Abstract

Purpose

The purpose of the paper is to investigate the potential relationships between emergency-care flow, patient mortality and healthcare costs using a patient-flow model.

Design/methodology/approach

The researchers used performance data from one UK NHS trust collected over three years to identify periods where patient flow was compromised. The delays’ root causes in the entire emergency care system were investigated. Event time-lines that disrupted patient flow and patient mortality statistics were compared.

Findings

Data showed that patient mortality increases at times when accident and emergency (A&E) department staff were struggling to admit patients. Four delays influenced mortality: first, volume increase and mixed admissions; second, process delays; third, unplanned hospital capacity adjustments and finally, long-term capacity restructuring downstream.

Research limitations/implications

This is an observational study that uses process control data to find times when mortality increases coincide with other events. It captures contextual background to whole system issues that affect patient mortality.

Practical implications

Managers must consider cost-decisions and flow in the whole system. Localised, cost-focused decisions can have a detrimental effect on patient care. Attention must also be paid to mortality reports as existing data-presentation methods do not allow correlation analysis.

Originality/value

Previous studies correlate A&E overcrowding and mortality. This method allows the whole system to be studied and increased mortality root causes to be understood.

Abstract

Purpose

The purpose of this paper is to describe the growth of the early ski market and the marketing strategies that the Union Pacific Railroad took in promoting Sun Valley ski resort, one of the most popular early destination ski resorts in the USA.

Design/methodology/approach

The paper uses primary and secondary source material, including ski periodicals, national magazines and the manuscript collection of W. Averell Harriman, the Chairman of the Union Pacific Railroad during the creation of Sun Valley.

Findings

This paper finds that Sun Valley pioneered the western ski vacation by conducting careful market research into not only the snow and weather conditions of western mountains, but also into the habits and economic potential of skiers and winter tourists.

Originality/value

Scholarly work on skiing has primarily looked at the sport from the social and cultural perspective of skiers. Work on entrepreneurial objectives of ski resort designers has largely focused on the period after the Second World War. This is among the first works to analyze entrepreneurial activities and marketing strategies in the ski industry before the Second World War. As a result, the paper challenges the idea that big business only began to shape the ski industry during the Cold War. Instead, this paper shows that large corporations like the Union Pacific Railroad were influential in growing the ski market by building resorts that illustrated the importance of market segmentation to the success of ski areas. In this way, the paper challenges the popular idea that Sun Valley was merely a media sensation and shows that it was a carefully designed business that exhibited a nuanced approach to changes in the ski market.

Abstract

Purpose

The purpose of this paper is to provide an historical perspective to help understand the forces that resulted in the Sarbanes‐Oxley Act. It aims to provide an historical vindication of the taxonomy developed by Charles Conrad in 2003.

Design/methodology/approach

The paper applies a framework developed by Charles Conrad in 2003 to explain the events that led to the corporate meltdown in 2002‐2003 and compare it to a similar scandal in the insurance industry at the beginning of the twentieth century.

Findings

A number of parallels were found between the two incidents. Additionally, the framework developed by Charles Conrad in 2003 was vindicated by the historical comparison. Lessons for practicing managers, domestic and international, are presented along with avenues for possible future research.

Practical implications

Recent changes in the political landscape, particularly in the USA, may indicate that Sarbanes‐Oxley will, indeed, be with us for a longer, rather shorter time. However, the real lesson for managers and scholars of management may be to concentrate on the three trends that foreshadow scandals and meltdowns to prevent similar problems, with their inevitable legal backlash in the future.

Originality/value

This paper uses the framework developed by Charles Conrad in 2003 to explore to different corporate meltdowns separated by a century in the USA. Lessons learned from these incidents as well as a perspective on the probable effective life of the Sarbanes‐Oxley Act are suggested.
