Abstract

Purpose

This study aims to explore the factors influencing electrical accidents. Here, the authors aim to understand and model the causes of electrical accidents at multiple levels.

Design/methodology/approach

In the study, the authors have tried to put causes of accidents in the electricity distribution segment, in the framework of the Swiss Cheese model. Delphi kind of expert survey was conducted to find the Cheese Slice (level) and the causes (holes) for electrical accidents. Inputs from a hundred experts having more than five years of experience in electrical utility companies have been used to find Cheese Slice and holes, to explain the occurrence of an electrical accident.

Findings

Effective training for safe work practices, safe knowledge and closer supervision would go a long way to plug the holes in the Cheese Slice in human factors. The difference in perception of managers, supervisors and workers on the importance of various causes of electrical accidents are also presented and discussed.

Research limitations/implications

This research is based on expert opinion and survey where respondent perception is reported. Actual accident data has not been used here.

Practical implications

The holes or causes of accidents at different levels (Cheese Slice) have been identified for plugging or removal for better safety.

Social implications

Electrical energy is widely used, and therefore, electrical safety is a social concern and also improving it is a social need.

Originality/value

The study contributes to electrical safety issues in the electrical utility sector.

Abstract

Purpose

Statistics show that the construction sector has the second-highest number of accident cases in Malaysia. A total of 100, 000 construction workers suffer from work-related bad health each year. Scaffolding accidents are the second cause of accidents on construction sites. Therefore, this present research provided answers to the following questions: (1) what are the causes of scaffolding accidents and (2) what are the possible measures to reduce scaffolding accidents?

Design/methodology/approach

The research developed a questionnaire instrument that included 24 causes of scaffolding accidents and 21 remedial actions. The research was based on a cross-sectional survey questionnaire administered to 129 members of construction organizations.

Findings

Data revealed that scaffolding-related cases caused a total of 70% of the deaths/injuries on sites. Furthermore, scaffolding accidents were mainly caused by a lack of guard rails on scaffoldings, poor inspections, improper assembly, a poor safety culture, poor attitudes towards safety, poor footing of scaffoldings and unsecured planking. To reduce scaffolding accidents, there must be a lifeline on scaffolding, proper guardrails and proper assembling of scaffoldings, and preventing access to incomplete or defective scaffoldings. The 24 causes are structured into six factors through factor analysis and the 21 remedial actions into six factors.

Originality/value

This research serves as the first attempt to conduct broad research on the causes and remedial actions concerning scaffolding accidents on construction sites in Malaysia. Theoretically, the research has provided fresh insights into the impact of scaffolding accidents.

Abstract

Purpose

The operation safety of the high-speed railway has been widely concerned. Due to the joint influence of the environment, equipment, personnel and other factors, accidents are inevitable in the operation process. However, few studies focused on identifying contributing factors affecting the severity of high-speed railway accidents because of the difficulty in obtaining field data. This study aims to investigate the impact factors affecting the severity of the general high-speed railway.

Design/methodology/approach

A total of 14 potential factors were examined from 475 data. The severity level is categorized into four levels by delay time and the number of subsequent trains that are affected by the accident. The partial proportional odds model was constructed to relax the constraint of the parallel line assumption.

Findings

The results show that 10 factors are found to significantly affect accident severity. Moreover, the factors including automation train protection (ATP) system fault, platform screen door and train door fault, traction converter fault and railway clearance intrusion by objects have an effect on reducing the severity level. On the contrary, the accidents caused by objects hanging on the catenary, pantograph fault, passenger misconducting or sudden illness, personnel intrusion of railway clearance, driving on heavy rain or snow and train collision against objects tend to be more severe.

Originality/value

The research results are very useful for mitigating the consequences of high-speed rail accidents.

Abstract

Purpose

Scientific knowledge is usually regarded as the basis for the management of natural environment and wildlife in ecotourism. However, recently, social construction approaches challenge the domination of natural science. This study aims to examine the effectiveness of the social construction paradigm in ecotourism management, through conducting a content analysis of social media comments on an accident caused by a monkey in a Chinese ecotourism area. The results show that people commented on the accident from five aspects. First, the public expressed their compassion and mourning for the deceased. Second, people thought that the death was casual and absurd, yet life is full of uncertainty and people should cherish the present. Third, people commented much on the deceased tourist’s company, which is a famous sugar brand well entrenched in many Chinese people’s childhood memories. Fourth, people constructed the monkey as Monkey King, Golden Monkey (another famous sugar brand in China) and as a criminal. Fifth, people also gave their opinions about possible causes of the accident, namely, it was caused by “the mandate of heaven,” company competition, conspiracies or poor management. This study only seriously considers the comments about the mandate of heaven. This explanation is consistent with the Chinese traditional construction of nature as “heaven,” which is believed to dominate the natural and human worlds. Most people, including the managers, accepted the accident and did not explore further about the reasons for the accident. In this case, such a social construction of nature does not aid effective ecotourism management.

