Abstract

Purpose

Nurses work in a wide range of roles and settings, each with inherent stressors that can impact mental well-being. High levels of stress, burnout and clinical errors have been linked to the levels of support nurses have in their workplaces. Both consumers and health organisations benefit when nurses can engage in their work with support. Digital applications offer a promising option for supporting the mental well-being of nurses. This paper aims to provide analysis and synthesis of studies from the emerging field of research investigating the potential of digital applications to support the mental well-being of nurses.

Design/methodology/approach

An integrative literature review was conducted focused on studies published within the last ten years that employed digital applications specifically included to support nurses’ mental well-being. As part of the review inductive thematic analysis was then undertaken identifying four main themes diversity of digital applications; wide applicability across nursing; differing dimensions of well-being; and challenges to integration.

Findings

The study of digital applications used among nurses is a modern emerging field of research. Digital applications have considerable design variability and are being rolled out across multiple nursing specialities. They target differing aspects of mental well-being, offering promise despite the challenges to integration.

Originality/value

Digital applications are an emergent and promising option for supporting nurses' mental well-being. They have the potential to be personalised, cost-efficient and engaging platforms. The fast-paced technological advancement of digital applications will lead to the adoption of novel means of supporting nurses.

Abstract

Purpose

Healthcare organizations have been facing challenges due to high costs and low efficiency in health services. The growth of costs and losses caused by avoidable mistakes lead to the search for solutions, and Health Lean Management appears as a potential solution to help in solving service quality problems, as well as reducing risks. This study aims to analyse the state of the art in the literature centred on the Lean approach in the context of risk management in healthcare organizations, and to identify new research opportunities, highlighting possible lines of future research.

Design/methodology/approach

Following a systematic literature review approach, 51 papers were considered relevant for this research, and reviewed to explore the development of literature in this area.

Findings

Based on the results, five main research streams were identified: (1) risk management oriented towards patient safety; (2) risk management oriented towards employee safety; (3) importance of attitudes and behaviours in risk reduction projects; (4) Lean tools used in healthcare risk management and (5) Integration of approaches. Moreover, several shortcomings were identified in literature.

Originality/value

Identified shortcomings represent significant opportunities for further research development.

Abstract

Purpose

Interventions that improve timely access to outpatient health services are essential in managing demand. This process evaluation aimed to describe the implementation, mechanism of impact and context of an intervention to reduce waiting for first appointments in an outpatient epilepsy clinic.

Design/methodology/approach

The UK Medical Research Council framework was used as the theoretical basis for a process evaluation alongside an intervention trial. The intervention, Specific Timely Appointments for Triage (STAT), is a data-driven approach that combines a one-off backlog reduction strategy with methods to balance supply and demand. A mixed methods process evaluation synthesised routinely collected quantitative and qualitative data, which were mapped to the domains of implementation, mechanisms of impact and contextual elements.

Findings

The principles of the STAT model were implemented as intended without adaptation. The STAT model reached all patients referred, including long waiters and was likely generalisable to other medical outpatient clinics. Mechanisms of impact were increased clinic capacity and elimination of unwanted variation. Contextual elements included the complexity of healthcare systems and the two-tier triage practice that contributes to prolonged waiting for patients classified as non-urgent.

Originality/value

This process evaluation shows how a data-driven strategy was applied in a medical outpatient setting to manage demand. Improving patient flow by reducing waiting in non-urgent, outpatient care is a complex problem. Understanding how and why interventions work is important for improved timeliness of care, and sustainability of public health services.

Abstract

Purpose

In most Sub-Saharan African countries, a widely adopted policy to increase access to healthcare is the complete or partial removal of health user fees. This paper examines the effect of removing health user fees on the cognitive development of children in Ghana.

Design/methodology/approach

The study uses the Ghana Socioeconomic Panel Survey (GSPS) and a difference-in-differences method to analyze the impacts of free healthcare.

Findings

The findings show that improved access to healthcare enhances the cognitive ability of children. More importantly, children who received free healthcare in early life experienced significant improvement in their cognitive ability. The benefits are also huge for girls and children from low-income families. The findings further show that maternal health, the timely receipt of vaccinations and regular infant health visits are important operative channels through which improved access to healthcare affects children’s cognitive ability.

