Abstract

Purpose

While educators impart FMEA instruction, its practical implementation within the educational sector remains limited. This study investigates the application of FMEA within higher education institutions. Implementing FMEA in these institutions is difficult due to statutory requirements, schedule restrictions, and stakeholder participation challenges. Moreover, higher education institutions struggle with preserving education quality, faculty training, and resource management, complicating organised methods such as FMEA.

Design/methodology/approach

This research conducted a global survey to identify the critical success factors, benefits, and common challenges in using FMEA in the higher education sector.

Findings

The outcomes highlighted that lack of awareness regarding the tools’ benefits is the primary barrier to FMEA implementation. However, respondents perceive that FMEA can improve process reliability and quality in higher education institutions. Further, the analyses found that knowledge about the FMEA tools is the prime critical success factor, and the lack of time due to other priorities in the organisation is a significant challenge in tapping the potential of FMEA.

Research limitations/implications

A limitation of the study is the relatively low number of HEIs surveyed globally. Further, the study provides a broad perspective rather than a focused study on one HEI.

Practical implications

This study addresses this gap by exploring the potential benefits, challenges, and factors associated with the successful adoption of FMEA in academic settings. Using this information, HEIs can become more successful in applying FMEA.

Originality/value

This study is unique in its exploration of the application of FMEA with higher education institutions for service quality improvement.

Abstract

Purpose

In light of the recent changes in the internal audit (IA) landscape, the role of auditors has undergone a significant transformation. This paper aims to investigate the effects of applying Lean Six Sigma (LSS) techniques on the effectiveness and efficiency of IA.

Design/methodology/approach

The study used a quantitative approach, surveying Iranian internal auditors with a sample size of 384 participants. Data analysis involved confirmatory factor analysis and structural equation modeling.

Findings

The analyses demonstrate a significant association between LSS application and IA effectiveness and efficiency. In addition, an exploratory analysis indicates that the application of LSS techniques by less experienced internal auditors had a reverse effect on IA function quality as a component of IA competency. However, IA motivation factors, including education and position, did not mediate the impact of LSS on IA effectiveness and efficiency.

Research limitations/implications

This study was conducted with Iranian internal auditors, which may limit the generalizability of the findings to other countries. However, the primary academic implication of this research lies in its novel perspective on emphasizing the concept of continuous improvement in IA through the use of LSS techniques. By focusing on the need for internal auditors to add value to the business in new ways, this research contributes to the literature on IA quality.

Practical implications

This study has significant implications for the effective management of IA departments. By promoting the application of LSS techniques in IA, lean auditing is enhanced, and IA can create value by improving the quality of its functions. Moreover, IA regulators can benefit from this study as it emphasizes providing guidance and training on LSS techniques to enhance IA skills.

Originality/value

This research is pioneering in applying LSS methodology to enhance the effectiveness and efficiency of internal auditing. It also considers the integration of lean thinking into current audit practices, making it unique and valuable in internal auditing research.

Abstract

Purpose

This paper aims to analyse the current state of research to identify the link between Lean Manufacturing and Industry 4.0 (I4.0) technologies to map out different research themes, to uncover research gaps and propose key recommendations for future research, including lessons to be learnt from the integration of lean and I4.0.

Design/methodology/approach

A systematic literature review (SLR) is conducted to thematically analyse and synthesise existing literature on Lean Manufacturing–I4.0 integration. The review analysed 60 papers in peer-reviewed journals.

Findings

In total, five main research themes were identified, and a thematic map was created to explore the following: the relationship between Lean Manufacturing and I4.0; Lean Manufacturing and I4.0 implication on performance; Lean Manufacturing and I4.0 framework; Lean Manufacturing and I4.0 integration with other methodologies; and application of I4.0 technologies in Lean Manufacturing. Furthermore, various gaps in the literature were identified, and key recommendations for future directions were proposed.

Research limitations/implications

The integration of Lean Manufacturing and I4.0 will eventually bring many benefits and offers superior and long-term competitive advantages. This research reveals the need for more analysis to thoroughly examine how this can be achieved in real life and promote operational changes that ensure enterprises run more sustainably.

Originality/value

The development of Lean Manufacturing and I4.0 integration is still in its infancy, with most articles in this field published in the past two years. The five main research themes identified through thematic synthesis are provided in the original contribution. This provides scholars better insight into the existing literature related to Lean Manufacturing and I4.0, further contributing to defining clear topics for future research opportunities. It also has important implications for industrialists, who can develop more profound and richer knowledge than Lean and I4.0, which would, in turn, help them develop more effective deployment strategies and have a positive commercial impact.

