Abstract

Purpose

The purpose of this paper is to explore the influence of hospital’s visibility for sensing (VFS), learning, coordinating and integrating on hospital-supplier collaboration. Second, it explored the influence of hospital-supplier collaboration on hospital supply chain performance. The author also explored how the technology orientation of the medical chain units influences the above linkages.

Design/methodology/approach

The study adopted a multi-unit study of different hospital supply chains. Consequently, perceptual data were gathered from seven dominant entities in a typical medical/hospital supply chain: hospitals and clinics, accommodation (i.e. hotels), chemistry and pharmaceutical, marketing/public relations/promotion, medical equipment manufacturers, food and beverage and insurance. The responses were gathered using e-mail survey and were analyzed using structural equation modeling.

Findings

Based on 192 completed responses, the author found positive influences of VFS, learning and integrating on hospital-supplier collaboration and a positive impact of hospital-supplier collaboration on hospital supply chain performance. An insignificant influence of hospital’s visibility for coordinating was noted on hospital-supplier collaboration. The study argued hospitals to invest more for enriching their dynamic capabilities to diagnose the changes in the environment so as to sustain their collaborative relationships leading to positive performance implications.

Originality/value

The study is the foremost to investigate the effects of hospital’s dynamic capabilities on its collaborative efforts with its key supplier and their influence on hospital supply chain performance. Also the study is foremost in exploring the importance of technology orientation on hospital dynamic capabilities and hospital-supplier collaboration. An important contribution of the research is the conceptualization of supply chain visibility core components (visibility of sensing, visibility of learning, visibility of coordinating and visibility of integrating) in the context of hospital supply chains.

Abstract

Purpose

The study aims to examine the role of health-care supply chain management during the COVID-19 pandemic in a cross-section of 42 selected sub-Saharan African (SSA) countries.

Design/methodology/approach

The study used cross-sectional robust least square regression for parameter estimates.

Findings

The results confirmed the N-shaped relationship between the health-care logistics performance index (HLPI) and COVID-19 cases. It implies that initially HLPI increases along with an increase in COVID-19 cases. Later down, it decreases COVID-19 cases by providing continued access to medical devices and personal protective equipment. Again, it increases due to resuming economic activities across countries.

Practical implications

The continuing health-care supply chain is crucial to minimize COVID-19 cases. The international support from the developed world in providing health-care equipment, debt resettlement and resolving regional conflicts is deemed desirable to escape the SSA countries from the COVID-19 pandemic.

Originality/value

The importance of the health-care supply chain during the COVID-19 pandemic is evident in the forecasting estimates, which shows that from August 2021 to April 2022, increasing the health-care supply chain at their third-degree level would reduce coronavirus registered cases. The results conclude that SSA countries required more efforts to contain coronavirus cases by thrice increasing their health-care logistics supply chain.

Abstract

Purpose

The past decade has witnessed a tremendous and progressive growth in the number of Nigerians who engage in medical tourism from Nigeria to India. Various commentators have advanced diverse reasons for this trend. However, there is a dearth of research that has sought to provide empirical insights. This paper aims to investigate the decision-making process of Nigerian medical tourists and why they prefer medical tourism to India to medical care locally.

Design/methodology/approach

Eight Nigerian medical tourists are interviewed on a one-on-one basis with open-ended questions using purposive criterion sampling technique from an interpretivist mind-set.

Findings

The paper identifies two major motivators, namely, inadequate medical infrastructure and poor medical, and customer service from health workers in Nigeria, which spurred medical tourism from Nigeria to India. Further, it finds that first timers premise their decisions on advice from reference groups, while previous personal experiences guide decisions on subsequent medical travels. Findings are explained using the template provided by the theory of planned behaviour.

Originality/value

This exploratory nature of this research provides a useful basis to elucidate the course of decision-making of Nigerian patients so that appropriate marketing communication channels can be applied. It improves the process of recruiting and engaging Nigerian patients and nurturing wholesome relationships between Nigerian patients and hospitals.

