Abstract

Purpose

This paper aims to explore the lived experience of people with a chronic non-healing wound and to explore what it means to live with a chronic wound.

Design/methodology/approach

A descriptive phenomenological study design was adopted to explore the living experience of person with chronic wound. A sample of 15 individuals of both genders was selected using a purposive sampling technique. To collect data, in-depth interviews were conducted, and all the interviews were audio-taped and transcribed verbatim. Data were analysed using the seven-step process described by Colaizzi (1978).

Findings

The findings were organized into 6 themes clusters and 12 themes. The six themes clusters were limiting mobility; receiving care; explaining causes of wounds; contending with chronic illnesses; adapting and mal-adapting; and economic burden of the wound.

Research limitations/implications

Chronic wound had a profound impact on participants’ lives by affecting their activities of daily living, their mobility, their income and their personal relationships.

Originality/value

Understanding the lived experiences of people with chronic wounds is crucial for health-care providers, including nurses. Investigating the chronic wound experience has become even more pressing given the projected increase in the number of elderly individuals and those with chronic illnesses such as diabetes mellitus. In Jordan, for example, the prevalence of diabetes mellitus is 17.1%, and it is projected to increase by 2050.

Abstract

Purpose

This paper aims to review state of the art of techniques for dimensioning chronic wounds, and validate the possibilities of employing a new proposed optical imaging approach for general task of wound assessment.

Design/methodology/approach

Current techniques used for quantifying wound surface are reviewed and evaluated from various perspectives to exam their usability in wound care clinical settings. A photometric stereo (PS) approach will be identified and verified to work as an alternative solution to better satisfy practical requirements on quantifying the dimension of real and mocked wounds.

Findings

Both contact and contactless approaches provide some useful functions for wound management; however, new imaging modalities are still required for achieving good portability, affordability and applicability in assisting decision-making in clinical settings. The PS approach can work as a potential solution to provide these functionalities as well as dense geometrical and color texture information of measured areas. The experiments demonstrate that the new approach is able to conveniently produce comparable results to those from latest stereo vision-based techniques.

Research limitations/implications

This work proposed and initially verified the potential of PS technique for the task of wound measurement. Substantial improvements on the prototype and more clinical trials are still required to validate the new technique before it is accepted as a tool for practical wound measurement.

Originality/value

This new PS approach has good potential to reliably measure the dimension of wounds as well as recover their color texture which could contain additional valuable information for predicting a healing procedure for those wound occurring deeper underneath the skin surface.

Abstract

Purpose

The purpose of this paper is to address three topical themes: user involvement in health services research; determining the value of new medical technologies in patient care pathways, furthering knowledge related to quality in health and social care; and knowledge exchange between manufacturers, health service supply chain networks and device users. The model is being validated in a case study in progress. The latter is a “proving ground” study for a translational research company. Medical devices play a pivotal role in the management of chronic diseases, across all care settings. Failure to engage users in device development inevitably affects the quality of clinical outcomes. A model of user engagement is presented, turning unmet needs for medical devices into viable commercial propositions.

Design/methodology/approach

A case study investigating the perceptions of individuals with Epidermolysis Bullosa (EB), their lay and professional carers into unmet needs. EB is an inherited condition affecting the skin and mucosal linings that leads to blistering and wounds.

Findings

Qualitative data are being collected to generate understanding of unmet needs and wound care products. These needs are being translated into new design concepts and prototypes. Prototypes will be evaluated in an n=1 experimental design, generating quantitative outcomes data.

Originality/value

There are generalisations from the case study, and the model outlined. New products for managing EB wounds can logically benefit other groups. The model is transferable to other clinical problems, which can benefit from research and technological advances that are integral to clinical needs and care.

Abstract

Purpose

As healthcare continues to become more expensive and complex, considering the voice of the patient in the design and operation of healthcare practices is important. Wound care and rural healthcare scenarios pose additional complexities for providers and patients. This study sought to identify key determinants of patient service quality in wound care.

Design/methodology/approach

Patients at the wound care/ostomy clinic (WOC) in a rural hospital were surveyed using the Kano model. The Kano model enables the categorization of quality attributes based on the attributes' contribution to the subject's overall satisfaction (and dissatisfaction). Chi-square goodness-of-fit testing, multinomial analysis and power analysis were then used to determine the Kano categories for each satisfaction-related attribute.

