Abstract

Purpose

The post-operative management of flexor tendon injuries has been the focus of considerable exploration and there continues to be variation in approaches and methods of mobilisation. The purpose of this paper is to explore therapy management following repair to flexor tendons at zone II and flexor pollicis longus (FPL) (all zones) in Ireland.

Design/methodology/approach

A descriptive survey questionnaire design through an online format was used. Therapists were recruited through the Irish Association of Hand Therapists, the national bodies for occupational therapy and physiotherapy, and therapy managers in acute hospitals, with 29 therapists participating in the study. Descriptive statistics were used to analyse the survey data.

Findings

Patients were generally seen three to five days following surgery. Early active mobilisation approaches were favoured by all but one therapist, with 62% using the Belfast protocol and 34% the Manchester Short Splint (MSS) protocol. Each early active protocol exercise session commences with passive motion followed by graded active flexion. Tenodesis is incorporated by the majority of respondents within the first four weeks. Therapy programme and splints are modified based on patient presentation. Resistance exercises are commenced from week seven. Patient compliance was identified as the most influential factor in the post-operative intervention approach taken.

Originality/value

This study provides the first Irish profile of current practice in the post-operative management of flexor tendon repairs at zone II and FPL which has not previously been reported. Further research should explore the reasoning behind the interventions chosen and also the implications for practice of changes to surgical techniques.

Abstract

Purpose

Wrist orthoses are used by occupational therapists to decrease pain, support weak muscles and protect tissues during healing. However, use of wrist orthoses has been observed to produce compensatory movements in other upper extremity joints. This paper aims to determine whether wearing wrist orthoses produced compensatory movements of the elbow in addition to the shoulder when performing drinking and hammering tasks.

Design/methodology/approach

Two twin-axis electrogoniometers were positioned on the elbow and shoulder to track joint movement. The four conditions were drink with orthosis, hammer with orthosis, drink without orthosis and hammer without orthosis. Joint movement was defined as total angular excursion of the joint throughout the performance of the task. Separate 2 × 2 (joint × orthosis) repeated measures analyzes of variance (ANOVA) were used to evaluate differences in joint excursion of the elbow and shoulder joints between orthosis conditions for each task.

Findings

Wearing a wrist orthosis did not change the amount of joint excursion compared to not wearing an orthosis during the drinking and hammering tasks.

Originality/value

Findings suggest that wrist orthoses do not result in statistically significant changes in elbow and shoulder joint movements during simulated drinking and hammering tasks.

Abstract

Purpose

The purpose of this study is to demonstrate the potential of three-dimensional printing technology for the remanufacturing of end-of-life (EoL) composites. This technology will enable the rapid fabrication of environmentally sustainable structures with complex shapes and good mechanical properties. These three-dimensional printed objects will have several application fields, such as street furniture and urban renewal, thus promoting a circular economy model.

Design/methodology/approach

For this purpose, a low-cost liquid deposition modeling technology was used to extrude photo-curable and thermally curable composite inks, composed of an acrylate-based resin loaded with different amounts of mechanically recycled glass fiber reinforced composites (GFRCs). Rheological properties of the extruded inks and their printability window and the conversion of cured composites after an ultraviolet light (UV) assisted extrusion were investigated. In addition, tensile properties of composites remanufactured by this UV-assisted technology were studied.

Findings

A printability window was found for the three-dimensional printable GFRCs inks. The formulation of the composite printable inks was optimized to obtain high quality printed objects with a high content of recycled GFRCs. Tensile tests also showed promising mechanical properties for printed GFRCs obtained with this approach.

Originality/value

The novelty of this paper consists in the remanufacturing of GFRCs by the three-dimensional printing technology to promote the implementation of a circular economy. This study shows the feasibility of this approach, using mechanically recycled EoL GFRCs, composed of a thermoset polymer matrix, which cannot be melted as in case of thermoplastic-based composites. Objects with complex shapes were three-dimensional printed and presented here as a proof-of-concept.

Abstract

Purpose

Many treatment methods for intra-articular fractures of the proximal interphalangeal (PIP) joint are described in the literature without a consensus on the most effective approach. The purpose of this study was to investigate the methods of treatment of PIP joint fractures being used by trauma surgeons in the Republic of Ireland currently and the timing of referral to therapy.

Design/methodology/approach

A cross-sectional descriptive study methodology was used to survey trauma surgeons, occupational therapists and physiotherapists in Ireland. An online platform was used. A total of 21 surveys were returned by surgeons and 37 by therapists. Descriptive statistical analysis was used to present the results.

Findings

Buddy strapping was reported as the primary treatment method for stable PIP joint fractures. All levels of fracture severity were reported to be treated using traction constructs, which include static and dynamic fixation and orthoses. Unstable fractures were managed using open reduction with internal fixation by 50 per cent of surgeons. Early timing of referral to therapy is reported by more surgeons than therapists. The majority of therapists indicated that they did not have the resources to see patients at the optimal time.

Originality/value

To the authors’ knowledge, this study provides the first description of the management of PIP joint fractures across the Irish health service. The findings of this study suggest that additional therapy resources are required within the health service executive to facilitate the desired early referral to therapy and to enable service development for this category of hand fractures.
