Abstract

Purpose

This purpose of this paper is to engage two enduring sets of assumptions within nursing: firstly, that woman with children should prioritise the care of children; and secondly, that nursing standards require nurses to put their profession above other priorities. Commitment is linked to full-time working which contrasts sharply with the reality for many women with children who need to work part-time and are not able to change or extend working hours

Design/methodology/approach

This qualitative research involved the use of 32 in-depth interviews with 32 female registered nurses with children and without children. They were employed in “acute” nursing, aged between 25 and 60 years, and in registered grades “D” to “senior nurse manager”. They worked or had worked on a variety of employment conditions, some, but not all, had taken career breaks. The rationale for exclusively selecting women was based on the need to identify and describe organisational, situational and individual factors related to women and the associations and barriers which affected their careers.

Findings

In a female-dominated profession, we find the profession resisting attempts to make the profession more accessible to women with young children. The career progression of women with children is inhibited, and this is driven in part by a determination to maintain “traditional” employment practices.

Originality/value

This paper develops Heilman’s argument that gender perceptions, by both males and females, can be biased against women, and these produce gender inequalities in employment. These findings are relevant across many areas of employment, and they are significant in relation to broadening the debate around equal opportunities for women.

Abstract

Purpose

Through the discussion of two case studies, the purpose of this paper is to suggest that nurses may drive themselves to achieve the impossible. Professional bodies and health care expert emphasise the importance of always putting the interests, health and wellbeing of patients first. Could this be at the expense of nurses’ health, thus limiting their capacity to provide quality care for their patients?

Design/methodology/approach

The two case studies discussed offer examples of how two nurses nearly lost their lives in their personal drive to deliver quality nursing care in adverse conditions. The paper is based upon the hypothesis that an organisation which invests in the development of a healthy workplace culture, in which staff are put first, will not only enhance the health and wellbeing of its staff, but will ultimately result in the development of a culture which will empower staff to deliver the highest quality of care.

Findings

These case studies not only offer an example of the risk of neglecting self-care, the risk to Ben and Lily who quickly changed roles from nurse to patient, but also the risk to the quality of health care as a consequence of not paying due attention to the health and wellbeing of nurses (Crane and Ward, 2016). This paper suggests a radical and no doubt controversial shift of focus. Treat nurses as patients, obsess about the quality of their care, to create a culture in which nurses are nurtured so that they can in turn, and they will, nurture their patients.

Social implications

The Royal College of Nursing, the nursing professional body and trade union for all nurses have actively campaigned for a safe and health workplace for the last three years. They support RCN Safety Representatives to work with employers to develop a safe and health working environment for Staff. An important part of this work is the Healthy You Campaign. This has resulted in a series of learning and development workshops for nurses and the development of supporting resources to empower nurses to take care of themselves (www.rcn.org.uk/healthy-workplace).

Originality/value

These case studies and the resulting discussions are the author’s own original work, and have not previously been submitted for publication elsewhere.

Abstract

Purpose

The purpose of this article is to explore how one higher education institution (HEI) has embraced the apprenticeship agenda and is successfully providing an apprenticeship programme for nursing associates (NA) a new profession within health and social care in the United Kingdom.

Design/methodology/approach

This is a descriptive case study narrative about one programme from one HEI, selected for its relevance for the subject area of apprenticeships. This descriptive approach allows the provision of apprenticeships within HEI to be explored in depth by one institution.

Findings

This case study identifies the considerable value of apprenticeships within HEI for the students, the employers and the HEI. It also identifies that there are challenges within this. These findings will be of interest to those entering into the apprenticeship arena, especially within healthcare, and may inform an academic discourse in this area.

Research limitations/implications

It is acknowledged that this case study does not seek to compare apprenticeships with other programmes. However, there is value in providing an academic narrative around the challenges of this provision which will be informative for others developing higher apprenticeships in the United Kingdom or similar delivery of apprenticeship models internationally.

Abstract

With expanded technologic and communication resources there is growing awareness worldwide of the public health problems caused by alcohol, tobacco, and other drug use, misuse, abuse and addiction. Trends vary by culture and region but use of tobacco and alcohol is almost universal and is associated with high rates of mortality and morbidity. While nurses have not universally embraced the prevention and treatment of substance‐related disorders as their province, this is changing as a function of organisations, World Health Organization (WHO) and national initiatives, and the strengthening of nurse education. Actions to promote consensus, identify and review competencies for nurses must consider national and cultural variations, traditions of social change and the need for evidence‐based practice. Collective action by nurses in newly formed and existing organisations, which focus on addictions prevention and treatment, have resulted in initial professional steps. Such progress can be facilitated if achieved in the context of larger international policies and initiatives and in collaboration with members of other professional disciplines.