Abstract

Purpose

The purpose of this study is to investigate the influence of institutional dynamics on road accidents and whether this relationship is moderated by information and communication technology (ICT).

Design/methodology/approach

The study adopted a quantitative approach with 133 respondents. Research hypotheses were tested in AMOS version 21. In addition, moderated regression analysis was used to test the moderating role of ICT on the relationship between institutional dynamics and road accidents.

Findings

The results show that vehicle maintenance, policy enforcement, safety culture, driver training and driver management positively influence road accidents. Moreover, the study established that ICT moderates the relationship between institutional dynamics and road accidents.

Practical implications

The results of this study serve as a practical guideline for policymakers in the road haulage sector. Managers may gain insights on how to design effective interventions to reduce road accidents.

Originality/value

This research contributes to the existing body of knowledge by exploring previously unexplored moderating paths in the relationship between institutional dynamics and road accidents. By highlighting the moderating role of ICT, the study sheds new light on the institutional dynamics that influence road accidents in the context of road haulage companies.

Abstract

Purpose

At the US passenger stations, train operations approaching terminating tracks rely on the engineer’s compliant behavior to safely stop before the end of the tracks. Noncompliance actions from the disengaged or inattentive engineers would result in hazards to train passengers, train crewmembers and bystanders at passenger stations. Over the past decade, a series of end-of-track collisions occurred at passenger stations with substantial property damage and casualties. This study’s developed systemic model and discussions present policymakers, railway practitioners and academic researchers with a flexible approach for qualitatively assessing railroad safety.

Design/methodology/approach

To achieve a system-based, micro-level analysis of end-of-track accidents and eventually promote the safety level of passenger stations, the systems-theoretic accident modeling and processes (STAMP), as a practical systematic accident model widely used in the complex systems, is developed in view of environmental factors, human errors, organizational factors and mechanical failures in this complex socio-technical system.

Findings

The developed STAMP accident model and analytical results qualitatively provide an explicit understanding of the system hazards, constraints and hierarchical control structure of train operations on terminating tracks in the US passenger stations. Furthermore, the safety recommendations and practical options related to obstructive sleep apnea screening, positive train control-based collision avoidance mechanisms, robust system safety program plans and bumping posts are proposed and evaluated using the STAMP approach.

Originality/value

The findings from STAMP-based analysis can serve as valid references for policymakers, government accident investigators, railway practitioners and academic researchers. Ultimately, they can contribute to establishing effective emergent measures for train operations at passenger stations and promote the level of safety necessary to protect the public. The STAMP approach could be adapted to analyze various other rail safety systems that aim to ultimately improve the safety level of railroad systems.

Abstract

Purpose

For the purpose of reducing the incidence of hazardous materials transport accident, eliminating the potential threats and ensuring their safety, aiming at the shortcomings in the process of current hazardous materials transportation management, this paper aims to construct the framework of hazardous materials transportation safety management system under the vehicle-infrastructure connected environment.

Design/methodology/approach

The system takes the intelligent connected vehicle as the main supporter, integrating GIS, GPS, eye location, GSM, networks and database technology.

Findings

By analyzing the transportation characteristics of hazardous materials, this system consists of five subsystems, which are vehicle and driver management subsystem, dangerous sources and hazardous materials management subsystem, route analysis and optimization subsystem, early warning and emergency rescue management subsystem, and basic information query subsystem.

Originality/value

Hazardous materials transportation safety management system includes omnibearing real-time monitoring, timely updating of system database, real-time generation and optimization of emergency rescue route. The system can reduce the transportation cost and improve the ability of accident prevention and emergency rescue of hazardous materials.