Abstract

Purpose

Quality has always been a competitive advantage for every organisation that strives for customer satisfaction when offering services or products. Technological advancements during the industrial revolutions have enabled organisations to grub improvement and transition opportunities into a new paradigm in operating business processes. In light of the fourth industrial revolution (FIR), in which Quality 4.0 (Q4.0) leveraged its technologies, this study establishes the need for service organisations to transition to Q4.0. It unveils the awareness level within the organisation, the existing challenges and the benefits of transitioning to Q4.0.

Design/methodology/approach

The data for this study was acquired through a survey methodology; it involved qualitative and quantitative methodologies and a mix of primary and secondary sources. The challenges and benefits regarding Q4.0 adoption were obtained from the relevant literature and used as a base of assessment in the selected service organisations. Minitab version 20 and SPSS 21.0 software packages analysed the gathered data.

Findings

The study found a high level of awareness regarding Q4.0 among the selected service organisations. Despite the high level of awareness, it was revealed that neither of the Q4.0-related technologies have been implemented nor in the process within the selected service industries. This is due to numerous challenges, including inadequate high-speed internet, a high cost of investment, inadequate skilled personnel and inadequate scepticalness to the implementation outcomes. Despite these challenges, leveraged technologies, potential benefits and Q4.0 awareness all demonstrate the need for Q4.0.

Research limitations/implications

The study introduces the advent of the FIR and the disruptive nature of the associated technologies. It also unveils the potential of the contemporary technologies of the FIR that could elevate quality service provision to increase their competitiveness. Moreover, it also assists service organisations in planning and properly allocating their resources to ensure all the challenges are addressed in the Q4.0 adoption process. The study is limited in that it merely considers service organisations in Tanzania without categorising what type of service organisations were considered, i.e. banking, telecommunications, health, etc. thus generalising the findings.

Originality/value

Much has been discussed in the literature regarding Q4.0 in manufacturing organisations, focusing less on service organisations. This study uniquely assessed the need to transition to Q4.0 for service organisations, which has yet to be covered in the literature.

Abstract

Purpose

Health-care organisations face many challenges in delivering safe, high-quality services while experiencing significant pressure to increase productivity and reduce costs. In this context, hospitals have implemented lean six sigma (LSS) programmes to improve their performance. This study aims to explore the application of LSS in three different non-profit Ecuadorian hospitals to comprehend the effectiveness of the methodology under this context.

Design/methodology/approach

A multiple-case analysis was performed in four phases: selecting the cases, defining a data collection protocol, performing a within-case analysis of each case and performing a cross-case analysis.

Findings

This research found that the LSS application positively impacts hospital performance indicators by reducing service time. The most frequently used tools include the supplier input process output customer diagram, value stream mapping, cause-and-effect diagram, five-why analysis, Gemba walk and paired two-sample test. Lastly, the results show that the most common challenges faced were lack of top management engagement, technical training and data availability.

Research limitations/implications

The study is limited by the constraint of a single Latin American country from which the cases were analysed. Collaboration with external partners, like universities, and government policies promoting training in continuous improvement methodologies are crucial for success. Academic implications stress the importance of integrating soft skills in LSS implementation and engineering education.

Originality/value

This study shows a multiple-case analysis of LSS in a Latin American country highlighting the most commonly used tools, their impact on performance and the challenges of implementing LSS in health-care organisations in non-profit Ecuadorian hospitals.

Abstract

Purpose

This study assesses postreplanting oil palm farming risks, analyzes seed procurement parameters, investigates seed institutions' performance factors and develops a framework for improved sustainability.

Design/methodology/approach

Incorporating data from 219 smallholder farmers in designated replanting areas, our study comprehensively evaluates seed supply performance, examining the roles of stakeholders and identifying potential risks in seed management. We assess these risks using the Risk Priority Number (RPN) methodology and Multidimensional Scaling (MDS) techniques.

Findings

The results show that the timing and quantity of oil palm seed supply have a relatively small impact on postreplanting failure risk. To mitigate this risk, focus on monitoring seed purity using high-quality Tenera oil palm-type seeds and early detection technology. Encourage seed-producing cooperatives to become legal seed producers for an inclusive system and consider smallholders' variety preferences.

Originality/value

This study’s significance lies in its comprehensive assessment of the risks associated with oil palm replanting on smallholder plantations, detailed analysis of critical parameters in seed procurement, investigation into the performance of palm oil seed institutions across various dimensions and development of a strategic framework to strengthen inclusive seed institutions for sustainable oil palm farming. This strategy holds valuable potential for the development of oil palm in Indonesia, particularly in expediting the smallholders' replanting program.