Sets out to examine the \ill\ of how nursing college libraries in Scotland developed \ill\ services to cope with the new demands implied in the \ill\ 2000 curriculum. Looks at the experience for libraries \ill\ with the first wave of Project 2000 students. Seeks \ill\ whether many of the problems experienced in England \ill\ repeated in Scotland or whether lessons have \ill\ . A postal survey of nursing college libraries in \ill\ was conducted followed by in‐depth interviews with a \ill\ of libraries. Observes issues such as funding, \ill\ with other institutions, use of stock, etc. Results see\ill\ that Scottish nursing college libraries have coped \ill\ changes for a number of reasons including professional \ill\ and involvement in planning. However, the study \ill\ possible ongoing or future strategic and organiza\ill\ which may militate against a satisfactory \ill\ scenario.

Abstract

Purpose

One of the fastest growing service industries is long‐term care. Identifying the best performers in the industry in terms of service productivity is difficult because there is no single summary measure of outcomes, particularly quality outcomes. The purpose of the paper is to show the potential of data envelopment analysis (DEA) as a benchmarking method in long‐term care.

Design/methodology/approach

The paper provides background information on the long‐term care industry and describes the DEA methodology and applications to long‐term care. Data originated from two data sources with four databases furnishing information on 69 long‐term care facilities used.

Findings

In the hypotheses tested it was found that most of the models showed that for profit nursing homes were significantly more efficient than nonprofit. The exception was in the model that included the condition of patients as a co‐production input and then there was no significant difference in efficient performance between ownership types.

Originality/value

The paper shows the value of DEA as a method of benchmarking in the context of long‐term care.

Abstract

Purpose

The aim of this paper is to examine the leadership role of the ward sister/charge nurse in the English National Health Service and demonstrate that its influence in maintaining standards of care has been undermined.

Design/methodology/approach

This is achieved through a critical review of the relevant research and policy literature. Evidence relating to recent failures in the provision of nursing care is summarised to establish the nature of the problem. This is followed by an analysis of existing research concerned with the ward sister/manager role in the context of recent policy.

Findings

The evidence demonstrating that the ward sister/charge nurse is one of the key determinants of quality care has been overlooked in recent years and this in part accounts for the quality failures that have occurred in NHS hospitals of late.

Practical implications

Action is needed to place the ward sister/charge nurse at the heart of the hospital system. Systems of organisational and mutual support are needed to ensure that these ward-based leaders can reach their full potential as the guardians of care.

Originality/value

The perspective offered in this paper illustrates how a solution to pressing organisational problem can be found through accessing the appropriate literature. Although the recommended action may not be “new” it is novel in the sense that it is based on rediscovering a role that already exists and can be supported to bring about change, rather than seeking wholesale complex change.

Abstract

Purpose

A Psychiatric Intensive Care Unit (PICU) and or High Dependency Unit (HDU) is a locked, intensive treatment facility available to people experiencing acute psychiatric distress. For many people who access public mental health services in Australia, the PICU/HDU is the primary point of admission, and should represent and facilitate timely assessment and an optimum treatment plan under a recovery framework. Nurses are the largest health discipline working in this specialty area of care. The paper aims to discuss these issues.

Design/methodology/approach

A qualitative study aimed to investigate the skills, experience, and practice, of nurses working in the PICU/HDU in relation to a recovery model of care. Identifying how nurses provide care in the PICU/HDU will inform a clinical practice guideline to further support this specialty area of care. Four focus groups were facilitated with 52 registered nurses attending.

Findings

The nurse participants identified specific skills under four distinct themes; Storytelling, Treatment and recovery, Taking responsibility, and Safeguarding. The skills highlight the expertise and clinical standard required to support a recovery model of care in the PICU.

Research limitations/implications

The research findings highlight urgency for a National PICU/HDU clinical practice guideline.

Practical implications

A PICU/HDU practice guideline will promote the standard of nursing care required in the PICU/HDU. The PICU/HDU needs to be recognised as a patient centred, therapeutic opportunity as opposed to a restrictive and custodial clinical area.

Social implications

Providing transparency of practice in the PICU/HDU and educating nurses to this specialty area of care will improve client outcome and recovery.

Originality/value

Very few studies have explored the skills, experience, and practice, of nurses working in the PICU/HDU in relation to a recovery model of care. A dearth of research exists on what is required to work in this specialty area of care.
