Abstract

As the world continues to grapple with new and existing challenges every passing day, the need to identify new approaches to maximise impact with given limited resources has become a pressing issue. To this end, overhauling the purchase and delivery of public services by introducing new funding models like commissioning has gained significant attention within the public and policy sector in recent years. Commissioning does not mean merely procurement and purchasing but securing the most appropriate services to address the needs of the population through due diligence and planning to maximise value. Although Commissioning has been practised internationally for many years, it has emerged at scale in New Zealand through the establishment of the Whānau Ora Commissioning Agency. This chapter discusses the Whānau Ora Commissioning model and its development as an indigenous model to commissioning for impact. It highlights the unique characteristics embedded in and exhibited by the model, which enable it to function in an indigenous context and facilitate positive well-being for its population. It also describes the challenges faced to perform and drive momentum forward. Commissioning may not be the only solution to resolving societal challenges. However, it can be instrumental in generating and amplifying value tailored to the context and used in conjunction with other innovative practices.

In the United Kingdom under the New Labour government from 1997 to 2010, there was a shift of power away from local towards central government, leading to a situation in which the role of public service organisations could appear to be mainly implementing priorities set by central government and complying with national standards, enforced through the setting of targets and the use of performance management frameworks. The key performance management systems, and the standards within them, became increasingly focused on outcomes, defined as the results that services produce that have an impact on the lives of service users and citizens – the benefits of the service rather than the volume or quality of the outputs (Willis & Bovaird, 2011).

Abstract

What is our unit of analysis and by implication what are the boundaries of our cases? This is a question we grapple with at the start of every new project. We observe that case studies are often referred to in an unreflective manner and are often conflated with geographical location. Neat units of analysis and clearly bounded cases usually do not reflect the messiness encountered during qualitative fieldwork. Others have puzzled over these questions. We briefly discuss work to problematise the use of households as units of analysis in the context of apartheid South Africa and then consider work of other anthropologists engaged in multi-site ethnography. We have found the notion of ‘following’ chains, paths and threads across sites to be particularly insightful.

We present two examples from our work studying commissioning in the English National Health Service (NHS) to illustrate our struggles with case studies. The first is a study of Practice-based Commissioning groups and the second is a study of the early workings of Clinical Commissioning Groups. In both instances we show how ideas of what constituted our unit of analysis and the boundaries of our cases became less clear as our research progressed. We also discuss pressures we experienced to add more case studies to our projects. These examples illustrate the primacy for us of understanding interactions between place, local history and rapidly developing policy initiatives. Understanding cases in this way can be challenging in a context where research funders hold different views of what constitutes a case.

Abstract

This chapter aims to give an overview of key mental health policy and service provision, highlighting the need for specific attention to Black and minority ethnic children and young people. The focus is on mental health provision in the UK provided through the statutory sector and the voluntary and community sector, the issues raised are likely to have resonance across wider geographic locations. The themes examined include: the relevance of terminology regarding race and ethnicity and related impact on the planning and provision of services; the extent to which policy and commissioning of services give due focus and attention to the mental health of Black and minority ethnic children and young people; views young people themselves have contributed on the issue; and a case study illustrating work being undertaken to redress some of the imbalances encountered by young people in accessing appropriate support. The chapter argues that the supply chain to young people receiving support that is relevant and appropriate to their needs is a long and complex one. It is fundamental to take a holistic approach and consider how the components of this chain impact specifically on the mental health of children and young people from Black and minority ethnic communities.

Child and adolescent mental health services (CAMHS) – This term refers to all services that work with children and young people to address their behavioural and emotional wellbeing needs. The services may be provided by the National Health Service (NHS), local authority, school, private sector or charitable organisation and span early intervention support through to specialist treatment.

Care Quality Commission – The independent regulator of health and social care services in England.

Commissioning – The process by which health services are procured and should be based on an up-to-date assessment and understanding of needs of the target population.

Co-production – A process for planning and delivery of health and social care services that involves partnership working and power sharing between those responsible for the planning and provision of services, service users, their family members, carers and other citizens.

National Service Framework – Ten year programmes that, until the health and social care reforms started in 2010, defined standards of care in the NHS including measurable goals within set timeframes.

Population Needs Assessments – The collection and study of relevant data to understand and estimate current and future needs of a population in order to inform the planning of services that meet identified needs.

Voluntary and Community Sector – Also referred to as the Third Sector and encompassing a diverse range of organisations, services and groups that are seen as distinct from the public (also referred to as statutory) and private sectors.

Youth Information, Advice and Counselling Services (YIACS) – Most YIACS have charitable status and provide services to young people on a range of issues, a key feature associated with YIACS is the provision of holistic, young person centred support provided under one roof.

Since the introduction of the Public Services (Social Value) Act 2012, there has been a common misconception that Social Value is inherently about Procurement. We think that the process of Procurement is a means through which questions can be asked around Social Value commitments; however, Social Value should come into dialogue way before a tender exercise. It must be at the forefront of political visioning, strategy development, officer behaviour and the design of goods and services. In this chapter, we explore how Public Authorities and other Anchor Institutions can embed Social Value into everything they do, utilising Commissioning and Procurement as the basis.

The social value movement emerged in the 1990s from a desire to demonstrate the value of investment in people and society. To demonstrate, evidence or proof is required, thus the focus on measurement is central to the social value movement.

In addition to measurement, understanding and valuing impact upon all stakeholders is a core theme of the social value movement. With measurement, it is more possible to see when an action has led to increased inequality or has negatively impacted the environment. With measurement, one can identify opportunities to improve societal well-being.

In procurement, thinking about impact from the view of all stakeholders will illuminate pathways to achieving maximum positive impact. The journey towards the goal of achieving maximum impact begins at the design stage of the procurement process and will be informed by stakeholder experience overtime.

We should be demanding evidence of maximum positive impact – from our governments and government-funded institutions, and from all corporations and service providers. Building maximum impact capacity among purchasers of services is equal in importance to building measurement capacity at the service delivery level.

The act of procurement offers a tremendous opportunity for positive environmental and social impact. Measurement will inform and guide us on how to maximise that opportunity. This is essential if we are to successfully repair and sustain the planet. Real sustainability requires us to address inequality and to actively improve well-being in order to meet the goal of addressing climate change.
