Abstract

Purpose

This study aims to explore the area of learning materials used by students in their psychiatry placement in more detail, and to identify any gaps not adequately filled currently. It also seeks to explore student interest in psychiatry and whether they are considering pursuing it as a future career.

Design/methodology/approach

This was a cross sectional study, using questionnaires completed by medical students about their psychiatric undergraduate placement at Manchester University.

Findings

Of 144 students, 126 responded (response rate 87.5 per cent). Students spent a mean of 10.1 hours a week studying psychiatry (outside formal teaching). Handouts from lectures or tutors were most commonly used (by 95.2 per cent of respondents) with journals and podcasts being the least common (18.5 per cent and 17.5 per cent, respectively). Psychiatric textbooks were the most useful material for learning about psychiatry. Students identified the need for better quality, more structured and more frequent teaching (n=58). They also identified a need for greater clinical experience and increased availability of resources such as textbooks.

Research limitations/implications

Knowing what materials and resources students prefer to learn from will be crucial to maximize the delivery of an enjoyable and educational psychiatry module. This in turn may result in more students choosing psychiatry as a career. Larger studies are required to support the findings in this study.

Originality/value

To the authors' knowledge, there is very little published research into what materials medical students use to learn for their psychiatry placement.

Abstract

NCC? You Can Say That Again The 1987 Salary Survey from the National Computing Centre (NCC) pinpointed a requirement for over 5,000 new networking staff over the next five years, and confirmed that communications was emerging as one of the fastest‐growing job categories at around 13 per cent per annum. To meet this growing requirement the Institution of Electrical Engineers (IEE), the Telecommunications Users' Association (TUA) and NCC are to investigate the demand for a National Certificate in Communications. They believe that the introduction of such a Certificate would increase the supply of qualified communications specialists able to apply technical and management skills in a business environment. This joint initiative comes in response to concern over the current shortage of high‐quality staff competent to plan and operate communications facilities in user organisations. Communications in this context embraces telephone systems, data networks and the whole range of text and image equipment on which organisations rely for their day‐to‐day business. The three organisations called all interested parties in government, industry and education to a meeting in London on 16 June to discuss the issue, with a view to introducing a course in October 1989 at approved colleges and educational establishments throughout the UK.

Abstract

Purpose

There are a lot of discussions about privacy in relation to contemporary communication systems (such as Facebook and other “social media” platforms), but discussions about privacy on the internet in most cases misses a profound understanding of the notion of privacy and where this notion is coming from. The purpose of this paper is to challenge the liberal notion of privacy and explore foundations of an alternative privacy conception.

Design/methodology/approach

A typology of privacy definitions is elaborated based on Giddens' theory of structuration. The concept of privacy fetishism that is based on critical political economy is introduced. Limits of the liberal concept of privacy are discussed. This discussion is connected to the theories of Marx, Arendt and Habermas. Some foundations of an alternative privacy concept are outlined.

Findings

The notion of privacy fetishism is introduced for criticizing naturalistic accounts of privacy. Marx and Engels have advanced four elements of the critique of the liberal privacy concept that were partly taken up by Arendt and Habermas: privacy as atomism that advances; possessive individualism that harms the public good; legitimizes and reproduces the capitalist class structure; and capitalist patriarchy.

Research limitations/implications

Given the criticisms advanced in this paper, the need for an alternative, socialist privacy concept is ascertained and it is argued that privacy rights should be differentiated according to the position individuals occupy in the power structure, so that surveillance makes transparent wealth and income gaps and company's profits and privacy protects workers and consumers from capitalist domination.

Originality/value

The paper contributes to the establishment of a concept of privacy that is grounded in critical political economy. Owing to the liberal bias of the privacy concept, the theorization of privacy has thus far been largely ignored in critical political economy. The paper contributes to illuminating this blind spot.

Abstract

Purpose

The conceptualization of service sabotage failed to adequately tap the domain of interest. Phenomena like turnover and service sabotage are difficult to measure and are not suitable for individual-level study. However, “intention” is suitable for individual-level or management-oriented studies. The paper aims to discuss these issues.

Design/methodology/approach

A new scale (eight items) to measure the intention to sabotage was developed and tested using a sample of bank (n=313) and insurance (n=258) employees in Nigeria. Cynicism and the desire for justice are the roots of sabotage. As such, the inability to stabilize institutionalized work processes and procedures may cause employees to be overcome with the intention to sabotage service, prior to the actual sabotage. Drawing on conservation of resources theory, this paper investigates the impact of employee cynicism on intention to sabotage as moderated by procedural justice.

Findings

The analyses suggest that employee cynicism is related to the intention to sabotage, and procedural justice moderates the relationship between employee cynicism and intention to sabotage. The findings endorse the model of interest, and implications of this study for research and practice are discussed.