Purpose

Service quality (SQ) has become an essential and indispensable component in healthcare and many other industries. SQ can deliver guaranteed stakeholder value and consequent consumer delight in the healthcare sector. The purpose of this study is to identify the relationships of various SERVQUAL elements with respect to the SQ of surgical instrument suppliers among surgeons.

Design/methodology/approach

Data were collected from a sample of 112 surgeons working in the USA using the “snowball sampling” technique. A few standardised questionnaires, including SERQUAL, were used to collect the data. R-programming was used to perform structural equation modelling (SEM) analysis on the collected data.

Findings

The research study identified that service delivery factors and the SQ of surgical instruments contribute significantly towards medical practitioner sensitivity in the US healthcare industry. Word of mouth (WOM) did not have any significant impact on the medical practitioners' sensitivity.

Originality/value

A review of related literature revealed that studies that examine the surgeon's perspectives of SQ are scarce. Thus, the present study is directed towards this gap in literature. The findings of the study are significant in nature and have made a substantial contribution to management literature.

Abstract

Purpose

The purpose of this paper is to develop a business model to generate quantitative evidence of the benefits of implementing radio frequency identification (RFID) technology, limiting the scope to outpatient surgical processes in hospitals.

Design/methodology/approach

The study primarily uses the define‐measure‐analyze‐improve‐control (DMAIC) approach, and draws on various analytical tools such as work flow diagrams, value stream mapping, and discrete event simulation to examine the effect of implementing RFID technology on improving effectiveness (quality and timeliness) and efficiency (cost reduction) of outpatient surgical processes.

Findings

The analysis showed significant estimated annual cost and time savings in carrying out patients' surgical procedures with RFID technology implementation for the outpatient surgery processes in a hospital. This is largely due to the elimination of both non‐value added activities of locating supplies and equipment and also the elimination of the “return” loop created by preventable post operative infections. Several poka‐yokes developed using RFID technology were identified to eliminate those two issues.

Practical implications

Several poka‐yokes developed using RFID technology were identified for improving the safety of the patient and cost effectiveness of the operation to ensure the success of the outpatient surgical process.

Originality/value

Many stakeholders in the hospital environment will be impacted including patients, physicians, nurses, technicians, administrators and other hospital personnel. Different levels of training of hospital personnel will be required, based on the degree of interaction with the RFID system. Computations of costs and savings will help decision makers understand the benefits and implications of the technology in the hospital environment.

Abstract

Purpose

Since mid-2000s, hospitals have begun implementing radio-frequency identification (RFID) technology in order to improve their operations. The purpose of this paper is to explore the potential of RFID technology in improving the traceability of surgical instruments in a hospital environment.

Design/methodology/approach

A case study is conducted at a teaching hospital in Montreal, Canada. Business process reengineering approach and simulation techniques are used to assess the realistic potential of the RFID technology. The application of different scenarios and how they influence the efficiency of process flow between the central sterilization department and operating rooms of the hospital is investigated.

Findings

Research outcomes demonstrated how tagging individual instruments or their sets lead to reduction of the time spent in re-processing the soiled instrument as well as the reduction of costs related to staff. Furthermore, specific key performance indicators are identified and eventual issues related to implementation of the re-designed processes are discussed.

Originality/value

Implementing RFID-enabled solutions in hospital context is still an emerging phenomenon that involves various stakeholders in a change management project. While implementing RFID technology can benefit hospitals by improving business processes and workflows, the adoption is still slow, especially for managing surgical instruments. It is, hence, crucial to compare the advantages and drawbacks of RFID-enabled surgical instruments solutions with other well-established traceability technologies such as barcoding.

