Abstract

Purpose

The purpose of this study was to explore the extent to which first-line managers' (FLMs') support of employee learning activities influence the learning outcomes and the transfer of training from a work-based vocational education and training (VET) programme.

Design/methodology/approach

The study was designed as a multiple-case study including six service units in elder care. Data were collected through interviews with managers and care workers in the six units.

Findings

The study demonstrated that the role of FLMs was a significant condition for the learning outcomes attained and for positive transfer of training. For care workers to attain benefits from training that went beyond the individual level, that is, benefits at the level of the work team or the organisation, a supportive FLM role appeared to be necessary.

Research limitations/implications

The statistical generalisability of the study is limited by its character of a multiple-case study of six work units. At the same time, a strength of the study lies in its potential for analytic generalisations, that is, for identifying and verifying theoretically meaningful patterns across a number of cases.

Practical implications

The findings suggest a need for management development that promotes a broader understanding of the task as FLM, and, specifically, improved knowledge and skills regarding how to lead and organise learning and development processes in a workplace.

Originality/value

Using a qualitative approach, this study adds to the limited knowledge of FLMs' influences on the learning outcomes and the positive transfer from work-based training.

Abstract

Purpose

This article analyzes recent Japanese efforts to recruit care labor from seven Asian countries to identify the relative contributions to migrants and their respective countries' health systems. Besides considering the factors affecting migration from, and benefits to, sending countries, it asks how differences in the role of public and private actors may matter.

Design/methodology/approach

The study uses two stages of analysis. The first uses quantitative and qualitative data for seven countries that send care labor migrants to Japan to identify differences in benefits for individual migrants and health care systems in the sending countries. The second stage examines recent initiatives for funding care worker training in Japan to assess the relative impacts of different public-private cooperative arrangements, especially in terms of Vietnam.

Findings

In addition to general migration policy mechanisms provided by the destination country, bilateral relationships and foreign assistance, along with economic, demographic and health care conditions in the origin countries, contribute to the relative benefits of migration. Among countries supplying care labor to Japan, Vietnam is obtaining the most benefits for its health care system in return.

Originality/value

Responding to central concerns surrounding care labor migration, the article compares across countries sending care workers to a single country. The comparison highlights a constellation of factors that contribute the greatest benefits. The article identifies how different types of public and private relationships can influence this process. The study provides observations applicable to other welfare states developing care labor migration relationships.

Abstract

This chapter addresses the grand challenge of an aging society and the subsequent growing demand for in-home care for the elderly – often referred to as homecare. It examines how emergent homecare models in England differ from the “time and task” model and how they are shaping the care market. These models offer new approaches regarding what, how, and when care is delivered at home. Homecare providers face rising demand driven not only by population aging but also by market demand for personalized care, choice, continuity of care, and real-time availability. The landscape presents an opportunity for innovative models to become established, by offering a more inducing service design and value propositions that respond to customers' needs. Using the “business model canvas” to guide data collection, this study presents an ethnographic case analysis of four homecare organizations with distinct emergent homecare models. The study includes 14 months of field observation and 33 in-depth interviews. It finds that providers are becoming increasingly aware of evolving customer needs, establishing models such as the “uberization,” “community-based,” “live-in,” and “preventative” described in the chapter. These models are becoming more pervasive and are mostly market-driven; however, some of their innovations are market shaping. The major innovations are in their value propositions, partnership arrangements, and customer segments. Their value propositions focus on well-being outcomes, including choice and personalization for care users; their workforces are perceived to be a major stakeholder segment, and their networks of partners offer access to complementary services, investments, and specialist knowledge.

Abstract

Purpose

Despite the large number of Indonesian health-care workers working as migrant care workers in Japan, there is a lack of studies that have investigated this phenomenon or assessed the working conditions of Indonesian care workers working in Japanese aged-care facilities. This study aims to explore the work experiences of Indonesian care workers in Japan.

Design/methodology/approach

A qualitative descriptive research design was used, and a purposive sample of 18 Indonesian health-care workers (12 nurses and six midwives) who work as care workers in Japan participated in this study. Semistructured interviews were conducted with these participants between December 2022 and January 2023. The data were analyzed using content analysis. The Consolidated Criteria for Reporting Qualitative Research checklist was used to report this study.

Findings

Four key themes emerged from the data: easy but mentally challenging, being a care worker is a blessing, working in a partially Muslim-friendly environment and enjoy living: prefer to stay longer.

Originality/value

The work experiences of Indonesian care workers in Japan are complex and multifaceted. By understanding their experiences and needs, the authors can work toward creating more inclusive and supportive workplaces for all.

Abstract

Purpose

The demand for ambulance services in Hong Kong is increasing and first aid plays an important role in saving lives. The government recognized first aid training courses offered by four organizations. This study aims to review and discuss the past, present and future of first aid and health training in Hong Kong.

Design/methodology/approach

This study adopted a case study approach to investigate the operations and development of the first aid and health training in one of the largest training providers in Hong Kong. Publicly available secondary data, including annual reports, websites, newspapers and social media, were collected and analysed. Using the thematic analysis procedure, researchers familiarised the data collected, generated initial codes, searched for themes, reviewed themes, defined themes and reported the results.

Findings

Through data analysis, researchers identified six themes which help to address the research questions regarding the types of first aid and health training courses, quality issue, awareness of first aid in the community and the future delivery of first aid training.

