Abstract

Purpose

This paper aims to offer a commentary on Psychologically Informed Services: A Good Practice Guide, a recently published operational guidance document on developing psychologically informed environments (PiEs) in services for homeless people.

Design/methodology/approach

The paper is an invited opinion piece and comment based on the author's specialist knowledge gained from undertaking research addressing the experiences and needs of young runaways.

Findings

The new operational guidance is welcomed but should be extended so that psychologically informed environments are provided in services for young runaways under the age of 16, who often encounter abuse, mental health issues and problematic substance misuse and experience homelessness post 16 and into adulthood.

Originality/value

Commissioners and practitioners should encourage the development of psychologically informed environments for young runaways to meet their needs, aid recovery and reduce the likelihood of experiencing homelessness as they move into adulthood.

Abstract

Purpose

Appreciative Inquiry (AI) has its roots in the world of Organisational Development (OD), however, as a strength-based approach which is intrinsically creative and generative, it has been found to work well in many other fields. The purpose of this paper, Part 1 of 2, is to provide an introduction to AI and suggests its potential in homeless work. Part 2 reports a pilot study of implementation and evaluation.

Design/methodology/approach

This is a conceptual paper suggesting a new approach to the development of a Psychologically Informed Environment (PIE). The background of the approach (AI) is given, the pilot project described, and the way it contributes to a PIE is discussed. The beneficial and practical link with Positive Psychology is also explored. Part 2 will describe the outcomes of the implementation of AI in pilot at a homelessness hostel in Westminster.

Findings

AI has its roots in the world of OD, however, it has been found to work well in many other fields. The paper highlights some of the aspects of AI and Positive Psychology which could be meaningful to hostel residents, and give it robustness and psychological sophistication when used by staff and residents. The paper also considers the benefits of using AI with staff as a tool for organisational learning, there by making it an ideal approach for hostels which want to become a PIE.

Originality/value

AI is well-established as an OD process and less well known as a personal development approach and has not previously been articulated as a tool for working with hostel residents or for developing PIEs. The openness of PIEs to alternative psychological approaches is indicated. AI is a strength-based approach, and a well-structured alternative to some of the problem-based psychological approaches that have been used. In addition, AI supports defining features of a PIE such as reflective awareness.

Abstract

Purpose

This study aims to share reflections on psychologically informed practice and research that has taken place within Opportunity Nottingham and in collaboration with local hostels and agencies. This study conveys a call to action to all stakeholders not to be complacent, but to use learning from Fulfilling Lives as a foundation for future developments.

Design/methodology/approach

This study will use the psychologically informed environments (PIEs) 2.0 elements to structure points of learning from working within Opportunity Nottingham, drawing on wider literature as appropriate. This will allow focus on core areas of interest for all stakeholders and illustrate how the PIE elements can be embedded in thinking.

Findings

Research-informed reflections from a Fulfilling Lives Clinical Psychologist will be offered across the domains of: psychological awareness; staff training and support; learning and enquiry; spaces of opportunity; and rules, roles and responsiveness.

Originality/value

To the best of the author’s knowledge, this will be the first paper to use PIE domains to structure a reflective account of learning from a Fulfilling Lives project, contextualised within broader literature.

Abstract

Purpose

This paper seeks to offer a commentary on Psychologically Informed Services: A Good Practice Guide, a recently published operational guidance document on developing psychologically informed environments (PIEs) in services for homeless people.

Design/methodology/approach

The paper is an invited opinion piece and comment, based on the specialist experience and viewpoint of the authors in education on mental health issues in primary care.

Findings

The new operational guidance is welcomed, with some provisos. Psychologically informed environments are needed not just in homelessness resettlement, but in all areas where services are commissioned; and all commissioned services should take particular care to collect suitable data on the outcomes they aim for and intend to achieve.

Originality/value

There is a great opportunity with GP commissioning to commission services that are psychologically informed and holistic, although it may be hard to ensure services remain user‐focused and not disjointed if split up for tendering. Public health leadership on the new Health and Wellbeing Boards will be crucial to integrate services so the new system will not make things worse. Given the need, hopefully a similar drive for PIEs everywhere – the criminal justice system, care homes and community wellbeing – will follow.

Abstract

Purpose

This final article in the series of three identifies and illustrates the links between the “enabling environment” approach and contemporary social policy themes such as relational health, public health, social inclusion; and the “Big Society”.

Design/methodology/approach

The paper begins with an example of the standards‐based approach in practice. It then indicates other areas where this approach is currently being explored. Finally it summarises recent contemporary policy frameworks for which the EE approach, it is suggested, supplies both a shared vocabulary and a practical, evidence‐able agenda.

