Abstract

Purpose

This survey was initially conceived as an audit to highlight the National Institute for Clinical Excellence (UK), guidelines on depression in 2004. In particular, the audit was to examine the guidance on the prescription of Venlafaxine, which required an electrocardiogram (ECG), and blood pressure measurements.

Design/methodology/approach

A retrospective case note review by the authors, of 123 patients prescribed Venlafaxine within Leicestershire Partnership NHS Trust was completed.

Findings

Nearly 50 per cent of patients had a blood pressure measurement prior to starting Venlafaxine compared to only 6 per cent of patients receiving an ECG.

Practical implications

The audit cycle could not be completed following the publication of a further set of guidelines that removed the need for ECG recordings.

Originality/value

This paper highlights how difficult it is for clinicians to keep up‐to‐date, and implement rapidly changing guidelines.

Abstract

Purpose

This study aims to investigate the potential of pumpkin seed oil (PSO) and zinc to attenuate oxidative stress and neuroinflammation caused by chronic mild stress (CMS) in the cerebral cortex of male rats.

Design/methodology/approach

The rats were submitted to stress for six weeks and then the behavior of the rats was tested by forced swimming test (FST) and novel cage test. The treated groups were given venlafaxine (20 mg/kg), pumpkin seed oil (40 mg/kg) and zinc (4 mg/kg). The cortex homogenate was used for the detection of the oxidative stress parameters, the concentration of neurotransmitters, tumor necrosis factor-α (TNF-α) and interleukin 1β (IL-1β), Na+/K+-ATPase activity, and the expression of histamine N-methyltransferase (Hnmt) and tyrosine hydroxylase (Th).

Findings

CMS causes a significant increase in immobility time in the FST and a significant decrease in the number of rearing in the novel cage test. CMS group showed a significant increase in alanine aminotransferase (ALT) activity, levels of cortisol, TNF-α, IL-1β, nitric oxide and malondialdehyde. CMS caused a significant decrease in the concentrations of serotonin, GABA, norepinephrine, and the activities of glutathione peroxidase, catalase, superoxide dismutase and Na+/K+-ATPase. CMS caused a marked reduction in the expression of Hnmt and Th in the cortex. PSO and zinc attenuated the Na+/K+-ATPase activity, oxidative parameters and neuroinflammation induced by the CMS, and this was reflected by the elevation of the concentration of neurotransmitters and reduction of cortisol and ALT, in addition to the behavior normalization. PSO and zinc attenuated the CMS by improving the antioxidant milieu and anti-inflammatory status of the cerebral cortex.

Abstract

Purpose

The purpose of this paper is to check the geranium oil and anise oil effect to inhibit inflammation in brain cerebral cortex and hippocampus areas in depression.

Design/methodology/approach

Depression defined as psychiatric disease and chronic mild stress (CMS) model a well-known animal model of depression that represented major symptoms occurred in human depression. Geranium oil and anise oil selected for such a study to check their anti-inflammatory effect in brain tissues in depressed animal model.

Findings

The brain cerebral cortex and hippocampus neurotransmitters serotonin, dopamine, norepinephrine, gamma aminobutyric acid (GABA) and interleukin (IL)-10 significantly decreased (p < 0.001) while brain cerebral cortex and hippocampus IL-1ß, IL-6, tumor necrosis factor-alpha (TNF-α) and Ki-67 levels significantly increased (p < 0.001) in CMS rats compared to control. The oral intake of venlafaxine drug, anise oil and geranium oil significantly increased (p < 0.001) serotonin, dopamine, norepinephrine, GABA and IL-10 while significantly decreased (p < 0.001) IL-1ß, IL-6, TNF-α and Ki-67 levels to approach normal levels in brain cerebral cortex and hippocampus areas compared with CMS rats.

Originality/value

Antidepressants used in depression treatment but these drugs are either too expensive or had side effects. Folklore and complementary medicine used in different diseases treatment due to cheap and available source. Geranium oil and anise oil had anti-inflammatory effect in brain cerebral cortex and hippocampus areas in CMS rats.

Abstract

Purpose

The purpose of this study was designed to investigate anise oil and geranium oil to amend body weight, serum bile acid and vitamin D, and liver histology in depressed rats.

