Abstract

Purpose

This study aims to explore the empirical correlation between patient flow issues, quality of green health services and patient satisfaction in specialist medical department factors from patients’ perspectives as service consumers.

Design/methodology/approach

This research is a type of nonintervention empirical research that uses an open survey to explore the views and experiences of users of specialist medical department services. The targeted population is hospital patients included in the top five national PERSI (Indonesian Hospital Association) Award 2022 Green Hospital Category, with a total number of respondents of 572 people. This study uses the partial least square-structural equation modeling analysis method with the SmartPLS application.

Findings

Patient flow problems generally affect the quality of eco-friendly health services, except for the waiting time problem, which affects service quality. It should be understood as a top priority for patients to receive services from medical specialists without risking time as a core service aspect from the patient’s perspective. In addition, all variables in eco-friendly hospital services affect patient satisfaction, except in the case of visits to specialist medical departments, which do not affect medical support services and hospital practices that are responsive to the delivery of care services resulting from medical support services that are inseparable in integrated services as well as health care following medical ethics.

Originality/value

This study has a novelty in understanding the implications of green practice in determining patient satisfaction in medical specialist department as the epicenter of hospital services and the main object of assessment for the quality of hospital services.

Abstract

Purpose

Doctor–medical institution collaboration (DMIC) services are an emerging service mode in focal online health communities (OHCs). This new service mode is anticipated to affect user satisfaction and doctors' engagement behaviors. However, whether and how DMIC occurs is still ambiguous because the topic is rarely examined. To bridge this gap, this study explores doctors' participation in DMIC services and its effects on their online performance, as well as its effect on patients' evaluation of them on OHC platforms.

Design/methodology/approach

The authors propose hypotheses based on structural holes theory. A unique dataset obtained from one of the most popular OHCs in China is used to test the hypotheses, and difference-in-differences estimation is adopted to test the causality of the relationship.

Findings

The results demonstrate that providing DMIC services improves doctors' online consultation performance and patients' evaluations of them but has no significant effect on doctors' knowledge-sharing performance on OHC platforms. Doctors' knowledge-sharing performance and consultation performance mediate the relationship between participation in DMIC services and patients' evaluation of doctors. Regarding doctors' participation in DMIC services, its impact on doctors' consultation performance and patients' evaluation of them is weaker for doctors with higher professional titles than for doctors with lower professional titles.

Originality/value

The findings clarify the value creation mechanisms of online collaboration between doctors and medical institutions and thereafter facilitate doctors' participation in DMIC services and enhance the sustainable development of OHCs.

Abstract

Purpose

The medical tourism industry has become one of the most profitable industries around the world wherein most of the countries exploit every effort to attract medical tourists to take advantage of its medical care benefits. The purpose of this study is to explore the determinants of medical tourists’ perceived services and their satisfaction for medical care in hospitals.

Design/methodology/approach

The quantitative method was conducted to examine the three groups of foreign travellers such as medical tourists, expats and normative medial tourists’ perceived medical services quality and satisfaction. Data from a survey of 266 respondents were analyzed using the structural equation modelling technique.

Findings

The findings of this study provide evidence that hospital accessibility and interpersonal behaviour are the most critical constructs that influence medical tourists’ attributes. Additionally, medical costs and health-care technicality have a significant relationship with the perceived services of medical tourists. The study also demonstrated that medical tourists’ satisfaction highly attributed to their perceived services towards the quality of medical care they received and experienced in the hospitals.

Practical implications

The results have important implications for managerial considerations at hospitals. Hospital accessibility is a crucial dimension to be measured at hospitals when measuring service quality. Service providers need to be mindful that all aspects of medical services are essential and need to be delivered satisfactorily to ensure customer satisfaction. Patients’ perceived services and their satisfaction is a crucial bridge in determining the likelihood of future return among patients to the hospitals.

Originality/value

This study has managed to convincingly secure findings to provide useful information and understanding of the hospital accessibility and interpersonal manner of health-care professionals at hospitals in Malaysia. Particularly, when any hospital offers quality services, they must consider the reasonable medical expenses that can be affordable by the average people and update their medical equipment that are necessary for technical and diagnostic purposes. By ensuring these, they can attract the medical tourists.

Abstract

Purpose

The purpose of this paper is to explore the rationality differences of cognition of non-technical medical services in different groups, and to provide countermeasures for improving non-technical medical services.