Findings

There is growing recognition of the need for a “sea change” away from a simple clinical interventions framework for community mental health and addressing health inequalities. There are clear linkages in this new approach with past and emerging policy frameworks in public health and corporate social responsibility, including the need for more “bottom up” solutions with local ownership.

Originality/value

This series of three papers is the first introduction to psychologically informed environments and the enabling environment approach to be made available to the general and specialist public.

Abstract

Although the idea of a therapeutic community (TC) has lost none of its dynamism, there are many modern‐day environments in which the original TC model has been unable to make headway. In recent years, new ideas have been emerging for the development of institutions and services that can be adapted to a wide range of psychological needs and settings, such as homelessness hostels and refuges. The psychologically‐informed environment (PIE) arises from the scope for reflective practice, leading to changes in day‐to‐day working ‐ including a more planned variant for high secure services. The PIE approach seems to offer greater flexibility in scope than the TC model. Nevertheless, such new approaches may yet need a clear values base; and the next article in this series will explore new ideas for the creation of ‘enabling environments’ in a still wider range of settings.

Design/methodology/approach

The paper is an invited opinion piece and comment based on the specialist experience and viewpoint of the author in working with advocacy services in the UK and in the USA.

Findings

The new operational guidance is welcomed, with some provisos. Specifically, any emphasis on training for staff should certainly include an equal weighting on training for service users themselves in order to support positive self‐direction and development.

Originality/value

Services that wish to develop as PIEs will need to ensure that service users are fully engaged and enabled through the process to identify their own needs.

Purpose

The purpose of this paper is to outline the challenges to achieving positive outcomes for young people within the secure estate in England, and introduces a psychologically informed framework, SECURE STAIRS (SS), aimed at improving outcomes.

Design/methodology/approach

The paper argues that there is a need for a fundamental shift in the way care and intervention for young people within the secure estate is delivered. It gives an overview of current challenges and needs and summarises the theoretical concepts and evidence base which can guide practice and form the foundations of the SS framework.

Findings

The framework recommends that intervention shift from focussing primarily on individual assessment and treatment to a greater emphasis on supporting the work of the wider system of care. Recommendations include promoting trauma-informed care, a focus on the system dynamics within institutions and how these impact on the care young people receive, and on the collaborative development with residential staff and young people of formulation-led care plans that include a focus on issues of sustainability after leaving the secure estate.

Practical implications

These include the establishment of discrete residential groupings with truly integrated and trauma-informed work across residential, mental health, education and criminal justice agencies. This involves addressing governance issues around shared record keeping, and challenges to sustainability and the accompanying need for local implementation plans for each establishment alongside central support at a strategic level.

Originality/value

This paper describes a new and innovative way of working within secure settings to ensure children and young people’s needs are better met.

Abstract

Purpose

This study aims to explore the perceptions of staff in four teams regarding the implementation of psychologically informed environments (PIE) across a community service and three hostels supporting individuals facing severe and multiple disadvantage.

Design/methodology/approach

Using a pre-post design, the PIEs Assessment and Self-Development for Services (known as the Pizazz) was completed by staff before the implementation of PIEs and at a six-month follow-up. A narrative review of the results and thematic analysis of the qualitative data are presented.

Findings

The majority of the Pizazz elements were rated as improved following PIE implementation. Thematic analysis developed three themes influencing staff members’ ability to develop a PIE: Complexities of Our and Wider Systems; Ready-made or Baked from Scratch; and Reflective and Responsive Staff.

Research limitations/implications

A planned one-year follow-up was obstructed by the coronavirus pandemic, limiting understanding of longer-term impact. Having only gathered data from staff members, the results cannot corroborate staff members’ perceptions. Further research could explore other stakeholder perspectives, as well as the impact of PIE implementation on staff perception of resources, and of a possible ceiling effect for hostels trying to develop PIE.

Originality/value

To the authors’ best of knowledge, this is the first UK study to use the Pizazz to evaluate the implementation of PIE.

Abstract

Purpose

The purpose of this paper is to inspire services to create Psychologically Informed Environments (PIEs) to support people experiencing homelessness, complex trauma and multiple exclusion.

Design/methodology/approach

It outlines key elements of PIEs and how these have been implemented at the Waterloo Project; a 19 bed hostel and integrated health and social care partnership in Lambeth. It considers the importance of meeting individuals emotional and psychological needs to support them out of homelessness.

Findings

The service review found a reliable reduction in residents’ mental distress and improved health and wellbeing. Repeat homelessness, anti-social and self-harming behaviour decreased whilst engagement with services, self-care and esteem increased. There are early indications of cost benefits to health and social care services. Staff reported professional development and an increase in their sense of personal accomplishment.

Originality/value

The paper aims to share learning, good practice and the outcomes of a psychologically informed approach to working with individuals who have not had their needs met by existing services. It illustrates the actual and potential social and economic impact and value on both individuals and public services.
