Abstract

Purpose

One of the major challenges facing health systems in developing countries is the international migration of professional nurses, coupled with migration from rural to urban areas and gravitation to the private sector from the public sector. This study aims to determine what the future work plans of professional nurses in South Africa are and to determine appropriate, contextually relevant strategies to retain nurses where they are needed most.

Design/methodology/approach

This is a cross‐sectional survey of professional nurses conducted throughout South Africa using a pre‐tested and self‐administered questionnaire. A total of 569 professional nurses participated in the study.

Findings

Of the respondents, 34.8 percent indicated an intention to change their sector of employment within the next five years while only 30.2 percent reported that they would most likely be still in their current positions as professional nurses in five years' time. Younger nurses, nurses in the public sector and nurses from the more rural provinces were also significantly less likely to be in their current positions within the next five years. Public sector nurses felt that employment security, workplace organisation and the working environment were the most important factors. Private sector nurses, however, rated workplace organisation, employment security and professional practice as being most important.

Originality/value

The paper presents evidence that health provision in South Africa is facing an imminent crisis with the overwhelming majority of nurses planning to leave their current positions. The findings suggest that most nurses, irrespective of whether they intend to stay or leave, feel that the same issues need to be addressed if they are to be persuaded to remain in their current positions. This implies that a homogeneous approach could be adopted across sectors and geographic regions to promote health organisations in a more attractive way to nurses.

Abstract

Purpose

The purpose of this paper is to operationalize the concepts of total quality management (TQM) and market orientation (MO) and to analyze the relationship between them in a major economic sector of activity, namely, the social services sector.

Design/methodology/approach

The paper analyzes the aforementioned relationships by means of a quantitative study carried out with information from managers of 137 Spanish organizations in the field of social service provision and, more specifically, in nursing homes for the elderly. Structural equation models were used to contrast the relationships.

Findings

Evidence is provided for the multi-dimensional structure of TQM and MO. The results confirm that TQM contributes to a greater degree of MO in the organizations analyzed, since a significant positive relationship between a set of components of the first concept (namely, processes, leadership and people) and the second is found.

Practical implications

Managers need to take into account the fact that the application of a management system in nursing homes that is based on TQM dimensions favors the development of an MO culture. The staff are the key factor in orientating the residential home toward the market.

Originality/value

The research covers a gap in the literature since these relationships have not been studied empirically in depth in service sectors such as social services and nursing homes, despite great and growing economic importance of this sector.

Abstract

Purpose

Strategic socio-political views are notably scarce among contemporary discourses on the causes of abuse of vulnerable older people in care and nursing homes. This paper aims to catalyse higher order consideration and discussion of one socio-political characteristic that has relevance to the issue of abuse, that is, the market-like environment in which care and nursing homes in England operate. In doing so, the paper argues that the now firmly established but imbalanced “quasi-market” of care that has developed over many years fosters conditions under which both poor care and abuse are more likely to occur. The evidence presented in the paper focusses primarily on the rise to dominance of for-profit care home provision and the contraction of public sector provision. The paper does not examine in detail the characteristics and market presence of the not-for-profit sector because it has not held a numerically significant market share either historically or contemporarily.

Design/methodology/approach

Outcomes of the marketisation of the care home sector that has its origins in the political landscape prevailing in 1979 and thereafter, along with the concurrent development of its regulation and oversight, are narrated and analysed. From this, a mythos of the motives behind the transition to a market-like economy that has taken place over four decades is developed and used to explain how prevailing market conditions contribute to the perpetuation of poor care and abuse.

Findings

In the opinion of the author, there are identifiable consequences arising from the evolution of the current care economy that dispel the beliefs that providing care by means of current market-like arrangements is advantageous, that the independent regulation and monitoring of such a market is unproblematic and effective, and that the “consumers” in this market are exercising personal choice, in accord with classical economic theory.

Practical implications

The paper offers the opportunity for the reader to consider how the development of a “quasi” market of care and nursing home services that has come to be dominated by for-profit private providers, and that is subject to ineffective oversight, may have contributed to conditions where abuse is more likely to occur and endure.

Originality/value

This is a conceptual paper that explores the consequences of the creation of the market-like economy of care in which care and nursing homes now operate and suggests that it is time for prevailing market conditions to be re-visited and subjected to remedial strategic intervention.

Abstract

Purpose

The purpose of this paper is to use generational cohort and professionalism theories as the framework to examine the interaction between supervisor‐subordinate relationships, work‐family conflict, discretionary power and affective commitment at the work‐life interface for Northeast Brazilian public sector professional nurses.

Design/methodology/approach

Quantitative data were collected from 550 public hospital nurses in North‐Eastern Brazil. Path and multivariate analysis were used to test the hypotheses.

Findings

The findings demonstrate that the impact of the independent variables on affective commitment was statistically significant but low. The impact of NPM factors such as contracting out and multiple job‐holding was a major influence on affective commitment and work‐life interface across generational cohorts. The analysis revealed significant differences between generational cohorts and suggested that affective commitment may well be enhanced by improving the quality of the work‐life interface and consequently, the wellbeing of nurses.

Research limitations/implications

This study is confined to the Northeast of Brazil and confined to public sector hospitals. The self‐reporting techniques used in this study to gather information may be open to common method bias.

Originality/value

The contribution of this research includes the provision of new information about the working context of professional nurses in Brazil, which is a fast growing BRICS economy where the issues surrounding the practice of nursing and nurse management are not well studied to date (i.e. NPM impact on nurse environment). North‐eastern Brazilian managers need to be more aware of generational differences and their impact on levels of affective commitment and the quality of the work‐life interface and wellbeing.

