Abstract

Purpose

People with an intellectual disability (ID) are more at risk of experiencing adverse childhood events. Moreover, prolonged exposure to ACEs results in enduring changes and impairments in neurological, physiological and psycho-social systems and functioning. In response, van der Kolk et al. (2009) have put forward the concept of developmental trauma disorder (DTD) to reflect the “constellation of enduring symptoms” and complex care needs of this population. The purpose of this paper is to ascertain the level of exposure to adverse childhood events and the prevalence of DTD in an inpatient forensic ID population.

Design/methodology/approach

A retrospective file review and consensus approach to diagnosis were used in a sample of adults with an ID detained in a secure forensic service.

Findings

Results revealed that 89 admissions (N=123) had been exposed to at least one significant ACE, with 81 being exposed to prolonged ACEs. A total of 58 admissions (47 per cent) met criteria for PTSD and 80 (65 per cent) met the criteria for DTD. Significant gender differences were noted in MHA status, primary psychiatric diagnoses, exposure to ACEs and DTD.

Research limitations/implications

The discussion explores the implications for working with forensic ID populations who report high incidents of childhood trauma and the utility, strengths and weaknesses of the proposed DTD, its relationship to ID diagnoses is explored.

Originality/value

The study outlines the prevalence of DTD and PTSD in ID forensic populations and suggests additional key assessment and treatment needs for this population.

Abstract

Purpose

Knowledge about the prevalence and impact of Adverse Childhood Experiences (ACEs) is pivotal to trauma-informed approaches, yet the impact of ACEs training is rarely investigated. This study reports a qualitative investigation of police perceptions of ACEs training in relation to conceptualisations of ACEs and trauma-informed working, practical applications of ACE knowledge and service-level support.

Design/methodology/approach

Four focus groups were conducted with 29 police officers, who had participated in an ACEs-awareness training. Based on the qualitative data, themes were generated using reflexive thematic analysis (Braun and Clarke, 2019).

Findings

Analysis generated seven themes, conceptualised into three domains of conceptual understanding, police culture and operationalising ACEs.

Research limitations/implications

The sample is limited to Scottish police officers and is ethnically non-diverse. Further evaluation of higher quality interventions is warranted.

Practical implications

The study highlighted that a lack of conceptual framework, officer concerns and police culture may present barriers to officers incorporating ACEs knowledge into their day-to-day work. Future trainings should address these issues to achieve maximum benefits.

Originality/value

To the authors’ knowledge, this is the first in-depth qualitative study of police officers' perceptions of ACEs training. Focus groups facilitated the expression of cultural norms. The results provide insight into tailoring trauma-informed interventions in police in future, as well as raising broader service-level issues.

Abstract

Purpose

This conceptual article presents a schematic for use with extended cybernetic recursion in living systems and applies it to the issue of hyper vigilance as a demonstration of its utility.

Design/methodology/approach

The test-operate-test-exit (TOTE) schematic of Miller et al. (1960) is critically evaluated along with other schematics, including those of ordered cybernetics, and a new schematic is proposed, a recursive test-operate-test (rTOT), which emphasizes teleological purpose and hierarchical structure. The background psychophysiology of trauma is reviewed and then rTOT is applied to hyper vigilance, a cardinal component of post-traumatic stress disorder (PTSD).

Findings

Once the schematic was developed, it was applied to the behavior of hyper vigilance. Other applications are suggested.

Research limitations/implications

As demonstrated, the rTOT schematic has potentially wide application because of its pragmatic and detailed structure.

Practical implications

The rTOT requires careful consideration of teleological purposes for its application and is simple enough, but also complex enough, for relevant utilization. Its compact nature and adjustable hierarchy scope are good mini-max complexity solutions for cybernetic, information modeling schematics.

Social implications

The revealed teleological purpose of the trauma adaptation of hyper vigilance presents significant alternative formulation options for prevention and intervention.

Originality/value

While the rTOT schematic is derived from previous schematics, it is original in its emphasis on information processing, the teleological aspects of extended recursion and on the provision of a hierarchical structure for those recursions. It is considerably more compact than other schematics associated with the ordered cybernetics literature. The explication of the adaptation model for post-trauma consequences is significantly enhanced by the rTOT application.

Literature of trauma in the workplace after an organizational catastrophe often focuses on various aspects of a critical incident where data are more readily available because of organizational “pressure points” – public relations, technology and the financial impact of a disaster on a corporation. Highlights the fact that external public rarely express concern or even interest in the mental health of workers in an organization perceived as being responsible for a catastrophe. Provides results from recently published studies that reveal workers experience some trauma after a critical incident, ranging from mild depression, to the onset of manic disease, to suicidal thoughts. Scrutinizes the impact on departmental and organizational morale, production and sustainability of key projects to comprehend the organizational behaviour dimension of critical incidents in an appropriate context. Overviews the relationship of organizational behaviour to crisis management and analyses the impact of trauma upon workers at one department of large oil exploration company operating in Alaska. Reaches beyond anecdotal surveys to include an analysis of employee turnover in the immediate department of that company after four workers had been badly injured in a serious industrial accident. Results suggest attention must be paid to stress and trauma by employees who witness organizational catastrophes.