Originality/value

There are several studies on the effects of eliminating health user fees on various outcomes, but only a few focus on children’s outcomes. This study contributes to the literature by focusing on a vital child outcome, cognitive ability and exploring the timing and pathways through which abolishing health user fees affect cognitive development in children. Because cognitive ability is crucial for educational and labor market outcomes, the findings are useful for policymakers in determining the population to target and the timing of interventions that yield huge impacts when designing health intervention programs in developing countries.

The chapter is focused on the multifaceted challenges and concerted efforts aimed at improving health, within the context of SDG 3, which aims at ensuring healthy lives and promoting well-being for all. It explores the intricate interconnections among physical, mental, and social well-being, acknowledging their reciprocal influence on overall health. Focusing on Africa, a particularly vulnerable region, the chapter sheds light on the complex landscape of healthcare, where governments, non-governmental organizations, professionals, institutions, and communities play pivotal roles. The African continent, though marked by progress, still lags behind in terms of healthcare performance. Several barriers, including limited financial resources, high treatment costs, fragile healthcare systems, and cultural resistance exist. Starting from the analysis, the chapter concludes with a comprehensive picture of the healthcare landscape in Africa, navigating challenges through collaborative efforts, innovative strategies, and tailored interventions. It underscores the imperative of culturally sensitive approaches, community involvement, and international partnerships in realizing the SDG 3 and ultimately achieving improved health and well-being for all African populations.

Abstract

Purpose

Technology-enabled healthcare focuses on providing better information flow and coordination in healthcare operations. Technology-enabled health services enable hospitals to manage their resources effectively, maintain continuous patient engagement and provide seamless services without compromising their perceived quality.

Design/methodology/approach

This study investigates the role of technology-enabled health services in improving perceived healthcare quality among patients. Data are collected from the users (n = 418) of health platforms offered in multi-specialty hospitals. Multiple learners are employed to accurately represent the users' perceived quality regarding the perceived usefulness of the features provided via these digital health platforms.

Findings

The best-fitted model using a decision tree classifier (accuracy = 0.86) derives the accurate significance of features offered in the digital health platform in fostering perceived healthcare quality. Diet and lifestyle recommendations (30%) and chatting with health professionals (11%) are the top features offered in digital health platforms that primarily influence the perceived quality of healthcare among users.

Practical implications

The predictability of perceived quality with the individual features existing in the digital health platform, the significance of the features on the perceived healthcare quality and the prediction rules showing the combined effect of features on healthcare quality can help healthcare managers accelerate digital transformation in hospitals by improving their digital health platform, designing and offering new health packages while strengthening their e-infrastructure.

Originality/value

The study represents perceived healthcare quality with the features offered in digital health platforms using machine learners based on users' post-pandemic experience. By advancing digital platforms with more patient-centric features using emerging technologies, this model can further foresee its impact on the perceived quality of healthcare, offering valuable directions to healthcare service providers. The study is limited to focusing on digital health platforms that can deal with people's general healthcare needs.

Abstract

Purpose

This work aims to integrate the concepts generated by a systematic literature review on patient flows in emergency departments (ED) to serve as a basis for developing a generic process model for ED.

Design/methodology/approach

A systematic literature review was conducted using PRISMA guidelines, considering Lean Healthcare interventions describing ED patients’ flows. The initial search found 141 articles and 18 were included in the systematic analysis. The literature analysis served as the basis for developing a generic process model for ED.

Findings

ED processes have been represented using different notations, such as value stream mapping and workflows. The main alternatives for starting events are arrival by ambulance or walk-in. The Manchester Triage Scale (MTS) was the most common protocol referred to in the literature. The most common end events are admission to a hospital, transfer to other facilities or admission to an ambulatory care system. The literature analysis allowed the development of a generic process model for emergency departments. Nevertheless, considering that several factors influence the process of an emergency department, such as pathologies, infrastructure, available teams and local regulations, modelling alternatives and challenges in each step of the process should be analysed according to the local context.

Originality/value

A generic business process model was developed using BPMN that can be used by practitioners and researchers to reduce the effort in the initial stages of design or improvement projects. Moreover, it’s a first step toward the development of generalizable and replicable solutions for emergency departments.

