Abstract

Purpose

Community health centres (CHCs) play a vital role in healthcare service delivery in rural India and act as a crucial link between the primary and tertiary healthcare systems. The rural population in the union territory of Jammu and Kashmir primarily depends on CHCs for healthcare services due to the scarcity of private healthcare infrastructure and the lack of access to tertiary hospitals. The purpose of this study is to analyse the impact of management capability, staff competence, waiting time and patient satisfaction on revisit intention among patients visiting CHCs for care needs. It further examines the mediational role of patient satisfaction between antecedents of patient satisfaction and revisit intention.

Design/methodology/approach

A survey by questionnaire was used to collect data from 318 inpatients and outpatients visiting CHCs. Partial least square-structural equation modelling was performed with the help of SmartPLS 3 software to evaluate the causal relationships between variables.

Findings

The findings of the study ascertain that staff competence and waiting time are strong predictors of patient satisfaction while management capability was reported as an insignificant factor. Patient satisfaction significantly affects revisit intention and successfully mediates the impact of management capability, staff competence and waiting time on revisit intention.

Originality/value

CHCs play a significant role in bridging the gap between primary healthcare and tertiary healthcare and in delivering healthcare services to the vast rural population in India. This study necessitates the active participation of management to ensure the smooth functioning of CHCs. There is a need to provide adequate staff and necessary infrastructural facilities to reduce the treatment waiting time.

Abstract

Purpose

Medical disruptive innovation is essential for deepening the reform of health-care system. The theory of general disruptive innovation assumes that innovations can diffuse by benefiting and attracting consumers through observed and objective relative advantages. Yet decision-makers for adoption in health-care settings are safety-sensitive professionals whose cognitions barriers about underperformance in focal attributes will impede further evaluation of innovation's ancillary performance. Existing studies do not answer the question of how such innovations can overcome safety barriers, find early adopters and grow to the early majority. The purpose of this study is to investigate the process, mechanism, and path of early diffusion of medical disruptive innovation.

Design/methodology/approach

The authors conduct a longitudinal case study of the diffusion of Enhanced Recovery After Surgery (ERAS) in China during 2011–2018.

Findings

The authors find that the diffusion process of medical disruptive innovations can be viewed as a cognitive evolutionary process that sequentially establishes conformity, differentiation and normalization. Cognition reframing of expert, meaning and benefit for professionals is its implicit mechanism. When adoption may trigger cognitive concerns, actors’ very early (dis)adoption is driven by a combination of structural position, innovation attributes and performance perceptions; central actors then play amplifier roles in the development from early adopters to the early majority.

Originality/value

This study proposes a process theoretical framework for the early diffusion of disruptive innovation. By dissecting the key processes and mechanisms from a cognitive perspective, the study offers theoretical contributions and practical insights into the diffusion of disruptive innovation in professional settings.

Abstract

Purpose

The purpose of this paper is to improve the understanding of the social contexts of sustainable Lean culture in healthcare by examining self-efficacy (SE) as a fundamental construct related to the value of perceived readiness, prior education of Lean and the importance of leadership’s system-level support.

Design/methodology/approach

A descriptive correlational study was conducted to identify the relationships between SE and Lean readiness factors, SE and prior Lean training, SE and clinical vs administrative roles and SE and perceived system-level support in a large health system.

Findings

There was a statistically significant difference in self-reported readiness to use Lean tools between individuals who had received Lean training during their academic education and those who had not; however, their level of education did not impact SE. Lastly, and perhaps most important, the learner who embodies SE also has system-level support.

Research limitations/implications

Future directions of this research, in addition to assessing team readiness as other studies suggest, would be to evaluate individual team member readiness by gauging SE and addressing deficits prior to the deployment of process improvement (PI) projects to promote success and sustainability.

Practical implications

This contributes to the ongoing scholarship of Lean management systems, providing clinical and non-clinical leaders with a contextual understanding of their supportive role in the SE of teams.

Originality/value

This study demonstrates the value of understanding SE of individual team members and how it can contribute to overall improved team outcomes, directly impacting the sustainability of Lean change culture and its promotion of improved patient safety, cost efficiencies and access to care.

Abstract

Purpose

The current research elucidates the role of empathy in design of artificial intelligence (AI) systems in healthcare context, through a structured literature review, analysis and synthesis of academic literature published between 1990 and 2024.

Design/methodology/approach

This study aims to advance the domain of empathy in AI by adopting theory constructs context method approach using the PRISMA 2020 framework.

Findings

The study presents a current state-of-the-art literature to review the connections between empathy and AI and identifying four clusters showing the emerging trajectories in the field of AI and empathy in healthcare setting.

Originality/value

Despite a rise in empirical research, the potential pathways enhancing AI accountability by incorporation of empathy is unclear. The research aims to contribute to the existing literature on AI and empathy in the healthcare sector by carving out four distinct clusters depicting the future research avenues.