Findings

The analyses resulted in 14 one-dimensional attributes and 3 indeterminable attributes. For the one-dimensional attributes, customer satisfaction is directly proportional to the level of performance for that attribute. The one-dimensional attributes included providing correct care on the first, provision of necessary supplies for care, appropriately qualified medical staff and confidence in care provided by medical staff, among others. Understanding the attributes important to the patient drive patient-centered care, which improves positive patient outcomes and recovery. These attributes can then be used by healthcare professionals to design patient-centric processes and services. This research provides a framework for incorporating the voice of the patient into healthcare services.

Research limitations/implications

While the research methodology can be used in other healthcare settings, the findings are not generalizable to other wound care clinics. This research was conducted in one small, rural hospital. In addition, the sample size was small due to the size of the wound clinic; therefore, an analysis of the differences between demographics could not be performed.

Practical implications

Considering the perspectives of rural wound care patients is important, as the patients are an under-served population with unique challenges related to patient care. The research findings detail rural patients' expectations during wound care treatments, which enable the clinic to focus on improving patient satisfaction. This research contributes to understanding the factors that are important to patient satisfaction in wound care. Further, the methodology presented can be applied to other healthcare settings.

Originality/value

While studies exist using the Kano model in healthcare and the literature is sparse in rural healthcare, this is the first case study using the Kano model in wound care to understand patient preferences.

Abstract

Purpose

Whilst there is a growing body of research which discusses the use of remotely piloted aircraft systems (RPAS) (otherwise known as “drones”) to transport medical supplies, almost all reported cases employ short range aircraft. The purpose of this paper is to consider the advantages and challenges inherent in the use of long endurance remotely piloted aircraft systems (LE-RPAS) aircraft to support the provision of medical supplies to remote locations – specifically “medical maggots” used in maggot debridement therapy (MDT) wound care.

Design/methodology/approach

After introducing both MDT and the LE-RPAS technology, the paper first reports on the outcomes of a case study involving 11 semi-structured interviews with individuals who either have experience and expertise in the use of LE-RPAS or in the provision of healthcare to remote communities in Western Australia. The insights gained from this case study are then synthesised to assess the feasibility of LE-RPAS assisted delivery of medical maggots to those living in such geographically challenging locations.

Findings

No insuperable challenges to the concept of using LE-RPAS to transport medical maggots were uncovered during this research – rather, those who contributed to the investigations from across the spectrum from operators to users, were highly supportive of the overall concept.

Practical implications

The paper offers an assessment of the feasibility of the use of LE-RPAS to transport medical maggots. In doing so, it highlights a number of infrastructure and organisational challenges that would need to be overcome to operationalise this concept. Whilst the particular context of the paper relates to the provision of medical support to a remote location of a developed country, the core benefits and challenges that are exposed relate equally to the use of LE-RPAS in a post-disaster response. To this end, the paper offers a high-level route map to support the implementation of the concept.

Social implications

The paper proposes a novel approach to the efficient and effective provision of medical care to remote Australian communities which, in particular, reduces the need to travel significant distances to obtain treatment. In doing so, it emphasises the importance in gaining acceptance of both the use of MDT and also the operation of RPAS noting that these have previously been employed in a military, as distinct from humanitarian, context.

Originality/value

The paper demonstrates how the use of LE-RPAS to support remote communities offers the potential to deliver healthcare at reduced cost compared to conventional approaches. The paper also underlines the potential benefits of the use of MDT to address the growing wound burdens in remote communities. Finally, the paper expands on the existing discussion of the use of RPAS to include its capability to act as the delivery mechanism for medical maggots.

Purpose

Real-time monitoring of wound or injured tissues is critical for speedy recovery, and the onset of a cascade of biochemical reactions provides potential biomarkers that facilitate the process of wound monitoring, e.g. pH, temperature, moisture level, bacterial load, cytokines, interleukins, etc. Among all the biomarkers, pH has been known to have a profound impact on the wound healing process, and is used to determine the incidence of bacterial infection of the wound (persistently elevated alkaline pH), proteolytic activity at the site of injury, take rate in skin grafting, wound healing stage and preparation for wound debridement.

Design/methodology/approach

This review highlights the significance of pH in determination of clinical parameters and for selection of an appropriate treatment regime, and it presents an in-depth analysis of the designs and fabrication methods that use integrated pH sensors, which have been reported to date for the real-time monitoring of wound healing.

