Abstract

Purpose

The purpose of this paper is to examine the barriers and facilitators to health and social service access and utilization for immigrant parents raising a child with a physical disability, in order to understand their specific needs and experiences of care.

Design/methodology/approach

The research involves qualitative interviews with five immigrant parents of four children with a physical disability in two cities in Ontario, Canada.

Findings

The findings indicate that divergent perceptions of health care and disability, language, knowledge of service systems, and finances may create obstacles to family-centered and culturally sensitive care. Formal supports, including a third person to navigate the system and services provided in families’ homes, are facilitators that enhance the service care experiences of parents.

Practical implications

Service providers need to accommodate and work with immigrant parents who have a child with a disability to overcome the barriers to their care. To enhance service experiences formal support and home services should be provided when possible.

Originality/value

There is little research on immigrant families’ experiences with pediatric rehabilitation services and this study contributes to this area. This paper shows that immigrant families of children with disabilities have barriers to service access, utilization, and service care experiences that are similar to those of non-immigrant families. There are few studies on facilitators to service access and utilization and positive experiences; therefore, this study makes a contribution to this area.

The purpose of this paper is to examine the service accessibility of Somali Australians suffering Autism Spectrum Disorder (ASD) by using both quantitative and qualitative methods.

Abstract

Purpose

The purpose of this paper is to examine the service accessibility of Somali Australians suffering Autism Spectrum Disorder (ASD) by using both quantitative and qualitative methods.

Design/methodology/approach

Using Geographic Information System (GIS) and the 2011 census data a total of 19,178 people reporting Somali ancestry were mapped to SA1 level with most being in the three capital cities of original migration; Sydney, Melbourne and Perth.

Findings

Access to primary services pertinent to ASD was measured using the GIS software, some 15 per cent of these cities Somali children were within 500 m of a General practice and 89 per cent within 2,000 m. A quarter of children were within 2,000 m of a speech pathology service access point and nearly a third (31 per cent) within 2,000 m of a psychologist. Qualitative analysis found a quite negative perspective on mental illness and ASD within the community with 85 per cent of respondents reporting a “Bad” perception of ASD within the community.

Research limitations/implications

Clearly, the opportunity these data provide is to develop service models targeting need and changing perspectives of ASD within an at risk community.

Originality/value

This is the first time in Australia that issues of service access (health) for Autism suffers and their families has been analysed in a detailed geographic manner.

Abstract

Purpose

The paper examined the mediating effect of social capital in the relationship between social intermediation and financial services in Ugandan micro finance industry. The purpose of this paper is to establish the role of social capital in the relationship between social intermediation and financial services access.

Design/methodology/approach

The paper adopted the MedGraph program, Sobel tests and Kenny and Baron approach to test for mediation effects.

Findings

It is clear that the true drivers of access to financial services in the micro finance industry are social intermediation and social capital. However, social capital exhibits partial form of mediation in the relationship between social intermediation and access to financial services.

Research limitations/implications

A single research methodological approach was employed in the study. Owing to limitations associated therein, future research through interviews could be undertaken to triangulate.

Practical implications

Since social capital is found to be a causal chain in the relation between social intermediation and financial serves access in this study, managers in the micro finance industry should endeavor to reinforce agents of social capital (i.e. trust and social networks) since the lending relationships between the micro‐finance operators and marginalized communities are driven by social collateral.

Originality/value

This is the first study that focuses on testing the mediating effect of social capital in the relationship between social intermediation and financial services access in the Ugandan microfinance industry.

Abstract

Purpose

This paper seeks to investigate how the concept of a trust level is used in the access control policy of a web services provider in conjunction with the attributes of users.

Design/methodology/approach

A literature review is presented to provide background to the progressive role that trust plays in access control architectures. The web services access control architecture is defined.

Findings

The architecture of an access control service of a web service provider consists of three components, namely an authorisation interface, an authorisation manager, and a trust manager. Access control and trust policies are selectively published according to the trust levels of web services requestors. A prototype highlights the incorporation of a trust level in the access control policy as a viable solution to the problem of web services access control, where decisions of an autonomous nature need to be made, based on information and evidence.

Research limitations/implications

The WSACT architecture addresses the selective publication of policies. The implementation of sophisticated policy‐processing points at each web service endpoint, to automatically negotiate about policies, is an important element needed to complement the architecture.

Practical implications

The WSACT access control architecture illustrates how access control decisions can be made autonomously by including a trust level of web services requestors in an access control policy.

Originality/value

The WSACT architecture incorporates the trust levels of web services requestors and the attributes of users into one model. This allows web services providers to grant advanced access to the users of trusted web services requestors, in contrast with the limited access that is given to users who make requests through web services requestors with whom a minimal level of trust has been established.

This study aims to highlight the processes that private organisations implement to improve access to health care services for low-income communities in Brazil.

Abstract

Purpose

This study aims to highlight the processes that private organisations implement to improve access to health care services for low-income communities in Brazil.

Design/methodology/approach

A qualitative research based on a comparative case study was conducted in São Paulo. A for-profit organisation and a not-for-profit one were compared to scrutinise how they adapt themselves to the social context they are embedded in; while improving their service accessibility.

Findings

Both kinds of organisations have succeeded to reach their goal of improving the access and reducing the time frame of health care services to low-income populations. Their initial business model (BM) makes them face their own challenges that they face with different strategies. It affects their way of communicating, their organisational culture, the patients’ expectations and their level of inclusiveness.