Abstract

Purpose

The purpose of this paper is to consider surgical robotics, with a focus on technology and design issues for remote‐mode operation assistance. The investigation leads to the definition of the technical characteristics of a co‐robotic positioning device (CRPD), to be developed in support of a split‐duty approach to planning. The expected characteristics and advantages are outlined, including the operation potential of special‐purpose devices (e.g. an automatic changer for surgical tools) and of scope‐driven enhancers (e.g. the exploration of the intervention theatre).

Design/methodology/approach

The paper addresses example developments based on projects performed with the co‐operation of other robot laboratories in Munich and Paris. The CRPD concept is applied in relation to the DLR KineMedic® arm (developed by the Munich laboratory), and with the LRP prototype mini‐arm (built by the Paris laboratory).

Findings

Minimally‐invasive surgery deserves increasing attention to reduce post‐operative hospital stays and to reduce complications. This leads to new trends in robotics, to facilitate safe, fast and accurate remote manipulation, and integrated computer‐aided implements. The features of the example CRPD design are summarised for the two cases.

Practical implications

The overall comments consider minimally‐invasive robotic surgery as a given intervention practice in the near future, and the split‐duty approach, supported by the CRPD technology, as a valuable aid for human‐robot co‐operation, according to the “best‐of‐skills” idea, supporting intervention under the surgeon's control.

Originality/value

This investigation shows new results aimed at expanding the operation versatility of robotics with integrated intelligence, to enhance scope‐driven alternatives and out‐of‐reach handling with improved dexterity and safe autonomic processing.

Abstract

Purpose

Current thinking about “patient safety” emphasises the causal relationship between the work environment and the delivery of clinical care. This research draws on the theory of normal accidents to extend this analysis and better understand the “organisational factors” that threaten safety.

Design/methodology/approach

Ethnographic research methods were used, with observations of the operating department setting for 18 month and interviews with 80 members of hospital staff. The setting for the study was the Operating Department of a large teaching hospital in the North‐West of England.

Findings

The work of the operating department is determined by inter‐dependant, “tightly coupled” organisational relationships between hospital departments based upon the timely exchange of information, services and resources required for the delivery of care. Failures within these processes, manifest as “breakdowns” within inter‐departmental relationships lead to situations of constraint, rapid change and uncertainty in the work of the operating department that require staff to break with established routines and work with increased time and emotional pressures. This means that staff focus on working quickly, as opposed to working safely.

Originality value

Analysis of safety needs to move beyond a focus on the immediate work environment and individual practice, to consider the more complex and deeply structured organisational systems of hospital activity. For departmental managers the scope for service planning to control for safety may be limited as the structured “real world” situation of service delivery is shaped by inter‐department and organisational factors that are perhaps beyond the scope of departmental management.

Abstract

Purpose

Overall equipment effectiveness (OEE) is a performance indicator that is been used to measure manufacturing productivity. The purpose of this paper is to propose the operating room effectiveness for hospital operating rooms (ORs), adapted from the OEE, to measure performance and identify losses based on lean health-care principles.

Design/methodology/approach

The present study is an exploratory, descriptive and applied research work. Literature review, documents of the hospital, observation and interviews with employees of a large university hospital in southern Brazil were analyzed to organize the proposed effectiveness indicator. After that, historical data of the ORs was collected and the usefulness of the indicator was analyzed. The indicator was applied for 10 months and validated with an expert committee from the hospital.

Findings

The present study describes an adaptation of a performance indicator to ORs of hospitals, allowing to classify its types of operational losses in a lean health-care context. The application of this indicator and the development of improvement actions to a university hospital, resulted in operational efficiency gains of 12 per cent and estimated annual savings of US$400,000.

Practical implications

ORs are a critical service for hospitals. This paper presents a new way to measure the performance of ORs and identify their main types of wastes. It also shows how to implement it and the potential gains of its application. The main research limitations are related to technical analysis of care data from doctors and nurses involved.

Originality/value

This paper fulfills the need to study how ORs performance can be measured and its operational wastes can be identified. In addition, this paper classifies the planning, performance and quality related losses, which can be used by researchers and practitioners to improve the performance of operation rooms.