Findings

For an expedited wound healing process, the significance of pH mandated the need of an integrated sensor system that would facilitate real-time monitoring of healing wounds and obviate the requirement of redressing or complicated testing procedures, which are both labor-intensive and painful for the patient. The review also discussed different types of sensor systems which were developed using hydrogel as a pH-responsive system coupled with voltammetry, potentiometry, impedimetric and flex-circuit inductive transducer systems. All of the mentioned devices have considerable potential for clinical applications, and there is need of in vivo testing to validate their efficiency and sensitivity under practical scenarios.

Abstract

Purpose

Intense interest has been shown in creating new and effective biocide agents as a result of changes in bacterial isolates, bacterial susceptibility to antibiotics, an increase in patients with burns and wounds and the difficulty of treating infections and antimicrobial resistance. Woven, nonwoven and knitted materials are used to make dressings; however, nonwoven dressings are becoming more popular because of their softness and high absorption capacity. Additionally, textiles have excellent geometrical, physical and mechanical features including three-dimensional structure availability, air, vapor and liquid permeability, strength, extensibility, flexibility and diversity of fiber length, fineness and cross-sectional shapes. It is necessary to treat every burn according to international protocol and along with it has to focus on particular problems of patients and the best possible results.

Design/methodology/approach

The objective of this paper is to conduct a thorough examination of research pertaining to the utilization of textiles, as well as alternative materials and innovative techniques, in the context of burn wound dressings. Through a critical analysis of the findings, this study intends to provide valuable insights that can inform and guide future research endeavors in this field.

Findings

In the past years, there have been several dressings such as xeroform petrolatum gauze, silver-impregnated dressings, biological dressings, hydrocolloid dressings, polyurethane film dressings, silicon-coated nylon dressings, dressings for biosynthetic skin substitutes, hydrogel dressings, newly developed dressings, scaffold bandages, Sorbalgon wound dressing, negative pressure therapy, enzymatic debridement and high-pressure water irrigation developed for the fast healing of burn wounds.

Originality/value

This research conducts a thorough analysis of the role of textiles in modern burn wound dressings.

Abstract

Purpose

In advancing the academic discourse around the theory of field, place and space in ethnographic research, this paper proposes a carnal sociological reading of the meaning and form of the Lindsay Leg Clubs – third-sector community leg care centres for older adults with leg problems – as a therapeutic space-construct.

Design/methodology/approach

The paper draws on non-formulaic, polymorphic ethnographic research in the UK Lindsay Leg Clubs conducted through multiple on-site and extramural engagements with the Leg Club community between 2019 and 2023 and an interview with the Leg Club founder and president, Professor Ellie Lindsay OBE. Wacquant's (2015) reflexive, enactive ethnographic approach is applied to develop an intellectual and carnal know-how of Leg Clubs as therapeutic space-constructs.

Findings

The researcher's “flesh and blood” experience of the Leg Clubs reveals the importance of cognitive and embodied appreciation of sounds, smells, sights, movements, the structures of wound stigma, centre-stage physical bodies of members and the volunteers, the material arrangement of the place and the researcher's own visceral and intellectual, biographical relation to the fieldwork to understand the therapeutic form and meaning of Leg Club spaces.

Originality/value

Applying the carnal sociology approach to reveal the therapeutic form and meaning of the Leg Club spaces makes concrete the abstract distinctions between field, place and space in ethnographic research, hence advancing the discourse around the theory of field in ethnography. A carnal sociological reading of the Leg Club spaces has implications for an embodied understanding of broader community care spaces.

Abstract

Purpose

Wound healing is a dynamic process that relies on coordinated signaling molecules to succeed. Silk has proven to be a promising biomaterial for the development of a novel product. The purpose of the study is development of silk films, augmented functionality can be provided to silk by means of loading honey and recombinant human epidermal growth factor (rhEGF).

Design/methodology/approach

In this research work, the authors set out to explore possibilities of silk-based biomedical device development with particular attention to different fabrication strategies that can be leveraged for this purpose. They have produced a novel silk-based drug delivery material, in the form of silk films. Scanning electronic microscope was used to observe the morphology and the highly specific surface area. The structure was studied by Fourier-transform infrared spectroscopy. This methodology is accomplished using in vivo study data using Wister albonia rats.

Findings

The developed films also provided a significant higher healing rate in vivo, with well-formed epidermis with faster granulation tissue formation when compared to the controls. Biodegradable polymeric materials based on blending aqueous dispersions of natural polymer sodium alginate, Chitosan and rhEGF complex, which allow controlled antiseptic release, are presented.

Originality/value

These results suggest that silk-based controlled release of Chitosan-rhEGF may serve as a new therapy to accelerate healing of burn wounds.
