Abstract

Anger has been shown to be associated with aggression and violence in adults with learning disabilities in both community and secure settings. Emerging evidence has indicated that cognitive behavioural anger treatment can be effective in reducing assessed levels of anger in these patient populations. However, it has been suggested that the effectiveness of these interventions is significantly affected by verbal ability. In this service evaluation study the pre‐ and post‐treatment and 12‐month follow‐up assessment scores of 83 offenders with learning disabilities who received cognitive behavioural anger treatment were examined in order to investigate whether participants' responsiveness to treatment was a function of measured verbal IQ. The results indicate that, overall, the effectiveness of anger treatment was not the result of higher verbal ability as reflected in verbal IQ scores. It is concluded that cognitive behavioural therapy for anger control problems can be effective for people with moderate, mild and borderline levels of intellectual functioning and forensic histories.

Abstract

There is growing evidence for the efficacy of anger treatment programmes, which aim to reduce inappropriate aggression in people with learning disabilities. To date they have been provided in both group and individual formats, but the differential efficacy of these approaches is yet to be assessed. Individuals with a learning disability and inappropriately expressed anger were assigned to either group or individual treatment or a waiting list control, depending on the availability of treatment options. In this way, 23 participants completed group treatment, 18 individual treatment and 21 were included in a waiting list control. Participants were assessed both before and after treatment using an anger provocation inventory. Improvements were found in the scores obtained on the anger inventory for both the group and individual treatments compared to the control using a 2 by 3 split plot ANOVA. There was no difference between group and individual treatments. These results replicate previous findings that suggest that both group and individual cognitive behavioural interventions are effective treatment options for people with learning disabilities, but do not support either method of delivery as the preferred option. Limitations of the data are discussed.

Abstract

Cognitive behavioural therapy (CBT) is being used increasingly with people with learning disabilities. The evidence base to support these developments comes from uncontrolled trials of CBT in a variety of psychological disorders and eight to nine controlled trials of CBT for anger (plus a single controlled study in depression). This paper reviews the evidence for the effectiveness of group‐based anger management and the acquisition of anger coping skills, and the effectiveness of individual anger treatment, with some discussion of the status of CBT for other indications and the difficulties of conducting outcome research in this area.

Abstract

Purpose

This study aims to evaluate differences between substance dependent men who were arrested for intimate partner violence (IPV) with and without concurrent psychiatric problems.

Design/methodology/approach

In total, 63 participants were randomly assigned to manual‐guided group behavioral therapies (cognitive behavioral therapy for substance dependent domestic violence offenders (SADV) or drug counseling (DC)) and assessed across 12 weeks of treatment.

Findings

Despite excluding psychotic disorders and bipolar disorders at baseline, 23 percent of clients reported prior mental health treatment. SADV participants with co‐occurring mental health were different than SADV participants without co‐occurring mental health symptoms on both baseline and post‐treatment time periods. SADV participants with co‐occurring mental health symptoms had more pre‐treatment impairments, more drug use during treatment, a trend for more problems with aggressive behaviors throughout treatment and significantly more impairments in anger control subscales at post treatment compared to SADV participants without co‐occurring mental health symptoms.

Practical implications

The findings suggest the need to target and treat co‐occurring psychiatric symptoms and distress among SADV offenders. Perhaps offering clients targeted behavioral or pharmacotherapy adjunctive treatments can help improve treatment outcomes among substance dependent offenders of IPV when mental health problems are present.

Originality/value

This study is one of the first to evaluate differences between substance dependent men arrested for IPV with and without concurrent psychiatric problems. It shows that men who are violent toward their partners tend to have a co‐occurring problem with addiction, yet little is known about differences between substance dependent male offenders who have dual mental health problems.

Surveys of offender treatment programmes have revealed that practitioners delivering specialist programmes are often unaware of much of the theoretical and research literature behind the interventions they deliver. For practitioners working less routinely with offenders, such as forensic nurses, this problem is likely to be more acute. In this paper, we provide a general overview of the theoretical and research literature relevant to programmes targeting sexual and aggressive offending that should be of interest to a range of professionals working with offenders in different clinical settings.

Abstract

De‐institutionalisation and resettlement have had a significant impact on offenders with learning disabilities (LD) who are now more visible in the wider community than before. Perhaps because of the challenges presented by people who were previously contained in institutions, there has been a growth of interest in their characteristics, the services and clinical interventions required to support them.This narrative review presents and discusses recent developments concerning offenders with LD. It looks at the historical association between crime and low intelligence, and then examines the evidence concerning the prevalence of offending by people with LD and recidivism rates. Recent research concerning service pathways for this population is summarised and progress in the development of actuarial, dynamic and clinical assessments of the future risk of offending is outlined.The second half of the paper focuses on a review of the evidence for and recent developments in the treatment of offending behaviour (anger/aggression, sexual offending and fire‐setting), utilising broadly cognitive behaviourally‐based approaches. Finally, future directions for research and practice innovation are proposed.

