Abstract

Thorough assessment of communication is of utmost importance for successful working with people with learning disabilities. Indeed, recent legislation now makes it incumbent on professionals working with people with learning disabilities to address issues of communication, both their own communication to clients and that of their clients. There are clear associations between learning disabilities, emotional, behavioural and psychiatric problems, and impaired communication. The nature of the communication impairments experienced by individuals with learning disabilities is likely to be complex, with cumulative and inter‐related features associated with the general developmental delay, the phenotypical presentation in a particular syndrome, the presence of specific linguistic impairments and the indirect impact of other difficulties on communication skills. A number of parameters need to be addressed in a communication assessment in order to take account of the complexity and dynamism of the communication process. They concern not only the underlying developmental and communication impairments affecting particular areas of functioning, but also many aspects of the wider context in which the communication takes place.

Abstract

Purpose

Research into the communication skills of individuals with Cornelia de Lange syndrome (CdLS) is extremely limited. This paper aims to evaluate the nature of these skills and impairments in CdLS using a detailed informant assessment of pre-verbal communication skills.

Design/methodology/approach

The study used the Pre-verbal Communication Schedule to evaluate communication skills in individuals with CdLS (n=14), aged five to14 years. The group was compared with a contrast group of individuals with Cri du Chat syndrome (CdCS; n=14) who were matched for age and intellectual ability.

Findings

A significant difference was identified in understanding non-vocal communication (p<0.005), with the CdLS group showing a greater deficit. These findings indicate the presence of a syndrome-specific deficit in understanding non-verbal communication in individuals with CdLS and suggest that there may be a dissociation between the processing of verbal and non-verbal communication.

Originality/value

The findings indicate that, in many ways, these two syndrome groups are not dissimilar in terms of their communication skills. However, individuals with CdLS show a syndrome-specific deficit in understanding non-vocal communication relative to the CdCS group.

Students with communication disorders present unique challenges to educators working toward fostering an inclusive classroom. For children with speech/language impairments, expressing themselves either academically or socially may present obstacles requiring communicative support and facilitation. For children with hearing loss, full access to educational material will be difficult without technological and/or visual support. Many children may have a combination of disorders, requiring a team of educators and other professionals to provide educational content and classroom support in the most inclusive way possible. This chapter is intended to provide an overview of variety of communication disorders, along with guidelines for improving student access across educational settings.

Abstract

Purpose

The study was to develop and test a new technology-aided program to support basic communication and leisure of people with extensive neuro-motor and speech impairments.

Design/methodology/approach

The study was carried out according to a non-concurrent multiple baseline design across three participants. During the intervention, the participants used the new program. This relied on a smartphone, a tablet, and radio frequency identification tags, and allowed the participants to make communication and leisure choices via small hand movements. Communication choices involved the possibility of sending messages or making telephone calls to preferred persons. Leisure choices involved the possibility of accessing preferred music, comedy, and sport or television shows.

Findings

During the intervention, the participants were highly successful in activating communication and leisure events independently. They had mean cumulative engagement times of about 7–8.5 min per 10-min session.

Originality/value

The new program is easily accessible and affordable in terms of the technology components involved and appears highly suitable and friendly for individuals who have only limited hand movement.

Abstract

Writings about language and speech impairments (SLI) have been present for many centuries (Smith, 2004). Unfortunately, early historical accounts tended to reflect negatively upon individuals with SLI. For example, Van Riper and Erickson (1996) related that during the Roman times, an individual who stuttered was placed into a cage for entertainment purposes. According to these authors, citizens passing would throw coins into the person's cage to get him to talk. During the late 1800s, the profession of speech-language pathology began as an avocation of certain professionals, notably doctors, educators, and elocutionists (public speakers), who were interested in helping others improve their speech. American doctors studied under the auspices of European doctors who treated people with communication disorders. The two most common disorders that were treated then were dysfluency (stuttering) and speech sound errors (articulation) (Duchan, 2002). Treatment was available for the above disorders, however, the programs were not in public schools and the results of intervention were mixed (Smith, 2004).