Abstract

Concerns about quality and standards of care in the nursing and residential home sector have exercised policymakers, managers and practitioners in both Australia and the UK for some years. While Australia is a relatively young country, demographically speaking, it has in place a coherent ‘aged care’ policy. The UK on the other hand, with its rapidly ageing profile, has only recently made a serious policy commitment to the health and social care agenda for older people. Australia therefore has several years of experience to be shared with the UK when it comes to policy and practice of quality improvement. In particular, there are valuable lessons to be learnt from Australia's national outcome standards and monitoring system for care homes, and its more recent introduction of a care homes accreditation system. Apart from identifying any issues associated with the implementation of such approaches to quality improvement, it is important to establish whether they have an impact on the quality of care and life of older residents. As the UK moves to implementing national minimum standards in 2002, lessons from Australia are timely and may help inform best practice and policy in long‐term care in the future.

Purpose

Taking into account strong intra‐European discrepancies between workforce models for women, such as part‐time work rates and the ability to have uninterrupted careers, this article aims to show how nursing careers are shaped differently by employment and gendered social policies in France and Britain. Using the case of state hospitals, it explores how French and British nurses reconcile sector‐specific work constraints and family life at different stages of their life and career cycle.

Design/methodology/approach

This paper compares nurses' career patterns using age, “early exit” and part‐time data drawn from national and European official sources, large‐scale surveys and secondary literature.

Findings

Although hospital employment policies prioritise entry‐level recruitment over retention of mature nurses in both countries, French nurses are more likely to have uninterrupted full‐time careers than their British counterparts thanks to the protective effect of state employment regulations and interventionist gendered policies. However, the positive impact of the French social model on nurses' careers is limited to the childbearing and rearing stage.

Research limitations/implications

The analysis of nursing careers is constrained by the scarcity of official longitudinal data in both countries. Although the study is limited to one profession in two countries, the method used is transferable to other professional groups and national contexts.

Originality/value

Thanks to the cross‐national comparative approach, the paper broadens profession‐centred analyses, highlighting the impact of employment and social policies on female professional careers.

Abstract

Purpose

All nursing students have the opportunity to gain experience in the Independent Sector. The Health and Social Care Act (2012) in the UK has led to more health services being provided by the Independent Sector. The purpose of this paper is to explore the perceptions of first year student nurses of their learning experience in nursing homes placements within the Independent Sector.

Design/methodology/approach

Two focus groups were undertaken with first year student nurses, one with six students and the other with seven students. All students had completed four week clinical placements in nursing homes. The focus groups were semi structured. Thematic analysis was undertaken on the data.

Findings

Important themes that emerged from the data included: leadership issues, poor practice relating to infection control and manual handling, team dynamics, commitment to addressing issues when raised and challenges to raising issues within the placement area. The findings provided an insight into some of the barriers for student nurses when raising issues. The decision to raise concerns was multifactorial and complex. Attention was also focused on the relationship between the mentor and the student and the impact that can have on the learning environment.

Research limitations/implications

The limitations for this study included: this was a small scale study, the findings were from a small number of placement areas within a small geographical area of the UK. Some students may have felt reluctant to share their views in a focus group. The findings reflect the views of first year student nurses only. However, first year students can provide an alternative view of a placement area. Their lack of experience can be an opportunity to question accepted norms with the mentor, acting as a resource of new ideas.

Originality/value

This paper identified opportunities and challenges for student nurses and education providers alike. For the student it was the complexities of raising issues and for the education provider it was equipping students with the knowledge and skills to raise such issues not only as a student but also upon qualification.

Purpose

The purpose of this paper is to present findings from a research project designed to determine the qualifications held by those staff who had perpetrated abuse in private sector care and nursing homes for older people during a 12-month period.

Design/methodology/approach

A self-completion, postal questionnaire was issued to the safeguarding teams of all local authorities in England with adult social care responsibilities to determine the qualifications held by staff who were proven to have perpetrated abuse in these facilities.

Findings

Though findings with respect to qualified nurses who had perpetrated abuse when considered in isolation were inconclusive in numerical terms, the proportion of all nursing and care staff who had perpetrated abuse, and who held either a professional or vocational qualification was high.

Research limitations/implications

Responses to the postal questionnaire represented 21.8 per cent of local authorities with social services responsibilities, yet the data secured suggests that care providing staff who have received recognised training are disproportionately represented among those proven to have perpetrated abuse.

Originality/value

Findings indicate that recognised training for those who provide care in care and nursing homes is of limited efficacy in the prevention of abuse.

Abstract

The purpose of this study is to investigate factors surrounding low Emirati student recruitment and retention within a nursing programme in the Abu Dhabi Emirate. A sequential explanatory mixed methods research design was used to collect data from 140 Emirati students at different levels of the nursing program. Questionnaires, semi-structured interviews and related document analyses were used to collect information for this study. Quantitative analysis revealed that overall participants perceived the Emirati society as holding a positive image of nursing, though a majority of them indicated that nursing as a profession, would be a third or last choice of career for them. Inferential analysis on the reasons to choose or not choose nursing, revealed an opportunity to work as part of the team; and as a stepping stone to other professions as significant reasons, versus perceived program difficulty level, and perceived unpleasant tasks. Thematic analysis of interviews highlighted: 1) personal interest; 2) parental support; 3) awareness of the profession; 4) perceived barriers; 5) potential benefits, and 6) policies at the local and national levels. The study has several implications for nursing education and policy in the United Arab Emirates and the region.