Abstract

Purpose

Thousands of Eritrean youth flee due to extreme domestic discontent with Eritrean Government. Little research has been done on Eritrean refugees in Ethiopia. The purpose of this paper is to explore the difficulties facing Eritrean refugees in the Shimelba Refugee Camp in northern Ethiopia. The study explores the refugees’ pre-migration experiences as well as their life difficulties in the refugee camp.

Design/methodology/approach

This study employed a cross-sectional qualitative approach. Relevant data were collected through personal interviews with a sample of 15 refugees. The study was also supplemented by the researcher’s personal observations regarding the living conditions of the refugees in the camp.

Findings

Gross human rights violations at home forced the Eritreans to flee in to Ethiopia. Refugees reported their experiences of arrest, torture, and abuse, due to their dissenting political and religious opinions. Moreover, they fled Eritrea to escape harsh compulsory conscription into the Eritrean military service. Unemployment and lack of income were also important push factors. Factors identified as threats to the psychosocial health of refugees at the refugee camp include the feeling of isolation, the absolute uncertainty of the future, fears concerning the safety of the family left behind, the strictness of the structure within the camp, and the fear of health-related problems associated with the limited health care facilities.

Research limitations/implications

Further research investigating the mental health problems of the refugees employing quantitative methods is needed. Also, research about the potential avenues for ameliorating the challenges faced by these refugees is desirable.

Originality/value

This paper gives an insight to the situation of Eritrean refugees in Ethiopia, which includes the reasons why they flee Eritrea, their experiences during flight, and the conditions in which Eritrean refugees live in the camp from the point of view of the refugees themselves. Understanding the challenges facing the refugees has implications for how short- and long-term policies can be altered to better serve them.

Recent research including advances in neuro‐imaging indicates a profound effect on brain development as a result of exposure to abuse and neglect in childhood. This new area is called developmental traumatology. Areas of the brain that may particularly be affected are important in arousal control and executive function. Many adolescents presenting to forensic services have histories of neglect and abuse and deficits in key brain developmental functions. The management and treatment options for young people with developmental trauma and forensic needs are explored.

Abstract

Purpose

Borderline personality disorder (BPD) is a complex condition characterized by a number of psychosocial difficulties that typically involve considerable suffering for individuals with the condition. Recovery from BPD may involve specific processes such as work on how the self is perceived by the individual with BPD and his or her relationships which differ from those common to recovery from other mental health conditions. The details of the processes that may best promote changes within the self and relationships are yet to be established. The paper aims to discuss these issues.

Design/methodology/approach

In total, 17 consumers from a specialist BPD service were interviewed to identify factors they have experienced that contribute to recovery from BPD. Thematic analysis within a grounded theory framework was used to understand key themes within the interview data. The emphasis was on specific conditions of change rather than the more global goals for recovery suggested by recent models.

Findings

Key themes identified included five conditions of change: support from others; accepting the need for change; working on trauma without blaming oneself; curiosity about oneself; and reflecting on one’s behavior. To apply these conditions of change more broadly, clinicians working in the BPD field need to support processes that promote BPD-specific recovery identified by consumers rather than focusing exclusively on the more general recovery principles previously identified within the literature.

Originality/value

The specific factors identified by consumers as supporting recovery in BPD are significant because they involve specific skills or attitudes rather than aspirations or goals. These specific skills may be constructively supported in clinical practice.

Abstract

Purpose

This paper seeks to explore three key aspects of the education of children in care: the composition of that population of children and the extent to which they differ from the general population of children due to difficulties most of them have experienced prior to as well as after entering care; issues relating to the identification of causal relationships and the extent of “underachievement” by children in care; and any evidence that care may provide more positive opportunities than is often supposed.

Design/methodology/approach

The paper's approach is an extensive literature review of existing published research into social policy and practice of caring for looked after children.

Findings

The significant factors that contribute to better achievement for children in care are: placement stability and support at school but for some children therapeutic help and specialist assessments are necessary to improve outcomes. Different analyses produce different results and the scrutiny of children's trajectories indicates better outcomes than one‐off comparisons with children not in care.

Originality/value

Extensive research has established that children in care achieve less educationally than their peers not in care, but does not explain why. This paper helps to fill this gap.

Abstract

The aim of this paper is to gather information that will be useful to practitioners who are assessing and trying to understand the difficulties of people with intellectual disabilities who may have experienced sexual abuse. In the first part of this paper the research into the effects of sexual abuse on people with learning disability is reviewed. In the second part of this paper, the major clinical implications of these findings are explored. These include the implications for abuse evaluations, identification of individuals at increased risk of disturbance, implications for treatment and provision of psychotherapeutic services.