Design/methodology/approach

Eighty male albino rats were divided into normal and depressed rats. Normal rats (40 rats) were divided into four equal groups: control, venlafaxine drug, anise oil and geranium oil groups. Depressed rats (40 rats) were divided into four equal groups: depressed rats, depressed rats + venlafaxine drug, depressed rats + anise oil and depressed rats + geranium oil groups. Body weight, food consumption and water intake were detected. Animal behavior, cerebral cortex and hippocampus neurotransmitters, serum bile acid and vitamin D and liver histology were also investigated in this study.

Findings

Body weight (117 ± 7.6 g), food consumption (5.6 ± 1.4 g/day) and water intake (8.7 ± 1.2 ml/day) were significantly decreased (p < 0.001) in depression compared to body weight (153 ± 7.6 g), food consumption (12.7 ± 1.6 g/day) and water intake (15.3 ± 1.6 ml/day) in control. Animal behavioral tests, e.g. sucrose preference (48.8 ± 1.5) test, distance traveled (70.0 ± 16.3), center square entries (0.20 ± 0.10), center square duration (52.18 ± 11.9), tail suspension (54.70 ± 2.9 s) test and forced swimming (134.4 ± 5.5 s) test were significantly decreased (p < 0.001) in depression compared to sucrose preference (89.2 ± 1.7) test, distance traveled (226 ± 90.1), center square entries (1.4 ± 1.8), center square duration (3.6 ± 2.0), tail suspension (19.3 ± 2.1 s) test and forced swimming (83.7 ± 3.6 s) test in control. Cerebral cortex and hippocampus areas neurotransmitters such as serotonin (7.4 ± 1.7 and 1.2 ± 0.54 pg/g tissue), dopamine (6.3 ± 1.5 and 0.86 ± 0.07 pg/g tissue), norepinephrine (8.1 ± 1.7 and 1.4 ± 0.41 pg/g tissue) and gamma aminobutyric acid (GABA) (1.3 ± 0.41 and 0.08 ± 0.04 µmole/g tissue), serum bile acid (46.8 ± 3.5 µmole/L) and vitamin D (1.3 ± 0.37 ng/ml) were significantly decreased (p?0.001) in depression compared to cerebral cortex and hippocampus areas neurotransmitters such as serotonin (16.8 ± 2.1 and 4.0 ± 1.4 pg/g tissue), dopamine (15.7 ± 2.0 and 1.8 ± 0.49 pg/g tissue) norepinephrine (18.2 ± 2.3 and 3.8 ± 1.3 pg/g tissue) and GABA (2.7 ± 0.62 and 0.16 ± 0.06 µmole/g tissue), serum bile acid (90.5 ± 4.3 µmole/L) and vitamin D (2.7 ± 0.58 ng/ml) in control. Depression induced injury to hepatic tissues. Oral supplementation with anise oil and geranium oil ameliorated body weight, serum bile acid and vitamin D and liver histology in depressed rats.

Originality/value

Depression treatment persists for a long time, so the search for a new herbal treatment is of concern due to available sources, cheap and no side effects of herbal plants. Anise oil and geranium oil improved body weight, food consumption, water intake, animal behavioral tests, cerebral cortex and hippocampus areas neurotransmitters, serum bile acid and vitamin D and liver histology in depression.

Abstract

Purpose

Micropollutants in the aquatic environment pose threats to both ecosystems and human health. Traditional wastewater treatment plants (WWTP) reduce some micropollutants, especially those who adhere to sludge or suspended matter. The hydrophilic micropollutants, on the other side, which may be non-biodegradable and resistant to UV-treatment etc. are typically transported untreated into the water recipients. This paper contains a literature study on the state of the art of advanced wastewater treatment technologies for reducing micropollutants such as pharmaceutical degradation products, personal care products, surfactants and industrial chemicals including heavy metals.

Design/methodology/approach

This literature study is completed using the most extensive and expansive literature database in the World to date, Google Scholar (GS). Published papers in recognized scientific journals are sought out in GS, and for relevance for this literature study, papers published here from 2016 and onwards (the last 5 years) have been chosen to eliminate irrelevant studies.

Findings

The result of the study is that there are many promising technologies on the market or emerging; however, no one solution treats every micropollutant equally well. Since advanced technologies often require expensive investments for municipalities and companies, it is important to identify which micropollutants pose the highest risk towards human health and the environment, because choosing systems to eliminate them all is not economically wise, and even choosing a system combining the existing technologies can be more expensive than states, municipalities and private companies are capable of investing in.

