Abstract

Purpose

Pre-pandemic research demonstrated the challenges of the nursing workforce and the provision of quality of patient care. Such challenges have been significantly intensified during the COVID-19 pandemic, not least in the workplace and fear of staff catching and transmitting COVID-19. We draw on conservation of resources (COR) theory to examine the impact of the fear of COVID-19 on nurses and the role of well-being-HRM (WBHRM) in negating the fear of COVID-19 and its impact on job stress and perceived quality of patient care.

Design/methodology/approach

We collected data from 260 nurses (treating COVID-19 patients) employed in US hospitals across two-waves. Data were analyzed using mediated regression and moderated mediation.

Findings

The results indicated that when nurses report higher levels of fear of COVID-19, this translates into higher levels of nursing job stress. This, in turn, reduces nurses’ perceptions of quality of patient care they can provide. As previous research has found, decreased perceptions of quality of patient care is a significant factor driving intentions to leave the profession. The results demonstrated that WBHRM practices buffer the negative impact of fear of COVID-19 on job stress, and in turn, the perceived quality of patient care.

Originality/value

Our paper contributes to new knowledge for healthcare managers on WBHRM bundles and their efficacy in buffering the effects of fear on job stress and quality of patient care. We contribute new knowledge on fear at work and how to manage employees’ fear through WBHRM practices.

Abstract

Purpose

Emotional intelligence (EI) and authentic leadership are essential attributes for effective healthcare management. Understanding their interplay can inform leadership development and improve patient care outcomes. This study aims to determine the relationship between EI and authentic leadership among nurse managers.

Design/methodology/approach

Study conducted a cross-sectional study, collecting data from 198 nurse managers using surveys assessing EI and authentic leadership. Correlation and regression analyses were used to explore the relationship and impact of EI on authentic leadership. STROBE checklist used to report the study.

Findings

The findings revealed a positive and significant relationship between nurse managers’ EI and their authentic leadership characteristics. Higher EI scores were associated with increased authenticity in leadership. EI, particularly interpersonal EI, plays a crucial role in authentic leadership. Enhancing nurse managers’ EI can foster more authentic leadership, positively impacting healthcare settings.

Originality/value

This research underscores the importance of EI in leadership development and its potential for improving patient care and organizational outcomes.

Abstract

Purpose

This paper aims to assess the influence of nursing leadership style on error management culture (EMC).

Design/methodology/approach

This scoping review was conducted following the integrative review methodology of the Joanna Briggs Institute (JBI) and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). PubMed, CINAHL, Scopus, Web of Science, Embase and EBSCO databases were systematically searched to identify studies on nursing leadership, error management and measurement, and error management culture. The studies’ methodological quality was then assessed using the JBI Critical Appraisal Checklist for Analytical Cross-Sectional Studies.

Findings

Thirteen manuscripts were included for review. The analysis confirmed that nursing leadership plays an important role in EMC and nurses’ intention to report errors. Three emerging themes were identified: 1) leadership and EMC; 2) leadership and the intention to report errors; and 3) leadership and error rate.

Research limitations/implications

A major limitation of the studies is that errors are often analyzed in a transversal way and associated with patient safety, and not as a single concept.

Practical implications

Healthcare managers should promote training dedicated to head nurses and their leadership style, for creating a good work environment in which nurses feel free and empowered to report errors, learn from them and prevent their reoccurrence in the future.

Originality/value

There is a positive relationship between nursing leadership and error management in terms of reduced errors and increased benefits. Positive nursing leadership leads to improvements in the caring quality.

Abstract

Purpose

Nurse turnover in critical care units (CCU) significantly affects patient outcomes and health systems worldwide. To safeguard patient care quality, hospitals must address the underlying reasons for turnover and strategize to retain their skilled nursing workforce. The study proposes a prescriptive framework to reduce nurse turnover in CCUs.

Design/methodology/approach

In this study, the integrated methodology of Delphi-AHP-Entropy was used for the comparative prioritization of factors and subfactors that influence nursing staff turnover in CCUs.

Findings

Study findings reveal that “Organizational factors” and “Individual factors” dictate critical care nurse attrition rate. At the subfactor level, staffing policy, chronic fatigue, and perceived career are the leading concerns for the decision of nurses whether to work or leave.

Research limitations/implications

This study is valuable for both researchers and healthcare professionals. It examines whether actions related to nurse retention align with existing theory and identifies areas requiring further theoretical or applied studies to enhance understanding in this area. This insight can bolster the field’s knowledge base and integrate theoretical and applied knowledge effectively. Additionally, for healthcare professionals, the study provides an overview of key factors conducive to retaining nursing staff in the CCU, offering valuable guidance for implementing effective strategies.