Abstract

Purpose

With the growing need for employees to be innovative, public-sector organizations are investing in employee training. This study aims to examine the effects of a combined Lean Six Sigma and innovation training, using action learning, on public-sector employees’ creative role identity and innovative work behavior.

Design/methodology/approach

The authors studied a public service agency in Singapore in which a five-day Lean Innovation Training was implemented, using a combination of Lean Six Sigma and Creative Problem-Solving tools, with a simulation on day one and subsequent team-based project coaching, spread over six months. The authors administered pre- and postintervention surveys among all the employees, and initiated group interviews and observations before, during and after the intervention.

Findings

Creative role identity and innovative work behavior had significantly improved six months after the intervention, enabled through senior management’s transformational leadership. The training induced managers to role-model innovative work behaviors while cocreating, with their employees, a renewal of their agency’s core processes. The three completed improvement projects contributed to an innovative work culture and reduced service turnaround time.

Originality/value

Starting with a role-playing simulation on the first day, during which leaders and followers swapped roles, the action-learning type training taught all the organizational members to use various Lean Six Sigma and Creative Problem-Solving tools. This nimble Lean Innovation Training, and subsequent team-based project coaching, exemplifies how advancing the staff’s creative role identity can have a positive impact.

Purpose

Patient safety is a top priority globally. A robust healthcare system requires strategic collaboration between research and development. The author analysed over 300 cases from seven hospitals using the failure modes, effects, and criticality analysis (FMECA) tool to understand the underlying causes of medical errors.

Design/methodology/approach

The author studied seven hospitals and 300 cases using FMECA to prioritise activities. The findings showed that high-priority events occurred less frequently but had the potential to cause the most harm. Team members evaluated independently to ensure unbiased evaluations. This approach is useful for setting priorities or assessing difficulties.

Findings

Poor communication and lack of coordination among staff in a healthcare organisation caused misunderstandings, ineffective decision-making, delays in patient care, and medical errors. Implementation of effective communication and coordination protocols can help avoid these problems.

Practical implications

The study recommends using FMECA to identify and prioritise failures and conducting in-depth analyses to understand their root causes. It also highlights the importance of interdisciplinary knowledge and soft skills for healthcare staff.

Originality/value

This study reveals the significance of FMECA in healthcare risk management and benchmarking. FMECA helps identify system failures, develop prevention strategies, and evaluate effectiveness against industry benchmarks. It offers healthcare professionals a valuable tool to enhance patient safety and improve healthcare quality.

Purpose

Hydraulic slide valve failure often results from competing failure modes, termed competitive failure. To enhance prediction accuracy for hydraulic slide valve remaining useful life, the authors propose a method incorporating competitive failure and Monte Carlo simulation. This method allows for more accurate prediction of hydraulic slide valve remaining useful life.

Design/methodology/approach

In this paper, the competitive failure mode of the hydraulic slide valve is analyzed by studying the two failure modes of the hydraulic slide valve, and the prediction of the remaining useful life of the hydraulic slide valve is studied by using the sample set generated by Monte Carlo simulation and the competitive failure joint model.

Findings

The results show that the proposed prediction method based on competitive failure and Monte Carlo simulation is more accurate than the traditional Bayesian joint model prediction method when dealing with the failure mode competition phenomenon of hydraulic slide valve.

Originality/value

In this paper, the remaining useful life prediction of hydraulic slide valve with competitive failure characteristics is studied, which provides a new idea for the remaining useful life prediction method.

Purpose

One of the primary challenges associated with excavation near buildings is the significant decrease in the bearing capacity of nearby foundations during the initial stages before the stabilization of the excavation wall. This study aims to investigate the correlation between excavation height and foundation-bearing capacity under actual field conditions.

Design/methodology/approach

This paper uses a three-dimensional rotational failure mechanism to propose a novel method for estimating foundation-bearing capacity using the upper bound limit analysis approach.

Findings

The study delineates two distinct zones in the excavation height versus bearing capacity diagram. Initially, there is a significant reduction in foundation-bearing capacity at the onset of excavation, with decreases of up to 80% compared to its undisturbed state. Within a specific range of excavation heights, the bearing capacity remains relatively constant until reaching a critical height. Beyond this threshold, the entire soil mass behind the excavation wall becomes unstable. The critical excavation height is notably influenced by the soil's internal friction angle, excavation slope angle and soil cohesion parameter. Notably, when the ratio of excavation height to foundation width is less than 0.4, changes in slope angle have no significant impact on bearing capacity.

Originality/value

The bearing capacity estimates derived from the method proposed in this paper are deemed to reflect real-world scenarios closely compared to existing methodologies.
