Abstract
Purpose
Pharmaceuticals have played an important role in improving the quality of life of the human population in modern times. However, it must also be acknowledged that both the production and use of pharmaceuticals have a significant, negative impact on the environment and consequently, a negative impact on the health of humans and wildlife. This negative impact is due to the embedded carbon in pharmaceuticals' manufacture and distribution and the waste generated in their manufacture, consumption and disposal. Pharmaceutical waste is comprised of contaminated waste (unwanted pharmaceuticals and their original containers) and non-contaminated waste (non-hazardous packaging waste). The paper aims to discuss these issues.
Design/methodology/approach
The article is a literature review.
Findings
The article identified a gap in the literature around pharmacist attitudes and behaviour toward the environmentally responsible handling of pharmaceutical waste.
Originality/value
Pharmacists, with their professional commitment to the quality use of medicines and their active participation in the medicines management pathway, already play an important role in the more sustainable use of pharmaceuticals. Even so, they have the potential to play an even greater role with the environmentally responsible disposal of pharmaceutical waste (including packaging waste) and the education of other health professionals and the general public on this topic.

Abstract
Purpose
The purpose of this study is to explore circular economy (CE) initiatives and apply the stimulus-organism-response theory to find the socio-political drivers and enablers of CE in the pharmaceutical industry. CE as a concept was relatively not studied much with respect to socio-political interests from operations management perspectives. This was especially so in the pharmaceutical industry.
Design/methodology/approach
This research study was anchored in the theoretical conversation of stimulus-organism-response theory to find the socio-political interests and enablers of the regenerative CE principles. These were the functions of remanufacturing, reuse and recycle. For this research study, data was collected in two steps. First, eight industry practitioners were interviewed to understand the CE practices in the pharmaceutical industry. Then 166 chiefs of production and operations functions from 124 pharmaceutical companies were surveyed. The quantitative data was empirically analyzed using SmartPLS3 software.
Findings
This research study revealed that pressure from suppliers and other public stakeholders were driving regenerative CE practices in the pharmaceutical industry. The results further stated that CE enablers such as green information technology systems and internal environmental management were critical for making pharmaceutical manufacturing operations circular.
Research limitations/implications
This research study measured the constructs on a formative scale. Studies measuring socio-political interests, CE enablers and sustainability practices constructs on a formative scale were much required for the development of the CE theory. This research study output could be applied across geographies and industries to measure the indicators of CE.
Practical implications
This research study indicated that in the context of the pharmaceutical industry, there was an overemphasis on the remanufacture and reuse principles. However, the focus on recycling principles was mostly subdued. For managers and regulators in the pharmaceutical sector, this research study provided clear insights that for more effective CE implementation. This was based on an effective application of recycling practices in the critical functions in pharmaceutical industry.
Originality/value
Earlier research studies on green and environmental manufacturing were focused on linear production models. To provide clear and robust foundations for CE theory, this research study considered operations management from the perspective of the value chain. This comprised the entire circular production model. Earlier research studies had treated socio-political interests, CE enablers and sustainability practices as reflective constructs. This study was one of the foremost to measure these constructs on a formative scale.

Abstract
Purpose
This research aims to design a green pharmaceutical supply chain (PSC) that reduces preventable pharmaceutical waste and effectively disposes of inevitable pharmaceutical waste. The main output of this study is the formulation of an integrated green PSC model involving all critical stakeholders, leading to improved environmental, economic and safety performance in medication management and delivery.
Design/methodology/approach
The research is based on literature and on secondary resources.
Findings
To green the PSC, every producer of waste is duty bound to facilitate the safe handling and disposal of waste. A cross boundary green PSC (XGPSC) approach is proposed to identify participants' contribution to the PSC. Peripheral influences are also recognised from professional and regulatory bodies.
Research limitations/implications
This study focuses solely on community PSC in the UK where patients receive medication from local community pharmacies and thus may be limited. The proposed XGPSC approach also needs to be tested and validated in practice. It may also be difficult to transfer some of the environmental practices proposed in this research into practice.
Practical implications
The environmental practices and actions proposed provide invaluable insight into various PSC activities, including purchasing, product design, prescription patterns and processes, medication use review, and customer relationship management.
Social implications
The proposed environmental actions encourage firm commitment from everyone to reduce, recycle or effectively dispose of pharmaceutical waste, with patients becoming stewards of medication rather than only consumers.
Originality/value
A cross boundary approach is developed to green the PSC, which encourages total involvement and collaboration from all participants in PSC.