Originality/value

This study uniquely positions itself by presenting a comprehensive and prescriptive framework for critical care nurse retention in the UAE.

Abstract

Purpose

Nurse turnover is perceived to be a major challenge for health-care administrators. The turnover is because of various factors like poor working conditions, low salaries, poor work–life balance and international opportunities. The purpose of conducting a bibliometric analysis on “Nurses’ Talent Retention” is to comprehensively analyze the trends, patterns and research contributions related to this area. By synthesizing and analyzing the existing literature, this study aims to identify key areas for nurse retention, thereby providing a foundation for further research, practical interventions and policy implications.

Design/methodology/approach

This research investigates talent retention practices in nursing from 1989 to 2023 by analyzing 427 documents from 213 sources sourced through Elsevier Scopus. Using the PRISMA framework, the study uses bibliometric analysis using the R studio’s biblometrix package. Key findings reveal a growing scholarly interest with an annual growth rate of 11.46%, an average document age of 7.93 years and an average citation rate of 43.26. Notably, international collaboration represents 17.56% of co-authorships, indicating the global relevance of the research. The study addresses three main research questions, including factors contributing to nurse retention in the Indian context, and implications for nurse well-being and turnover rates across different health-care contexts and countries.

Findings

The bibliometric analysis of nurse retention highlights several key findings. Magnet hospitals, accredited by the ANCC, demonstrate superior work environments, staffing levels and nurse outcomes, emphasizing the significance of nursing leadership and professionalism. The factors such as resilience, influenced by perceived competence, collaboration and coping strategies, highlight the importance of addressing psychological well-being in retention efforts. Transformational leadership and supportive work environments are associated with increased intentions for nurses to remain in their roles, suggesting the need for organizational practices fostering positive workplace cultures. Diversity management initiatives promoting inclusivity correlate with positive organizational outcomes, indicating the potential for diverse and inclusive workplaces to enhance nurse retention.

Practical implications

These findings provide substantial implications for health-care organizations and policymakers. Investing in nursing leadership and nurturing supportive work environments can enhance nurse satisfaction and retention. Implementing diversity management in recruitment practices can create inclusive workplaces, positively impacting nurse retention and organizational outcomes. Mandating staffing ratios may also improve nurse retention and patient care quality. Understanding the economic and organizational costs associated with nurse turnover emphasizes the need for effective retention strategies and healthy work environments.

Originality/value

This study contributes to the understanding of nurse retention dynamics by synthesizing findings from the literature. By highlighting the importance of nursing leadership, resilience and diversity management, it provides valuable insights for future research and practice in the field. The study also emphasizes the economic and organizational costs of nurse turnover, enhancing the urgency of implementing effective retention strategies.

Purpose

To map nurses’ actions performed during the care transitions from hospital to home of Covid-19 patients.

Design/methodology/approach

A scoping review based on the Joanna Briggs Institute guidelines was carried out. We searched in seven databases: PubMed/MEDLINE, BDENF, LILACS, SciELO, Embase, Scopus, Web of Science and Google Scholar. A two-step screening process and data extraction was performed independently by two reviewers. The findings were summarized and analyzed using a content analysis technique.

Findings

Of the total 5,618 studies screened, 21 were included. The analysis revealed nurses’ actions before and after patient’ discharge, sometimes planned and developed with the interprofessional team. The nurses’ actions included to plan and support patients’ discharge, to adapt the care plan, to use screening tools and monitor patients’ clinical status and needs, to provide health orientation to patients and caregivers, home care and face-to-face visiting, to communicate with patients, caregivers and other health professionals with phone calls and virtual tools, to provide rehabilitation procedures, to make referrals and to orient patients and families to navigate in the health system.

Practical implications

The results provide a broader understanding of the actions taken and challenges faced by nurses to ensure a safe care transition for Covid-19 patients from hospital to home. The interprofessional integration to discharge planning and the clinical nursing leadership in post-discharge monitoring were highlighted.

Originality/value

The nurses’ actions for Covid-19 patients performed during care transitions focused on coordination and discharge planning tailored to the needs of patients and caregivers at the home setting. Nurses monitored patients, with an emphasis on providing guidance and checking clinical status using telehealth tools.

Abstract

Purpose

The objectives of the proposed model are: aiding nursing staff in documentation tasks, which can be onerous and stressful; and helping management by offering an estimate of the nursing workload, which can be considered for administrative purposes, such as staff scheduling.

Design/methodology/approach

An exploratory-descriptive study was conducted in order to identify, investigate, and describe the problem of documenting nursing activities and workload estimation in an intensive care unit. Technological solutions were explored, and models were proposed to address these issues.

