The original concept for this book arose from Michael Calnan's sociological analysis of health policy. Both authors contributed equally to the development of the central ideas and the analysis of documentary material comprising this book. Chapters One and Four were co-authored. Michael Calnan led the analysis and writing for Chapter Two. Tom Douglass led the analysis and writing for Chapter Three. Author order is alphabetical.

Michael Calnan: Thank you to the undergraduate and postgraduate students from the School of Social Policy, Sociology and Social Research at the University of Kent for interesting discussion and debate about some of the themes covered in this book.

Tom Douglass: Thank you to my parents, and my partner, Abbie, for many years of love and support. I must additionally thank Dr Catrin Rhys and the rest of the ‘Real Complaints’ team at Ulster and Stirling universities for their encouragement and for facilitating the space to work on this book.