Purpose
Pharmaceutical donations are a practical approach to increase medicine availability during disasters such as disease outbreaks. However, often donated pharmaceuticals are inappropriate and unsuitable. This convergence of inappropriate pharmaceuticals is a severe operational challenge and results in environmental hazards. This study explores the pharmaceutical supply chains (PSCs) during a disease outbreak to relieve the negative impact of the material convergence problem (MCP).
Design/methodology/approach
This study adopts a situation-actors-process learning-action-performance (SAP-LAP) linkage framework to understand the PSC dynamics. The problem-solving component of the SAP-LAP analysis provides the strategies catering to MCP. The findings from the SAP-LAP helped to develop the causal loop diagram (CLD). This study conducts several experiments on the proposed strategies by integrating CLD into a stock and flow diagram. Later, a disease outbreak case study accessed the pharmaceutical donations effect on PSC performance.
Findings
The study synthesises and evaluates propositions and strategies to incorporate circular economy (CE) principles in PSC. This study proposed two strategies; one to sort and supply and the other to sort, supply and resell. The reuse policy improves humanitarian organisations' finances in the simulation study. This study verified the operational improvement of PSC by reducing the transport and storage burden due to MCP.
Originality/value
This study comprehensively approaches the issue of drug donation and uniquely produced several propositions for incorporating a CE perspective in PSC. The study also proposed a unique simulation approach to model the donation arrivals in response to a disease outbreak using susceptible, exposed, infectious and recovered modelling.

Abstract
Purpose
Millions of people die of chronic diseases within inpatient settings annually in the United States, despite patient preferences for dying at home. This medicalization of dying has received social and economic critiques for decades. This chapter offers a further analysis to these critiques by examining the ecological impacts of inpatient end-of-life care on the natural environment and occupational and public health.
Methodology
We compare the ecological health outcomes of medical care in three inpatient units (conventional cancer unit, palliative care ward, and hospice facility) using ethnographic observations, semi-structured interviews, and institutional records on medical supply use, waste generation, and pharmaceutical administration and disposal.
Findings
Care provided on all three medical units had significant socioecological impacts. Cumulative impacts were greatest on the conventional unit, followed by palliative care, and lowest on the hospice unit. Variations in impacts mirrored differences in dependence on material interventions, which arose from variations in patient needs, institutional policies, and nursing cultures between the three units.
Practical implications
Social and economic concerns have been major drivers in reforming end-of-life medical care, and our analysis shows that ecological concerns must also be considered. Transitioning terminal patients to less materially intensive modes of care when appropriate could mitigate ecological health impacts while honoring patient preferences.
Originality
This chapter describes how the medicalization of dying has converged with institutional policies, practices, and actors to increase the negative consequences of medical care, and recognizes that the far-reaching impacts of clinical decisions make the provision of medical care a socioecological act.

Abstract
Purpose
This study aims to explore the downstream pharmaceutical supply chain (PSC) and provides insight to the delivery process of medicines and associated operational inefficiencies.
Design/methodology/approach
An exploratory, qualitative approach was adopted to examine PSC inefficiency within two European contexts, namely, the UK and Greece. Data was gathered through interviews and a thematic analysis conducted to analyse the data and identify challenges faced by both supply chains(SCs).
Findings
The medicines delivery system needs to be enhanced in terms of quality, visibility, speed and cost to perform effectively. The findings demonstrated that although the healthcare SCs in the two European contexts have different operational structures, the results are in concordance with each other. Financial, communication, waste and complexity issues were the major concerns.
Research limitations/implications
To the knowledge this is the first study to examine aspects of the medicines SC via a cross-case analysis in the UK and Greece and extends the body of knowledge. A broader sample of responses is warranted to further validate these findings.
Practical implications
The study outputs can inform pharmacies’ strategic to instigate targeted improvement interventions. The implications of which may be extrapolated further to other European healthcare organisations.
Originality/value
This research contributes to the academic literature by adding further theoretical insights to SC strategy development, especially those that have been characterised as highly complex. The study identifies four key areas of intervention needed within this SC (in both countries) to promote higher level efficiencies and effectiveness.