Abstract

Purpose

We propose an alternative rationale for why some firms employ veterans, driven not solely by benevolence but also by the prospect of enhanced outcomes. Financially, hiring veterans could correlate with improved stock market performance for the hiring company while aligning with corporate social responsibility (CSR) initiatives. Our study centers on the stock market performance of companies hiring veterans. It aims to underscore a lesser-known facet of the veteran employment discourse and its connection to the hiring firm's financial performance.

Design/methodology/approach

This paper evaluates the stock market performance of three VETS portfolios (made of companies that hire veterans) compared to the benchmark SPDR S&P 500 ETF. Using a modular approach, we create three VETS passive indices: VETSEW (equal-weighted index), VETSPW (price-weighted index) and VETSVW (value-weighted index). The study analyzes the annual returns, portfolio allocations, risk-adjusted performance metrics and style analysis of the portfolios from January 1, 2020, to December 31, 2022.

Findings

The findings indicate that all three VETS portfolios outperformed the benchmark, with higher ending balances and superior risk-adjusted ratios such as the Sharpe and Sortino ratios. Notably, the portfolios demonstrated resilience during challenging periods, including the COVID-19 pandemic, subsequent recovery and an inflationary period.

Research limitations/implications

Limitations include the paper's focus solely on stock returns, suggesting a need for broader financial and management ratios. Moreover, a deeper exploration into how veterans contribute during turbulent times is suggested for further investigation. Although the study touches upon the financial performance of veteran-focused companies during challenging economic times, it does not extensively delve into the specific ways in which veterans add value under such circumstances, presenting an opportunity for further exploration.

Practical implications

Firms that employ veterans amid the COVID-19 pandemic demonstrate favorable risk-adjusted returns, underscoring the potential of veterans as valuable crisis-time assets. Our research further underscores the correlation between veteran hiring and enhanced financial prowess. These insights carry significant policy implications, including CSR initiatives for hiring veterans, skill translation and training and collaboration with veteran organizations.

Social implications

The paper's findings suggest significant implications: (1) Policymakers could incentivize firms to hire veterans through tax benefits or grants, leveraging their skills for organizational resilience. (2) Collaborative efforts between policymakers and firms can promote responsible hiring, boosting a company's reputation through diversity and inclusion, positively impacting society. (3) Support for skill translation from military to civilian jobs is crucial. Programs certifying skills and tailored education aid veterans' successful transition into the workforce. (4) Collaborations between policymakers, veteran organizations and private sector entities can create networks, job placements and support systems for veterans' employment.

Originality/value

Numerous prior studies within the domain of corporate social responsibility have predominantly neglected the contributions veterans offer to businesses and the underlying reasons behind firms' decisions to employ them. Our research uniquely concentrates on the stock market performance of companies that choose to hire veterans.

Abstract

Purpose

Experts may adjust their assessments through communication and mutual influence, and this dynamic evolution relies on the spread of internal trust relationships. Due to differences in educational backgrounds and knowledge experiences, trust relationships among experts are often incomplete. To address such issues and reduce decision biases, this paper proposes a probabilistic linguistic multi-attribute group decision consensus model based on an incomplete social trust network (InSTN).

Design/methodology/approach

In this paper, we first define the new trust propagation operators based on the operations of Probability Language Term Set (PLTS) with algebraic t-conorm and t-norm, which are combined with trust aggregation operators to estimate InSTN. The adjustment coefficients are then determined through trust relations to quantify their impact on expert evaluation. Finally, the particle swarm algorithm (PSO) is used to optimize the expert evaluation to meet the consensus threshold.

Findings

This study demonstrates the feasibility of the method through the selection of treatment plans for complex cases. The proposed consensus model exhibits greater robustness and effectiveness compared to traditional methods, mainly due to the effective regulation of trust relations in the decision-making process, which reduces decision bias and inconsistencies.

Originality/value

This paper introduces a novel probabilistic linguistic multi-attribute swarm decision consensus model based on an InSTN. It proposes a redefined trust propagation and aggregation approach to estimate the InSTN. Moreover, the computational efficiency and decision consensus accuracy of the proposed model are enhanced by using PSO optimization.

Abstract

Purpose

Notwithstanding their potential benefits especially for individuals with low health literacy, users are still somewhat skeptical about the reliability of healthcare chatbots. The present study aims to address this challenge by investigating strategies to enhance users’ cognitive and emotional trust in healthcare chatbots. Particularly, this study aims to understand the effects of chatbot design cues in increasing trust and future chatbot use intention for low health literacy users.

Design/methodology/approach

We conducted two experimental studies with a final sample of 327 (Study 1) and 241 (Study 2). Three different chatbots were developed (Chatbot design: Bot vs Male-doctor vs Female-doctor). Participants were asked to have a medical consultation with the chatbot. Participants self-reported their health literacy scores. The PROCESS model 7 was used to analyze the hypotheses.

Findings

The results showed that the female-doctor cues elicited greater cognitive and emotional trust, whereas the male-doctor cues only led to greater cognitive trust (vs bot-like cues). Importantly, this study found that users’ health literacy is a significant moderating factor in shaping cognitive and emotional trust. The results indicated that both the female and male-doctor cues’ positive effects on cognitive trust were significant for those with lower levels of health literacy. Furthermore, the positive effect of the female-doctor cues on emotional trust was also significant only for those whose health literacy level was low. The increased cognitive and emotional trust led to greater future intention to use the chatbot, confirming significant moderated mediation effects.