Abstract

Purpose

Railway-level crossings (RLCs) are the point of intersection between rail and road users and are therefore hotpots of road-rail user conflict and catastrophic collisions. The purpose of this study is to assess the risks associated with RLCs and suggest probable reduction measures. Through questionnaires and visual inspection, the authors identify the safety risks, hazards and hazardous events at some railway crossing of Addis Ababa light rail transit (AA-LRT) north-south (N-S) route. The identified risky events are then categorized based on As Low As Reasonably Practicable (ALARP) principle and generic risk ranking matrix. The authors then examine existing safety management measures at railway crossing and assess the need for additional safety management. Five major crossings on the 16.9 km (10.5 mi) N-S line, starting from Menelik II Square to Kality, were considered for the study. This study is carried out by data collection from about 145 stakeholders and the application of statistical data and risk analysis methods. The major findings of this study and the recommendations for improvement are suggested.

Design/methodology/approach

The research followed a case study approach. Through questionnaires and visual inspection, the authors identify the safety risks, hazards and hazardous events at some railway crossing of AA-LRT N-S route. The identified risky events are then categorized based on ALARP principle and generic risk ranking matrix. Collected data was then analyzed using SPSS to deduce relationships.

Findings

The study findings reveal human factors as the greatest cause of accidents, injury or death. About 22% of hazards identified by category are human factors, whereas 20% are because of technical problems. Intolerable risks stand at 42%, whereas the tolerable risks are at 36% according to risk classification results as per the ALARP model. Because the process of risk management is a long-term cycle, its importance should not be missed at any time.

Research limitations/implications

Because of design considerations of RLCs and the difference in generalized human behaviors for people of a given region, the results are limited to AA-LRT RLCs. This study opens a discourse for detailed evaluations, qualitative and quantitative analysis into the categorized identified hazards. There is also room for additional research into the performance of RLCs aimed at formulating standard necessary features that should be included on RLCs for proper risk control especially in emerging economies.

Abstract

Purpose

This paper will examine the unsafe work practices that are plaguing the construction industry. Statistics show that four out of five of all workplace accidents are attributed to unsafe behaviour. Research studies have sought to understand worker self-protection. For example, it is difficult to make predictions of conditions that influenced worker’s behaviour to act unsafely or safely in a given work situation. It is evident there is a gap in the literature in this area of research, most notably failing to understand the underlying “why” factors. The aim of the study is to identify and examine the proximate set of contributing factors most likely to have an influence on workers’ decisions about participation in unsafe behaviour.

Design/Methodology/Approach

To perform the study, questionnaires were adopted, and 225 construction workers from 9 construction companies participated in the study.

Findings

Results showed that both underlying organisational factors and individual factors could affect the risk aversion among construction workers. The paper also highlights measures to create a safe work environment to minimise unsafe behaviour among construction workers. Results from the study are important to help organisation to systematically plan for a good working environment.

Research limitations

As the results were based only from the questionnaires, a deeper understanding behind the workers’ responses was not probed.

Practical implications

Construction companies should work at several organisational levels at the same time. It is necessary to include levels such as individual, group, workplace and management levels, thus taking a system perspective on risk behaviour and safety.

Abstract

Purpose

The purpose of this paper is to develop a structural relationship model to study the relationship between causes and effects of poor communication and information exchange in construction projects using Smart-PLS.

Design/methodology/approach

The first method of this research is to identify the causes and effects factors of poor communication in construction projects from the extant of literature. The data used to develop the model was collected using a questionnaire survey, which targeted construction practitioners in the Malaysian construction industry. A five-point Likert type scale was used to rate the significance of the factors. The factors were classified under their relevant construct/group using exploratory factor analysis. A hypothetical model was developed and then transformed into Smart-PLS in which the hypothetical model suggested that each group of the cause factors has a direct impact on the effect groups. The hypothesis was tested using t-values and p-values. The model was assessed for its inner and outer components and achieved the threshold criterion. Further, the model was verified by engaging 14 construction experts to verify its applicability in the construction project setting.

Findings

The study developed a structural equation model to clarify the relationships between causes and effects of poor communication in construction projects. The model explained the degree of relationships among causes and effects of poor communication in construction projects.

Originality/value

The published academic and non-academic literature introduced many studies on the issue of communication including the definitions, importance, barriers to effective communication and means of poor communication. However, these studies ended up only on the general issue of communication lacking an in-depth investigation of the causes and effects of poor communication in the construction industry. The study implemented advanced structural modeling to study the causes and effects. The questionnaire, the data and concluding results fill the identified research gap of this study. The addressed issue is also of interest because communication is considered one of the main knowledge areas in construction management.
