Abstract

Purpose

Past research has shown individuals with autism spectrum disorder (ASD) visit hospital emergency departments (ED) at high rates. In order to assist individuals with ASD, their families and health care providers to improve ED care, it is important to understand these encounters in greater detail. The purpose of this paper is to provide a descriptive summary of the ED experiences of adolescents and adults with ASD, from the perspective of their families.

Design/methodology/approach

A subset of data from a larger prospective cohort study was used. Specifically, 46 parents of adolescents and adults with ASD provided details concerning 49 ED visits over a 12-month period.

Findings

Results suggest a range of presentations requiring ED use, and also diverse profiles of those with ASD who visited the ED, in terms of age, gender, and ASD severity. While overall degree of satisfaction with care received in the ED was high, parents provided recommendations to improve the ED experiences for their family members with ASD.

Originality/value

This is the first study to provide detailed accounts of ED visits from the perspective of parents of adolescents and adults with ASD. Families play an important role in the lives of individuals with ASD across the lifespan and it is important to include their perspective to improve hospital-based care for those with ASD.

Abstract

It is customary these days to denounce our society for its unconscionable neglect of the elderly, while we look back romantically to some indeterminate past when the elderly were respected and well cared for. Contrary to this popular view, old people historically have enjoyed neither respect nor security. As Simone de Beauvoir so effectively demonstrates in The Coming of Age (New York: Putnam, 1972), the elderly have been almost universally ill‐treated by societies throughout the world. Even the Hebrew patriarchs admonished their children to remember them as they grew older: “Cast me not off in time of old age; when my strength fails, forsake me not” (Psalms 71:1). Primitive agrarian cultures, whose very existence depended upon the knowledge gleaned from experience, valued their elders, but even they were often moved by the harsh conditions of subsistence living to eliminate by ritual killing those who were no longer productive members of society. There was a softening of societal attitudes toward the elderly during the period of nineteenth century industrial capitalism, which again valued experience and entrepreneurial skills. Modern technocratic society, however, discredits the idea that knowledge accumulates with age and prefers to think that it grows out‐of‐date. “The vast majority of mankind,” writes de Beauvoir, “look upon the coming of old age with sorrow and rebellion. It fills them with more aversion than death itself.” That the United States in the twentieth century is not alone in its poor treatment of the aged does not excuse or explain this neglect. Rather, the pervasiveness of prejudice against the old makes it even more imperative that we now develop programs to end age discrimination and its vicious effects.

Abstract

Purpose

The purpose of this paper is to critically examine the instruments used in the screening process, with particular attention given to supporting research validation. Psychological screening is a well-established process used in the selection of employees across public safety industries, particularly in police settings. Screening in and screening out are both possible, with screening out being the most commonly used method. Little attention, however, has been given to evaluating the comparative validities of the instruments used.

Design/methodology/approach

This review investigates literature supporting the use of the Minnesota Multiphasic Personality Inventory (MMPI), the California Personality Inventory (CPI), the Inwald Personality Inventory (IPI), the Australian Institute of Forensic Psychology's test battery (AIFP), and some other less researched tests. Research supporting the validity of each test is discussed.

Findings

It was found that no test possesses unequivocal research support, although the CPI and AIFP tests show promise. Most formal research into the validity of the instruments lacks appropriate experimental structure and is therefore less powerful as “evidence” of the utility of the instrument(s).

Practical implications

This research raises the notion that many current screening practices are likely to be adding minimal value to the selection process by way of using instruments that are not “cut out” for the job. This has implications for policy and practice at the recruitment stage of police employment.

Originality/value

This research provides a critical overview of the instruments and their validity studies rather than examining the general process of psychological screening. As such, it is useful to those working in selection who are facing the choice of psychological instrument. Possibilities for future research are presented, and development opportunities for a best practice instrument are discussed.

Abstract

Purpose

The paper's aim is to consider the effects of recurrent economic crisis on the management and organisational structures of transnational companies based in the UK by considering contemporary evidence and scholarly views of the processes involved, and especially to consider the contributions of the papers that follow in this special issue of the journal.

Design/methodology/approach

The paper provides a summary overview of some of the key evidence and arguments concerning the origin of recent continuing crises in Western capitalism. The paper places in context and assesses the contribution of five papers especially selected by the editors to be included in this special issue which bear on different aspects.