Originality/value

The study differentiated service sabotage from intention to sabotage, and developed and tested a scale to measure the intention to sabotage.

Abstract

Purpose

The purpose of this paper is to outline a conceptual plan for innovative, integrated care designed for people living with long-term conditions (LTCs).

Design/methodology/approach

The conceptual plan delivers a partnership between the health system, the person with LTCs (chronic), their family, and the community. The partnership aims to support people at home with access to effective treatment, consistent with the New Zealand Government Health Strategy. This concept of people-owned care is provided by nurses with advanced practice skills, who coordinate care across services, locations and multiple LTCs.

Findings

With the global increase in numbers of people with multiple chronic conditions, health services are challenged to deliver good outcomes and experience. This model aims to demonstrate the effective use of healthcare resources by supporting people living with a chronic condition, to increase their self-efficacy and resilience in accordance with personal, cultural and social circumstance. The aim is to have a model of care that is replicable and transferable across a range of health services.

Social implications

People living with chronic conditions can be empowered to manage their health and well-being, whilst having access to nurse-led care appropriate to individual needs.

Originality/value

Although there are examples of case management and nurse-led coordination, this model is novel in that it combines a liaison nursing role that works in partnership with patients, whilst ensuring that care across a number of primary and secondary care services is truly integrated and not simply interfaced.

Abstract

Purpose

Cannabis use among young adults is increasing, despite being associated with several negative consequences. Protective behavioral strategies (PBSs) are a potential mechanism of behavior change for reducing substance use, yet PBS use for cannabis is not well understood. The purpose of this paper is to further define and measure the PBS construct for cannabis.

Design/methodology/approach

A community sample of cannabis users (n=54) participated in eight focus groups discussing the use of PBSs. Participants completed surveys regarding demographics, cannabis use habits and cannabis problems. The authors also administered an existing measure of cannabis PBS and asked them to generate new or unique protective strategies that they had used or had heard of others using.

Findings

Thematic analysis of qualitative focus group data provided information about cannabis users’ reasons for regulating cannabis use (e.g. health or legal problems, interpersonal) as well as strategies to moderate cannabis use or attenuate their risk for experiencing adverse consequences (e.g. distraction, existential/spiritual strategies). Analyses of quantitative survey data revealed that use of PBSs was negatively correlated with cannabis outcomes. Perceived helpfulness of strategies was an important predictor of decreased cannabis use and adverse consequences.

Research limitations/implications

Findings expand the understanding of the definition and measurement of strategies for regulating cannabis use and reducing related risk of experiencing adverse consequences.

Originality/value

This is the first study to examine cannabis-related PBS using both qualitative and quantitative methods, which provide insights into the definition of PBS and for future refinements of PBS measurement.

Abstract

Purpose

The COVID-19 pandemic is unprecedented for young people within the UK. The pandemic has presented particular challenges for vulnerable children and young people. For example, a recent study in the UK indicated that 83% of young people with existing mental health conditions said the pandemic had made their condition worse (Young Minds, 2020). To date, the impact upon populations such as young people in Secure Children’s Homes (SCHs) is unknown. This study aims to elucidate this area.

Design/methodology/approach

SCHs provide a safe, supportive environment for vulnerable young people who frequently present with multiple and complex needs. Young people residing within a SCH may be residing at the setting because of a Secure Accommodation Order under a Section 25 Order of the Children’s Act (1989) or for criminal justice reasons, i.e. serving a Remand period or custodial sentence. Preliminary research compared a baseline period to a follow-up period after the commencement of COVID-19 national lockdown measures within a SCH in the North of England to develop understanding of the impact for young people.

Findings

A significant decrease in overall incidents (t (5) = −6.88, p < 0.001), restraints (t (5) = −9.07, p < 0.001) and other incidents including assaults occurred during follow-up. The SECURE STAIRS framework supports trauma-informed care and enhances support within the setting. Consistent with the framework, provision of formulation meetings was significantly increased within the follow-up period (Welsh’s t (74) = −2.74, p < 0.001). Reflections and future recommendations are outlined.

Originality/value

The unanticipated results highlight the value of examining incident data within secure environments and could lead to effective practice changes for practitioners working within this domain. This research also demonstrates how frameworks such as SECURE STAIRS can be beneficial for vulnerable young people during periods of change and stress in mitigating some of the potential negative effects. The implementation of such frameworks within SCHs is still novel and thus evaluative research is valuable.

Abstract

Purpose

The purpose of this paper is to draw attention to the need to treat negative workplace behaviours which are not perceived as bullying as seriously as those which are. The paper also aims to examine whether or not the National Health Service (NHS) appears to experience a higher level of negative behaviour than private sector organisations and whether lower frequency behaviour has similar levels of effect as higher frequency behaviour.

