Abstract

Purpose

Readmissions to the hospital are expensive and can have negative health consequences for patients. Older adults are at greater risk of readmission. Patient perspectives are valuable in identifying areas for improvement in the transition of care. The purpose of this qualitative study is to increase our understanding of patients’ perspectives on the transition of care from hospital to primary care.

Design/methodology/approach

This study employs a qualitative methodology to conduct semi-structured interviews with patients who have been discharged from hospitals in the Ireland East Hospital Group region. Remote interviews were conducted with 18 participants from eight general practices. Transcripts were analysed using thematic analysis as described by Braun and Clarke.

Findings

The three main themes identified were communication, outpatient supports and patient education. Gaps in communication do occur, but patients are often too external to comment. Patients benefit from a wide variety of outpatient supports including general practice, family, carers, allied health professionals and voluntary organisations. Access and cost are barriers to these supports. Participants were generally positive towards proposed primary care-based interventions such as follow-up appointments with general practitioners (GPs) and education sessions.

Originality/value

This study highlights a number of areas for improvement in the transition of care in current practice including communication between services and access to outpatient care. It also suggests directions for further research, such as explorations of healthcare provider perspectives and pilot studies of readmission reduction interventions.

Abstract

Purpose

This research demonstrates the theoretical merit of a reference architecture-based approach to life cycle cost (LCC) analysis system provision in the built environment. LCC insight is considered fundamental to sustainable decision making by asset managers; however, the current capabilities in practice do not align with the political ambition and the scale of competencies required to realise sectoral emissions–reduction targets.

Design/methodology/approach

In pursuing practical outcomes, the study employs a custom design science research-inspired methodology. Domain requirements are gathered via literature research as an initial top-down software reference architecture which is refined, bottom-up, through testing and implementation in a representative case study. A prototype IT system and reference architecture artefact are developed and used to evaluate the concept qualitatively through broad practitioner focus groups.

Findings

Sentiment analysis of the expert opinions is broadly positive and helps to substantiate the proposal’s theoretical suitability in addressing the scalability challenge. Additionally, constructive feedback provides guidance towards this trajectory, highlighting the importance of aligning with existing communities and standards, broadening future research scope to consider further scenarios and prioritisation of efforts to build trust around contracts and data quality.

Originality/value

The novelty of the work is the provision of the reusable LCC reference architecture development methodology.

Practical implications

The concept has the potential to provide LCC capabilities to industry at scale while the artefacts developed herein can be appended to existing LCC standards as implementation guidance to support IT system developers. Furthermore, the developed methodology can be employed in harmonisation efforts between policy and practice.

Abstract

Purpose

The aim of this study was to explore and understand the leadership experiences of medical consultants prior to a major hospital move. Health and care is becoming increasingly complex and there is no greater challenge than the move to a new hospital. Effective leadership has been identified as being essential for successful transition. However, there is very little evidence of how medical consultants experience effective leadership.

Design/methodology/approach

A qualitative methodology was utilized with one-to-one semi-structured interviews conducted with ten medical consultants. These were transcribed verbatim and analyzed using inductive thematic analysis. The research complied with the consolidated criteria for reporting qualitative research (COREQ).

Findings

Four themes were found to influence medical consultants’ experience of leadership: collaboration, patient centredness, governance and knowledge mobilization. Various factors were identified that negatively influenced their leadership effectiveness. The findings suggest that there are a number of factors that influence complexity leadership effectiveness. Addressing these areas may enhance leadership effectiveness and the experience of leadership in medical consultants.

Research limitations/implications

This study provides a rich exploration of medical consultants’ experience of collective leadership prior to a transition to a new hospital and provides new understandings of the way collective leadership is experienced in the lead up to a major transition and makes recommendations for future leadership research and practice.

Practical implications

The findings suggest that there are a number of factors that influence complexity leadership effectiveness. Addressing these areas may enhance leadership effectiveness and the experience of leadership in medical consultants.

Social implications

Clinical leadership is associated with better outcomes for patients therefore any interventions that enhance leadership capability will improve outcomes for patients and therefore benefit society.

Originality/value

This is the first research to explore medical consultants’ experience of collective leadership prior to a transition to a new hospital.

Abstract

Purpose

The aim of this study is to investigate the contribution of tangible and intangible investments in driving labour productivity growth in the European Union over the period 2000–2017 and their role in the short and medium run. Additionally, heterogeneity across countries is accounted for by performing estimates separately for Eastern and Western European countries.

Design/methodology/approach

The methodology used to conduct the analysis of the determinants of productivity is the two-way fixed-effect and the system generalised method of moments. We also include country-specific dummies in place of our variable on national innovative capacity as a means to further reduce the number of instruments.

Findings

The results reveal a long-term relationship of investment in intangible assets with labour productivity growth, more specifically of investment in R&D. This relationship holds both when considering the whole set of European countries and for Western European countries, demonstrating that R&D is key to enhancing labour productivity growth. On the contrary, the effect for Eastern countries is negative, probably due to the lack of capacity to turn this investment into an efficient and effective way to foster productivity.

Originality/value

Besides confirming the well-known role of tangible and intangible assets in productivity, the heterogeneity shown in our analysis highlights the need for improving capabilities in Eastern countries. Diversifying the decisions on the investments in European countries, depending on the specific needs and their heterogeneity, could help bridge the productivity gap and enhance specific capabilities of the country systems.
