Abstract

Purpose

There is substantial evidence that young people moving from child and adolescent mental health services to adult services are more likely to experience poor transitions. However, little is known about the care pathways of young people transitioning from forensic services. This retrospective case note review sought to examine the clinical characteristics, transition pathways and psychosocial indicators of transition outcomes amongst young people in forensic medium secure services discharged to adult services.

Design/methodology/approach

The electronic records of 32 young people, who transitioned from six adolescent medium secure units in England to adult services between May 2015 and June 2016, were examined.

Findings

Approximately 65% of young people were between 18 and 19 years at the time of transition and the average waiting time from referral to discharge was six months. A total of 63% young people transitioned to community placements and adult medium secure services. Four pathways describing the journey into and out of adolescent medium secure services were identified in a subsample of 12 young people. A total of 25% young people with neurodevelopmental problems moved to specialist services.

Practical implications

The results suggest that diagnosis, severity of offence and clinical background are associated with transition pathway. Promoting a person-centred approach and gradual independence of the young person may improve current practice.

Originality/value

These results inform existing policy and clinical practice in an effort to reform transition guidelines around young people’s needs during transition times. Further studies in adolescent forensic services are needed to understand complex neurodevelopmental problems and comorbidities.

Abstract

Purpose

The purpose of this paper is to explore not only the academic measures such as grade point average of success of college-to-university transfer programs (Pathway Programs), but also the social-cultural facilitators and barriers throughout the students’ Pathway experience.

Design/methodology/approach

The experience of students and academic advisors moving between Queensdale College and North Star University (NSU) (pseudonyms) were analyzed using a mixed-methods approach including analysis of data from online surveys, secondary data (course performance), and focus group interviews.

Findings

Students who are able to enter the Pathway Programs at NSU perform on average better than their four-year traditional program peers. There remain a number of social-cultural barrier which need to be addressed to improve the overall experience of these transfer students.

Practical implications

The results from this study will assist the administrative decision makers in designing Pathways and their associated communication plans in order to meet the needs of the students with tools and supports that are both perceived by the students as valuable and are improving their Pathway experience and ultimately their academic performance.

Originality/value

The move to develop Pathway Programs in Ontario is a new phenomenon, even in provinces where this is more common, few studies exist which consider the social-cultural aspects of the student journey between the two institutions. This study moves beyond the standard academic performance data and provides insight into the critical role played by the social aspects in higher education experiences.

Abstract

Multiple sclerosis (MS) is a chronic condition with variable physical, cognitive, and quality of life impacts. Little research has investigated how MS outcomes vary by social identity (race, gender, disability, age, sexual orientation, and nationality) and social location (place within systems of power and privilege). However, emerging evidence points to racial and ethnic group disparities in MS outcomes. This chapter integrates core concepts from the life course perspective and an intersectional feminist disability framework to interrogate the role of diagnosis pathways in determining differential MS outcomes. MS diagnosis pathways (the time from symptom onset to the point of diagnosis) are a logical place to begin this work given the varying nature of symptom onset and the importance of a quick diagnosis for optimal MS outcomes. Whereas the life course perspective provides a framework for understanding disability transitions and pathways across the life span, an intersectional feminist disability framework centers disability within an axis of overlapping social identities and locations. The combination of both frameworks provides an approach capable of examining how MS disparities and inequities emerge in different contexts over time. The chapter begins with an overview of MS and current knowledge on disparities (mainly racial) in MS prevalence, diagnosis, and outcomes. The chapter proceeds to describe the utility of key concepts of both the life course perspective and intersectional frameworks when researching health disparities. Finally, the chapter ends with a theoretical application of an intersectional feminist disability life course perspective to investigate disparities in MS diagnosis pathways.

Abstract

Purpose

The purpose of this article is twofold. First, this study characterises the current state of the bio-packaging market's development. Second, it identifies key factors influencing and possible scenarios of the bio-packaging market transition to increase the market share of compostable packaging.