Abstract

Purpose

The purpose of this paper is to evaluate the effectiveness of a brief, Cognitive Behaviour Therapy-based group intervention in anger management with male offenders. All participants were the subject of a stipulation to attend the programme under a probation order, and were at the time of the study being managed in the community.

Design/methodology/approach

Totally, 105 offenders attended the anger management programme, which was delivered by the Probation Board for Northern Ireland (PBNI), between 2008 and 2010 across a range of centres, representing most regions of the province. Prior to treatment, the offenders completed two measures: The State Trait Anger Expression Inventory (STAXI), and the Stages of Change Scales (SCS). Both these measures were also completed at the end of the programme of treatment.

Findings

It was found that the programme significantly reduced the expression of anger as well as state and trait anger among offenders referred to the programme as measured by the STAXI. Both the action and maintenance subscales of the SCS were significant predictors of improvement in anger expression. The action subscale was shown to be a valuable predictor of readiness for change amongst the offenders.

Originality/value

Assessing an offender’s readiness to change may enhance selection for specific rehabilitation programs thus reducing drop-out rates leading to a more efficient use of resources. This study demonstrates that those participants who were found to be more ready for change, benefited most from the intervention programme.

Abstract

Purpose

This study aims to explore the effects of political information and anger on the public's cognitive processing and foreign policy preferences concerning third‐party interventions in ethnic conflict.

Design/methodology/approach

The study employs an experimental design, wherein the authors manipulate policy‐specific information by generating ad hoc political information related to ethnic conflict. The statistical methods of analysis are logistic regression and analysis of covariance.

Findings

The results demonstrate that both political information and anger have a significant impact on an individual's cognitive processing and policy preferences regarding ethnic conflict interventions. Specifically, political information increases one's proclivity to choose non‐military policy options, whereas anger instigates support for aggressive policies. Both factors result in faster decision making with lower amounts of information accessed. However, the interaction of political information and anger is not significant. The study also finds that policy‐specific information – rather than general political information – influences the public's policy preferences.

Originality/value

This study confronts and advances the debate over whether political information is significant in influencing the public's foreign policy preferences and, if so, whether such an effect is the product of general or domain‐specific information. It also addresses an under‐studied topic – the emotive repercussions of ethnic conflicts among potential third‐party interveners. In addition, it tackles the argument over whether political information immunizes people against (or sensitizes them to) the effects of anger on their cognitive processing and foreign policy preferences. The study also introduces a novel approach for examining political information through an experimental manipulation of policy‐specific information.

Purpose

Although studies suggest alcohol abuse is not the major problem among offenders and others with intellectual disabilities (ID), it is still a significant problem. There are also suggestions that alcohol may have a more serious effect on those with ID. The purpose of this paper is to describe a treatment for alcohol-related difficulties designed for people with ID.

Design/methodology/approach

A programme for alcohol-related problems is described and four case studies are presented to illustrate the sessions and review the way in which people with ID have responded to the methods. The cases have a mixture of alcohol-related problems including anger, anxiety, social withdrawal and depression. The alcohol programme is coordinated with a range of person centred interventions for specific difficulties.

Findings

All cases responded to the programme positively. Two cases showed reductions in anger, two reported reductions in anxiety and one reported reductions in depression. All cases increased their alcohol knowledge considerably.

Research limitations/implications

The programme seems promising in its approach to alcohol-related difficulties. It is noted that alcohol education alone is likely to improve participants’ wellbeing in the absence of coordinated intervention for other relevant personal difficulties. A controlled treatment trial for effectiveness is clearly required.

Originality/value

The paper describes a programme for alcohol-related problems and may be the first such programme that has contained pilot evaluation.

Abstract

Purpose

High levels of anger have been associated with forensic learning disabled populations. The role of gender within the experience of anger is not clear. This study aims to start exploring the question “Do female forensic patients with learning disabilities have different needs in relation to anger?”

Design/methodology/approach

This study used a between‐subjects design with one independent variable: gender. The Novaco Anger Scale and Provocation Inventory was applied. Scores obtained by 12 females were compared with those of 23 males within a forensic psychiatric service for learning disabled patients.

Findings

Significant differences were found between scores, suggesting female forensic patients with learning disabilities experience higher levels of anger than do males, particularly in the arousal domain, and demonstrate difficulties in regulating anger.

Research limitations/implications

The use of a single measure of anger and the small, forensic nature of the sample limits this study. It is recommended that further research address these issues and extend the exploration of this issue to non‐forensic populations.

Practical implications

Gender specific differences, and the potential influence of learning disabilities should be considered when assessing and treating anger difficulties. Female forensic patients with learning disabilities may benefit from a greater emphasis on anger arousal reduction work.

Social implications

This study highlights how the impact of gender and learning disabilities on the experience of anger is currently insufficiently understood.

Originality/value

This study forms a preliminary study of anger in the under‐researched population of females with learning disabilities.
