Abstract
Purpose
Homelessness and psychoactive substance (PS) use are both determinants of physical and mental ill health, with the homeless population using, and dying of PSs more frequently than the general population. However, there is a gap in research on the real-world implications psychoactive substance use (PSU) has on the homeless population. This study aims to explore the experiences of PSU from the perspective of homeless users.
Design/methodology/approach
Purposive sampling was adopted to recruit participants and semi-structured interviews collected data from participants, with interpretive phenomenological analysis (IPA) generating common themes from the data gathered.
Findings
Four participants were interviewed. The themes generated were family and close relationships; cyclical patterns; mistrust in people and services; and low self-worth.
Research limitations/implications
This study is limited by potential bias from researchers who built relationships with participants through the data collection process. Despite efforts to remove this bias, through reflexivity throughout data collection and analysis, some bias may be still present. The researchers saw the participants as vulnerable people who were striving to overcome adversity. Such conception of the participants is reflective of how the participants portrayed themselves. The small sample is suitable for IPA purposes. Of course, it could have been possible that if different participants had been recruited or more participants had been recruited, then there could have been different themes and findings. IPA prides itself on its idiographic focus.
Practical implications
More research is needed on a wider scale to assess the extent and cause of these issues. Increased education and dissemination of research such as this is required to break down stigma within the public and guide policy change in professional services.
Originality/value
This paper interpretatively presents themes generated by semi-structured interviews with four homeless PSUs. As such, these individuals are vulnerable and have faced adversity throughout life from both society and the services they use. Their vulnerability leads to a cycle of substance use and a feeling of low self-worth, which is perpetuated by the perceived views of those around them.

Abstract
Purpose
Using social learning theory and the model of innovation diffusion, this study aims to provide reflections on how new information and knowledge can be shared and adopted by farmers in collectivist rural areas.
Design/methodology/approach
Firstly, the researcher selected 76 farmers from four rural villages in Perak, Malaysia, and, using semi-structured, probing interviews, explored the underlying factors that contribute to information and knowledge transfer. Secondly, the researcher analysed 452 questionnaires to validate the qualitative interview findings. Thirdly, the researcher analysed 487 questionnaires after nine months to determine whether differences had occurred in knowledge acceptance and adoption.
Findings
Social learning and local integration play prevalent roles in information and knowledge spread among individuals. However, the data also suggest that care must be taken to ensure that the knowledge spread does not jeopardise the prevailing collective structure; rather, it must begin with innovators who show evidence of improved yield.
Practical implications
The findings suggest strategies for researchers and practitioners to transfer knowledge to farming communities using innovators and the social learning process.
Social implications
Members of a collectivist society often find it difficult to deviate from the norm; therefore, understanding how local integration, sequencing of information and knowledge spread can be accomplished through proper protocols and ethics is important.
Originality/value
While prior research has produced insights into knowledge management among individuals, the field still lacks a comprehensive understanding of the germinal stages of how individuals initiate norm-breaking behaviour while continuing to adhere to societal norms.

Abstract
Purpose
Society is critically dependent on an adequate supply of hospital doctors to ensure optimal health care. Voluntary turnover amongst hospital doctors is, however, an increasing problem for hospitals. The aim of this study was to systematically review the extant academic literature to obtain a comprehensive understanding of the current knowledge base on hospital doctor turnover and retention. In addition to this, we synthesise the most common methodological approaches used before then offering an agenda to guide future research.
Design/methodology/approach
Adopting the PRISMA methodology, we conducted a systematic literature search of four databases, namely CINAHL, MEDLINE, PsycINFO and Web of Science.
Findings
We identified 51 papers that empirically examined hospital doctor turnover and retention. Most of these papers were quantitative, cross-sectional studies focussed on meso-level predictors of doctor turnover.
Research limitations/implications
Selection criteria concentrated on doctors who worked in hospitals, which limited knowledge of one area of the healthcare environment. The review could disregard relevant articles, such as those that discuss the turnover and retention of doctors in other specialities, including general practitioners. Additionally, being limited to peer-reviewed published journals eliminates grey literature such as dissertations, reports and case studies, which may bring impactful results.
Practical implications
Globally, hospital doctor turnover is a prevalent issue that is influenced by a variety of factors. However, a lack of focus on doctors who remain in their job hinders a comprehensive understanding of the issue. Conducting “stay interviews” with doctors could provide valuable insight into what motivates them to remain and what could be done to enhance their work conditions. In addition, hospital management and recruiters should consider aspects of job embeddedness that occur outside of the workplace, such as facilitating connections outside of work. By resolving these concerns, hospitals can retain physicians more effectively and enhance their overall retention efforts.
Social implications
Focussing on the reasons why employees remain with an organisation can have significant social repercussions. When organisations invest in gaining an understanding of what motivates their employees to stay in the job, they are better able to establish a positive work environment that likely to promote employee well-being and job satisfaction. This can result in enhanced job performance, increased productivity and higher employee retention rates, all of which are advantageous to the organisation and its employees.
Originality/value
The review concludes that there has been little consideration of the retention, as opposed to the turnover, of hospital doctors. We argue that more expansive methodological approaches would be useful, with more qualitative approaches likely to be particularly useful. We also call on future researchers to consider focussing further on why doctors remain in posts when so many are leaving.