Design/methodology/approach

Literature analysis, expert interviews, questionnaire survey and frequency analysis were taken to reveal the influencing factors of non-technical medical services. Grey correlation methods were taken to compare the rationality differences of cognition of non-technical medical services by analysis influencing factors’ scores marked by different groups.

Findings

A total of 12 influencing factors of non-technical medical services were obtained, including “doctor’s working career”, “doctor’s strict implementation of medical treatment norms and medication guidelines”, “doctor’s service awareness”, etc. And rationality differences of cognition of non-technical medical services were confirmed as follows: the doctors’ cognition was more reasonable compared with patients; the women’s cognition was more reasonable compared with men; the lower aged groups’ cognition was more reasonable compared with higher aged groups; and people with doctoral degree had a less reasonable cognition compared with others.

Originality/value

The authors systematically discussed the cognition differences of non-technical medical services among different people, and provided some countermeasures reasonably.

Abstract

Purpose

The research topicality is stipulated by the need to study the legal regulation problems of the state financial guarantees of medical services for the population in Ukraine and to consider positive experience of the developed countries in this area. The aim of the article is to define the main trends in the legal regulation of healthcare in Ukraine; analyze the state financial guarantees of medical services for the population, and positive and negative aspects of the healthcare reform; to explore the international experience in medical services for its implementation to national law enforcement in the sphere of healthcare.

Design/methodology/approach

During the research, systematization and generalization of the positive international experience in the legal regulation of the state financial guarantees of medical services for the population were realized.

Findings

The analysis of the population's perception of healthcare innovations was carried out. The level of medical service was assessed. The results showed that in order to improve Ukrainian legislation on healthcare, it is necessary to remove absolute conflicts of normative legal acts; to establish public confidence in innovations and changes; to take into account international experience adapting it to specific features of Ukrainian legislation.

Originality/value

The defined theoretical and applied provisions constitute the practical significance for scholars and practicians, who deal with the legal regulation problems of the state financial guarantees of medical services for the population, university professors training highly qualified personnel, and all interested people concerned with healthcare.

Abstract

Purpose

In North America, delegated practice “medical direction” models are often used as a proxy for clinical quality and safety in paramedic services. Other developed countries favor a combination of professional regulatory boards and clinical governance frameworks that feature paramedics taking lead clinician roles. The purpose of this paper is to bring together the evidence for medical direction and clinical governance in paramedic services through the prism of paramedic self-regulation.

Design/methodology/approach

This narrative synthesis critically examines the long-established North American Emergency Medical Services medical direction model and makes some comparisons with the UK inspired clinical governance approaches that are used to monitor and manage the quality and safety in several other Anglo-American paramedic services. The databases searched were CINAHL and Medline, with Google Scholar used to capture further publications.

Findings

Synthesis of the peer-reviewed literature found little high quality evidence supporting the effectiveness of medical direction. The literature on clinical governance within paramedic services described a systems approach with shared responsibility for quality and safety. Contemporary paramedic clinical leadership papers in developed countries focus on paramedic professionalization and the self-regulation of paramedics.

Originality/value

The lack of strong evidence supporting medical direction of the paramedic profession in developed countries challenges the North American model of paramedics practicing as a companion profession to medicine under delegated practice model. This model is inconsistent with the international vision of paramedicine as an autonomous, self-regulated health profession.

Abstract

Purpose

The purpose of this paper is to propose a general model that examines the contextual factors underlying the decision‐making process of international hospital outshopping.

Design/methodology/approach

Patients who chose medical treatment abroad were selected to analyze the drivers of internationalization in medical services. A total number of 27 international patients who traveled from developed countries to receive medical treatment at the largest hospital in Thailand were interviewed and their responses assessed through narrative analysis.

Findings

The narrative analysis reveals that while high costs and the deteriorating conditions of health care in developed countries are initially driving consumers to leave their local service area and choose foreign service providers, pull factors such as innovation, organizational efficiency, emotional service quality and patient‐doctor relationships in service encounters are nurturing a real preference for choosing foreign health care providers.

Research limitations/implications

This exploratory study is limited to the largest hospital in South East Asia. Future research could expand upon its findings and comparisons be made with other different foreign health care providers.

Practical implications

In order to attract foreign customers a personal service, such as medical treatment, must be based on and sustained by continuous innovation in service quality.