Research limitations/implications

The research is context-dependent because of the specific conditions of the health public system in Brazil. When shaping health care BMs, the national context must be taken into account and the service marketing components should be used to enhance patients’ value co-creation in the health care service delivery process.

Practical implications

The research gives insights to organisations that seek to adapt their BM to improve health-care access to low-income populations.

Social implications

Health-care access plays a key role in improving populations’ living conditions and reach one of the sustainable development goals of the United Nation.

Originality/value

Health care services access at the bottom of the pyramid remains under-studied. The paper brings value by comparing for-profit and non-profit organisations, which have the same social goal of improving health-care access to low-income populations while developing different practices to deal with their own challenges.

Abstract

Purpose

Accessing adequate healthcare to all people is one of the main goals of the health sector. The purpose of this paper is to investigate healthcare services access development of the provinces in Iran during 2007 and 2013.

Design/methodology/approach

This is a descriptive cross-sectional study. The study population included all the provinces in Iran. The data for 13 variables, including physical and human health resources, was collected from the Ministry of Health and Medical Education and the Statistics Center of Iran. Taxonomy technique was used to determine the degree of healthcare services access development in the provinces.

Findings

The findings show that Semnan was the province with the most developed healthcare services access with development score of 0.342 while Sistan Balocehstan province was the least developed with development score of one in 2007. In the year 2013, Chahar-Mahal Bakhtiari and Sistan Baluchestan were the least and most developed provinces with scores of 0.551 and 0.989, respectively. The mean and standard deviation of the development scores in access to healthcare services in 2007 and 2013 were 0.7463±0.1268 and 0.7766±0.1058, respectively.

Originality/value

Most previous studies that examined disparities in access to healthcare resources in Iran only considered one resource. This study applied a taxonomy technique to investigate the disparity and changes in access using 13 main healthcare resources. This approach helped the authors to investigate whether the decisions of the policy makers were intended to eliminate the disparities.

Abstract

Purpose

Undergraduates are highly susceptible to the development of mental health difficulties. Afro-Caribbean students are particularly vulnerable to the pressures of university yet are less likely than other ethnic groups to receive early intervention. This paper aims to understand the barriers and facilitators that Afro-Caribbean undergraduates perceive towards accessing mental health services in the UK.

Design/methodology/approach

Critical Incident Technique was used as the qualitative method because it explores the critical factors that contribute to or detract from a specific experience. Seventeen Afro-Caribbean undergraduates participated in five focus groups. This involved engaging in a novel psychosocial activity that incorporated vignettes to encourage the identification of barriers and facilitators to service access. The data were analysed thematically to generate categories of critical incidents and wish-list items.

Findings

Analysis revealed rich data from a sub-group rarely researched within UK literature. Fifteen barriers, eleven facilitators and five wish-list items were identified. The importance of mental health literacy, social networks, cultural sensitivity and concerns surrounding services underpinned many categories.

Originality/value

Findings provide a new perspective on barriers reported in previous literature. Novel facilitators were highlighted where, although psychological and sociocultural factors were deemed valuable, structural changes were most desired. Recommended changes illustrate innovative interventions that could make services accessible for young adult Afro-Caribbean populations. Future research should explore the barriers and facilitators identified by Afro-Caribbean undergraduates across various universities who have successfully accessed and engaged with services. This could provide a holistic perspective on viable facilitators enabling access despite the presence of barriers.

Abstract

Purpose

The purpose of this paper is to examine the extent to which social intermediation influences access to financial services in Uganda's microfinance industry.

Design/methodology/approach

The paper adopts analysis of moment structures (AMOS), a form of structural equation modeling (SEM) to test hypotheses.

Findings

It was established that social intermediation together with antecedents of social capital and managerial competence, account for 32 percent of the variance in access to financial services in the microfinance industry.

Research limitations/implications

Only a single research methodological approach was employed and future research through interviews could be undertaken to triangulate. Furthermore, the findings from the present study are cross-sectional, future research should be undertaken to examine the social intermediation and its effects on access to financial services across time.

Practical implications

In order to boost the wealth of the active poor and microfinance institutions in Uganda, Uganda should always endeavor to build the human and institutional capacities through social intermediation so as to encourage the marginalized people to fully participate in formal financial intermediation in the microfinance industry.

Originality/value

This is the first study that focuses on testing the influence of social intermediation on access to financial services in Uganda's microfinance industry.

Purpose

The purpose of this paper is to highlight some initial efforts within Her Majesty’s Prison and Probation Service (HMPPS) to develop digitally enabled services supporting the rehabilitation of service users. It is not designed to set out either HMPPS policy on digital rehabilitative services or the position of HMPPS Interventions Services on this subject. Rather, it is a short exposition of the authors’ views on the potential of digitally enabled strategies to enhance interventions in forensic settings. In this context, the authors will also describe the development of the first digitally enabled rehabilitation service accessed via HMPPS in-room computer terminals.

Design/methodology/approach

The authors have reviewed current literature and outlined how the authors have and are aiming to add to this area of work.

Findings

This general review outlines the authors’ views on the potential of digitally enabled strategies for improving interventions in forensic settings.

Originality/value

This paper is a short exposition of the authors’ views on the potential of digitally enabled strategies to enhance interventions in forensic settings. In this context, the authors will also describe the development of the first digitally enabled rehabilitation service accessed via HMPPS in-room computer terminals.

Before anything can be achieved by such connections a great deal of effort must go into homogenising service access procedures. This is going to be a crucial area of development. On an experimental basis attempts have been made to tackle this problem with encouraging results. A rethink of the distribution of functions in the network environment is necessary to optimise this homogenising of service access procedures.
