The original concept for this book arose from Michael Calnan's sociological analysis of health policy. Both authors contributed equally to the development of the central ideas and the analysis of documentary material comprising this book. Chapters One and Four were co-authored. Michael Calnan led the analysis and writing for Chapter Two. Tom Douglass led the analysis and writing for Chapter Three. Author order is alphabetical.