Findings

Cross-dataset experiments were performed, and the model was able to offer an adequate estimate of the nursing workload. The results suggest that continuous retraining is essential for maintaining high accuracy. While the proposed model was considered in the context of an adult ICU, it can be adapted to other contexts, such as elderly care.

Research limitations/implications

While the proposed solution seems promising, further research is required, such as deploying this system in an ICU and facing challenges in the areas of computer security, medical ethics, and patient data privacy. More patients’ variables could also be collected to improve the workload estimates.

Originality/value

Nursing workload assessment is critical to improve the cost-benefit ratio in health care, offer high-quality patient care, and reduce unnecessary expenses, and this process is usually manual. An automated device can automatically document the amount of time spent in patient care activities in a more transparent, efficient, and accurate manner, freeing staff for more urgent activities and keeping management better informed about day-to-day nursing operations.

Abstract

Purpose

Previous research has demonstrated that social-relational factors are instrumental to employee voice. An essential aspect of this relates to notions of respect or disrespect. Although nurses commonly report experiencing professional disrespect in their interaction with doctors, earlier studies have focused on how the professional status hierarchy and power imbalance between doctors and nurses hinder speaking up without considering the role of professional disrespect. Addressing this gap, we explore how professional disrespect in the doctor–nurse relationship in surgical teams influences the willingness of nurses to voice legitimate concerns about threats to patient safety.

Design/methodology/approach

Fifty-seven semi-structured interviews with nurses drawn from a range of specialities, ranks and surgical teams in three hospitals in a West African Country. In addition, two interviews with senior representatives from the National Registered Nurses and Midwifery Association (NRNMA) of the country were undertaken and analysed thematically with the aid of NVivo.

Findings

Disrespect is expressed in doctors’ condescending attitude towards nurses and under-valuing their contribution to care. This leads to safety concerns raised by nurses being ignored, downplayed or dismissed, with deleterious consequences for patient safety. Feeling disrespected further motivates nurses to consciously disguise silence amidst speech and engage in punitive silence aimed at making clinical practice difficult for doctors.

Originality/value

We draw attention to the detrimental effect of professional disrespect on patient safety in surgical environments. We contribute to employee voice and silence by showing how professional disrespect affects voice independently of hierarchy and conceptualise the notion of punitive silence.

Abstract

Purpose

In emergency departments, effective communication is of utmost importance to ensure the safety of patients. However, communicating can be quite challenging when dealing with high-stress situations. This study aims to assess the efficacy of coaching workshops, informed by a transtheoretical coaching model, in managing communication challenges perceived by emergency nurses and enhancing their communication skills.

Design/methodology/approach

The study involved seven emergency room nurses working at a public hospital in Morocco. The data were gathered through various instruments, including observation grids, interviews and pre- and post-test questionnaires.

Findings

The study identified prevalent challenges in communication among nurses, notably difficulties in accurately interpreting messages when faced with confrontational attitudes from colleagues or superiors. Additionally, some nurses exhibited asymmetrical communication patterns, prioritizing their own perspectives over others' during interactions. The findings revealed a statistically significant disparity between pre- and post-test scores (P = 0.017). The nurses’ mean score has improved by 5.14 after attending the four workshop coaching experience, passing from 5.71 in the pre-test to 10.85 in the post-test.

Originality/value

This is the first study in Morocco to evaluate the effectiveness of coaching workshops guided by a transtheoretical coaching model in improving communication skills and overcoming communication barriers among working emergency nurses.

Abstract

Purpose

This study aims to analyse the role of work–life balance (WLB) experiences and job satisfaction on turnover intentions (TI) among nurses working in private sector hospitals.

Design/methodology/approach

The research followed the analytical research design with a self-administered questionnaire survey using a five-point Likert scale. Responses from 386 nurses working in different positions in private sector hospitals were collected. The collected data were examined using descriptive and inferential statistics using structural equation modelling. Data validation, path coefficient analysis and a mediation effect test were conducted using Smart PLS 4 with a 5% significance level. WLB was examined with three dimensions: work interference with personal life, personal life interference with work and work–personal life enhancement.

Findings

The study established a significant relationship between personal life interference with work and work–personal life enhancement with job satisfaction. Also, the result revealed a significant negative relationship between interferences of WLB and TI. The study also established a partial and full mediation of job satisfaction about two WLB dimensions with TI.

Originality/value

This research suggests emphasizing WLB and job satisfaction to discourage TI. This research can be used by managers and policymakers alike to improve the scenario and take measures accordingly. This study also provides theoretical implications based on the boundary theory.