Findings

The paper suggests several key processes are at work in contemporary capitalism, which can be summed up as increasing financialisation. This is the process by which businesses are increasingly orientated to the extraction of value, and success is primarily assessed in terms of the rate of return to capital employed. In their different ways the papers in the special issue illustrate aspects of the process of financialisation, including: the operation of a new set of financial institutions including private equity and hedge funds, and the effects of these on the various policies and priorities of the executive leaders of large businesses in such areas as human resource management and the adoption of new organisational forms. The discussion extends to the consideration of the effects of change on international finance and argues for the origins of changes in changed class relations.

Research limitations/implications

The implications of this work are that the character and reach of current economic change are further illuminated – including especially the underlying causes of the present economic crisis.

Social implications

Financialisation represents both a reorganisation of the processes of capitalist production and a class strategy of international elites to entrench their advantages in the new conditions of international political economy opened through the intellectual and policy triumph of neoliberal thinking.

Originality/value

The work brings together scattered insights and viewpoints and builds them into a coherent synthesis. It thus moves beyond limited conceptions and insights.

Abstract

Purpose

There is inadequate health care for patients with developmental disabilities (DD), due to a number of systemic issues. This case study describes the establishment of a medical-dental clinic in Montréal, Québec for adults with DD. The purpose of this paper is to describe the model of interdisciplinary care based on best practices, as an example to encourage a growing community of trained health professionals to serve this population.

Design/methodology/approach

Interviews with all the clinic staff and leadership were conducted on-site at the clinic, followed by document review and discussions with an embedded researcher in the organization.

Findings

The clinic was established through a series of events that led to public and government interest to act, the timely emergence of major donors, and bringing together several dedicated individuals and organizations. The core team engaged in consultation with clinics, followed by extensive billing analyses and iterative process mapping as a learning organization. Prior to patient visits, the clinic conducted detailed intake processes to adequately plan for each patient interaction. Desensitization visits were undertaken to improve patient tolerance for examination and treatment. The continual collection of data fed into an evaluation framework to facilitate continuous improvement and articulate a model for replication.

Originality/value

To the best of the authors knowledge, there is not a clinic of this nature serving this population in Canada. This work can serve to inform the efforts of other care providers looking to create a medical – dental home for this population.

Abstract

Purpose

The purpose of this paper is to analyze determinants of farmers’ participation and credit rationing in microcredit programs using survey data from Ghana.

Design/methodology/approach

The authors use the Garrett Ranking Technique to analyze farmers’ reasons for participation or non-participation in credit programs, a probit regression model to estimate factors influencing farm households’ participation, and the Heckman’s sample selection model to identify factors influencing farm households’ probability of being credit rationed by microcredit programs.

Findings

The results reveal that farm households participate in credit programs because of improved access to savings services and agricultural loans. Fear of loan default and lack of savings are reasons for non-participation in credit programs. Furthermore, membership in farmer-based organizations (FBOs) and the household head’s formal education are positively associated with farmers’ participation in credit programs. The likelihood of farmers being credit rationed (i.e. their loan applications were either rejected or the amount of credit they applied for was reduced) is less likely among higher income farmers and members of FBOs such as farmer cooperatives and savings clubs.

Practical implications

The findings suggest that policy strategies aiming to improve access to savings and credit services should educate farmers and strengthen FBOs that could serve as entry points for financial service providers. Such market smart strategies have the potential to improve farmers’ access to financial services and reduce rural poverty.

Originality/value

Although existing studies have examined farmers’ participation in credit markets and credit rationing separately, the unique contribution of this paper is the analysis of participation in microcredit programs as well as the likelihood of farmers being credit rationed in Ghana.

Abstract

Purpose

The purpose of this paper is to assess the wide-ranging implications of the global economic crisis and provide a comprehensive assessment of the how the structure of the market and competition within it is changing as a result.

Design/methodology/approach

The research methodology draws on a “financialization” market construct and adapts it to include the public-private interfaces (PPIs) that have appeared since the global economic crisis.

Findings

The crisis has turned the global system on a dime. The decades-long surge of globalization, as characterized by market liberalization and ever more fast-paced investment flows, has abated and, in some cases, been dramatically reversed. It has altered the international investment paradigm. Firms have revised their risk functions and are re-arranging their stakeholder relationships.

