Abstract

Purpose

Psychodynamic psychotherapy is increasingly adapted and used with individuals with intellectual disability (ID) and mental health difficulties. However, the evidence base is still small and largely based on case studies and small trials whose participants mainly have mild to moderate ID. This paper aims to review and critique the literature in regards to the adaptations; and the effectiveness of psychodynamic psychotherapy for those with severe and profound ID.

Design/methodology/approach

A systematic literature search of PsycINFO, Social Policy and Practice, Medline, Cumulative Index to nursing and allied health literature and applied social sciences index and abstracts was conducted. Six studies met inclusion criteria and underwent a quality evaluation and critical review.

Findings

Six papers (all case studies) met inclusion criteria and underwent a quality evaluation and critical review. Some adaptations to therapy were reported, such as a more flexible therapeutic frame and increased use of the physical environment as a therapeutic tool. Due to significant methodological weaknesses of the included studies, it is yet unclear whether psychodynamic psychotherapy is an effective intervention for individuals with severe and profound ID.

Research limitations/implications

Only a small number of case studies met the inclusion criteria. Further research should use more robust outcome measures, larger samples and compare psychodynamic psychotherapy to alternative interventions.

Originality/value

This paper is the first to review the psychodynamic psychotherapy literature with regard to its effectiveness as a treatment specifically for individuals with severe and profound ID and mental health difficulties.

Abstract

Purpose

Many referrals to our mental health of learning disability service focus on concerns about behaviours that present risks to the referred person and to those around them, including support workers. If the referred person has good verbal ability, psychological therapy may be requested and offered, but the person may find it too difficult to engage for a number of reasons. Even when they do engage in therapy, the authors recognise the importance of helping staff better understand their attachment needs. This paper aims to demonstrate an innovative approach to helping staff provide Trauma-Informed Care (TIC).

Design/methodology/approach

The authors developed a training programme for support workers using psychodynamic concepts, such as splitting, transference and counter-transference, to help them develop insight into the processes that get enacted during their work. In this paper the authors have generally used the term “care staff”, recognising that an important aspect of the role of those staff is to provide care within an attachment framework; as well as support to maximise independence.

Findings

The feedback from staff who have attended has been very positive, both at the time and later. Staff who have attended have talked to multidisciplinary colleagues about the impact their learning has had on their ability to work with service users who present great challenges, in the context of their trauma histories.

Originality/value

The importance of providing TIC is gaining traction across varied settings. The authors are in the process of developing both qualitative and quantitative research programmes to evaluate this approach to increasing TIC for adults with learning disabilities, reducing staff burn out and placement breakdown.

Abstract

Purpose

The aim is to explore important perspectives and structures that can inform leadership in a therapeutic environment based on psychodynamic principles.

Design/methodology/approach

The experiences of the author as Director of the Mulberry Bush (1991‐2001) are used as a focus for inquiry into the challenges and issues that emerge in relation to leadership.

Findings

It was found that certain key features of leadership when exercised – attending to beginning and endings, emotional containment and holding in mind, “holding the line” and tolerating uncertainty – more reliably ensure the continuity of a healthy therapeutic environment.

Practical implications

Leadership informed by a psychodynamic perspective and reflections can assist the leader, the team, the residents and the entire organisation to focus on not only creating but also sustaining a way of living and learning together that will produce learning, changing, growing and “going”.

Originality/value

The exploration of the subject of leadership in this context contributes to broadening and deepening understanding in the sector of the significance of leadership and how best to engage with its challenges in a way that more reliably ensures the continuity of going on being and growing that is needed at the heart of a therapeutic environment.

Abstract

Purpose

The purpose of this paper is to detail the impact and efficacy of Her Majesty’s Prison and Probation Service ((HMPPS) Therapeutic Communities (TCs) (both democratic and hierarchical). This paper outlines recent developments in the TC literature, to provide readers with an up-to-date overview of the outcomes of prison-based TC treatment, while highlighting the strengths and challenges of this treatment approach. Trends within the research are discussed, and the authors draw attention to any gaps in the current knowledge.

Design/methodology/approach

This paper uses a narrative literature review approach to review the most current literature around the effectiveness of prison-based TCs in HMPPS. Academic literature published predominantly from 2010 onwards is discussed because of limited literature review publications on this topic post-2010. To obtain literature, searches of relevant databases were conducted, and/or clinical leads at prison sites were contacted for relevant publications.

Findings

There is a body of research which demonstrates that TCs are an effective form of treatment for people with an offending history and personality difficulties. Evidence indicates that Democratic TC treatment plays a part in reducing reoffending rates, as well as improving psychological features. Further research is needed in a number of areas, specifically with female offenders and individuals who undertake treatment in hierarchical TCs in the UK. It is also suggested that TC treatment aftercare may help to further the positive outcomes identified.

Originality/value

To the best of the authors’ knowledge, there is no up-to-date review of the impact and efficacy of HMPPS TC treatment. This paper reflects on available research within the current context of TC treatment and provides an original overview of the current UK TC practice. It has value in recommending areas for further research and consideration.