Originality/value

Despite the strong economic and educational benefits of healthcare chatbots for low health literacy users, studies examining how healthcare chatbot design cues affect low health literate users surprisingly remained scarce. The results of this study suggest that healthcare chatbots can be a promising technological intervention to narrow the health literacy gap when aligned with appropriate design cues.

Abstract

Purpose

Depressive symptoms are a risk factor for loneliness, while sources of perceived social support from family and friends are protective factors; however, the complex mechanisms behind these factors have not been examined among health-care employees in Portugal. The purpose of this study is to evaluate the indirect effect of perceived social support from family and friends on the association between depressive symptoms and loneliness.

Design/methodology/approach

Health-care employees (n = 279; 242 female and 37 male) from 12 health-care institutions in Portugal participated in this cross-sectional study. The SPSS program with PROCESS macro (Model 6) was used to test the hypothesis regarding the indirect effect analysis.

Findings

Bootstrap analysis found that family as a source of social support had an indirect effect in the relationship between depressive symptoms and loneliness. Similarly, depressive symptoms had a significant indirect effect on loneliness through the indirect role of social support from friends. Moreover, it was found that the relationship of family and friends as sources of social support mediated the association of the aforementioned variables.

Originality/value

The findings of this study underscore the important role of depressive symptoms on perceived social support from family and friends, leading to loneliness, which is a public health concern among health-care employees in Portugal.

Abstract

Purpose

This paper aims to strengthen the connection between therapeutic built environments and tourism research and practice. While there is evidence in the importance of the Built Environment (BE) of cities, workspaces and health-care facilities to health, the BE of facilities for tourism in relation to health remains relatively unexplored.

Design/methodology/approach

The paper conducts an exploratory search on architecture and tourism BE and narrowed it down to a scoping review on wellness tourism and architectural health impacts from 2010 to 2024. This would highlight lessons learned from the field of medical architecture, i.e. a cross-disciplinary field combining BE research, public health and health-care services research, to explore potential synergies of cross-pollination with the field of hospitality and medical architecture.

Findings

Principles and theories of medical architecture can be incorporated into the BE of wellness hospitality, tourism for ageing and pandemic preparedness.

Originality/value

The paper sets the basis of a novel cross-disciplinary collaboration between therapeutic architecture and hospitality for increasing the societal impact of the latter. This is particularly important in a post-Covid and an ageing society.

Objetivo

Este documento pretende reforzar la conexión entre los entornos construidos terapéuticos y la investigación y la práctica del turismo. Aunque existen pruebas de la importancia del entorno construido (EC) de las ciudades, los espacios de trabajo y las instalaciones sanitarias para la salud, el EC de las instalaciones para el turismo en relación con la salud sigue estando relativamente inexplorado.

Diseño/metodología/enfoque

Realizamos una búsqueda exploratoria sobre arquitectura y el EC turístico y la acotamos a una revisión de alcance sobre el turismo de bienestar y los impactos arquitectónicos en la salud desde 2010 hasta 2024. Esto pondría de relieve las lecciones aprendidas en el campo de la arquitectura médica, es decir, un campo interdisciplinar que combina la investigación de la EC, la salud pública y la investigación de los servicios sanitarios, para explorar posibles sinergias de polinización cruzada con el campo de la hostelería y la arquitectura médica.

Resultados

Los principios y teorías de la arquitectura médica pueden incorporarse a la EC de la hosteleria para el bienestar, el turismo para el envejecimiento y la preparación ante pandemias.

Originalidad/valor

Sentamos las bases de una novedosa colaboración interdisciplinar entre la arquitectura terapéutica y la hostelería para aumentar el impacto social de esta última. Esto es especialmente importante después de la crisis y en una sociedad que envejece.

Abstract

Purpose

The purpose of this study focused on a global longitudinal bibliometric mapping of research in the field of health biotechnology between 1990 and 2023 to determine who is leading this field of knowledge and to estimate the sub-disciplines that are emerging and project those that will prevail in the future.

Design/methodology/approach

The study identified the most relevant countries, institutions and researchers, as well as the type of scientific collaborations. The applied steps applied in the study were the following: identification and selection of keyword terms by a panel of experts; design and application of an algorithm to identify these selected keywords in titles, abstracts and keywords using Web of Science terms to contrast them; performance of JCR data processing during 2023 using R, Python and VOSviewer.

Findings

Among the most relevant conclusions of the study are the following exponential growth has been observed in the study period; new branches of knowledge have emerged in which the subjects have been acquiring their own autonomous capabilities; and R&D in this field is still concentrated in a small group of core countries, and the trend is for it to remain so due to the capacity needs required.

Originality/value

This contribution seeks to systematize the existing scientific knowledge in the field of biotechnology, specifically in the area of health, using the technique of scientific mapping based on a logical model of indicators that aims to determine potential thematic ramifications.