Originality/value

The paper is the first empirical in‐depth study that examines the factors underlying the decision‐making process of international hospital outshopping.

Abstract

Purpose

To test the validity of the presumed characteristics of professional services by studying their manifestation in the problem solving that occurs in service production.

Design/methodology/approach

The paper uses medical research as secondary data to study the existence of associations between the presumed characteristics of professional services and problem solving in the medical context. A systematic review of empirical studies concerning physicians' prescribing decisions is conducted.

Findings

Supporting assumptions presented in the literature, specialist knowledge of professional and customer participation was found to influence prescribing decisions. The assumption regarding collegial control was partially supported. Some degree of contradiction was found with respect to the presumed professional autonomy and altruism. Whilst the professional services literature emphasises factors related to the client's problem, the service encounter and the profession, we conclude that problem solving is influenced also by factors embedded in the related organisational, market and institutional environments.

Research limitations/implications

Further empirical validation of the presumed professional characteristics is needed. The results indicate that professional services research should pay more attention to the role of the wider context in professional problem solving. Medical researchers might also benefit from a broader perspective on patient participation.

Practical implications

An holistic view of factors that influence physicians' prescribing decisions is of use to managers of health care organisations, marketers of pharmaceuticals, and policy makers and third‐party payers.

Originality/value

By using an interdisciplinary approach, the paper contributes to professional services research by providing empirical support for the often repeated characteristics of professional services and outlining factors that potentially influence problem solving within professional services.

Abstract

Purpose

Researchers recommend a reorganization of the medical profession into larger groups with a multispecialty mix. We analyze whether there is evidence for the superiority of these models and if this organizational transformation is underway.

Design/Methodology Approach

We summarize the evidence on scale and scope economies in physician group practice, and then review the trends in physician group size and specialty mix to conduct survivorship tests of the most efficient models.

Findings

The distribution of physician groups exhibits two interesting tails. In the lower tail, a large percentage of physicians continue to practice in small, physician-owned practices. In the upper tail, there is a small but rapidly growing percentage of large groups that have been organized primarily by non-physician owners.

Research Limitations

While our analysis includes no original data, it does collate all known surveys of physician practice characteristics and group practice formation to provide a consistent picture of physician organization.

Research Implications

Our review suggests that scale and scope economies in physician practice are limited. This may explain why most physicians have retained their small practices.

Practical Implications

Larger, multispecialty groups have been primarily organized by non-physician owners in vertically integrated arrangements. There is little evidence supporting the efficiencies of such models and some concern they may pose anticompetitive threats.

Originality/Value

This is the first comprehensive review of the scale and scope economies of physician practice in nearly two decades. The research results do not appear to have changed much; nor has much changed in physician practice organization.

Abstract

Purpose

According to the importance of strategic purchasing as an effective tool for resource allocation and service procurement, this study examines national laws, regulations and other related documents related to the strategic purchasing of health services related to the advanced medical equipment in Iran.

Design/methodolgy/approach

It was a national qualitative document analysis conducted in 2019 applying content analysis approach. The four-step Scott method was used to include the documents in terms of authenticity, credibility, representation and meaningfulness. After retrieving the related documents, they were coded with the implicit and explicit approach. MAXQDA10 was used for content analysis.

Findings

The findings show that according to the framework of effective factors on the strategic purchasing of health services, seven main factors are determined as the main essential factors in purchasing advanced medical equipment. These factors consist of health care providers, health service buyers, purchaser and service provider contracts, payment mechanisms, organization and management evaluation of health technology including expensive medical equipment and technology-related.

Research limitation/implication

The study had some limitations as follows: the proposed method should be tested and its feasibility has to be investigated through appropriate tools for Iranian insurance companies and those with the similar settings.

Practical implication

The results of this study can shed more light for policy makers affiliated in Ministry of Health as the main service provider, Ministry of Welfare and the insurance agencies as the main purchasers of health services on paying attention to these seven main themes extracted from the upstream documents and laws and regulations of the Islamic Republic of Iran.

Social implication

The strategic purchasing of expensive high technology-based medical equipment is a necessity for Iranian public health insurance organizations that is emphasized in national documents in the way of implementing this necessity.

Originality/value

This study examines all the laws and regulations and all related documents in the strategic purchasing of health services related to advanced medical equipment, giving an analysis of the most important challenges and requirements of implementing strategic purchasing in the health services provision sector with expensive medical equipment.