Abstract

Purpose

Describes how actionable knowledge is created to successfully initiate consulting relationships designed to promote changes in the composition of corporate boards and, ultimately, social change to eliminate exclusionary practices that are keeping women from consideration for board seats.

Design/methodology/approach

Examines the work of The Boston Club in promoting women on corporate boards to build needed theory to guide change efforts.

Findings

Concludes that no one theoretical perspective supplies the necessary guidance. Presents a model that combines psychodynamic, organizational learning, open systems, and critical management studies views.

Originality/value

Presents the push/pull approach taken by The Boston Club that addresses perceived social constraints and psychological needs involved in changing behavior to create commitment to adding women to boards. Suggests that the sequencing of push and pull techniques may be an important consideration in designing change efforts.

Purpose

The purpose of this study is to seek answer to the following question: Does participating in storytelling changes participants’ life experiences in their journey of recovery? The study explores participants’ experiences of engaging in a storytelling programme in a community mental health team in a large New Zealand city. The programme aims to provide a safe environment to support and increase participants’ engagement with services and the community. Currently, there is limited literature on consumer’s experiences of engaging in therapeutic storytelling programmes in the international or New Zealand context.

Design/methodology/approach

Following ethical approval, eight adult participants were recruited to the study. Data were collected through semi-structured interviews and analysed using the six-phase process of thematic analysis, developed by Braun and Clarke.

Findings

Three main themes emerged from the data. The first theme, breaking barriers, was described by participants as to how they overcame barriers. The second theme, creating positive memories, uncovered the positive experiences that participants gained from the programme. The final theme, becoming a new person, described some of the transformative experiences that emerged through participation in the programme.

Originality/value

This is the first qualitative study in the New Zealand context that has explored the value of a storytelling programme from a consumer’s perspective. The findings suggest that participating in a storytelling programme can contribute to a participant’s journey of recovery; the use of myths, legends and group work is fundamental to those from collective cultures with oral traditions. The ongoing value of storytelling as a therapeutic tool requires further research and the development of a clearer evidence base to inform practice.

Abstract

Purpose

The purpose of this paper is to provide an overview of a rare phenomenon of physical aggression in a Therapeutic Community (TC) and the clinical results of its management.

Design/methodology/approach

The case vignette illustrates the psychodynamic and socio-therapy elements of the treatment model worked within the service prior to, during and after the incident. The information demonstrates how the TC model finds significant value in the living-learning experience that is possible in physical aggression and its management.

Findings

The dynamic administration at the interface between TC with preparation groups has a potential pitfall which might increases the risk of aggression in TC.

Originality/value

It is an original study focusing on the interface between new notion of preparation groups and TC following a case of Physical aggression. Physical aggression is a rare incident in TCs and there are very limited literature about it. The paper has a distinctive feature of being prepared in collaboration with TC service users.

Abstract

Purpose

Effective treatments for juvenile sexual offenders are needed to reduce the societal impact of sexual crimes. The purpose of this paper is to review the empirical literature on treatments for this clinical population.

Design/methodology/approach

The authors searched PsycInfo and MEDLINE (via PubMed) for studies that evaluated outcomes of treatments with juvenile sexual offenders.

Findings

There are a small but growing number of treatment studies (n=10) with juvenile sexual offenders, and all of these studies evaluated cognitive-behavioral therapy or multisystemic therapy for problem sexual behaviors. The results of these studies are promising, although conclusions about treatment effectiveness have been frequently limited by methodological problems.

Originality/value

The authors provide recommendations for treatment providers and policymakers to consider in their decisions about interventions for juvenile sexual offenders. Furthermore, the authors offer suggestions for researchers who seek to develop effective interventions targeting this clinical population.

Purpose

This paper aims to address how one Doctorate in Clinical Psychology (DClinPsy) programme contributes to the shaping of attitudes of its trainee clinical psychologists (TCPs) towards cognitive behavioural therapy (CBT).

Design/methodology/approach

A total of 28 TCPs completed an online, mixed-methods questionnaire relating to their attitudes towards CBT, what factors had influenced their attitude and how competent they felt in applying CBT to clinical practice.

Findings

The majority of respondents reported a positive attitude towards CBT. There was a statistically significant positive change at an individual level in TCPs’ views of CBT between the point at which they applied for the DClinPsy and the present day. Thematic analysis of qualitative data identified influential factors on the development of TCP attitudes towards CBT. The vast majority of TCPs reported that they felt competent applying CBT in their clinical practice.

Research limitations/implications

Overall, the DClinPsy has a positive effect on TCPs’ attitudes towards CBT. However, the influence of placements has a more mixed effect on attitudes. A small sample size reduced the reliability of these conclusions. Recommendations for further evaluation have been made.

Originality/value

This paper evaluates the effect of a DClinPsy programme on TCPs’ attitudes towards CBT. The value is that it establishes which components of the course have different effects on trainee attitudes.

In part 1 of this paper we used the concept of the occupational role to illustrate some of the contributions social scientists make to the better understanding of the problems confronting members of work organisations. The problem we were particularly interested in commenting upon was the difficulties surrounding the inter and intra‐organisational mobility of labour, and we used an analysis of the structure of the occupational role to examine the processes which result in the inability and/or unwillingness of employees to change their job.
