Abstract
This paper argues that there is a need for an independent specialist women's refuge sector to address the housing needs of BMER (Black, minority ethnic and refugee) women. It will consider barriers to equal access that BMER women have and how these could be resolved by providing specialist services tailored to their specific needs. Specifically, the paper shows how such services, attuned to concerns of race, class, and gender, could positively help resolve additional barriers confronting BMER women due to housing inequality. The primary research, based on an analysis of questionnaire responses and a focus group with service users, offers a snapshot of the impact that the lack of access to housing provision has for BMER women including increasing their social exclusion and vulnerability if need remains unmet. A case is made for a strengthened independent specialist sector to deal with the housing needs of women fleeing domestic violence. Key recommendations are identified on how housing policies, practices and service provision can be strengthened through the implementation of a specialist sector.

Abstract
Purpose
This study aims to explore the empirical correlation between patient flow issues, quality of green health services and patient satisfaction in specialist medical department factors from patients’ perspectives as service consumers.
Design/methodology/approach
This research is a type of nonintervention empirical research that uses an open survey to explore the views and experiences of users of specialist medical department services. The targeted population is hospital patients included in the top five national PERSI (Indonesian Hospital Association) Award 2022 Green Hospital Category, with a total number of respondents of 572 people. This study uses the partial least square-structural equation modeling analysis method with the SmartPLS application.
Findings
Patient flow problems generally affect the quality of eco-friendly health services, except for the waiting time problem, which affects service quality. It should be understood as a top priority for patients to receive services from medical specialists without risking time as a core service aspect from the patient’s perspective. In addition, all variables in eco-friendly hospital services affect patient satisfaction, except in the case of visits to specialist medical departments, which do not affect medical support services and hospital practices that are responsive to the delivery of care services resulting from medical support services that are inseparable in integrated services as well as health care following medical ethics.
Originality/value
This study has a novelty in understanding the implications of green practice in determining patient satisfaction in medical specialist department as the epicenter of hospital services and the main object of assessment for the quality of hospital services.

Abstract
Specialist CAMHS provide skilled assessment and interventions for children, young people and their families who have mental health disorders. The training needs of the staff who work in specialist CAMHS are not always clear or prioritised, due to the complexities and differing contexts in which specialist CAMHS are provided. The aim of this paper was to establish stakeholders' experiences of service complexities and challenges that affect training within specialist CAMHS. The project employed interviews to gain wide‐ranging consultation with key stakeholder groups. The sample consisted of 45 participants recruited from policy departments, professional bodies, higher education providers, commissioners, service managers, and practitioners. The participants identified a number of themes that limit training, and put forward solutions on how these could be facilitated in the future. Emerging themes related to leadership and the role of service managers, strategic management of training, commissioning, levels of staff training, resources, impact of training on service users, and availability of training programmes. The findings emphasise the need for the strategic workforce planning of training to meet service delivery goals. Policy, commissioning, workforce training strategies, service needs, and delivery of training should be integrated and closely linked.

Abstract
This paper explores the findings from a recent study about the assessment and management of care for older people who may have a sensory impairment. Using qualitative research methods, the work focused on non‐specialist practitioners who are responsible for the assessment and management of care for older people and their carers. The findings are based upon the analysis of in‐depth interviews with non‐specialist practitioners, specialist workers and managers from statutory and voluntary sector agencies. Older people with a hearing impairment or a visual impairment are not a homogenous group of people with a single set of needs or service support networks. It is the existence of non‐specialist practitioners, carrying out the assessment and management of care for older people that draw together in one study the three areas of visual impairment, hearing impairment and dual impairment. The findings relate to practitioners' awareness of sensory impairment in their local community; how practitioners assess and manage care; access to services; staff training and development; and, information strategies. The interface between non‐specialists and practitioners with particular expertise in sensory impairment is also examined. The implications for policy and practice are identified.

The paper examines how housing inequality creates additional barriers for BMER women fleeing domestic violence, and provides arguments for the way in which specialist services address inequality from the perspective of race, class and gender. Abstract
Purpose
The purpose of this paper is to study, inequalities between general physicians’ and specialists’ visits in Shiraz. Also, the factors effecting the utilization of visits were determined.
Design/methodology/approach
Concentration index and curves, ranked by income and quality of life were used to estimate the amount of inequality in the utilization of services. Health utilization data which had been gathered already were used for this purpose. Poisson regression was used to construct the models.
Findings
Results of the study showed that, inequalities in specialists’ visits were higher than GPs’. Complementary insurances users and females used more specialist services. People with higher quality of life utilized fewer GPs’ and specialists’ services.
Originality/value
New evidences about inequality in health services utilization and its components in Iran was surveyed.

This article describes a process undertaken to develop a set of commissioning principles to support the commissioning of secure learning disability services across England. The principles, shaped around the 11 competencies laid down in the World Class Commissioning competencies framework (Department of Health, 2008a), were produced following a scoping exercise that looked at provision and commissioning of secure learning disability services within each strategic health authority (SHA) area in England. Specific details were collected about types of services provided, including detailed service specification, quality indicators, how these (specialist) services link with local services (secure and non secure) and cost of services. Information collected about commissioning concentrated on strategic vision, practical commissioning arrangements, how the quality of services was monitored, how access to services was controlled and how ‘secure’ service users are reintegrated back into local (non secure) services and communities. This scoping exercise was augmented by qualitative data obtained from interview with a group of former service users. Themes generated through the interviews were integrated within the general guidance. A quality assurance framework based on the World Class Commissioning Competencies is proposed, against which specialist and local commissioners can benchmark their current commissioning arrangements.

There are high rates of emotional, behavioural and psychiatric problems (Cooper et al, 2007) in the learning disability population. This paper describes the Mental Health in Learning Disabilities (MHiLD) service for adults with learning disabilities in four South London boroughs. This service has been in operation in two South East London boroughs from 1982, and was extended to a third in 1999 and a fourth in 2006.