Research limitations/implications

Much needs to be done to assess the wide-ranging implications of the most recent crisis. This is just one set of “snapshots”, if you will, of the way in which market structure and competition are being altered.

Originality/value

The re-arrangement of stakeholder relationships of both privately owned firms and sovereign enterprises will have far-reaching effects on market structure in such areas as market access and competition, as well as on civil society, writ large.

Abstract

Purpose

The purpose of this article is to examine the role of social capital and higher order meta-skills in developing the employability of marketing students at a UK university.

Design/methodology/approach

This conceptual article, bolstered by illustrative primary data, provides a broader conceptualisation of employability. This is to address the specific research question on how social capital (contacts and connections) is deployed (via capability-based higher order meta-skills) in a UK university developing the employability of a specific group of students. The article is situated in the highly fraught context of teaching excellence measurement schemes [such as the teaching excellence framework (TEF) in the UK].

Findings

The research findings highlighted the role of social capital and higher order meta-skills in developing the employability of marketing students at a UK university.

Research limitations/implications

While the illustrative primary data are not generalisable, as they are limited to one group of marketing students in one UK university; the conceptual development, including a new social capital based definition of employability that incorporated the capabilities, provided by higher-order meta-skills, is widely applicable.

Practical implications

The article has highlighted how the impact of social capital, etiquette and meta-skills, while being “between the lines” of the employability discourse and the metrics of the TEF, explains the differing perceptions of the value of employability initiatives. The article highlights the grey area of between the reasons given as to why some candidates are valued over others. Perhaps no rhyme or reason sometimes, just the “hidden” perception/interpretations of the interview panel of the “qualities” of one candidate over another.

Originality/value

The difficulty in ascertaining the influence of social capital (and how it can be deployed through higher-order meta-skills as capabilities) results in challenges for universities as they endeavour to respond to the data requirements of “learning gain” within teaching excellent measurement schemes such as the UK teaching excellence framework.

Abstract

Purpose

Neo-liberal “reform” has in many countries shifted services across the boundary between the public and private sector. This policy re-opens the question of what structural and managerial differences, if any, differences of ownership make to healthcare providers. The purpose of this paper is to examine the connections between ownership, organisational structure and managerial regime within an elaboration of Donabedian’s reasoning about organisational structures. Using new data from England, it considers: how do the internal managerial regimes of differently owned healthcare providers differ, or not? In what respects did any such differences arise from differences in ownership or for other reasons?

Design/methodology/approach

An observational systematic qualitative comparison of differently owned providers was the strongest feasible research design. The authors systematically compared a maximum variety (by ownership) sample of community health services; out-of-hours primary care; and hospital planned orthopaedics and ophthalmology providers (n=12 cases). The framework of comparison was the ownership theory mentioned above.

Findings

The connection between ownership (on the one hand) and organisation structures and managerial regimes (on the other) differed at different organisational levels. Top-level governance structures diverged by organisational ownership and objectives among the case-study organisations. All the case-study organisations irrespective of ownership had hierarchical, bureaucratic structures and managerial regimes for coordinating everyday service production, but to differing extents. In doctor-owned organisations, the doctors’, but not other occupations’, work was controlled and coordinated in a more-or-less democratic, self-governing ways.

Research limitations/implications

This study was empirically limited to just one sector in one country, although within that sector the case-study organisations were typical of their kinds. It focussed on formal structures, omitting to varying extents other technologies of power and the differences in care processes and patient experiences within differently owned organisations.

Practical implications

Type of ownership does appear, overall, to make a difference to at least some important aspects of an organisation’s governance structures and managerial regime. For the broader field of health organisational research, these findings highlight the importance of the owners’ agency in explaining organisational change. The findings also call into question the practice of copying managerial techniques (and “fads”) across the public–private boundary.

Originality/value

Ownership does make important differences to healthcare providers’ top-level governance structures and accountabilities and to work coordination activity, but with different patterns at different organisational levels. These findings have implications for understanding the legitimacy, governance and accountability of healthcare organisations, the distribution and use power within them, and system-wide policy interventions, for instance to improve care coordination and for the correspondingly required foci of healthcare organisational research.
