Abstract

This paper is the first to use the individual level, longitudinal catch-up growth of boys and girls in a historical population to measure their relative deprivation. The data is drawn from two government schools, the Marcella Street Home (MSH) in Boston, MA (1889–1898), and the Ashford School of the West London School District (1908–1917). The paper provides an extensive discussion of the two schools including the characteristics of the children, their representativeness, selection bias and the conditions in each school. It also provides a methodological introduction to measuring children’s longitudinal catch-up growth. After analysing the catch-up growth of boys and girls in the schools, it finds that there were no substantial differences between the catch-up growth by gender. Thus, these data suggest that there were not major health disparities between boys and girls in late-nineteenth-century America and early-twentieth-century Britain.

Abstract

Purpose

There is a considerable gap in the literature examining the effect of parental international migration on children’s health in Pakistan. The author aims to examine the impact of parental international labour migration on the health (anthropometric measures) of children left-behind in Pakistan.

Design/methodology/approach

The author investigates the impact of parental international labour migration on the health (anthropometric measures) of children left-behind in Pakistan using econometric estimation techniques and the latest wave of the Pakistan Demographic and Health Survey 2017–2018. The main child health measures the author uses include weight-for-age (WAZ) and height-for-age (HAZ) Z-scores for children under five years of age. Since unobservable factors may be vital determinants of the child nutrition outcomes, the author uses two unique instrumental variables to address the potential endogeneity problem.

Findings

The author finds that international migration improves the left-behind children’s weight-for-age and height-for-age nutritional outcomes, generating essential policy insights.

Originality/value

To the best of the auhtor’s knowledge, no previous study has been conducted on parental migration and left-behind children in Pakistan using the latest Demographic and Health Survey.

Abstract

Purpose

Stunting and being underweight in children are major nutritional problems especially in developing countries. The purpose of this study is to evaluate the effect of egg supplementation on a vitamin and mineral fortification program for growth, cognitive development and hemoglobin in underweight and stunted children.

Design/methodology/approach

This was an experimental study using a crossover design conducted in Yogyakarta province, Indonesia. A total of 39 subjects were randomly selected from two community health centers and provided with two types of intervention: vitamin and mineral fortification sprinkle (Taburia) and Taburia with egg supplementation (Taburia PLUS). Each intervention was conducted for three months with one-month of washout period in between interventions.

Findings

There were no changes in weight-for-age Z-score in Taburia and Taburia PLUS (all p > 0.05). The height-for-age Z-score was increased in Taburia PLUS (p = 0.022) but not in Taburia (p > 0.05). Hemoglobin level was significantly increased in Taburia (p = 0.039) but not in Taburia PLUS (p > 0.05). There were no significant changes in visual motoric score in Taburia, but there were slight increases in Taburia PLUS (all p > 0.05).

Originality/value

The authors concluded that egg supplementation combined with multivitamins and minerals fortification program, Taburia PLUS, is beneficial to improve linear growth but not hemoglobin in stunted and underweight children.

Abstract

Purpose

This study aims to assess the nutritional status of children with sickle cell diseases using anthropometric measurements, biochemical markers and dietary intakes.

Design/methodology/approach

The study was conducted in 100 children of 3-12 years of age with sickle cell diseases (SCDs) at the Komfo Anokye Teaching Hospital in the Kumasi Metropolis of Ghana. Weight, height and age of participants were used to calculate body mass index-for-age. The mid-upper-arm-circumference-for-age, weight-for-age (percentiles) and height-for-age (percentiles) were compared with standards growth charts for children. Biochemical measures such as serum albumin and ferritin, as well as full blood count, were assessed. Dietary intake was assessed using 24-h dietary recall and food frequency questionnaire.

Findings

From the study, 73 and 37 per cent of the children with SCD recorded inadequate intake of iron and vitamin E, respectively, when compared to the recommended daily allowance. Out of the 100 participants, 37 per cent were underweight and 22 per cent were stunted. There was significant difference (p < 0.05) in underweight (weight-for-age) prevalence by gender. A multiple variate regression showed a significant association between zinc (r2 = 0.763, p < 0.05) and haemoglobin levels.

