Abstract

This chapter describes Andrews and Bonta's (2006) Principles of Effective Offender Treatment and its relevance for the treatment of sexual offenders. The three principles of this model are Risk, Needs and Responsivity. Each of these is described in some detail with the greatest emphasis being placed on general responsivity which is one of the two parts of the Responsivity Principle. Our interpretation of general responsivity differs from the view of others (e.g. Hanson et al., 2009) who define this aspect of Responsivity in terms of Cognitive Behaviour Therapy (CBT). While Andrews and Bonta indicate that within their meta-analyses, CBT programmes were the ones most likely to succeed; such programmes were not at all effective. It seems to us that a far more important aspect of general responsivity is what Andrews and Bonta describe as the Core Correctional Practices (CCP) which have to do with the way in which treatment is delivered. We review the CCPs in some detail and provide other evidence indicating that the style of treatment delivery is the crucial factor in determining effectiveness.

Abstract

Purpose – Treatment for alcohol and drug addiction in correctional settings has become commonplace throughout much of the United States. The delivery of treatment services in prisons is a promising approach and has certain advantages relative to outpatient and voluntary treatment, including (i) certainty of program enrollment and participation by individuals who would not likely seek treatment on their own (i.e., coerced participation/guaranteed delivery of treatment); (ii) program modalities specific to residential settings as treatment options – in effect, more intensive treatment; and (iii) the parole process ensures participation in post-release aftercare services. During this era wherein reentry is a pronounced theme throughout American corrections, substance abuse treatment is fundamental in terms of rehabilitating offenders, increasing public safety, and lowering recidivism rates and, ultimately, the overall prison population.

Methodology – Using data from a process evaluation of an in-prison alcohol treatment program in Texas, this study examines the environmental barriers to effective recovery present in correctional settings and considers the strengths and weaknesses of coercive treatment, generally.

Findings – Findings indicate that offenders can indeed become motivated to change through coerced treatment. However, study findings also suggested that a certain number of offenders will not become engaged in treatment and fail to develop any internal motivation, which can be problematic for a number of reasons.

Practical implications – The highly coercive and restrictive nature of correctional facilities may negate the overall rehabilitative intent of treatment programs.

Abstract

In this chapter, we first describe the all governing principles of treatment for sexual offenders that maximise effectiveness. These are derived from Andrews and Bonta's (2006) summary of a variety of meta-analyses of outcome studies. From this source and others, we then claim that there are three elements essential to effective treatment: (1) targeting criminogenic features; (2) employing empirically sound procedures to modify these targets; and (3) delivering treatment in an effective psychotherapeutic way. Next we describe our treatment approach that emphasises these crucial elements within a strength-based programme that is motivational and provides Ward's (2002) Good Lives Model as the framework. We then challenge the broadly accepted idea that the Random Controlled Trial (RCT) is the only basis upon which inferences about treatment effectiveness can be derived. We point to methodological, practical and ethical problems associated with implementing an RCT study and offer at least two alternatives: the so-called ‘incidental design’ which compares the outcome of the treated group with a matched (but not randomly assigned) group from the same or similar setting to the treated group; and a strategy where the recidivism rate of treated group is compared with what would be expected on the basis of risk assessments of each of the treated subjects.

Abstract

Although healthcare and healthy living are integral to the Sustainable Development Goals (SDGs) for 2030, the coronavirus epidemic has dealt a devastating blow to these efforts. As governments and policymakers were compelled to shift their focus to lockdowns, sustenance, procurement, and distribution of vaccines, the momentum for health initiatives slowed, and the already fragile health systems of emerging markets were subjected to additional shocks. However, in many underserved regions of the globe, the introduction of technology has greatly facilitated the distribution and adoption of healthcare services.

This chapter highlights mini-cases from four emerging nations: Bangladesh, Nigeria, Vietnam, and the Philippines. Although the countries are emerging, each one of them are in a distinct stage of development and face a unique set of healthcare-related challenges. The chapter showcases how four different organizations based in these countries leveraged the use of technology to take healthcare services to underserved populations. In doing so, they addressed the key challenges of imparting healthcare: geographic accessibility, availability, financial accessibility, and acceptability.

This chapter concludes with a discussion of the implications of expanding healthcare industries leading to increased healthcare waste. To prevent mass population exposure to hazardous substances, the emergence of intelligent healthcare waste collection and disposal systems will be an absolute necessity. Hence, with the development of healthcare services, governments and policymakers need to mechanize smart waste management systems to safeguard humans, animals, and the environment.

