Abstract

Every month this Journal records prosecutions arising out of the finding of glass in bottles of milk delivered to schools and it would be easy to get the impression that this particular hazard to which school‐children are daily exposed was one of considerable magnitude, and even that among those responsible for the bottling of milk there was a great deal of carelessness and negligence. In the Appendix to his recently published report for the year ending March 31st, 1957, Mr. J. A. O'Keefe, Chief Officer of the Public Control Department of the County of Middlesex, puts this matter of glass in school milk more in its proper perspective, and records the results of a nine months survey of the known incidence of glass in school milk supplied in Middlesex. Apparently head‐masters of all schools receiving milk were asked to report every case brought to their notice, and during the period April to December, 1956, 104 instances were recorded. The total number of bottles distributed during this time ran to something over 36 millions. During the same period, some 1,639 bottles were also sampled by the County Inspectors, all being examined for glass but with negative results. It would seem, then, that certainly as far as Middlesex is concerned, and doubtless this is true of other districts, the chances of a child finding glass in his or her milk are really very small indeed.

Abstract

The Protection of Consumers (Trade Descriptions) bill which, owing to the General Election, did not quite make the Statute Book in the last Parliament, is, at the moment of writing, passing through its readings, with every likelihood of becoming law in the near future. It has been criticised for the extent of the control to be exercised over general trading and that in “coddling the customer” it will place unreasonable responsibilities upon retailers. In fact, it is impossible to foresee just how far its provisions may extend, but there will be few who will disagree that new and more searching requirements are long overdue.

Abstract

At a recent meeting of the Glasgow Grocers' and Provision Merchants' Association, it was alleged that there are provision merchants in Glasgow who are doing a large business in selling margarine as butter at 1s. 2d. per pound. In commenting upon this statement The Grocer very properly urges that the officials of the Association referred to should take prompt steps to place the facts in their possession before the Glasgow authorities and their officers, and observes that in certain cities and towns—Birmingham, for example—the grocers' associations have co‐operated with the authorities in their efforts to suppress illegal trading, particularly in regard to the sale of margarine as butter. It appears that one of the members of the Glasgow Association expressed the opinion that the Margarine Act has been a failure and that shopkeepers who sell margarine as butter should be charged with obtaining money under false pretences.

Abstract

Since its origins during the Second World War, the computer industry has grown more rapidly than any other technology in history, and this growth has spawned a wealth of new terms and manners‐of‐speaking to describe computers and the uses to which they can be put. Such terms are often referred to collectively as computerese. The thesis of Barry's entertaining book is that the use of computerese is increasingly being extended to a wealth of other subjects that are often totally unrelated to computing. Barry refers to this use (or abuse) of language as technobabble: the subject matter and the pleasingly tongue‐in‐cheek style can be judged from the introduction, which starts as follows: ‘This paper‐based, productized bookware module is designed to support the robust implementation of a friendly, context‐driven interface between the developer and the end‐user. Did you understand this sentence? If so, you are fluent in technobabble’.

Abstract

Purpose

The purpose of this paper is to introduce librarians who are not necessarily music specialists to four books on a subset of American popular song literature (“the American songbook” or “standards”), all of which assume some degree of musical literacy on the reader's part. This assumption sets these books apart from most writing on the subject, and the paper suggest reasons why this is the case.

Design/methodology/approach

The books are grouped into pairs. The first two books are by a single author whose previous scholarship may make his interest in the American songbook seem surprising. In the other pair the second book is both a continuation of the first and a response to it. In both pairs the earlier book breaks new ground.

Findings

All four books should be regarded as essential parts of a library's music literature collection. The first book discussed is more technical than the others, but its pioneering status makes it a landmark.

Originality/value

The author's research indicates that the oldest of the books considered was not discussed in scholarly journals until many years after its publication, while the most recent appears to have received only glancing consideration in peer‐reviewed literature.

Abstract

Purpose

The purpose of this paper is to explore different health care professionals’ discourse about privacy – its definition and importance in health care, and its role in their day-to-day work. Professionals’ discourse about privacy reveals how new technologies and laws challenge existing practices of information control within and between professional groups in health care, with implications not only for patient privacy, but also for the role of information control in professions more generally.