Originality/value

The evidence in this paper is relevant for treatment, health education and nutritional counselling of parents with children who have SCD.

Abstract

This study uses migrant household survey data from 2008 to 2009 to examine how parental migration decisions are associated with the nutritional status of children in rural and urban China. Results from instrumental variables regressions show a substantial adverse effect of children’s exposure to parental migration on height-for-age Z scores of left-behind children relative to children who migrate with their parents. Additional results from a standard Blinder–Oaxaca decomposition, a quantile decomposition, and a counterfactual distribution analysis all confirm that children who are left behind in rural villages – usually because of the oppressive hukou system – have poorer nutritional status than children who migrate with their parents, and the gaps are biggest at lower portions of the distribution.

Abstract

Purpose

The purpose of this paper is to assess the impact of household food security and nutrition programme (HFSN) on the nutritional status of children (0–5 years) in Oyo state, Nigeria.

Design/methodology/approach

Pre‐tested structured and unstructured questionnaires were used to elicit information from 201 programme and 201 non‐programme women participants. The participants who are farmers were selected using systematic random sampling technique from each of the six local government areas covered by the programme, while non‐participants were selected using simple random sampling technique. Also, anthropometry technique was used to assess the nutritional status of the children of both groups. Descriptive statistics such as frequency counts, percentages and means were used to describe the findings while analysis of variance (ANOVA) was used to determine the significant difference in the nutritional status of the children of the two groups.

Findings

The results show that the women farmers who participated in programme activities possessed some personal and household characteristics which influenced the nutritional status of the children. The participants utilized most of the prograrmme recommendations and significant differences existed in the weight for age and height for age indices of their children.

Originality/value

The results indicate the need for constant surveillance and collaborative effort of the health, agricultural extension and education departments in providing women farmers with the education and training needed to reduce the incidence of malnutrition and food insecurity.

Abstract

Purpose

Developing countries like India, accounts for about 40 percent of undernourished children in the World and it is largely due to the result of dietary inadequacy in relation to their needs. The purpose of this paper was to evaluate the changes in the nutritional status of the children, from their preschool days to the present primary school days.

Design/methodology/approach

The present investigation was conducted at 20 Integrated Child Development Service (ICDS) centers (Center-A) and 15 primary schools (Center-B) in Bali Gram Panchayat, Arambag, Hooghly District of West Bengal, India, at an interval of three to four years. A total of 1,012 children (boys=498; girls=514) aged two to six years old enrolled in these ICDS centers and a total of 603 children (boys=300, girls=303), aged five to ten years were studied from the 15 primary schools who were the beneficiaries of ICDS centers. Underweight (weight-for-age Z-score (WAZ)) and wasting (weight-for height Z-score (WHZ)) were used to assess the nutritional status.

Findings

The nutritional situation (both in case of underweight and wasting) was better in Center B as compared with Center A. In general, the nutritional condition of boys was better than girls. Center had a very significant effect on both WAZ as well as WHZ, irrespective of age and sex. Sex has a significant impact only on WAZ. Interestingly, there was no significant sex-center interaction for both WAZ as well as WHZ. The children of the area were getting Mid Day Meal supplied through the school authorities which was comparatively better than the ICDS centers’ food supplementation. Better monitoring of nutritional supplementation at primary schools may be an important factor.

Practical implications

In ICDS centers, only the Anganwari worker is responsible in running and implementing the programs offered by the Government. However, at primary schools, the active involvement of all the teachers to run the program may have effectively led to have better results. Furthermore, the Government's focus should not be only on the increase the area covered by the ICDS program, but focus should be to increase the quality of food supplied, proper monitoring of the implementation and increase the allocation of funds. Appropriate measures may be taken by the authorities regarding this.

