Abstract

Purpose

In healthcare, there is limited knowledge of and experience with patient choice management. The purpose of this paper is to focus on patient choice, apply and test demand-supply-based operating (DSO) logic integrated with clinical setting in clarifying choice contexts, investigate patient’s choice-making at different contexts and suggest context-based choice architectures to manage and develop patient choice.

Design/methodology/approach

Prenatal screening and testing in the Helsinki and Uusimaa Hospital District (HUS), Finland, was taken as an example. Choice points were contextualized by using the DSO framework. Women’s reflections, behaviors and experience at different choice contexts were studied by interviewing women participating in prenatal screening and testing. Semi-structured interview data were processed by thematic analysis.

Findings

By applying DSO logic, four choice contexts (prevention, cure, electives and continuous care) were relevant in the prenatal screening and testing episode. Women had different choice-making in prevention and cure mode contexts regarding choice activeness, information needs, social influence, preferences, emotion status and choice-making difficulty. Default choice was widely accepted by women in prevention mode and individual counseling can help women make informed choice in cure mode.

Originality/value

The authors apply the DSO model to contextualize the patient choice in one care episode and compare patient choice-making at different contexts. The authors also suggest the possible context-based choice architectures to manage and promote patient choice

Abstract

Purpose

The main aim of this paper is to examine how commercial sustainability innovation projects in real estate and construction industries utilise the contemporary market‐oriented innovation models based on the service‐dominant logic.

Design/methodology/approach

A survey of a large set of sustainability innovation projects was conducted and analysed.

Findings

The results show that over a third of the examined projects lack all the necessary innovation components recognised in the literature. Furthermore, very few projects utilised concurrent market feedback in the development phase. The study suggests that sustainability innovations fail commercially in built environment, because they lack active customer participation and value network involvement, and they aim for incremental instead of radical improvements.

Research limitations/implications

Future research should address available ways for companies in the real estate and construction industries to take advantage of their unused potential in climate change mitigation business through innovation.

Practical implications

Organisations striving for sustainability innovations should aim at radical instead of incremental innovation, and focus on customer participation and involvement of the value network. The innovation processes in the construction sector could be organised better by utilising tested service‐dominant models.

Originality/value

This study is among the first to address innovation elements based on the service‐dominant logic in analysing projects aiming at sustainability innovation in real estate and construction industries.

Abstract

Purpose

The rise of knowledge work has entailed controversial characteristics for well-being at work. Increased intensification, discontinuities and interruptions at work have been reported. However, knowledge workers have the opportunity to flexibly adjust their work arrangements to support their concentration, inspiration or recuperation. The purpose of this paper is to examine whether the experienced well-being of 46 knowledge workers was subject to changes during and after a retreat type telework period in rural archipelago environment.

Design/methodology/approach

The authors conducted a longitudinal survey among the participants at three points in time: one to three weeks before, during, and two to eight weeks after the period. The authors analyzed the experienced changes in psychosocial work environment and well-being at work by the measurement period by means of repeated measures variance analysis. In the next step the authors included the group variable of occupational position to the model.

Findings

The analysis showed a decrease in the following measures: experienced time pressure, interruptions, negative feelings at work, exhaustiveness of work as well as stress and an increase in work satisfaction. There were no changes in experienced job influence, clarity of work goals and work engagement. Occupational position had some effect to the changes. Private entrepreneurs and supervisors experienced more remarkable effects of improvement in work-related well-being than subordinates. However, the effects were less sustainable for the supervisors than the other two groups.

Originality/value

This paper provides insights into how work and well-being are affected by the immediate work environment and how well-being at work can be supported by retreat type telework arrangements.

Abstract

Purpose

The purpose of this paper is to examine the roles and dynamics of different institutional actors in the innovation process and to demonstrate how discontinuous sustainability innovation depends on the support of a group of actors wider than the innovating consortium. The paper explores how discontinuous sustainability innovation comes about in the construction industry. More specifically, it focuses on the understudied area of low-technology innovation.

Design/methodology/approach

The paper reports the findings of a detailed longitudinal single case study. The case in question is the K3 houses initiative: the Finnish Cultural Foundation’s attempt to point out a low-technology path to energy-efficient detached houses. The empirical data consist of semi-structured interviews and extensive archival data. A visual mapping strategy is used to structure the qualitative case analysis.

Findings

The paper reports two main findings. First, environmentally oriented regulation can sometimes hamper sustainability innovation by encouraging a lock-in into a technological trajectory that limits opportunities for discontinuous innovation. Second, an innovation-centric rather than a firm-centric approach to discontinuous innovation can benefit both firms managing and researchers studying innovation.

Originality/value

The paper is one of the few discussing the potential of low-technology innovation in construction for sustainable development. The introduction of the concept of change-based momentum adds a novel dynamic element to the inter-organisational models of construction innovation. Its implications for discontinuous innovation are illustrated with a gravitational slingshot analogy.

Abstract

Purpose

The study aims to investigate the effects of a remote energy management service to the energy consumption of retail buildings. The study focuses on analysing the changes in energy consumption after the implementation of a facility service concept where building processes are optimized with a remote energy management system. The paper seeks to demonstrate that remotely operated building management practices, which allow high competence service for all facilities, have a positive impact, beyond traditional facility services, on energy and environmental performance of buildings.

Design/methodology/approach

The research analyses the metered energy consumption of two retail building portfolios comprising altogether 44 properties. Additionally, secondary data are collected from archive reviews, observation and interviews.

Findings

The research shows that remote energy management service reduced the total energy consumption during the two-year service period by 12 and 6 per cent depending on the portfolio. Electricity consumption was found to decrease by 7 per cent and heating energy by 26 per cent on the average in the first portfolio, and 7 and 4 per cent in the second one, respectively.

Research limitations/implications

Variation between buildings was found to be relatively high as the individual characteristics and history of the different buildings inevitably affect the achieved results.

Practical implications

The study indicates that remote energy management offers an effective means to reduce the energy consumption and costs, and ultimately climate impacts derived from buildings.

Originality/value

The study adds to the knowledge of facilities management in context to energy management and environmental performance of buildings.
