious job status increased presenteeism, but decision authority did not. Workers reporting high psychological distress and more severe psychosomatic complaints were also more likely to report higher rates of presenteeism. These results suggest that stress research should not only include absenteeism as an outcome indicator, but also consider presenteeism.

Abstract
Purpose
There is a growing awareness within organisations of the significance of “green” issues. This paper aims to examine the impact of a public sector organisation's sustainable transport policies on the sickness absence levels of its employees. It focuses on those policies (such as workplace travel plans) which include methods of increasing the use of public transport for the journey to work.
Design/methodology/approach
This is an exploratory study, using a quantitative methodology, which analyzes data from a survey of a sample of employees within a large public sector organisation.
Findings
The key finding is that sickness absence levels are higher among employees who use public transport to travel to work. A profile of public transport user characteristics most associated with higher absence levels is offered. Moves to act “green” by encouraging greater use of public transport may unintentionally have an adverse effect on sickness absence levels.
Research limitations/implications
This is a study of a sample of employees within a large public sector organisation. The size and character of the sample were restricted by organisational constraints. The results are indicative of a potential area of concern which needs wider investigation.
Practical implications
HR practitioners need to be more closely involved in workplace travel plans and sickness absence variations between different modes of travel to work require careful monitoring.
Originality/value
These results suggest a link between the use of public transport for the journey to work and above average sickness absence levels which may have significant implications for the implementation of workplace travel plans.

Abstract
Purpose
Sickness absence is a major concern for healthcare services and their employees with healthcare workers having higher sickness absence rates compared to the general population. The purpose of this paper is to systematically review randomised control trials (RCTs) that aimed to reduce sickness absence among healthcare workers.
Design/methodology/approach
A systematic review was conducted that aimed to include RCTs with study participants who were employed in any part of the healthcare sector. This review included any type of intervention with the primary outcome measure being sickness absence.
Findings
Seven studies were included in the review and consisted of one exercise-only intervention, three multicomponent intervention programmes, two influenza vaccination interventions and one process consultation. Three studies (exercise-only, one multicomponent intervention programme and one influenza vaccination intervention) were able to demonstrate a reduction in sickness absence compared to control.
Research limitations/implications
Due to the lack of high-quality evidence, this review identified that there are currently no interventions that healthcare organisations are able to use to effectively reduce sickness absence among their employees. This review also highlights the importance of a standardised measure of sickness absence for healthcare staff, such as shifts.
Originality/value
To the authors’ knowledge, this is the first systematic review to synthesise such evidence among healthcare workers.

Purpose
The purpose of this paper is to examine the impact of wage dispersion on sickness absenteeism observed in Belgian firms.
Design/methodology/approach
The authors use detailed linked employer-employee panel data for the period 1999-2006 that allow the authors to compute a conditional wage dispersion indicator following the Winter-Ebmer and Zweimüller (1999) methodology and to estimate the relationship between sickness absenteeism and wage dispersion while controlling for time-invariant workplace characteristics.
Findings
The authors find a positive and hump-shaped relationship between intra-firm wage dispersion and sickness absenteeism, the turning point of this relation being extremely high. In addition, the magnitude of the influence of wage dispersion on sickness absenteeism is found to be stronger in firms employing a larger share of blue-collar workers.
Practical implications
The results could therefore suggest that wage dispersion, suggestive of larger pay-for-performance mechanisms, decreases worker satisfaction and the workplace climate in general. Only a minority of workers, who are less sensitive to equity and cohesion considerations, would be less absent as pay-for-performance increases.
Originality/value
While numerous approaches analyse the link between wage dispersion and firm productivity, very few studies we are aware of are devoted to the relationship between wage dispersion and sickness absenteeism. Yet, the outcomes in terms of productivity and sickness absenteeism may be different. Furthermore, the influence of wage dispersion on sickness absenteeism does not seem unambiguous from a theoretical point of view. To the authors knowledge, it is the first time that this relation is analysed with Belgian data.

The first taxonomy enumerates the indicators of sickness both in the built environment of a city and in its citizens that should be prevented, and the second taxonomy focuses on the health aspects of the built environment of the city and the physical, mental, social health of its citizens, which should be protected and promoted. The practical implementation of these indicators can become the basis for the creation of an urban health policy.

Purpose
The purpose of this paper is to describe the relationship between job stress, stress coping ability and performance among Japanese nurses.
Design/methodology/approach
Health risk and organization environment as job stress factors, sense of coherence (SOC) as stress coping ability and medical risk indicator and sickness‐absence days as a performance proxy were used to investigate the relationship between stress and performance. Length of professional experience also was included in the investigation.
Findings
The findings suggest a possibility that enriching nurses' professional experiences reduces medical risk. There is also a possibility that raising the SOC, while improving organization environment, contributes to reducing sickness‐absence.
Research limitations/implications
A cross‐sectional study of nurses in a single institution was used. In order to generalize the study's results, it will be necessary to conduct multi‐institutional longitudinal studies.
Originality/value
The present study shows key factors affecting medical risk and sickness‐absence leading to a reduced nursing performance.

Purpose
The aim of this paper is to study age-related differences in how perceptions of two forms of organizational justice, i.e. procedural and interactional justice, are related to short (i.e. non-certified) spells and long (i.e. medically certified) spells of sickness absence.
Design/methodology/approach
The authors conducted a study on a large sample of Finnish public sector employees (n=37,324), in which they matched employees' 2004 survey data with their records-based sick absences in 2005 and 2006.
Findings
The results suggest that age moderates the association between perceptions of procedural justice and long sickness absences after controlling for gender, tenure, occupational group, work unit, job demands and health behaviors. When older employees experienced a high level of procedural justice, they were 12 percent less likely to miss work due to medically certified illnesses. Overall, older employees were less likely to take short, non-certified sickness absences from work. Finally, the results suggest that high-quality relationships with supervisors can prevent both short and long spells of sickness absence at all ages
Originality/value
The study contributes to the literature on age-related differences in the effects of psychosocial workplace conditions (organizational justice) on employee behavior (absenteeism).