Originality/value

The results of the study will help in policy making in reducing the prevalence of undernutrition.

Abstract

Purpose

The purpose of this paper is to determine the relationship between parental child feeding practices with growth status of Orang Asli (OA) children in Negeri Sembilan.

Design/methodology/approach

This is a cross-sectional study conducted amongst 190 OA children aged 4–6 years studying at Tabika KEMAS OA in Negeri Sembilan. Mothers were interviewed to obtain socio-demographic information while feeding practices were assessed using Malay version of Comprehensive Feeding Practices Questionnaire (CFPQ-M). Growth status of the children were determined using WHO ANTHRO Plus software. Three indicators of growth status assessed were weight-for-age (WAZ), height-for-age (HAZ) and body mass index (BMI)-for-age (BAZ) z-scores.

Findings

The mean score for feeding practices used by the mothers while feeding their child showed that most of the mothers practiced modelling (3.71 ± 0.82), encouraging balance and variety (3.52 ± 0.76) and environment (3.66 ± 0.66) while the practice of restriction to weight (1.85 ± 0.79) and emotional regulation (2.20 ± 0.96) were low. Most of the OA children had normal WAZ (70.0%), HAZ (71.1%) and BAZ (84.2%). Amongst the three indicators of growth status, BAZ was used to study the relationship with feeding practices. The use of feeding practices such as pressure (r = −0.34, p < 0.01), food as a reward (r = −0.23, p=<0.01), child control (r = −0.18, p = 0.01) and restriction to weight (r = 0.19, p = 0.01) were associated with BAZ.

Research limitations/implications

This study only focussed on OA children of Temuan and Semelai sub-ethnics studying at kindergarten specific for OA children, thus findings could not be generalised to the entire OA population in Malaysia.

Social implications

There is a need to address the OA population as they continue to be amongst the most disadvantaged population often affected with poor health and social outcomes.

Originality/value

In this study, a wide range of scales in that represents different feeding practices in CFPQ-M was assessed instead of focussing on limited type of feeding practices. Thus, a better understanding was observed regarding the healthy and unhealthy feeding practices. The result of this study could be useful to update the existing literature of OA research, and to plan suitable interventions with regards to feeding practices and growth status of the children, especially amongst this vulnerable community.

Abstract

Purpose

Identifying and addressing poor nutritional status in school-aged children is often not prioritized relative to HIV/AIDS treatment. The purpose of this paper is to elucidate the benefits of integrating nutrition (assessment and culturally acceptable food supplement intervention) in the treatment strategy for this target group.

Design/methodology/approach

The authors conducted a randomized, double blind pre-/post-intervention trial with 201 HIV-positive children (six to 15 years) in Botswana. Eligibility included CD4 cell counts < 700/mm3 (a marker for the severity of HIV infection), documented treatment with antiretroviral (ARV) drugs, and no reported evidence of taking supplemental food products with one or more added nutrients in the six-month period prior to the study. The intervention (12 months) consisted of two food supplements for ethical reason, one with a higher protein content, bean (bean-sorghum based) group (n=97) and a cereal (sorghum) group (n=104) both of which contained added energy- and micro- nutrients. Anthropometric and biochemical nutritional status indicators (stunting, wasting, underweight, skinfolds for fat and muscle protein reserves, and hemoglobin levels) were compared within and between the bean and the cereal groups pre- and post-intervention separately for children six to nine years and ten to 15 years.

Findings

Older children (ten to 15 years) fared worse overall compared to those who were younger (six to nine years) children in anthropometric and protein status indicators both at baseline and post-intervention. Among children six to nine years, the mid arm circumference and blood hemoglobin levels improved significantly in both the bean and cereal groups (p < 0.01 and p < 0.05, respectively). Although the BMI for age z-score and the triceps skinfold decreased significantly in the bean group, the post-intervention subscapular skinfold (fat stores) was significantly higher for the bean group compared to the cereal group (p < 0.05). Among children ten to 15 years, both the bean and the cereal groups also showed improvement in mid arm circumference (p < 0.001), but only those in the bean group showed improvement in hemoglobin (p < 0.01) post-intervention.

