Abstract

Purpose

Excessive drinking of fluids occurs across the autism spectrum but despite the detrimental and potentially hazardous consequences very few studies of this phenomenon have been published. Literature on the topic is sparse. The purpose of this paper is to explore the issues via a large on-line survey and a clinic sample. It is hoped this study will encourage further interest in and investigation including exploration of the links with the neurology underlying autistic conditions.

Design/methodology/approach

Results were obtained via a large on-line survey of autistic individuals and parents and carers (n=637) and data from referrals to a single diagnostic assessment and diagnostic clinic.

Findings

Of 634 respondents of the on-line survey 474 reported excessive drinking of fluids in severe form. Almost two-thirds stated that this started before the age of five years. Of the children and adults seen at the clinic approximately one-third had dunk fluids excessively in the past. The effects of gender, type of autism condition, intellectual disability, reported stress and associated conditions were examined as were the type of fluids drunk and rate of drinking. The response to having to wait for a drink and the occurrence of vomiting and diarrhoea were also examined.

Research limitations/implications

This is a preliminary study but with a large sample size. Limitations lie in the sparse amount of literature on this topic as it affects autism and reliance on parental and self-reports from an on-line survey, the majority of whom responded to an appeal for participants for whom excessive drinking of fluids was an established problem. The clinic sample comprised children and adults who had not been referred for reasons connected to excessive drinking of fluids but for whom this was still a significant problem. A further limitation concerns the absence of data on sensory profiles. This would be worth including in any follow up.

Practical implications

It is important to be aware of the implications of excessive drinking of fluids on the health and well-being of children and adults on the autism spectrum. As there are potentially lethal consequences associated with such behaviours it is essential that they are recognised, understood and responded to.

Social implications

Excessive drinking of fluids has implications for the development of the child and far reaching consequences for physical and social well-being.

Originality/value

This is an original paper that draws on the limited literature available but is primarily based on the results of a unique on-line survey and evaluation of a clinic sample.

Abstract

Purpose

Diabetes is one of the life-threatening chronic diseases, which is already affecting 422m people globally based on (World Health Organization) WHO report as at 2018. This costs individuals, government and groups a whole lot; right from its diagnosis stage to the treatment stage. The reason for this cost, among others, is that it is a long-term treatment disease. This disease is likely to continue to affect more people because of its long asymptotic phase, which makes its early detection not feasible.

Design/methodology/approach

In this study, the authors have presented machine learning models with feature selection, which can detect diabetes disease at its early stage. Also, the models presented are not costly and available to everyone, including those in the remote areas.

Findings

The study result shows that feature selection helps in getting better model, as it prevents overfitting and removes redundant data. Hence, the study result when compared with previous research shows the better result has been achieved, after it was evaluated based on metrics such as F-measure, Precision-Recall curve and Receiver Operating Characteristic Area Under Curve. This discovery has the potential to impact on clinical practice, when health workers aim at diagnosing diabetes disease at its early stage.

This study has not been published anywhere else.

Abstract

Classical definitions of motivation typically involve two main components: direction and activation. Motivated behavior is directed toward or away from particular stimuli (i.e., appetitive and aversive motivation). Furthermore, activational aspects of motivation refer to the observation that motivated behavior is characterized by substantial activity, vigor, persistence, and exertion of effort in both the initiation and maintenance of behavior. Although separate neural systems direct organisms toward distinct motivational stimuli (e.g., food, water, sex), there appears to be a common circuitry regulating behavioral activation and the exertion of effort. Mesolimbic dopamine is one of the brain systems mediating activational aspects of motivation and exertion of effort. This system integrates aspects of motivation and motor control functions involved in the instigation of action. Research on the neurobiology of effort has contributed to our understanding of the pathophysiology of neurological and psychiatric disorders that are characterized by motivational dysfunction.