Design/methodology/approach

The results of 29 in-depth interviews (IDIs) with representatives of the key groups of bio-packaging supply chains' (SCs') stakeholders were the input for the consideration of the research problem.

Findings

The main economic, legal, social and technological enablers and barriers to the bio-packaging regime transition are recognised, and their impact at the market level is explained. The authors recognised the hybrid transition scenario towards an increase in the market share of compostable packaging related to the three traditional pathways of transformation, reconfiguration and technological substitution.

Originality/value

This study contributes to a better understanding of the socio-technical system theory by examining interdependencies between landscape (external environment), market regime (bio-packaging market) and niche innovations (compostable packaging) as well as system transition pathways. The findings and conclusions on bio-packaging market developments can be important lessons learnt to be applied in different countries due to the same current development stage of the compostable packaging lifecycle worldwide.

Abstract

Purpose

Despite the growing emphasis on revitalizing the technical and vocational education and training (TVET) sector in Bangladesh, very little discussion has taken place on reforming the current inflexible transition pathways, from TVET to the universities. This paper aims to reflect critically on the existing literature on TVET, in the global and national context, and the experiences of students and TVET experts, to develop a model of collaboration between the polytechnic institutes and the universities in Bangladesh.

Design/methodology/approach

This paper follows a qualitative strategy of enquiry, using a mix of critical reflection on literature on TVET and higher education and unstructured interviews with two TVET experts, four TVET students and four students from a public university. It draws on the theories of collaboration and uses exemplary cases to illustrate and support the line of reasoning.

Findings

This paper identifies that there is resource dependency between the polytechnic institutes and universities in Bangladesh, and their institutional environment necessitates them to form collaboration to ensure flexible transition pathway, from polytechnic institutes to universities; this paper proposes a model for such collaboration.

Practical implications

This paper offers a guideline for forming collaboration among the relevant stakeholders.

Social implications

Collaboration between polytechnic institutes and universities in Bangladesh is likely to address the inequitable nature of TVET, by improving its social status and acceptance, as well as allowing higher income opportunity and greater mobility for the TVET graduates, coming especially from humble socio-economic backgrounds.

Originality/value

This paper contributes in the recent discussions on how collaboration among different stakeholders can contribute in achieving the sustainable development goals, with special emphasis on TVET.

Abstract

Purpose

This paper aims to illustrate relationships between stakeholders in the conceptualisation and implementation of water and sanitation services (WSS) projects in marginalised settings of Nairobi and Kiambu Counties. It outlines these relationships in a flow diagram that shows a pathway analysis in which social innovation strategies are used as markers in the program of inclusive water provision. The study’s objective is to map the processes of social innovation in an effort to highlight the role of marginalised communities in their access to the right to water.

Design/methodology/approach

The paper approached the study using a case study design, and thereafter, constructivist grounded theory was used to further delve into the comparative cases. To map the processes of social innovation, the case study design was seen to be appropriate, as it sequenced activities in a time-series analysis. From these two case studies, four comparative cases were used to pinpoint path-breaking transition in the pathway analysis. The methods used in both phases were, namely, in-depth interviews, observations and document analysis, and these were complemented by field notes.

Findings

The paper indicates an opportunity to use emergent patterns for a more context-specific analysis of WSS projects in marginalised communities. It advances the role of marginalised communities as vital stakeholders in the approach described as “the right to the city”. The model of spatial appropriation brings to the fore the binary yet separate processes that stakeholders engage in.

Research limitations/implications

Owing to the use of a grounded theory model, the results may not be transferable to other contexts. Therefore, further testing of the proposed pathway analysis and model is encouraged, as this model suggests ways of ensuring full community engagement which would result in greater success in projects involving marginalised communities.

Practical implications

The paper has implications for both the government and communities, in that more deliberate roles for the community-based organisation in the conceptualisation of WSS projects can lead to social learning opportunities for government institutions and greater success in implementation.

Originality/value

The paper justifies the need for government institutions to map and evaluate WSS projects using emergent patterns to highlight the role of marginalised communities as their right to the city.