Design/methodology/approach

The authors conducted in-depth, semi-structured interviews with n=83 doctors, nurses, and health information professionals in two academic medical centers and one veteran’s administration hospital/clinic in the Northeastern USA. Interview responses were qualitatively coded for themes and patterns across groups were identified.

Findings

The health care providers and the authors studied actively sought to uphold the protection (and control) of patient information through professional ethics and practices, as well as through the use of technologies and compliance with legal regulations. They used discourses of professionalism, as well as of law and technology, to sometimes accept and sometimes resist changes to practice required in the changing technological and legal context of health care. The authors found differences across professional groups; for some, protection of patient information is part of core professional ethics, while for others it is simply part of their occupational work, aligned with organizational interests.

Research limitations/implications

This qualitative study of physicians, nurses, and health information professionals revealed some differences in views and practices for protecting patient information in the changing technological and legal context of health care that suggest some professional groups (doctors) may be more likely to resist such changes and others (health information professionals) will actively adopt them.

Practical implications

New technologies and regulations are changing how information is used in health care delivery, challenging professional practices for the control of patient information that may change the value or meaning of medical records for different professional groups.

Originality/value

Qualitative findings suggest that professional groups in health care vary in the extent of information control they have, as well in how they view such control. Some groups may be more likely to (be able to) resist changes in the professional control of information that stem from new technologies or regulatory policies. Some professionals recognize that new IT systems and regulations challenge existing social control of information in health care, with the potential to undermine (or possibly bolster) professional self-control for some but not necessarily all occupational groups.

Abstract

Purpose

The purpose of this paper is to outline the “Hybrid” “International” Emergency Medicine (HIEM) programme, which is an ethical pathway for the recruitment, employment and training of Emergency Medicine doctors; with a rotation through the NHS on a two-year medical training initiative with a Tier 5 visa, “earn, learn and return” programme. The HIEM programme offers an advantage to the Tier 2 visa by combining training, education and employment resulting in new learning to help improve the health system in Nepal and provide continued cultural support, clinical and leadership development experience in the UK NHS. Finally, this programme also provides a Return on Investment to the NHS.

Design/methodology/approach

A shortage of doctors in the UK, combined with a need to develop Emergency Medicine doctors in Nepal, led to a UK Emergency Medicine Physician (PS) to facilitate collaboration between UK/Nepal partners. A mapping exercise of the Royal College of Emergency Medicine curriculum with the competencies for the health system and quality improvement leaders and partners with patients produced a “HIEM programme”. The HIEM programme aims to develop first-class doctors to study in Emergency Departments in the UK NHS while also building trainee capability to improve the health system in Nepal with a research thesis.

Findings

The HIEM programme has 12 doctors on its programme across years one and two, with the first six doctors working in the UK NHS and progressing well. There are reports of high levels of satisfaction with the trainees in their transition from Nepal to the UK and the hospital is due to save £720,000 (after costs) over two years. Each trainee will earn £79,200 over two years which is enough to pay back the £16,000 cost for the course fees. Nepal as a country will benefit from the HIEM programme as each trainee will submit a health system improvement Thesis.

Research limitations/implications

The HIEM programme is in its infancy as it is two years through a four-year programme. Further evaluation data are required to assess the full impact of this programme. In addition, the HIEM programme has only focussed on the development of one medical speciality which is Emergency Medicine. Further research is required to evaluate the impact of this model across other medical and surgical specialties.

Practical implications

The HIEM programme has exciting potential to support International Medical Graduates undertake a planned programme of development while they study in the UK with a Tier 5 visa. IMGs require continuous support while in the UK and are required to demonstrate continued learning through continuous professional development (CPD). The HIEM programme offers an opportunity for this CPD learning to be structured, meaningful and progressive to enable new learning. There is also specific support to develop academic and research skills to undertake a thesis in an area that requires health system improvement in Nepal.

Originality/value

This is the first time an integrated clinical, leadership, quality improvement and patient partnership model curriculum has been developed. The integrated nature of the curriculum saves precious time, money and resources. The integrated nature of this “hybrid” curriculum supports the development of an evidence-based approach to generating attitudes of collaboration, partnership and facilitation and team building in medical leadership with patient engagement. This “hybrid” model gives hope for the increased added value of the programme at a time of global austerity and challenges in healthcare.