Abstract

Purpose

The purpose of this paper is to describe the journey of a young person with severe and complex communication needs from no formal expressive communication system, to a point where he is motivated and able to use a text based voice output communication aid for a range of communication functions, in a variety of settings, and with a range of communication partners.

Design/methodology/approach

The paper is a descriptive single case study, documenting long‐term changes in speech, language, and communication needs and use, and discontinuation of use, of range of Augmentative and Alternative Communication (AAC) tools.

Findings

The paper describes the different AAC interventions and their success or otherwise in supporting the young person. It also describes key educational and therapeutic aspects of his management. Changes in the young person's interaction, language and literacy skills, and how his family and the professionals around him perceived the changes in his communication are highlighted.

Research limitations/implications

The paper is a description of one person without a known underlying diagnosis of his severe and complex communication impairment and might, therefore, be of restricted use when generalized.

Originality/value

There are few published longitudinal descriptions concerning how, why, and when young people use or discard AAC tools. This paper highlights the multiple and various factors of the factors that can be at work when actually providing intervention.

Abstract

The field of special education in Thailand is still in its infancy. This chapter provides a retrospect on special education in Thailand reflecting societal attitudes toward people with disabilities from the past to present. It also provides a list of factors impacting this population and members of the community who are involved with their lives. Special education law, definitions of various disability categories, types of educational settings, as well as issues and challenges in the field are discussed. A critical analysis of special education teacher preparation is also provided. Finally, recommendations and conclusions are offered.

Abstract

Purpose

The purpose of this paper is to critically review the current police training and criminal justice policy regarding the treatment of suspects with autism spectrum disorder (ASD) during the initial stages of the criminal justice system (CJS), and provide potential policy reform and areas for further research.

Design/methodology/approach

By reviewing extant literature, research and policy documents, this paper provides a critical review of the current policy and training for dealing with suspects with ASD in the current CJS in England and Wales for suspects with ASD.

Findings

This paper proposes that current policy and police staff training is insufficient during all initial stages of the criminal justice process. Although there are emerging policies and schemes which are promising, they require further research and national participation. Policy reform and improved training is required to ensure minimal opportunities for miscarriages of justice to those individuals with ASD.

Originality/value

This paper provides a chronological journey through the initial stages of the CJS in England and Wales for a suspect with ASD, and the challenges that they may face. Suggestions are made based on criminological and psychological research to remedy the potential opportunities for miscarriages of justice.

Abstract

Purpose

Adults who have autism spectrum disorders (ASD) experience a range of core and co-morbid characteristics which impede daily functioning and quality of life. Children and adolescents with ASD derive clinically meaningful benefits from psychological interventions, including those designed to reduce socio-communication deficits and mental health conditions. Relatively little is known about the effectiveness of these interventions for the adult ASD population. The paper aims to discuss this issue.

Design/methodology/approach

A selective search of English language, peer-reviewed publications was undertaken, in order to summarise the empirical data pertaining to psychological interventions for adults with high-functioning ASD (HF-ASD).

Findings

Thus far, social skills interventions, cognitive behaviour therapy techniques, and mindfulness-based approaches have been researched most extensively. Interventions have primarily sought to: reduce the impact of core ASD characteristics; enhance skills; and improve co-morbid mental health symptoms. Methodological and clinical heterogeneity render it difficult to generalise study findings across population samples, but overall, interventions appear to be associated with reductions in co-morbid symptom severity, and improved functioning.

Research limitations/implications

Further studies that seek to improve functioning, reduce co-morbid characteristics, and enhance the propensity for attaining and maintaining independence are now needed.

Practical implications

Adaptations to standard treatment protocols are likely required in order to enhance engagement and optimise treatment gains.

Originality/value

This is one of the first reviews to focus specifically on psychological interventions for adults with HF-ASD.