Abstract

Purpose

Healthcare systems are facing persistent challenges, such as dealing with an ageing population, related increases in chronic diseases and healthcare costs facilitated by technological progress. The authors argue that the boundaries of optimisation are being reached and a more fundamental change or transition is necessary. The purpose of this paper is to explore the contours of this transition in the Netherlands. The authors do this from the perspective of healthcare organisations that have participated in the “Expedition to Sustainable Healthcare”: a learning programme organised by the Dutch Network for Sustainable Healthcare aimed at creating frontrunners in this transition.

Design/methodology/approach

The paper combines conceptual with experimental empirical work. The authors use the transition research frameworks to conceptualise persistent problems and transitional dynamics in the healthcare system. In a longitudinal study, the authors analysed how the participating organisations developed after the expedition.

Findings

The process validated the initial understanding of persistent sustainability challenges. An integral approach to sustainable healthcare is translated as a transformation of culture, structures and practices and the development of capacity for crossing borders and domains, inside and outside of the organisation. To facilitate and stimulate such a process the authors found that problem structuring and collective identification of persistent problems and the unsustainability in the healthcare system is a crucial step towards a shared view and discourse that supports change.

Originality/value

A transition in the Dutch healthcare system is just starting to emerge and has barely been subject of research. This paper provides an empirical description of a transition management process in this context. The authors hope to lay a foundation for future work that seeks to explore transitions in healthcare in theory and practice.

Abstract

Purpose

Care pathways are being increasingly used in the national health service to outline an anticipated programme of care in relation to a particular illness, condition or set of symptoms. The purpose of this paper is to inform those using the service of what they might expect within what time frame. They are designed to reduce variation in practice and allow optimal quality of care across a variety of care settings. Care pathways map out a patient’s journey, providing coordination of services for users. They aim to have: “the right people, doing the right things, in the right order, at the right time, in the right place, with the right outcome”.

Design/methodology/approach

This paper outlines care pathways in relation to people with intellectual disabilities who present with behaviour that challenges.

Findings

It is likely that many people will have a lifelong need for support, so discharge from clinical services should only be considered if it is genuinely appropriate. Reductions in a person’s behaviours that challenge are likely to be a consequence of changes that have been made to the person’s environment and supports. Therefore, any reductions in the level or type of support that the person receives may lead to an escalation of the behaviour again.

Originality/value

Standards in relation to care pathways are presented.

Abstract

Purpose

This study discusses the application of Insider Action Research to the systematic integration of an environmental perspective into industrial product innovation processes and projects. Applying Insider Action Research, it aims at a broader integration between top-down and bottom-up and soft and hard perspectives in the intersection of ecodesign and innovation and project management disciplines.

Design/methodology/approach

The research design is based on action research and specificities from Insider Action Research to capture the change and transition aspects involving two main action research cycles. The longitudinal five-year research was carried out within a Brazilian cosmetics company. The outcomes of these cycles are analysed including customisation of ecodesign tools and related application, and views of different stakeholders of the challenge for the transition.

Findings

This paper describes an action research application aiming at faster learning loops in the field of sustainable innovation management. As a second contribution, an Ecodesign Transition Framework (ETF) is proposed, combining both technical and soft sides. The proposed framework is structured in three levels (strategic, tactical and operational), with two complementary perspectives of a mature ecodesign pattern and a transition pathway.

Practical implications

The action research led to the intended outcomes both on the applied side, with increased diffusion of ecodesign in the company, and on the research side, with the ETF formulation and application. Practitioners interested in sustainable innovation can follow the ETF procedures, challenges faced, lessons learnt and conclusions.

Originality/value

The Insider Action Research addressed the literature gap to better relate the recommendations in the field to business reality. The resulting ETF brings a novel perspective for integrating environmental considerations in the product innovation process of a company. It organises the soft issues to be considered for the necessary transition towards such integration, consolidating and advancing previous theoretical views of ecodesign, with associated new practical implications.
