Abstract

The last edition of the Journal carried an introduction to retirement communities. Hartrigg Oaks is a non‐profit‐making community, developed without government subsidy, developed by the Joseph Rowntree Housing Trust, and has become well‐known among those interested in different housing and social care for older people. Modelled on American continuing care retirement communities (CCRC), this retirement village has recently been the subject of a three‐year evaluation (Croucher et al, 2003). This article gives information about Hartrigg Oaks, and some of the findings of the evaluation.

Abstract

Purpose

Shanghai is currently faced with a rapid increase in the ageing population and demand for elderly homes. Continuing care retirement community (CCRC) has been emerging as a high-end alternative to offer specialised accommodation to the elderly in major cities. Since the first development in 2008, the industry is now still at the infancy stage. This study aims to examine the investment barriers hindering the supply and demand of CCRCs with an aim to recommend practical and senior housing policy measures to facilitate CCRC developments.

Design/methodology/approach

Multiple-case study method was used to confirm whether the literature findings on investment barriers apply to the context of Shanghai. Four representative CCRC development cases in Shanghai were examined, in which qualitative data were collected from interviews with experienced CCRC development managers and quantitative data from a questionnaire survey of the CCRC residents.

Findings

Operation management experience, financial risks and government support policy were found to be the main supply barriers. Chinese traditional family-oriented culture and affordability were not the main demand barriers of CCRCs in Shanghai. Poor quality of services and living environment were identified as the main barriers suppressing the demand for CCRC.

Research limitations/implications

Although common trends and views can be drawn from the representative cases in Shanghai to provide valid results, further research should be conducted on other major cities in China so that the results can be widely applied.

Practical implications

Successful CCRC investment strategy should focus on partnering with experienced professional eldercare management companies, provisions of high-quality medical professionals and trained care personnel and delivery of flexible care service, along with intensive capital flows for land, construction and operating costs.

Social implications

Additional senior housing policy support should be established to promote the CCRC supply to address the ageing needs, particularly granting lands for CCRC developments at Tiers 1 and 2 major cities where the land cost is high.

Originality/value

This research’s practical and policy measures can be applied to enable and promote CCRC developments in Shanghai, thus benefitting both housing investors and the government. The findings also form a baseline for CCRC developments in other major cities.

Abstract

Purpose

China’s population is ageing. Continuing care retirement communities (CCRCs) are an emerging living arrangement of older Chinese. Incorporating social sustainability features into CCRCs helps to create age-friendly residential environments for residents. However, it is still unclear what kinds of social sustainability features are incorporated into the residential environments of CCRCs in China. Therefore, this study aims to address this research gap.

Design/methodology/approach

Qualitative content analysis is adopted to analyse the retrieved business information of representative CCRC developers in China.

Findings

This study revealed 36 social sustainability features in CCRCs, with the top-ranked ones being health care and management, social connection and engagement, high-quality and diverse services and daily life support and assistance. Additionally, a preliminary social sustainability framework of CCRCs was proposed, and this framework includes the five components of care and health, environment and management, service and facility, age-friendly life philosophy and social support and inclusion.

Originality/value

In theory, this research’s findings clarify the meaning of social sustainability within the context of CCRCs, which supports future relevant explorations in the CCRC research community. In practice, these findings enhance stakeholders’ understanding of the social sustainability in CCRCs, which promotes the development of age-friendly living environments for older people in an ageing society.

Abstract

Purpose

Resident satisfaction is a catalyst for continuing care retirement communities (CCRCs) to enhance their competitiveness in the expanding senior living market. CCRC attributes play an imperative role in resident satisfaction when older adults reside in these communities. The purpose of this study was to identify the attributes that affect CCRC resident satisfaction and quantify their relative levels of importance. In addition, the impact of resident satisfaction on word of mouth (WOM) and satisfaction with life was examined.

Design/methodology/approach

The proposed hypotheses were tested by performing structural equation modeling on the data collected from 332 CCRC residents throughout the USA.

Findings

The study results revealed that all 10 of the CCRC attributes identified in this study significantly influenced resident satisfaction. Sociopsychological factors had the strongest influence on resident satisfaction, followed by cost and value, medical services, availability of activities, food and beverage services, design, rooms, management and staff, location, and spiritual environment. The study results further indicated that resident satisfaction had a positive impact on WOM and satisfaction with life.

Practical implications

This study provides significant practical implications for CCRC owners and operators to help them better identify the types of CCRC attributes and their relative levels of importance for resident satisfaction.

Originality/value

This study is one of the first comprehensive studies to identify CCRC attributes associated with resident satisfaction and examine the impact of resident satisfaction on satisfaction with life within the senior living field.

Abstract

Purpose

This paper aims to examine the trends among the elderly population in China about residential preferences and policy applications, as the elderly is a rapidly expanding demographic group that has increasing and diversifying inclinations for demanding the residential facilities for the elderly (RFEs) now and in the foreseeable future.

Design/methodology/approach

Based on a review of the existing literature and policies, a model is conceptualised for understanding the demands of the elderly. Their needs for functional supportiveness and richness of residential resources in RFEs are then categorised into focal groups.