Abstract
Purpose
A direct correlation exists between waste disposal, disease spread and public health. This article systematically reviewed healthcare waste and its implication for public health. This review identified and described the associations and impact of waste disposal on public health.
Design/methodology/approach
This paper systematically reviewed the literature on waste disposal and its implications for public health by searching Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA), PubMed, Web of Science, Scopus and ScienceDirect databases. Of a total of 1,583 studies, 59 articles were selected and reviewed.
Findings
The review revealed the spread of infectious diseases and environmental degradation as the most typical implications of improper waste disposal to public health. The impact of waste includes infectious diseases such as cholera, Hepatitis B, respiratory problems, food and metal poisoning, skin infections, and bacteremia, and environmental degradation such as land, water, and air pollution, flooding, drainage obstruction, climate change, and harm to marine and wildlife.
Research limitations/implications
Infectious diseases such as cholera, hepatitis B, respiratory problems, food and metal poisoning, skin infections, bacteremia and environmental degradation such as land, water, and air pollution, flooding, drainage obstruction, climate change, and harm to marine and wildlife are some of the public impacts of improper waste disposal.
Originality/value
Healthcare industry waste is a significant waste that can harm the environment and public health if not properly collected, stored, treated, managed and disposed of. There is a need for knowledge and skills applicable to proper healthcare waste disposal and management. Policies must be developed to implement appropriate waste management to prevent public health threats.

Because medical waste may present potential hazards to employees, waste handlers and the general public, all facilities generating this form of waste should develop and implement a medical waste management strategy. This strategy should be prepared after conducting a survey to determine the types and estimated amounts of medical waste generated by the facility. The strategy should address medical waste handling, including segregation, packaging, in‐house transport and storage. The management plan must also prescribe appropriate treatment procedures for contaminated waste, and designate proper methods for final disposal of medical wastes and treatment residues. Finally, the facility must ensure the implementation and monitoring of this strategy.

Abstract
Purpose
This study focuses on identifying potential challengers affecting cold chain performance in the pharmaceutical industry. Considering the history of humanity, every pandemic experienced could only be controlled with the vaccine found. While the world is fighting the unforgettable epidemic called COVID-19, scientists are also working to find the therapeutic vaccine. The vaccines studied have different temperature storage and transport properties. In the pharmaceutical industry, it is necessary to know and analyse every step of the cold supply chain to provide the most appropriate and safe cooling level. In addition, it is important to understand the relationship between all the facilities, equipment, tools and materials needed to avoid mistakes along the chain.
Design/methodology/approach
Hence, this study focuses on identifying potential challengers affecting sustainable cold supply chain performance in the pharmaceutical industry and proposing a conceptual framework that involves these main challengers. In this study, firstly, different main and sub-factors are defined from the literature, and fuzzy Decision Making Trial and Evaluation Laboratory method is applied to analyse the causal link between these factors for an effective application.
Findings
Results showed that packaging, transportation and shipping, storage specifications and handling practices, inventory management, technical issues and delivery delay are the most affected factors during the sustainable cold supply chain performance in the pharmaceutical industry. This study offers both managerial implications and a roadmap that are designed with a holistic view to guide manufacturer, logistics parties and even policymakers.
Originality/value
Some of the studies related to the pharmaceutical industry are monitoring and controlling the temperature in the cold supply chain steps; the other part is the studies where the chain steps are examined with a focus on production or transportation. While these issues are the focus, the requirements and conditions of each stage of the supply chain must be studied for a safe, effective and sustainable cold chain beyond the current global pandemic crisis. To the best of the authors’ knowledge, this is the first study that highlights identifying the potential challengers that affect cold supply chain performance for the pharmaceutical industry both theoretically and empirically, solving the causal relationships among these challengers and designing a holistic framework.

Abstract
Purpose
Hospital solid waste may cause serious health hazards and impair the quality of life of the community through transmission of diseases and injury if not properly managed. Therefore, the purpose of this paper is to examine solid waste management practice (SWMP) in Obafemi Awolowo University Teaching Hospital Complex.
Design/methodology/approach
Primary data were collected for this study through interview and questionnaire administration. Systematic sampling technique was used to select 60 students, 13 staffs, 43 cleaners, 8 contractors, and a management staff for questionnaire administration. Data obtained were analyzed using frequency distribution, pictorial analysis, and factor analysis.
Findings
Findings established that solid wastes components generated in the hospital were not segregated in line with the directive of the World Health Organization. The study further established that the factors influencing SWMP accounted for 79.9 percent variance in the following proportion: available storage and collection facilities (30.94 percent), number of patients’ factors (17.86 percent), transportation factor (15.39 percent), human and material resource factors (8.33 percent), and disposal (7.36 percent).
Originality/value
The study therefore concludes that the effectiveness of SWMPs depends on the facilities and equipment, human resources capacity, and frequency of waste collection and disposal.
