Abstract
Recent developments in medical treatments have resulted in the increased use of infusion devices for the administration of highly potent drugs. Drug administration is one of the highest risk areas of clinical practice and infusion devices are associated with a substantial number of adverse drug events. Locally, there was a perception that adverse drug events involving infusion devices appeared to be increasing, and there was anecdotal evidence to suggest that the available number of devices was inadequate to meet the increasing demand. A two‐part, observational audit, carried out in an acute district general hospital, was used to identify weak areas in the systems associated with the use of infusion devices and to implement actions to rectify the weaknesses and consequently reduce the risk to patients and staff.

BOURNEMOUTH fulfilled some of the high expectations of those who attended it. The welcome was cordial, the local arrangements good, as we were entitled to expect from so proved an organizer as Mr. Charles Riddle and from his committee and staff, and, when fine, the town was most attractive. The weather, however, was bad, and too warm at the same time for most of us. One thing that certainly emerged from this experience was the real need to change the time of the conference. Only librarians among similar bodies appear to meet in the summer season. The accountants, engineers and other professional people confer in late May or in June, when they do not compete with holiday‐makers for accommodation and attention. The Council might well consider the re‐arrangement of its year with such a change in view.

Abstract
The author argues that we must stop and take a look at what our insistence on human labour as the basis of our society is doing to us, and begin to search for possible alternatives. We need the vision and the courage to aim for the highest level of technology attainable for the widest possible use in both industry and services. We need financial arrangements that will encourage people to invent themselves out of work. Our goal, the article argues, must be the reduction of human labour to the greatest extent possible, to free people for more enjoyable, creative, human activities.

Purpose
The purpose of this article is to provide a review of the most recent literature concerning document supply and related matters.
Design/methodology/approach
The article covers the reading of over 150 journals as well as monographs, reports and websites.
Findings
That the fundamental debate on the direction of scholarly publishing continues intensely and that opposition is growing to DRM constraints. Electronic books remain a small minority market but the mass digitisation of books is proceeding apace. Open access continues to grow but with widely differing views on its impact – the publishers start to fight back.
Originality/value
The paper represents a useful source of information for librarians and others interested in document supply and related matters.

Abstract
Purpose
This paper aims to unravel the puzzle that the United Kingdom’s high-quality government accounting and fiscal architecture is associated with low-quality outcomes, including poor productivity growth, high public debt, public services which do not meet citizen expectations and historically high levels of taxation. It contributes to public sector accounting research in the fields of fiscal transparency and governance.
Design/methodology/approach
This paper uses Miller and Power’s (2013) economization framework and Dunsire’s (1990) concept of collibration to explain why being a global leader in public sector accounting reform and in fiscal and monetary architecture has not protected the UK from weak governance. The intersection of economization’s roles of accounting with modes of government accounting clarifies the puzzle.
Findings
Whereas accruals government accounting contributes to fiscal transparency, this is not a sufficient condition for well-judged policy and its effective application. Collibration is the dominant mechanism for mediation in the fiscally centralized UK, but it has failed to deliver stable outcomes, in part because Parliament is limited in its ability to hold back inappropriate behaviour by the Executive. Subjectivization has disrupted adjudication because governments at all levels resist constraints on their behaviour, with unpredictable and often damaging consequences.
Originality/value
This paper provides insights through the combined lens of economization and modes of government accounting, demonstrating the practical value of this conceptualization. Although some causes for unsatisfactory outcomes are specific to the UK, there are cautions for accounting and fiscal reformers in other countries, such as Member States of the European Union.