Design/methodology/approach

A mixed‐method approach is used whereby a mainly quantitative questionnaire is complemented by the inclusion of qualitative questions and the collection of qualitative data collected within the two NHS Trusts concerned.

Findings

The evidence collected draws attention to the considerable impact that workplace incivility, which may or may not be classed as bullying, has on the well‐being of employees and the effectiveness of organisations. Where aggression is present, the levels of effect are shown to be higher and the behaviour is always classed as bullying. The evidence also shows that the NHS does appear to experience a higher level of negative behaviour than private sector organisations, and that lower frequency behaviour does indeed appear to have similar levels of effect as higher frequency behaviour.

Originality/value

This article shows that the focus placed by many researchers and organisations on countering/eliminating behaviour purely perceived as bullying is unlikely to be effective unless they also adopt a similar approach to the full range of negative behaviours that employees experience/witness in organisations.

Abstract

Purpose

The purpose of this paper is to systematically identify, appraise and synthesise qualitative literature exploring the experience, both positive and negative, of clinical supervision from the supervisor’s perspective.

Design/methodology/approach

A systematic search of three databases, grey literature, reference lists and citations was conducted. Six articles met the inclusion criteria and their quality was critically appraised by using a modified version of the Critical Appraisal Skills Programme tool. Data extracted from the articles were synthesised using meta-ethnography.

Findings

Four key themes were identified: experiencing difficulties in clinical supervision, responsibility, similarities to therapy and capabilities as a supervisor. These demonstrated that the role of a supervisor has the potential to be both beneficial and harmful to personal and professional development.

Research limitations/implications

The quality of the studies was variable. Further research is required to explore how supervisors manage difficult experiences to ensure personal development and growth.

Practical implications

Clinical implications include the need for employers to consider the additional pressure associated with providing clinical supervision and to ensure that appropriate support is available. Results complement previous research on the bi-directionality of parallel process in clinical supervision.

Originality/value

This review presents an original synthesis of the supervisor’s experience of delivering clinical supervision to qualified therapists. This is achieved by utilising a systematic methodology and appraising the quality of the studies included. The review highlights how the effects of clinical supervision are not limited to the supervisee, but also experienced by the supervisor. The competing demands and responsibilities associated with clinical supervision impact upon the supervisor’s experience, both positively and negatively. When beneficial, delivering clinical supervision can lead to personal and professional growth in addition to the acquisition of new skills.

Abstract

Purpose

Public attitudes are considered influential in the successful reintegration of offenders into society after release, however research into attitudes towards offenders with intellectual disability (ID) has received little attention. The purpose of this study is firstly to see if people hold differing attitudes towards the reintegration of offenders with ID compared to those without ID and secondly, to investigate whether this difference in attitude is because of differing implicit theories of intelligence (TOI). The effects of familiarity with ID were also measured.

Design/methodology/approach

In total, 200 participants read crime vignettes depicting crimes committed by offenders with and without ID and completed Dweck’s “TOI” scale.

Findings

Participants were found to have greater entity views of intelligence towards ID yet displayed more positive attitudes towards their reintegration than offenders without ID. The influence of demographics was mixed. It would appear attitudes towards offenders with ID are not as negative as initially thought.

Research limitations/implications

Implications of these findings are discussed in terms of desistance and community integration.

Practical implications

While some care must be taken when interpreting the results, this study demonstrates positive results regarding the reintegration of offenders with ID. Attitudes may be changing for the better towards those with disabilities, which is positive in terms of the government and National Health Service (NHS) objectives to reintegrate people with ID successfully back into the community. Although limited in number, it is noted that community forensic mental health teams have been effective in managing offender risk and providing good quality care (Dinani,et al.,2010; Benton and Roy, 2008). They can provide more person-centred and specialist treatments options and have links with other community services, probation and the police (Royal College of Psychiatrists, 2014).

Social implications

Community care is thought to lead to more timely treatments with more accessible support teams and services that those with ID would struggle to access in prison (Bradley, 2009). It can also lead to greater well-being and support as individuals are in a less restrictive environment and are closer to their social networks, acting as a protective factor against further reoffending (Benton & Roy, 2008; Bradley, 2009). Furthermore, it has been indicated significant financial savings would be achieved for the criminal justice system by reducing inpatient care and increasing community services and sentences, in addition to reducing the demand on prison spaces (Bradley, 2009; Benton & Roy, 2008).

Originality/value

To date, very few studies have used TOI to examine attitudes towards offenders, with none examining attitudes towards offenders with ID.