Practical implications

This study provided two implications on both micro and macro levels. At the micro level, training providers can enhance training quality by adopting technology. At the macro level, training providers can continue to advocate and facilitate the integration of first aid into the formal education curriculum. The last section addressed the limitations of this study and provided future research directions.

Originality/value

This study provided insights about the development of the first aid and health education and training. Health-care training providers and policymakers could benchmark the findings from this study and develop appropriate measures for promoting health education.

Abstract

Purpose

This paper seeks to address the question, “How can private home care providers compete and drive their businesses forward to deliver best value to the community?” Public sector managers in local authorities need this question answered so that they can provide their part of the solution, facilitating best value.

Design/methodology/approach

A review was performed of relevant literature on the commissioning (outsourcing) of home care and of best value. This indicates that the future of home care services, taking into consideration commissioning and how best value will be achieved, has not been researched widely. Therefore, an exploratory approach to research was adopted here using in‐depth analysis of a small number of particularly informative local authorities and private providers selected by purposive/judgemental (extreme and critical case) sampling. Personal contact was deemed necessary in order to perform an intensive investigation to pursue in‐depth information.

Findings

To improve value one can cut costs and/or increase quality. It is argued here that there is little immediate opportunity for private home care providers to cut costs and with fixed pricing substantial improvements in quality cannot be funded by increased prices or cost cutting elsewhere. To address this impasse, two solutions have been identified; increased economies of scale through consolidation in the marketplace and radical improvements in efficiency through the exploitation of information and communication technology (ICT). Both of these strategies have major ramifications for the “enabling local authority” taking actions to see best value delivered to its community.

Research limitations/implications

This paper presents the findings of exploratory research. A more detailed study covering many more local authorities, private care providers and an international prospective will be conducted over the next two years.

Practical implications

This paper provides timely guidance to public sector managers in local authorities and private home care providers seeking best value in home care through commissioning.

Originality/value

Little has been found in the literature on strategies by which private home care providers can deliver best value, yet such strategies are needed urgently to achieve best value. This paper is a timely contribution to addressing this need.

Abstract

Purpose

The purpose of this paper is to elaborate a clinician's perspective on the shared integrated care of older people with dementia and/or frailty. In addition, it aims to describe the significance and value of dignity, kindness and compassion in care.

Design/methodology/approach

This is a general review of current policy, research and good practice, amplified from a clinical perspective.

Findings

The key components of effective integrated care are shared knowledge, understanding, training and support. Equally important are shared objectives, leadership, and governance.

Originality/value

The clinical perspective offered in the paper aims to link advances in medicine with advances in care. This confirms that comprehensive geriatric assessment, as well as working with individuals and their families, must underpin all integrated, humane and effective care for older vulnerable people.

Purpose

When public agencies seek to privatize a service, a commissioning process begins wherein public sector budgeters must decide how generous the funding will be while taking occupational standards into account so that the quality of service is assured. One important area of occupational standards is the required personnel and job sizes of certified employees. Not enough attention has been directed to how occupational standards’ related knowledge is treated in the process. The purpose of this paper is to: first, investigate how the commissioning process is experienced by Israeli, often female, occupational standards administrators. Second, proposing a gendered perspective on Sennett’s corrosion of character thesis.

Design/methodology/approach

As part of an institutional ethnography project, 16 interviews were conducted with (14 female and two male) occupational standards administrators at the Israeli Welfare, Education and Health Ministries.

Findings

The routine of commissioning involves a stage of using occupational standards’ knowledge and experience, and a stage of dismissing it. The “corrosion of character” embedded in the dismissal stage undermines historical achievements in the area of recognizing caring work and skills.

Research limitations/implications

The research is unable to distinguish between the specific caring occupations discussed.

Practical implications

Service delivery modes has to develop into more publicly visible forums where occupational standards’ are protected.

Social implications

The continuous corrosion of occupational knowledge may result in the demise of professionalization in care service occupations causing increasingly more polarization and poverty among their employees.

Originality/value

While Sennett’s thesis has already been found plausible for understanding public servants’ experiences of the “new public management,” until recently, not enough attention has been devoted to the commissioning processes’ gendered implications for contract-based delivery of services. This paper examines these implications for the power struggle between the feminist achievements protecting skill recognition in caring occupations, and policy makers.

Abstract

Purpose

The purpose of this paper is to provide an overview of the process of commissioning adult social care services in England. It reflects the literature on commissioning at the strategic level followed by a section on operational or micro-commissioning. The rest of the paper focusses on the emergence of ideas about outcomes-based commissioning (OBC) in the field of adult social care and ends with critical consideration of the effectiveness of OBC in adult social care as applied to support and care provided in extra care housing.

Design/methodology/approach

The review of strategic and operational commissioning in adult social care in England (and Scotland in brief) is based on both policy documents and a review of the literature, as are the sources addressing OBC in adult social care particularly in extra care housing settings.

Findings

The core of this paper focusses on the challenges to the implementation of OBC in adult social care in the context of provision for residents in extra care housing. Of central importance are the impact of the squeeze on funding, increasing costs as a result of demographic change and the introduction of a national living wage plus the focus on the needs of service users through the idea of person-centred care and resistance to change on the part of adult social care staff and workers in other relevant settings.

Originality/value

Addressing the implementation of OBC in adult social care in England in the context of extra care housing.