Findings

The Chinese elderly’s demand for specialised residential facilities is under a shift from seeking deficit relief to pursuing personal choices. It is suggested that there will be a continuing demand for affordable RFEs from a number of key focal groups, including: the functionally impaired; marginally housed; socially isolated; and the elderly requiring social relief. In addition, retirement housing in China is likely to be more affordable for the next elderly generation. However, the immature social welfare system and low average income level of the current elderly generation means that the Chinese Government has tough decisions to make about service priorities.

Practical implications

Policy and investment priorities may have to be inclusive of those who demand social relief (free-of-charge) and affordable professional long-term care in RFEs, whereas the rest of the demand could be released by growth in the development of community- and home-based service systems.

Originality/value

This study is one of the first to identify the diversifying demands of age-exclusive living facilities for the elderly that deserve priorities in China. The results can inform and guide future policy and project investment in China.

Abstract

Purpose

Expectations of an aging population increases the need for accurate models and reliable forecasts of housing demand in the later stages of life. This paper aims to examine whether the only published forecasting model predicts the actual movement of older Americans into retirement housing.

Design/methodology/approach

A logistic regression model is used to test which older movers chose retirement housing using data from the 2002 and 2004 Health and Retirement Survey.

Findings

Age is found to have a positive relationship with the choice to move into retirement housing, but the other socioeconomic variables are not significant and the model is not robust.

Research limitations/implications

More investigation is necessary to identify the variables that will provide a reliable estimate of those choosing retirement housing. Segmented by housing type may be necessary rather than estimating total retirement housing demand.

Originality/value

The paper tests whether a model previously used to forecast senior housing demand actually predicts housing choice. It demonstrates that additional research is needed to develop models that can more accurately estimate demand in this growing segment of the housing market in aging countries.

Factors such as retirement and declining health may trigger older Americans to move into retirement housing. Most mature consumers make this decision in consultation with their family. Understanding the timing and decision‐making process is necessary to properly position and promote retirement housing. A national survey of retirement housing residents found that most moves were prompted by financial considerations, retirement, and health problems. Although most seniors made the final decision to move themselves, children and physicians were influencers. Thus, retirement housing must be promoted to family members and health care workers as well as potential residents.

Abstract

Purpose

Appropriate housing for the growing population of older adults is becoming an international concern. The purpose of this paper is to report on a review of UK and international literature carried out as part of a project exploring the commissioning and delivery of social care in housing with care settings. The paper also considers housing with care in the context of UK policy and practice.

Design/methodology/approach

The peer literature review process used a range of academic databases as well as government and third sector web sites, drawing on relevant material in English published from 1990 to 2012.

Findings

Findings are presented within three main themes: how care and support is provided; the role of the built environment; and the benefits for resident well-being. The review found a paucity of literature focusing specifically on care and support in housing with care settings, particularly in terms of how social care is delivered, but the evidence base suggests that housing with care is in a strong position to deliver on most if not all UK government aspirations.

Practical implications

Despite a growing literature both in the UK and internationally exploring the characteristics and benefits of housing with care for older people, substantial gaps remain in the research evidence.

Originality/value

This paper presents an up to date review of the housing with care literature in the context of current UK policy.

Abstract

Purpose

The purpose of this paper is to stimulate a wider debate around the coordination of workforce planning in non-statutory services (in this case, specialist housing for older people or those with long-term health and social care needs, such as learning disabilities). The authors argue that current NHS reforms do not go far enough in that they fail to include specialist housing and its workforce in integration, and by doing so, will be unable to optimise the potential efficiencies and streamlining of service delivery to this group.

Design/methodology/approach

The paper used exploratory study using existing research and data, enhanced by documentary analysis from industry bodies, regulators and policy think tanks.

Findings

That to achieve the greatest operational and fiscal impact upon the health care services, priority must be given to improving the efficiency and coordination of services to older people and those requiring nursing homes or registered care across the public and third sectors through the integration of service delivery and workforce planning.

Research limitations/implications

Whilst generalisable and achievable, the model proposed within the paper cannot be fully tested theoretically and requires further testing the in real health and social care market to evidence its practicality, improved quality of care and financial benefits.

Originality/value

The paper highlights some potential limitations to the current NHS reforms: by integrating non-statutory services, planned efficiency savings may be optimised and service delivery improved.

Abstract

Purpose

Demand for long-term care services increases with population aging. This study aims to develop a conceptual model of elderly customers’ health-care experiences to explore the antecedents, mechanisms and outcomes of social participation in long-term care service organizations.

Design/methodology/approach

Using a two-phase data collection approach, this study collects data from 238 elderly customers in a long-term care service organization. The final data are analyzed through structural equation modeling.

Findings

The results show that care management efforts (i.e. customer education, perceived organization support, role modeling, perceived other customer support and diversity of activity) influence elderly customers’ psychological states (i.e. self-efficacy and sense of community), leading to increased social participation. In addition, high levels of social participation evoke positive service satisfaction and quality of life, both of which alleviate switching intention.

Originality/value

This study is one of the first conclusive service studies focused on the role of elderly customers’ social participation in their long-term care experience. The findings contribute to health-care service marketing and transformative service research, and expand understanding of elderly customers’ health-care experience, especially in long-term care service settings.
