Abstract

The aim of this study was to provide a pilot evaluation of novel, brief formulation development workshops with direct care staff supporting people with intellectual disabilities who display significant psychosocial difficulties. A series of workshops were designed and delivered to a staff team supporting an individual who had been referred to specialist intellectual disabilities health services. The workshops used a psychosocial framework to facilitate development with care staff of a case formulation for the individual they were supporting. Following the workshops, there were decreases in problematic behaviours displayed by the individual and in the staff team's perception of the severity of these behaviours. The staff team felt that the workshops had had a beneficial impact on their practice. The pilot indicated that the workshops were feasible, positively received and associated with changes in the psychosocial difficulties displayed by the individuals staff were supporting.

Abstract

Purpose

The purpose of this study is to evaluate a conflict management training that used a communication competence perspective. This addresses whether the training had an impact on role conflict, conflict resolution skills, horizontal violence, burnout, turnover intention and perceptions of consumers. It also assessed staff perceptions of the training.

Design/methodology/approach

A mixed-method analysis was used using survey data from multiple time points along with focus group interviews.

Findings

The program decreased role conflict, horizontal violence and burnout among direct-care workers, whereas feelings of safety and perceptions of workers’ ability to protect themselves and others in aggressive situations increased. Furthermore, staff felt the training was useful and increased feelings of safety and empowerment at the study.

Practical implications

These findings suggest that conflict management training may need additional refresher sessions. Administrative planning is also needed to ensure training of all staff is trained in an adequate timeframe.

Originality/value

These results, although positive, are somewhat time bound. Therefore, the content of training and knowledge dissemination of conflict management training need additional research to ensure best practices.

Abstract

Telephone interviews were conducted with 64 participants who attended an initial systemic consultation meeting within a community‐based psychology service for people with a learning disability. Most participants found the sessions helpful and liked the reflecting conversations, which they said broadened their views on the presenting problems and offered a positive approach to the meeting, as well as enabling as many views as possible to be heard. However, depending on whether they were carers, colleagues or client and family members, participants' views varied on how prepared and comfortable they felt about the format of the meetings. The authors felt that the results lend support to the overall helpfulness of using an initial systemic consultation meeting when working with this client group.

Abstract

Purpose

The majority of adults with intellectual disabilities (ID) are prescribed at least one, if not multiple medications, with psychotropic medications being the most commonly prescribed. Direct care staff play an important role in psychotropic medication administration and monitoring, yet little is known about their knowledge and comfort with medication. The paper aims to discuss this issue.

Design/methodology/approach

A 15-item survey, focusing on self-reported knowledge and comfort with psychotropic medication, was completed by 152 direct care staff employed at three agencies providing residential services for individuals with ID across Ontario.

Findings

In total, 62 per cent of staff respondents reported that psychotropic medications were among the top medications regularly taken by the individuals they support, with behaviour listed as the most commonly reported reason for taking this medication. The majority of staff reported monitoring medication, however, the frequency of monitoring varied considerably. Generally, staff reported feeling comfortable and knowledgeable about medication use, but, most reported a desire for additional medication training.

Originality/value

This is the first Canadian study to examine staff knowledge and comfort regarding medication use, and the first study to assess PRN (“as needed”) as well as regularly administered medications.

Abstract

Purpose

The paper reports on the implementation of the Revised Elderly Persons' Disability Scale use – a 53‐item, seven sub‐scale functional assessment tool – in a range of older adult care institutions. Staff education on the instrument's use is provided; patient assessments collated centrally; and results fed back to respective care facilities. This study explores the views of qualified and unqualified staff on the use of the scale in their respective UK National Health Service, Local Authority or Private Residential older adult care facilities.

Design/methodology/approach

An action research mode was adopted where the researcher was a member of a team responsible for planning, development, and co‐ordination of community care projects for adults leaving hospital care. Structured and semi‐structured interviewing was employed to explore personal experience of the instrument's use in older adult care institutions. A total of 20 care staff qualified in nursing or social work and nine unqualified staff participated in this research.

Findings

Participants reported that scale use contributed positively to the planning and delivery of care. Its use influenced the admission of individuals for care by the institutions studied and improvements in the quality of the transfer of clients between settings were reported. It proved straightforward to use and unqualified staff reported their greater involvement in the care planning process as a result of its implementation. Participants reported that instrument use contributed to the identification of staff training needs.

Originality/value

Evidence for functional assessment scale effectiveness is equivocal and little has been published on the experience of scale use by a range of elder care staff. This experience forms this study's sole focus and scale use is shown to have the potential to contribute to care improvement.

Abstract

Purpose

Drawing upon the job-demands resources and the job demands-control-support model, the authors examined the buffering effect of health-oriented leadership (HoL) in terms of staff care on the relationship between job demands and employee health and job satisfaction.

Design/methodology/approach

Cross-sectional data from two studies (N1 = 314 and N2 = 260) were analyzed using moderation analyses.

Findings

Study 1 showed that staff care mitigates the effect of job demands on strain and health complaints. Study 2 found that staff care also buffered the effect of job demands on general health and job satisfaction.

Practical implications

Particularly under high job demands, staff care is an important resource for employees' health and satisfaction. Organizations should promote leaders' staff care.

Originality/value

Findings provide further evidence for the beneficial role of leaders in terms of HoL.

Abstract

Purpose

Research into culture within intellectual disability services, has predominantly focussed on small group homes in Australia. The current investigation aimed to explore the transferability of the dimensions of culture identified in the literature, to a residential Intellectual Disability service in the UK. The purpose of this study was to better understand the impact of informal culture upon the behaviour of direct care staff, particularly around managing behaviour that challenges.

Design/methodology/approach

Semi-structured interviews were conducted with direct care staff. Interview transcripts were analysed thematically using template analysis.

Findings

Themes regarding leadership, perceptions of managers, team functioning, and relationships between direct care staff and service users, were identified, corroborating the existing literature. Additional themes relating to being aware of the risks posed by service users, and the emotional impact of behaviour that challenges, also emerged.

Originality/value

To the best of the authors’ knowledge, this is the first qualitative study to explore informal culture within a community residential Intellectual Disability service in the UK.

Abstract

Purpose

Active support (AS) influences the way staff support people with intellectual disabilities to take part in everyday activities. Changes in work practices may affect job satisfaction. The impact of AS on job satisfaction has not, however, been widely studied. Job satisfaction is linked with levels of staff turnover and the overall quality of services provided to people with intellectual disabilities (Coomber & Barriball, 2007; Hatton et al., 2001). The purpose of this paper is to describe an evaluation of job satisfaction amongst 38 direct care staff working in intellectual disability services before and after AS was implemented.

Design/methodology/approach

A single group, repeated-measures design was used. In total, 38 members of direct care staff received AS training. Data on job satisfaction were collected before, and after, AS was implemented. In total, 19 members of staff took part in a follow-up 12 weeks later.

Findings

There was a significant increase in reported job satisfaction following the implementation of AS. Subscale analysis revealed that the most significant increases in job satisfaction were related to areas directly targeted by AS, including satisfaction with skill level and satisfaction with amount of time spent with service users.

Originality/value

Implementing AS may provide an added benefit for direct care staff, who feel more satisfied at work. While a significant number of papers have been published focusing on the benefits of AS, no papers have specifically looked at the impact that the intervention can have on job satisfaction.