Abstract

Purpose

This article is the first in a two‐part series exploring challenging behaviour, its prevalence, risk factors and causes. The aim of this article is to provide an overview of prevalence studies and explore the various risk factors that have been associated with challenging behaviour. The authors also seek to explore methodological differences across studies that may contribute to the prevalence variations reported in the literature.

Design/methodology/approach

The article summarises the findings from frequently cited prevalence studies as well as more recent studies.

Findings

The prevalence of challenging behaviour reported in the literature has varied due to methodological differences across studies. Despite this, the best estimate is believed to be from 15 to 17.5 per cent. A range of factors have been associated with challenging behaviour and include gender, age, severity of disability and residential setting.

Originality/value

This article appears to represent the most current overview of research in this area.

Purpose

Non-communicable diseases such as type 2 diabetes, hypertension, cancers and cardiovascular diseases have become a major health concern globally. As literature claims that frequent consumption of fruits and vegetables can delay the onset of type 2 diabetes and its complications, this paper aims to evaluate the potential hypoglycemic properties in five types of non-leafy vegetables (pumpkin, sweet potato, bitter gourd, onion and lady’s finger), which are commonly available in Malaysia.

Design/methodology/approach

Articles were identified through several main search engines, including Pubmed, Google Scholar, Taylor and Francis Online, EDS, Wiley, ScienceDirect and Scopus. The search was limited to selected keywords to refine the outcome.

Findings

All the five types of non-leafy vegetables demonstrate hypoglycemic properties to some extent. Emerging findings indicate that there are several phytonutrients in the non-leafy vegetables contributing to the hypoglycemic effects. To date, the underlying mechanism of action remains to be elucidated, although a number of potential mechanisms of action have been proposed in the literature.

Originality/value

This review provides some insights into the hypoglycemic properties in non-leafy vegetables. In addition, phytonutrients that are responsible for the hypoglycemic effects and their mechanism of action are also highlighted.

Abstract

Background: Hyperglycemia in pregnancy (HIP) is a common medical complication during pregnancy and is associated with several short and long-term maternal-fetal consequences. We aimed to determine the prevalence and factors associated with HIP among Ugandan women.

Methods: We consecutively enrolled eligible pregnant women attending antenatal care at Kawempe National Referral Hospital, Kampala, Uganda in September 2020. Mothers known to be living with diabetes mellitus or haemoglobinopathies and those with anemia (hemoglobin <11g/dl) were excluded. Random blood sugar (RBS) and glycated hemoglobin A1c (HbA1c) were measured on peripheral venous blood samples. HIP was defined as an HbA1c ≥5.7% with its subsets of diabetes in pregnancy (DIP) and prediabetes defined as HbA1c1c of ≥6.5% and 5.7–6.4% respectively. ROC curve analysis was performed to determine the optimum cutoff of RBS to screen for HIP.

Results: A total of 224 mothers with a mean (±SD) age 26±5 years were enrolled, most of whom were in the 2nd or 3rd trimester (94.6%, n=212) with a mean gestation age of 26.6±7.3 weeks. Prevalence of HIP was 11.2% (n=25) (95% CI: 7.7–16.0). Among the mothers with HIP, 2.2% (n=5) had DIP and 8.9% (n=20) prediabetes. Patients with HIP were older (28 years vs. 26 years, p=0.027), had previous tuberculosis (TB) contact (24% vs. 6.5%, p=0.003) and had a bigger hip circumference (107.8 (±10.4) vs. 103.3 (±9.7) cm, p=0.032). However only previous TB contact was predictive of HIP (odds ratio: 4.4, 95% CI: 1.2–14.0; p=0.022). Using HbA1c as a reference variable, we derived an optimum RBS cutoff of 4.75 mmol/L as predictive of HIP with a sensitivity and specificity of 90.7% and 56.4% (area under the curve=0.75 (95% CI: 0.70–0.80, p<0.001)), respectively.

Conclusions: HIP is common among young Ugandan women, the majority of whom are without identifiable risk factors.