Research limitations/implications

The research is limited to published papers on GS, which may omit certain papers published in closed databases not sharing their work on GS.

Practical implications

The practical implications are that practitioners cannot find go-to solutions based on the conclusions of the research and thus need to use the results to investigate their own needs further in order to make the wisest decision accordingly. However, the paper outlines the state of the art in advanced wastewater treatment and explains the benefits and downsides of the technologies mentioned; however, more research in the field is required before practitioners may find a proper solution to their specific issues.

Social implications

The social implications are that the consequences of introducing a removal of micropollutants from the water environment can ultimately effect the citizens/consumers/end-users through added costs to the tariffs or taxes on advanced wastewater treatment, added costs on everyday goods, wares and products and added costs on services that uses goods, wares and products that ultimately produces micropollutants affecting the water environment.

Originality/value

This paper presents a much needed state of the art regarding the current advanced technologies to mitigate micropollutants in wastewater. The overview the paper provides supports politics on national as well as international levels, where larger unions such as the EU has stated that advanced wastewater treatment will be the next step in regulating pollutants for aquatic outlet.

Purpose

The management of individuals with autism spectrum disorders (ASDs) requires a multimodal approach of behavioural, educational and pharmacological treatments. At present, there are no available drugs to treat the core symptoms of ASDs and therefore a wide range of psychotropic medications are used in the management of problems behaviours, co-occurring psychiatric disorders and other associated features. The purpose of this paper is to map the literature on pharmacological treatment in persons with ASD in order to identify those most commonly used, choice criteria, and safety.

Design/methodology/approach

A systematic mapping of the recent literature was undertaken on the basis of the following questions: What are the most frequently used psychoactive compounds in ASD? What are the criteria guiding the choice of a specific compound? How effective and safe is every psychoactive drug used in ASD? The literature search was conducted through search engines available on Medline, Medmatrix, NHS Evidence, Web of Science and the Cochrane Library.

Findings

Many psychotropic medications have been studied in ASDs, but few have strong evidence to support their use. Most commonly prescribed medications, in order of frequency, are antipsychotics, antidepressants, anticonvulsants and stimulants, many of them without definitive studies guiding their usage. Recent animal studies can be useful models for understanding the common pathogenic pathways leading to ASDs, and have the potential to offer new biologically focused treatment options.

Originality/value

This is a practice review paper applying recent evidence from the literature.

Purpose

Electronic medical case files of male prisoners in a category B prison in London were studied to establish a prevalence during an eight-month period of the use of and the reasons for prescribing gabapentinoids in prison and also to establish prescribing standards in prison and compliance with these. In addition, the prevalence of co-prescription of gabapentinoids with opioids and antidepressants, particularly tricyclic antidepressants such as amitriptyline, was also assessed in light of the increased risk of respiratory depression resulting in death when these drugs are used in combination. The paper aims to discuss these issues.

Design/methodology/approach

A retrospective, SystmOne case-file based survey was undertaken searching by SNOMED CT supplemented by examination of free text, in a category B prison for males (Capacity 1,500 prisoners; Average turnover of prisoners up to 6,000 per year), to establish practice standards related to the prescription of Gabapentinoids in the prison and determine compliance with these.

Findings

In total, 109 cases were identified of prisoners having been prescribed gabapentinoids, pregabalin in 66 cases (61 per cent) and gabapentin in 43 cases (39 per cent). In 36 cases (33 per cent) prescriptions were for unlicensed indications. This in fact represented 50 per cent of the cases where the indications were documented. In 51 cases (47 per cent) gabapentinoids were prescribed with an opioid substitute. In 14 cases (13 per cent), prescribed gabapentinoids were diverted to other prisoners.

Practical implications

The initiation of gabapentinoids in prison should be avoided. For prisoners who are also receiving opioid substitutes or are abusing opiates, it may be unsafe to continue on gabapentinoids. Issues raised by this study are likely to apply to other prisons, secure forensic psychiatric facilities and indeed community mental health and primary care as well.

Social implications

Risk of dependance on gabapentinoids including risk of mortality when taken with opioids and opioid substitutes.

Originality/value

This is an original study conducted at a category B prison in London.