Originality/value

Similar significant nutritional status findings and trends were found for both food interventions and age within group pre- vs post-comparisons, except hemoglobin in the older children. Post-intervention hemoglobin levels for the type food supplement was higher for the “bean” vs the “cereal” food in the younger age group. The fact that all children, but especially those who were older were in poor nutritional status supports the need for nutrition intervention in conjunction with ARV treatment in children with HIV/AIDS, perhaps using a scaled up future approach to enhance desired outcomes.

Abstract

Purpose

The purpose of this paper is threefold: first, to measure the health inequalities among malnourished children; second, to decompose the health inequalities to identify key socioeconomic predictors for child malnutrition; and third, to assess the change in the proportional contribution of key predictors over time.

Design/methodology/approach

The study has used data of National Family Health Survey (NFHS) conducted in 1992-1993, 1998-1999 and 2005-2006. The information on anthropometric indicators for children below three years of age is provided; however the study is restricted to “weight-for-age,” as it is considered to be a comprehensive indicator of child nutritional status. In the first stage of analysis, health inequalities are measured among malnourished children using concentration indices (CI) for each round of NFHS. In second stage, the inequalities are decomposed to estimate the proportional contribution of socioeconomic predictors. In the third stage, change in the relative contribution of socioeconomic predictors over three rounds is assessed to suggest target-specific policies and programs.

Findings

The results highlight a slow decline of only seven percentage points in the proportion of malnourished children in India during 1992-2006. The increasing values of CIs from −0.13 (1992) to −0.18 (2006) demonstrates the concomitant rise in economic inequalities among malnourished children. The results of decomposition analysis point toward household poor economic status and mother’s illiteracy as the major contributor of inequalities during 1992-2006. During the study period, the economic status explained 50, 65 and 59 percent of inequalities, whereas mother’s illiteracy explained 40, 30 and 29 percent of inequalities, respectively. Overall, the contributors to health inequalities remained the same over time with a change in their relative contribution.

Research limitations/implications

The present study is focussed on three rounds of NFHS data conducted at different time period and so it has certain limitations which should be kept in mind while interpreting the results. The study has revealed mother’s education and economic status of the household as the major contributory factors toward child health inequalities. However, one should not forget that the level and quality of education has undergone tremendous change from 1992 to 2006 which the authors could not capture considering the availability of data in the form of years of schooling. Second, since the NFHS-1 has collected the information about the caste groups in only three categories, i.e. schedule caste, schedule tribe and others; the authors have to pool the other backward caste groups with the general caste groups. Third, the authors have used the broad classification of place of residence, i.e. rural and urban area to analyze the inequalities assuming the uniform level of development across the urban regions; however there exists huge disparities within urban areas which leave scope for further research. Fourth, though, the authors have estimated the wealth based inequalities, but NFHS does not provide the absolute level of wealth and so the authors have computed the proxy measure for wealth based on the household assets which has been extensively used in the field of research. Despite these limitations, the authors however believe that the present research work has appropriately decomposed the inequalities among malnourished children and have revealed the changes in the proportional contribution of socioeconomic factors over time.

Practical implications

The decomposition analysis brought into light that average health indicators are insufficient for determining the right approach to health intervention programs. Health policy interventions have to focus ideally on both health averages and within and between group inequalities based on varying contributions of socioeconomic determinants.

Social implications

Concentrated efforts along with the inter-sectoral concurrence, good nutrition governance, effective investment and unequal distribution of resources are pre-requisites to ameliorate the level and existing inequalities in child malnutrition in India.

Originality/value

The distinctiveness of this study can be primarily found in the use of all three rounds of NFHS data to estimate health inequalities among underweight children. The study has also decomposed the health inequalities to estimate and analyze the change in relative contribution of socioeconomic predictors for each round to facilitate the formulation of target-specific policies and programs.