Purpose

India has been on the pathway of improvement concerning healthcare and health outcomes of its population. However, India must overcome its unique challenges and cover a long journey ahead. This mandates a need for a high-quality, contemporary and community-based health system that promises consistent and quality healthcare, is trusted and valued by all its citizens, considers the changing population needs and should be affordable and accessible.

Design/methodology/approach

The study examines various dimensions and elements associated with the integrated healthcare system in India and uses input, process and output structural measures.

Findings

The present paper proposes an integrated, comprehensive healthcare system in India that endorses participation from diverse stakeholders such as the government, organizations, the community and individuals who can contribute uniquely. It also focuses on defined and measurable output that can make health a topic of social movement or “Jan Andolan” and create a sustainable and integrated care system.

Originality/value

The study is unique as it focuses on the role of stakeholders in health care. The research emphasized the involvement of the government, community, people and organizations in developing an integrated healthcare ecosystem that includes modern technology, skilled employees, enough finance, governance, efficient delivery platforms and top-tier infrastructure. The model’s output is focused on healthcare that is inexpensive, accessible, available, accountable and user-centered. This would gradually improve everyone’s health and well-being.

Abstract

Purpose

This survey explores the application of real options theory to the field of health economics. The integration of options theory offers a valuable framework to address these challenges, providing insights into healthcare investments, policy analysis and patient care pathways.

Design/methodology/approach

This research employs the real options theory, a financial concept, to delve into health economics challenges. Through a systematic approach, three distinct models rooted in this theory are crafted and analyzed. Firstly, the study examines the value of investing in emerging health technology, factoring in future advantages, associated costs and unpredictability. The second model is patient-centric, evaluating the choice between immediate treatment switch and waiting for more clarity, while also weighing the associated risks. Lastly, the research assesses pandemic-related government policies, emphasizing the importance of delaying decisions in the face of uncertainties, thereby promoting data-driven policymaking.

Findings

Three different real options models are presented in this study to illustrate their applicability and value in aiding decision-makers. (1) The first evaluates investments in new technology, analyzing future benefits, discount rates and benefit volatility to determine investment value. (2) In the second model, a patient has the option of switching treatments now or waiting for more information before optimally switching treatments. However, waiting has its risks, such as disease progression. By modeling the potential benefits and risks of both options, and factoring in the time value, this model aids doctors and patients in making informed decisions based on a quantified assessment of potential outcomes. (3) The third model concerns pandemic policy: governments can end or prolong lockdowns. While awaiting more data on the virus might lead to economic and societal strain, the model emphasizes the economic value of deferring decisions under uncertainty.

Practical implications

This research provides a quantified perspective on various decisions in healthcare, from investments in new technology to treatment choices for patients to government decisions regarding pandemics. By applying real options theory, stakeholders can make more evidence-driven decisions.

Social implications

Decisions about patient care pathways and pandemic policies have direct societal implications. For instance, choices regarding the prolongation or ending of lockdowns can lead to economic and societal strain.

Originality/value

The originality of this study lies in its application of real options theory, a concept from finance, to the realm of health economics, offering novel insights and analytical tools for decision-makers in the healthcare sector.

Abstract

Purpose

The purpose of this study is to test a model of healthcare professionals’ well-being seen as a consequence of a process of motivated professional identity construction, a variable that mediates the influence of the organizational identity (utilitarian or normative) and the perceived reputation of the profession on well-being.

Design/methodology/approach

Cross-sectional design, based on a survey of 384 healthcare professionals. Structural equation modeling with latent variables was used to test the model.

Findings

The data provide empirical evidence supporting the proposed model. We find that organizational identity (utilitarian and normative) and perceived professional reputation positively relate to professional identity, a variable that positively relates to well-being. Professional identity mediates the relationship between organizational identity (normative and utilitarian) and perceived professional reputation and well-being. Utilitarian organizational identity and perceived professional reputation are also directly related to well-being.

Originality/value

This research significantly departs from the current focus of explaining the well-being of healthcare professionals by resorting mainly to individual factors and introduces organizational and institutional determinants.