Abstract

Questions regarding the effectiveness of criminal justice efforts to reduce crime have dominated social and political thinking in this area for more than a century (Bowen, 2011). During this time a number of philosophical shifts regarding the aims of correctional systems have occurred, fuelled typically by the prevailing political standpoint (McGuire, 2005). At the start of the twentieth century, policymakers in the United States and United Kingdom placed faith in the rehabilitative ideal and offender ‘treatment’-dominated corrections policies (Cullen & Gendreau, 2000). In this context ‘treatment’ refers to a range of interventions designed to alter the individual, contextual and social factors that sustain offending behaviour (Hollin, 1999). This remained the prevailing perspective for the subsequent seven decades until questions arose regarding the quality of ‘state run’ corrections facilities in the United States in the early 1970s. At this point, evaluation science was one of many influences on a change of policy (Hollin, 1999). Martinson's (1974) now infamous research synthesis, described by Glaes (1998, p. 713) as ‘a watershed moment’, provided politicians and policymakers with greater justification for changing the focus of corrections policy. Although arguably it was observers misreporting of Martinson's claims about the evidence which were most influential, rather than the review itself. According to Martinson (1974):It is just possible that some of our treatment programs are working to some extent, but that our research is so bad that it is incapable of telling. Having entered this very serious caveat, I am bound to say that these data … give us very little reason to hope that we have in fact found a sure way of reducing recidivism through rehabilitation. This is not to say that we found no instances of success or partial success; it is only to say that these instances have been isolated, producing no clear pattern to indicate the efficacy of any particular method of treatment. (p. 49)

Treatment integrity (TI; also known as fidelity of implementation, treatment fidelity, and procedural reliability) refers to the degree to which an intervention is implemented as intended. TI data provides evidence of the internal validity of a study; without TI data, one cannot attribute observed effects to an intervention or distinguish whether interventions that fail do so because of problems with the intervention, its delivery, or both. Unfortunately, the field of intervention research has seen limited progress in the assessment and reporting of TI over time. This chapter describes the development of models of TI across fields, options for measuring TI, and important issues yet to be resolved.

Barriers to adequate healthcare in rural areas remain a grand challenge for local healthcare systems. In addition to patients' travel burdens, lack of health insurance, and lower health literacy, rural healthcare systems also experience significant resource shortages, as well as issues with recruitment and retention of healthcare providers, particularly specialists. These factors combined result in complex change management-focused challenges for rural healthcare systems. Change management initiatives are often resource intensive, and in rural health organizations already strapped for resources, it may be particularly risky to embark on change initiatives. One way to address these change management concerns is by leveraging socio-technical simulation models to estimate techno-economic feasibility (e.g., is it technologically feasible, and is it economical?) as well as socio-utility feasibility (e.g., how will the changes be utilized?). We present a framework for how healthcare systems can integrate modeling and simulation techniques from systems engineering into a change management process. Modeling and simulation are particularly useful for investigating the amount of uncertainty about potential outcomes, guiding decision-making that considers different scenarios, and validating theories to determine if they accurately reflect real-life processes. The results of these simulations can be integrated into critical change management recommendations related to developing readiness for change and addressing resistance to change. As part of our integration, we present a case study showcasing how simulation modeling has been used to determine feasibility and potential resistance to change considerations for implementing a mobile radiation oncology unit. Recommendations and implications are discussed.

Abstract

Workplace voice is well-established and encompasses behaviors such as prosocial voice, informal complaints, grievance filing, and whistleblowing, and it focuses on interactions between the employee and supervisor or the employee and the organizational collective. In contrast, our chapter focuses on employee prosocial advocacy voice (PAV), which the authors define as prosocial voice behaviors aimed at preventing harm or promoting constructive changes by advocating on behalf of others. In the context of a healthcare organization, low quality and unsafe patient care are salient and objectionable states in which voice can motivate actions on behalf of the patient to improve information exchanges, governance, and outreach activities for safer outcomes. The authors draw from the theory and research on responsibility to intersect with theories on information processing, accountability, and stakeholders that operate through voice between the employee-patient, employee-coworker, and employee-profession, respectively, to propose a model of PAV in patient-centered healthcare. The authors complete the model by suggesting intervening influences and barriers to PAV that may affect patient-centered outcomes.

Abstract

The contemporary focus on high fidelity implementation of research-based practices often creates tensions for educators who seek to balance fidelity with needed flexibility as they strive to improve learner outcomes. In an effort to improve how decisions are made such that flexibility is achieved while fidelity to core components is maintained, this chapter begins with a discussion of the role of fidelity in research and practice. Particular attention is given to current conceptualizations of fidelity that may help inform theoretically and empirically driven adaptations to research-based practices. Specifically, we describe adaptations based on the instructional context for implementation and the characteristics of the individual learners. A framework for adapting research-based practices is then presented with relevant examples from research designed to optimize learner responsiveness without sacrificing fidelity to core components. The chapter ends with implications and future directions for research and practice.
